    HIV prevalence in blood donors and recipients in Pakistan: a meta-analysis and analysis of blood-bank data

    Background: The first instances of HIV-antibody detection in donated blood in Pakistan were reported in 1988. Since then, documentation of HIV in blood donors and of rates of transmission via transfusion has been limited. Previously assumed to have a low prevalence, HIV is an increasing health concern in Pakistan. Since there is no national, centralized blood-banking system, there are no reliable data on which to base estimated risks of transfusion-associated HIV infection. This study was therefore conducted to estimate the prevalence of HIV in blood donors and recipients in Pakistan between 1988 and 2012. Methods: Meta-analyses were undertaken of reported prevalences of HIV in blood donors and recipients published during 1988-2012. Papers were identified by searching PubMed, Google, CINAHL and PakMediNet and the websites of the World Health Organization, the national HIV/AIDS Surveillance Project and the National AIDS Control Programme of Pakistan. In addition, the 1998-2012 records of the Aga Khan University blood bank were analysed. Results: The 254 abstracts identified at the preliminary search were reviewed and, after removal of duplications, case-reports, editorials and reviews, 32 papers were selected that met the inclusion criteria. All studies that reported on HIV antibodies in blood donors/recipients were included, irrespective of the methodology used. Since seroconversion had only been confirmed through supplemental testing in a few papers, the results were analysed separately for reports based on screening only and confirmed cases. A total of 142 of 2 023 379 blood donors and 4 of 3632 blood recipients were HIV positive, giving an overall pooled seroprevalence of 0.00111% in blood donors and 0.00325% in blood recipients. The annual prevalences of HIV in donors at the Aga Khan University blood banks were similar, ranging from 0.013% to 0.116%. Conculsion: Very few reports on HIV in blood donors in Pakistan could be retrieved, and the overall pooled prevalence is low. However, the limited data and confounding factors mean that that these results may significantly underestimate the true situation. It is recommended that a complete survey of blood banks should be conducted throughout the country, in order to provide a more reliable estimate of the risk of transfusion-associated HIV infection in Pakistan

    Since the Commercial off-the-shelf (COTS) software become widespread components in the market for building the systems with less time and cost, the COTS evaluation and selection becomes a non-trivial task. There are many models attempted to propose a set of characteristics for evaluating and selecting COTS software.However, these models have concentrated on the functional and quality characteristics of COTS software, leaving other effective characteristics related to vendor (the organization that developed, support, and realized COTS software) and user (the organization that integrated and used COTS software) organizations. Therefore, this paper proposed a set of important characteristics relevant to the vendor and user organizations that play important role to discriminate between COTS alternatives in COTS evaluation and selection process.Most of the related studies have been analyzed and carefully studied in the literature to identify and propose these characteristics together with their attributes and associated metrics

    Prevalence of hepatitis B in the blood donors of NW. F.P and FATA regions and the current scenario of HBV infection in Pakistan

    Hepatitis B is a fatal liver disease caused by the hepatitis B virus. In this study, blood donors from various districts of the North-western frontier province and the federally administered tribal area (FATA) of Pakistan were tested for HBsAg and HBV DNA by ICT (Immuno-chromatographic test), ELISA and RTPCR. Out of the 7148 blood donors, 244 (3.41%) were positive for HBsAg by ICT, 147 (2.05%) by ELISA while 132 (1.85%) were positive by PCR. Our data indicates that the incidence of hepatitis B has decreased in these regions in recent times.Key words: HBV, HBsAg, Pakistan

    Molecular analysis of genetic diversity in elite II synthetic hexaploid wheat screened against Barley yellow dwarf virus

    The presence of sufficient genetic diversity in the germplam is an important prerequisite for the improve-ment and efficient use of available material. Traditionally, the data on agronomic, morphological and physiological plants traits are used to estimate the genetic diversity. But now molecular markers are available for authenticated and reliable studies for genetic diversity. The present study was conducted to assess the genetic diversity of Elite-II synthetic hexaploid (SH) wheat by genome DNA fingerprinting as revealed by random amplified polymorphic DNA (RAPD) analysis. Ten decamer RAPD primers (OPG-1, OPG-2, OPG-3, OPG-4, OPG-5, OPA-3, OPA-4, OPA-5, OPA-8, and OPA-15) were used to evaluate the diversity profile of the selected SH entries. Primers OPG-2 andOPA-4 gave the highly polymorphic results. The pair wise similarity values shows that genotypes 1, 20 and 30 have most closest relationship with highest similarity values i.e., 100% while genotypes 9and 13 show dictinct relationship with minimum similarity value that is, 54%. Therefore, the allelic variation of the SH resistance germplasm is a potent means to enrich and improve bread wheat cultivars where BYDV is a production threat and these can be used in future wheat breeding programme

    Hepatitis B and C: prevalence and risk factors associated with seropositivity among children in Karachi, Pakistan

    Background Infections with hepatitis B virus (HBV) and hepatitis C virus (HCV) can lead to chronic liver disease and hepato-cellular carcinoma (HCC). This cross-sectional study estimated the prevalence and identified risk factors associated with Hepatitis B surface antigen (HBsAg) and HCV antibody (anti-HCV) sero-positivity among children 1 to 15 years of age. Methods The study targeted the low to middle socioeconomic population that comprises 80% to 85% of the population. Consent was obtained from parents of the eligible children before administering questionnaire and collected a blood sample for anti-HCV and HBsAg serology. Results 3533 children were screened for HBsAg and anti-HCV. 1826 (52 %) were males. 65 (1.8 %) were positive for HBsAg, male to female ratio 38:27; mean age 10 ± 4 years. 55 (1.6 %) were positive for anti-HCV with a mean age 9 ± 4 years. 3 (0.11%) boys were positive for both HBsAg and anti-HCV. The overall infection rate was 3.3 % in the studied population. Hepatitis BsAg was more prevalent in subjects who received therapeutic injections 45 (69.2%) positive [Odd Ratio OR = 2.2; 95% Confidence interval CI: 1.3–3.6] inspite of using new needle and syringe 44 (67.7%) positive [OR = 2.2; 95% CI: 1.3–3.7] and vaccination in the government healthcare facilities 46 (70.7 %) positive with [OR = 3.0; 95% CI: 1.4–6.4]. These factors were not significant in anti-HCV positive cases. Conclusion There is a need to educate general population regarding HBV and HCV infection and risks associated with inappropriate therapeutic injections. Hepatitis B vaccine should be administered to all newborns regardless of maternal HBsAg status

    Role of Family System, Positive Emotions and Resilience in Social Adjustment among Pakistani Adolescents

    AbstractThe present study explored the role of positive emotions, resilience & family system in social adjustment in Pakistani adolescents.  Through convenient sampling, data  collected from 150 adolescents (70 boys & 80 girls).In terms of family system 92 were living in nuclear family while 58 injoint family. Urdu version of Modified Differential Emotions Scale, Resilience Scale and Bell`s Adjustment Inventory were used. Statistical analysis revealed the presence of significant positive relationship between positive emotions and resilience (r = .39**) where girls were slightly more resilient (M ± SD = 120.77 ± 24.03) & socially adjusted (M ± SD = 17.41 ± 4.15). Moreover 52 % adolescents reported difficulty in talking with new people. Majority of participants from Joint family reported good social adjustment (38%) and high resilience (17%) as compared to those in nuclear family (21%, 13%).  The influence of these demographic variables is important indication to be explored further.Keyword: Family system, Positive emotion, and resilienc

    Temporal trends in hepatitis B and C infection in family blood donors from interior Sindh, Pakistan

    <p>Abstract</p> <p>Background</p> <p>Hepatitis B (HBV) and C (HCV) infections are a serious global and national public health problem. Earlier studies have reported increasing rates of hepatitis infection in Pakistan, particularly in rural areas. Pakistan has no active surveillance program to monitor trends of these infections. The objective of this study was to verify this trend in blood donors from the rural Sindh area of the country.</p> <p>Methods</p> <p>The study analysed the data of blood donors of interior Sindh who donated blood at JPMC blood bank from January 1, 2004 to September 15, 2007. HBsAg status was determined by using HBsAg Serodia kit and antibodies to HCV using the Detect HCV ™ V.3 Kit. Samples repeatedly reactive for HBsAg or anti-HCV were considered positive for HBV or HCV infection respectively.</p> <p>Results</p> <p>The overall seroprevalence of HBV infection among donors was 6.2 % (95% CI 5.5%–6.9%) and did not change significantly over the study period. Overall seroprevalence of HBV infection in literate blood donors was 5.7 %(95% CI 4.7%–6.8%). Prevalence decreased significantly in this group over the study period (p = 0.05). No other significant trends in seroprevalence of HBV infection were seen in the stratified analyses.</p> <p>The overall seroprevalence of HCV among donors was 7.5% (95% CI 6.8%–8.3%) and increased significantly over the study period from 7.2% (95% CI 5.8%–8.7%) in 2004 to 8.9% (95% CI 7.4%–10.6%) in 2007 (p = 0.02). Significant increase in seroprevalence was particularly seen in literate (p = 0.03), non–first time (p = 0.01) and Sindhi speaking (p = 0.01) donors.</p> <p>Conclusion</p> <p>Our study finds a steady increase in the prevalence of HCV infection in blood donors from interior Sindh between 2004 and 2007. On the contrary, decreasing prevalence of HBV was found, particularly in literate blood donors. There may be a need to have rural community-based epidemiological studies to identify the determinants of the spread of HCV infection and also those that are limiting the spread of HBV infection particularly in the literate blood donor population.</p

    Evidence for a Founder Effect among HIV-infected injection drug users (IDUs) in Pakistan.

    Background: We have previously reported a HIV-1 subtype A infection in a community of injection drug users (IDUs) in Karachi, Pakistan. We now show that this infection among the IDUs may have originated from a single source. Methods: Phylogenetic analysis was performed of partial gag sequences, generated using PCR, from 26 HIV-positive IDU samples. Results: Our results showed formation of a tight monophyletic group with an intra-sequence identity of \u3c 98% indicating a founder effect . Our data indicate that the HIV-1 epidemic in this community of IDUs may have been transmitted by an HIV positive overseas contract worker who admitted to having contact with commercial sex workers during stay abroad. Conclusion: Specific measures need to implemented to control transmission of HIV infection in Pakistan through infected migrant workers
