Is Cerebrospinal Fluid C-reactive Protein a Better Tool than Blood C-reactive Protein in Laboratory Diagnosis of Meningitis in Children?
Objectives: This study aimed to test whether C-reactive protein (CRP) measurement could differentiate between different types of meningitis and become a routine test. Methods: A prospective study included 140 children admitted to Manipal Teaching Hospital, Pokhara, Nepal, between July 2009 and June 2011. The subjects had a blood test and detailed cerebrospinal fluid (CSF) analysis, including blood and CSF CRP levels. Results: Of those admitted, 31.1% had pyogenic meningitis (PM), 26.2% partially treated meningitis (PPM), 33% viral meningitis (VM), and 9.7% tubercular meningitis (TBM), with 26.4% controls. Organisms were isolated in 12.5% of the cases by blood culture and 25% of cases through CSF culture. Blood CRP was positive in all groups, with the highest values in PM (53.12 ± 28.88 mg/dl) and PPM (47.55 ± 34.34 mg/dl); this was not statistically significant (P = 0.08). The CSF CRP levels were significantly higher (P <0.001) in PM (45.75 ± 28.50 mg/dl) and PPM (23.11 ± 23.98 mg/dl). The sensitivity and specificity of blood CRP was 90.62%, 88.88%, 64.7%, 70% and 32.4%, 30.97%, 24.52%, 26.12% and that of CSF CRP was 96.87%, 66.66%, 20.58%, 10% and 74.73%, 63.71%, 50.94%, 55.35% for PM, PPM, VM and TBM, respectively. Conclusion: Because of its high sensitivity, both CSF CRP and blood CRP can be used to screen for bacterial meningitis (both PM and PPM). CSF CRP screening yielded results with a higher specificity than blood CRP; hence, it can be a supportive test along with CSF cytology, biochemistry, and microbiology for diagnosing meningitis

THD Analysis of a Seven, Nine, and Eleven Level Cascaded H-Bridge Multilevel Inverter for Different Loads
A multilevel inverter is implemented for generating the required staircase AC voltage of output from various steps of voltages of DC sources. The multilevel inverter gives a better harmonic spectrum and a compatible quality of output. This article delves into an analytical analysis of the total harmonic distortion (THD) of different multilevel inverters which employ a multicarrier PWM technique. This technique is implemented for operating the switches at their respective angle of conduction. This paper deals with various cascaded H-Bridge multilevel inverters (CMI) with various loads that are modelled by implementing the MATLAB/Simulink platform. The output gives a better result of the proposed model in terms that it is helpful towards reducing the THD and the losses of switching
ANTIOXIDANT AND ANTIPROLIFERATIVE POTENTIALS OF MARINE ACTINOMYCETES
Objective: The aim of present studyis to isolate and characterize a novel actinomycetes strain from marine sediments collected at Kakinada coast. To evaluate antioxidant and antiproliferative activities of the crude methanolic extract of isolated actinomycetes.Methods: Actinomycetes was isolated from marine sediments collected at Kakinada coast using starch casein agar. Morphological, biochemical and molecular characterization was performed and the crude methanolic extract was used for determination of in vitro antioxidant activity and antiproliferative activity against MDA-MB 231 breast cancer cells.Results: The crude methanolic extract exhibited significant antioxidant activity with ICĂÂ50Ă value in the range of 30-46 Ă”g/ml in different radical scavenging assays. Moreover, the extract showed concentration dependent cytotoxicity against triple negative breast cancer cell line, MDA-MB 231 with IC50 of 42.5Ă”g/ml. The extract also inhibited cell proliferation with IC50 value 44.1 Ă”g/ml probably by arresting cell cycle at S phase. The antioxidant activity of the extract was strongly correlated with cytotoxic and antiproliferative activity.Conclusion: The isolated marine actinomycetes can be the potential source of antioxidants with the anticancer property
Hybrid Renewable Energy Sources based Mobile Charging Unit for Public Usage
Mobiles are becoming a part of our daily life and the usages of mobile phones are increasing day by day to simplify our lifestyle. The charging in the battery is very important to operate the mobile. However, the availability of charging points is very difficult to find in public places including bus stands, railway stations, most populated areas etc. Hence installing a charging device to charge various mobiles effectively is needed at many places in the society. Moreover, unlimited power supply is required to such a device for an effective operation. Further the utilizing of renewable energy sources to provide sufficient power supply to the charging device will be more effective. Renewable energy sources such as Photovoltaic (PV) and wind based hybrid power supply systemsare incorporated into the charging device to supply required reliable power. However, different voltage levels are required to charge batteries of various mobiles. Hence, a multi-output dc to dc converter along with a master control unit is implemented in this paper. OPAL RT modules are utilized to create a Hardware-in-the-Loop (HIL) system, which showcases outcomes across different operational modes
Estimation of the correlation coefficient using the Bayesian Approach and its applications for epidemiologic research
BACKGROUND: The Bayesian approach is one alternative for estimating correlation coefficients in which knowledge from previous studies is incorporated to improve estimation. The purpose of this paper is to illustrate the utility of the Bayesian approach for estimating correlations using prior knowledge. METHODS: The use of the hyperbolic tangent transformation (Ï = tanh Ο and r = tanh z) enables the investigator to take advantage of the conjugate properties of the normal distribution, which are expressed by combining correlation coefficients from different studies. CONCLUSIONS: One of the strengths of the proposed method is that the calculations are simple but the accuracy is maintained. Like meta-analysis, it can be seen as a method to combine different correlations from different studies
The High Burden of Cholera in Children: Comparison of Incidence from Endemic Areas in Asia and Africa
Cholera is an often forgotten disease affecting the world's forgotten people. When a large cholera outbreak occurs, the disease appears briefly on the radar of public attention. Some unfortunate populations around the world suffer recurrent episodes of cholera but their plight goes unnoticed. We established cholera surveillance in impoverished areas in Jakarta (Indonesia), Kolkata (India), and Beira (Mozambique) where the disease is known to occur regularly. The cholera burden was calculated using the site population as the denominator and the number of cholera cases as the numerator. The lowest overall rate was in Jakarta with 0.5 cases per 1000 population per year. The incidence was three times higher in Kolkata (1.6/1000/year) and eight times higher in Beira (4.0/1000/year), adding to the growing impression of the large cholera problem in Africa. In all sites, children are the most affected. Estimates such as these are useful when considering where and among whom interventions against the disease are most needed. Improvement of water supply and sanitation is the best strategy against cholera and other diarrheal diseases but may not be achievable in these impoverished areas in the near future. Other immediate, short- to medium-term strategies such as vaccination against cholera may be useful
