    Molecular and biochemical characterization of Xanthomonas axonopodis pv. citri pathotypes

    Prevalence of citrus bacterial canker caused by Xanthomonas axonopodis pv. citri in citrus groves is the major impediment and limiting factor in successful citrus production. Severity varies among different species, varieties and prevailing climatic conditions. Despite extensive studies on the biology, epidemiology and management of this disease, there is still little known about the role of different biocontrol agents for management of this disease. Traditional management of X. axonopodis pv citri is brought about by chemicals which have become complicated through the development of chemical resistance, and as such, it is hazardous for health. It is necessary to identify the pathotypes of X. axonopodis pv. citri through biochemical and molecular characterization and to determine the role of different biocontrol agents (antibiotics and plant extracts), in order to find out a safer way for controlling citrus canker as disease severity results in defoliation, dieback, premature fruit drop and blemished fruit that consequently decrease fruit production and market value.Keywords: Citrus, bacterial canker, Xanthomona

    Influence of proline priming on antioxidative potential and ionic distribution and its relationship with salt tolerance of wheat

    Mechanisms involved in salt tolerance urge exploration and investigation of genotypic variation to assist future breeding programs. Comparative examination of ten wheat cultivars for salt tolerance and their response towards proline-seed-priming was performed. Exposure of wheat seedlings to salinity resulted in prominent reduction in root and shoot growth attributes of all cultivars. Furthermore, decrease in the chlorophyll contents was evident although this varied among cultivars. Wheat seedlings grown from proline pre-treated seeds exhibited improved photosynthetic pigments, besides this response was also cultivar and concentration dependent. Generally, salt stressed plants exhibited higher antioxidant enzyme activities. Proline priming significantly influenced antioxidant activities, however, its magnitude varied. The peroxidase activity varied among wheat cultivars that were evident from the analysis of POD activity on Native-PAGE gel. Salinity caused the accumulation of Na+ in the roots and the magnitude of Na+ translocation to the shoot was cultivar dependent. Similarly, K+ uptake and its distribution among root and shoot varied. Priming treatments affected ion distribution of Na+ and K+ but inter-cultivar variations were evident. Conclusively, all the cultivars investigated exhibited differential response to salinity and proline seed pre-treatments. However, the proline-priming mediated improvements in growth and antioxidant enzyme activities contributed to stress tolerance which partly relied on the ability of the plant to uptake sodium and its partitioning in the roots. Of the cultivars tested, Faisalabad-08 and Bhakhar-2002 were ranked as relatively salt tolerant and the cvs. AARI-10, MH-97 and Auqab-2000 as relatively salt sensitive

    Alleviation of salinity effects by exogenous applications of salicylic acid in pearl millet (Pennisetum glaucum (L.) R. Br.) seedlings

    Experiments for the study of alleviation of salt stress effects by exogenous applications of salicylic acid (SA) in pearl millet (Pennisetum glaucum (L.) R. Br.) seedlings were conducted in soil filled earthen potshaving size 12 inches with 7 kg soil. Three treatments comprising, control (T0), 50-mol m-3 NaCl (T1) and T2 are having NaCl (50 mol m-3) + SA (50 mg l-1). Pots were arranged in completely randomized design (CRD) with 6 replicates. NaCl significantly reduced the plant and root lengths, plant fresh and dry weights. In contrast, NaCl did not show any adverse effect on plants treated with NaCl plus SA. Salicylic acidtreated pearl millet plants under NaCl salinity strongly reduced accumulations of Na+, K+, Ca2+ and Cl− and glycinebetaine (GB) and total soluble carbohydrates (TSC) as compared to NaCl treatments. Higher N and relative water contents (RWC) was noted in T2 (NaCl + SA) but it reduced in T1 (NaCl) as compared to control.     Advances in “Omics” Approaches for Improving Toxic Metals/Metalloids Tolerance in Plants

    Food safety has emerged as a high-urgency matter for sustainable agricultural production. Toxic metal contamination of soil and water significantly affects agricultural productivity, which is further aggravated by extreme anthropogenic activities and modern agricultural practices, leaving food safety and human health at risk. In addition to reducing crop production, increased metals/metalloids toxicity also disturbs plants’ demand and supply equilibrium. Counterbalancing toxic metals/metalloids toxicity demands a better understanding of the complex mechanisms at physiological, biochemical, molecular, cellular, and plant level that may result in increased crop productivity. Consequently, plants have established different internal defense mechanisms to cope with the adverse effects of toxic metals/metalloids. Nevertheless, these internal defense mechanisms are not adequate to overwhelm the metals/metalloids toxicity. Plants produce several secondary messengers to trigger cell signaling, activating the numerous transcriptional responses correlated with plant defense. Therefore, the recent advances in omics approaches such as genomics, transcriptomics, proteomics, metabolomics, ionomics, miRNAomics, and phenomics have enabled the characterization of molecular regulators associated with toxic metal tolerance, which can be deployed for developing toxic metal tolerant plants. This review highlights various response strategies adopted by plants to tolerate toxic metals/metalloids toxicity, including physiological, biochemical, and molecular responses. A seven-(omics)-based design is summarized with scientific clues to reveal the stress-responsive genes, proteins, metabolites, miRNAs, trace elements, stress-inducible phenotypes, and metabolic pathways that could potentially help plants to cope up with metals/metalloids toxicity in the face of fluctuating environmental conditions. Finally, some bottlenecks and future directions have also been highlighted, which could enable sustainable agricultural production

    Impact of age on outcome after colorectal cancer surgery in the elderly - a developing country perspective

    <p>Abstract</p> <p>Background</p> <p>Colorectal cancer (CRC) is a major source of morbidity and mortality in the elderly population and surgery is often the only definitive management option. The suitability of surgical candidates based on age alone has traditionally been a source of controversy. Surgical resection may be considered detrimental in the elderly solely on the basis of advanced age. Based on recent evidence suggesting that age alone is not a predictor of outcomes, Western societies are increasingly performing definitive procedures on the elderly. Such evidence is not available from our region. We aimed to determine whether age has an independent effect on complications after surgery for colorectal cancer in our population.</p> <p>Methods</p> <p>A retrospective review of all patients who underwent surgery for pathologically confirmed colorectal cancer at Aga Khan University Hospital, Karachi between January 1999 and December 2008 was conducted. Using a cut-off of 70 years, patients were divided into two groups. Patient demographics, tumor characteristics and postoperative complications and 30-day mortality were compared. Multivariate logistic regression analysis was performed with clinically relevant variables to determine whether age had an independent and significant association with the outcome.</p> <p>Results</p> <p>A total of 271 files were reviewed, of which 56 belonged to elderly patients (≥ 70 years). The gender ratio was equal in both groups. Elderly patients had a significantly higher comorbidity status, Charlson score and American society of anesthesiologists (ASA) class (all p < 0.001). Upon multivariate analysis, factors associated with more complications were ASA status (95% CI = 1.30-6.25), preoperative perforation (95% CI = 1.94-48.0) and rectal tumors (95% CI = 1.21-5.34). Old age was significantly associated with systemic complications upon univariate analysis (p = 0.05), however, this association vanished upon multivariate analysis (p = 0.36).</p> <p>Conclusion</p> <p>Older patients have more co-morbid conditions and higher ASA scores, but increasing age itself is not independently associated with complications after surgery for CRC. Therefore patient selection should focus on the clinical status and ASA class of the patient rather than age.</p

    Awareness and Use of Benzodiazepines in Healthy Volunteers and Ambulatory Patients Visiting a Tertiary Care Hospital: A Cross Sectional Survey

    Background: Indiscriminate prescription of Benzodiazepines in Pakistan and subsequent availability over-the-counter without prescription is a major public health problem, requiring systematic inquiry through research. Additionally, there is limited data on the awareness and use of Benzodiazepines from developing countries making it impossible to devise meaningful health policies. Methodology/Principal Findings: This was an Observational, Cross-Sectional study. conducted at Aga Khan University. A total of 475 (58.5 % males, 41.5 % females) people visiting a tertiary care hospital were interviewed by means of a structured questionnaire. The results showed that majority of population was aware of one or more Benzodiazepines (80.4%) and 30.4 % had used them at some point in life. 42.4 % of the users had been using it for more than a year. Commonest reason for use was sleep disturbance. Frequency of usage was higher for females, married individuals, educated (.Grade12), high socioeconomic status and housewives. More (59%) were prescribed than not and of them most by GP (58.5%). Only 36.5% of them were particularly told about the long-term addiction potential by the use of these drugs. Conclusion: Easy availability, access to re-fills without prescription and self prescription compounded with the lack of understanding of abuse potential of benzodiazepines constitutes a significant problem demanding serious consideratio

    The widening gap between the digital capability of the care workforce and technology-enabled healthcare delivery: a nursing and allied health analysis

    There is a need to ensure that healthcare organizations enable their workforces to use digital methods in service delivery. This study aimed to evaluate the current level of digital understanding and ability in nursing, midwifery, and allied health workforces and identify some of the training requirements to improve digital literacy in these health professionals. Representatives from eight healthcare organizations in Victoria, Australia, participated in focus groups. Three digital frameworks informed the focus group topic guide that sought to examine the barriers and enablers to adopting digital healthcare along with training requirements to improve digital literacy. Twenty-three participants self-rated digital knowledge and skills using Likert scales and attended the focus groups. Mid-range scores were given for digital ability in nursing, midwifery, and allied health professionals. Focus group participants expressed concern over the gap between their organizations’ adoption of digital methods, relative to their digital ability and there were concerns about cyber security. Participants also saw a need for the inclusion of consumers in digital design. Given the widening gap between digital innovation and health workforce digital capability, there is a need to accelerate digital literacy by rapidly deploying education and training and policies and procedures for dig-ital service delivery

    Bleeding disorders in the tribe: result of consanguineous in breeding

    <p>Abstract</p> <p>Objective</p> <p>To determine the frequency and clinical features of bleeding disorders in the tribe as a result of consanguineous marriages.</p> <p>Design</p> <p>Cross Sectional Study</p> <p>Introduction</p> <p>Countries in which consanguinity is a normal practice, these rare autosomal recessive disorders run in close families and tribes. Here we describe a family, living in village Ali Murad Chandio, District Badin, labeled as haemophilia.</p> <p>Patients & Methods</p> <p>Our team visited the village & developed the pedigree of the whole extended family, up to seven generations. Performa was filled by incorporating patients, family history of bleeding, signs & symptoms, and bleeding from any site. From them 144 individuals were screened with CBC, bleeding time, platelet aggregation studies & RiCoF. While for PT, APTT, VWF assay and Factor VIII assay, samples were kept frozen at -70 degrees C until tested.</p> <p>Results</p> <p>The family tree of the seven generations comprises of 533 individuals, 63 subjects died over a period of 20 years and 470 were alive. Out of all those 144 subjects were selected on the basis of the bleeding history. Among them 98(68.1%) were diagnosed to have a bleeding disorder; 44.9% patients were male and 55.1% patients were female. Median age of all the patients was 20.81, range (4 months- 80 yrs). The results of bleeding have shown that majority had gum bleeding, epistaxis and menorrhagia. Most common bleeding disorder was Von Willebrand disease and Platelet functional disorders.</p> <p>Conclusion</p> <p>Consanguineous marriages keep all the beneficial and adversely affecting recessive genes within the family; in homozygous states. These genes express themselves and result in life threatening diseases. Awareness, education & genetic counseling will be needed to prevent the spread of such common occurrence of these bleeding disorders in the community.</p

    Utilisation of an operative difficulty grading scale for laparoscopic cholecystectomy

    Background A reliable system for grading operative difficulty of laparoscopic cholecystectomy would standardise description of findings and reporting of outcomes. The aim of this study was to validate a difficulty grading system (Nassar scale), testing its applicability and consistency in two large prospective datasets. Methods Patient and disease-related variables and 30-day outcomes were identified in two prospective cholecystectomy databases: the multi-centre prospective cohort of 8820 patients from the recent CholeS Study and the single-surgeon series containing 4089 patients. Operative data and patient outcomes were correlated with Nassar operative difficultly scale, using Kendall’s tau for dichotomous variables, or Jonckheere–Terpstra tests for continuous variables. A ROC curve analysis was performed, to quantify the predictive accuracy of the scale for each outcome, with continuous outcomes dichotomised, prior to analysis. Results A higher operative difficulty grade was consistently associated with worse outcomes for the patients in both the reference and CholeS cohorts. The median length of stay increased from 0 to 4 days, and the 30-day complication rate from 7.6 to 24.4% as the difficulty grade increased from 1 to 4/5 (both p < 0.001). In the CholeS cohort, a higher difficulty grade was found to be most strongly associated with conversion to open and 30-day mortality (AUROC = 0.903, 0.822, respectively). On multivariable analysis, the Nassar operative difficultly scale was found to be a significant independent predictor of operative duration, conversion to open surgery, 30-day complications and 30-day reintervention (all p < 0.001). Conclusion We have shown that an operative difficulty scale can standardise the description of operative findings by multiple grades of surgeons to facilitate audit, training assessment and research. It provides a tool for reporting operative findings, disease severity and technical difficulty and can be utilised in future research to reliably compare outcomes according to case mix and intra-operative difficulty
