    Recession coverage using the modified coronally advanced tunnel and connective tissue graft with or without enamel matrix derivative: 5-year results of a randomised clinical trial.

    OBJECTIVES To evaluate the 5-year results of single and multiple recession type (RT) 1 and 2 (Miller I to III) recessions treated with the modified coronally advanced tunnel (MCAT) and connective tissue graft (CTG) with or without an enamel matrix derivative (EMD). The main outcome variable was the stability of obtained root coverage from 6 months to 5 years. MATERIALS AND METHODS In 24 patients, both complete and mean root coverage (CRC and MRC) and gain of keratinised tissue (KT) were assessed at 6 months and 5 years after recession coverage by means of MCAT and CTG with or without EMD. Aesthetic outcomes after 5 years were evaluated using the root coverage aesthetic score (RES). RESULTS At 5 years, 24 patients with a total of 43 recessions were evaluated. Eight patients (57.14%) of the test and 6 (60.0%) of the control group showed complete root coverage. MRC revealed no statistically significant differences between the two groups, with 73.87 ± 26.83% (test) and 75.04 ± 22.06% (control), respectively. KT increased from 1.14 ± 0.57 mm to 3.07 ± 2.27 mm in the test group and from 1.24 ± 0.92 mm to 3.02 ± 1.55 mm in the control group, respectively. CONCLUSION Treatment of single and multiple RT 1 and 2 recessions by means of MCAT and CTG with or without EMD yielded comparable clinical improvements which could be maintained over a period of 5 years. The additional use of EMD did not influence the clinical outcomes. CLINICAL RELEVANCE The use of MCAT + CTG yielded successful coverage of single and multiple RT 1 and 2 gingival recessions, while the additional application of EMD did not seem to influence the results

    Adjunctive laser or antimicrobial photodynamic therapy to non-surgical mechanical instrumentation in patients with untreated periodontitis. A systematic review and meta-analysis.

    AIM To compare the adjunctive effects of lasers or antimicrobial photodynamic therapy (aPDT) to non-surgical mechanical instrumentation alone in untreated periodontitis patients. MATERIALS AND METHODS Two focused questions were addressed using the Population, Intervention, Comparison and Outcome criteria as follows: in patients with untreated periodontitis, i) does laser application provide adjunctive effects on probing pocket depth (PPD) changes compared with non-surgical instrumentation alone? and ii) does application of aPDT provide adjunctive effects on PPD changes compared with non-surgical instrumentation alone? Both randomized controlled clinical trials (RCTs) and controlled clinical trials (CCTs) were included. Results of the meta-analysis are expressed as weighted mean differences (WMD) and reported according to the PRISMA guidelines. RESULTS Out of 1'202 records, 10 articles for adjunctive laser and 8 for adjunctive aPDT were included. With respect to PPD changes, 1 meta-analysis including 2 articles (total n = 42; split-mouth design) failed to identify a statistically significant difference (WMD = 0.35 mm; 95%CI:-0.04/0.73; p = .08) in favour of adjunctive aPDT (wavelength range 650-700 nm). In terms of adjunctive laser application a high variability of clinical outcomes at 6 months was noted. Two articles included patient-reported outcomes and 10 reported on the presence/absence of harms/adverse effects. CONCLUSIONS Available evidence on adjunctive therapy with lasers and aPDT is limited by (i) the low number of controlled studies and (ii) the heterogeneity of study designs. Patient-reported benefits remain to be demonstrated

    Buchweizenanbau in der Schweiz: neue Sorten für eine alte Nischenkultur

    Die Geschichte des Buchweizens reicht weit in die Vergangenheit zurück. Ursprünglich stammt Buchweizen aus China wo Buchweizenarten gemäss Pollenuntersuchungen bereits 2500 v. Chr. wuchsen (Zeller und Hsam 2004). Von dort breitete sich der Buchweizen vermutlich im Mittelalter über Kirgistan, Tadschikistan und Usbekistan in Richtung Europa aus (Zeller 2001). Mit der Intensivierung des Ackerbaus verlor er aber nach und nach an Bedeutung. Aktuell (Datengrundlage 2017) gilt Russland mit 1,52 Millionen Tonnen (von weltweit ca. 4 Millionen Tonnen) als führender Buchweizenproduzent (FAO 2019). In der russischen Küche ist Buchweizen – als Beilage oder Hauptgericht – auch heute noch ein wichtiger Nahrungsbestandteil (Miedaner und Longin 2012). Ab 1400 n. Chr. hat sich der Buchweizenanbau auch in der Schweiz etabliert. Bereits um 1800 n. Chr. beschränkten sich die Anbaugebiete aber nur noch auf einige Täler in Graubünden und im Tessin. (Schilperoord 2017). Hauptgrund war die verstärkte Verbreitung und Förderung des Kartoffelanbaus, was die gesamtschweizerische Anbaufläche von Buchweizen bis ins Jahr 1972 auf unter eine Hektare schrumpfen liess (Lustenberger et al. 1977). Seither taucht Buchweizen nicht mehr einzeln in der Anbaustatistik auf und bis auf die Sorte Brusio ist keine Schweizer Buchweizensorte in der Genbank eingelagert worden (BLW 2019). Seit einigen Jahren nimmt die Bedeutung von Buchweizen in der menschlichen Ernährung aber wieder zu. Gerade für Menschen mit einer Glutenunverträglichkeit (Zöliakie) bietet der glutenfreie Buchweizen eine Alternative zu herkömmlichen Getreideprodukten. Daneben ist Buchweizen auch reich an Spurenelementen wie Zink oder Mangan und wirkt Erkrankungen wie Bluthochdruck oder hohen Cholesterinwerten entgegen (Skrabanja et al. 2004). Aufgrund der geringen Ansprüche an Klima und Boden (Lustenberger et al. 1977) und der eher kurzen Vegetationsdauer von ca. 115 Tagen (Aufhammer et al. 1995) kann Buchweizen als Fruchtfolgeglied interessant sein. Da Buchweizen botanisch gesehen mit keiner anderen häufig in der Schweiz angebauten Kulturpflanze verwandt ist, kann er zur Auflockerung der Fruchtfolge und Aufwertung des Bodens genutzt werden. Durch die kurze Vegetationsdauer besteht zudem die Möglichkeit, Buchweizen als Zweitkultur z. B. nach Wintergerste anzubauen. Als Gründüngungs- und Untersaatenkomponente wird Buchweizen seit vielen Jahren eingesetzt. Unter guten Wachstumsbedingungen ist Buchweizen aufgrund der schnellen Jugendentwicklung ein guter Unkrautunterdrücker. Auch für Insekten ist die Ackerfrucht von Bedeutung, denn sie blüht – je nach Saatzeitpunkt und Sorte – über eine längere Zeit und v. a. während der tendenziell eher trachtarmen Sommermonate. Beim Anbau von Buchweizen als Reinkultur zur Körnernutzung stellen sich zahlreiche Herausforderungen. So ist z. B. die Bestimmung des richtigen Erntezeitpunkts aufgrund des unbegrenzten Wuchses der aktuell verfügbaren Sorten und deshalb auch die Ernte mit dem Mähdrescher schwierig. Neue Züchtungen aus Russland versprechen jedoch eine gleichmässige Abreife bei gleichzeitig hohen Kornerträgen und tieferen Erntefeuchtigkeiten. Um Lösungsansätze für den Buchweizenanbau in der Schweiz zu entwickeln, wurden in einem gemeinsamen Projekt von Agroscope und der Hochschule für Agrar-, Forst- und Lebensmittelwissenschaften HAFL Versuche mit verschiedenen Buchweizensorten und Saatdichten durchgeführt.     Verhalten verschiedener Zwischenkulturen in einem Netzwerk von On-Farm-Versuchen

    Um das Verhalten von Zwischenkulturen bei verschiedenen Umweltbedingungen besser zu verstehen, wurde ein Netzwerk von Versuchsparzellen entlang der Achse Genf- Zürich und im Jura eingerichtet. Einige der untersuchten Arten vermochten den Boden im Herbst schnell zu bedecken (z.B. Sareptasenf). Andere Arten produzierten eine bedeutende Biomasse oberirdischer Pflanzenteile (z.B. Sonnenblume). Wieder andere, im Herbst weniger leistungsfähige Arten, stellten eine gute Bodenbedeckung Ende Winter sicher, wie der Rauhafer. Mit einer multifaktoriellen Analyse konnten die Beziehungen zwischen den Leistungen der verschiedenen Zwischenkulturen und den Umweltbedingungen untersucht werden. Keine Art vereinigt alle über die gesamte Zwischenkultur erwünschten Eigenschaften, Mischungen verschiedener Arten sind jedoch vielversprechend

    Synovial hemangioma of the knee joint in a 12-year-old boy: a case report

    <p>Abstract</p> <p>Introduction</p> <p>Synovial hemangioma is a rare condition and is frequently misdiagnosed, leading to a diagnostic delay of many years.</p> <p>Case presentation</p> <p>We present a case of an atypical synovial hemangioma in a 12-year-old Caucasian boy with a diagnostic delay of 3 years.</p> <p>Conclusion</p> <p>It is important to know that synovial hemangioma mostly affects the knee joint, showing recurrent bloody effusions without a history of trauma. If there are no intermittent effusions, the diagnosis will be even more difficult. In cases of nonspecific symptoms and longstanding knee pain the diagnosis of a synovial hemangioma should also be considered in order to avoid diagnostic delay. Magnetic resonance imaging is the main diagnostic tool to evaluate patients with synovial hemangioma, showing characteristic lace-like or linear patterns.</p> <p>Angiography can identify feeder vessels and offers the possibility of embolisation in the same setting. Surgical excision, either done per arthroscopy or per arthrotomy, is recommended as soon as possible to avoid the risk of damage to the cartilage.</p

    Existing benchmark systems for assessing global warming potential of buildings – Analysis of IEA EBC Annex 72 cases

    Life cycle assessment (LCA) is increasingly being used as a tool by the building industry and actors to assess the global warming potential (GWP) of building activities. In several countries, life cycle based requirements on GWP are currently being incorporated into building regulations. After the establishment of general calculation rules for building LCA, a crucial next step is to evaluate the performance of the specific building design. For this, reference values or benchmarks are needed, but there are several approaches to defining these. This study presents an overview of existing benchmark systems documented in seventeen cases from the IEA EBC Annex 72 project on LCA of buildings. The study characterizes their different types of methodological background and displays the reported values. Full life cycle target values for residential and non-residential buildings are found around 10-20 kg CO2_2e/m2^2/y, whereas reference values are found between 20-80 kg CO2_2e/m2^2/y. Possible embodied target- and reference values are found between 1-12 kg CO2_2e/m2^2/y for both residential and non-residential buildings. Benchmark stakeholders can use the insights from this study to understand the justifications of the background methodological choices and to gain an overview of the level of GWP performance across benchmark systems

    Endochin-like quinolones (ELQs) and bumped kinase inhibitors (BKIs): Synergistic and additive effects of combined treatments against Neospora caninum infection in vitro and in vivo.

    The apicomplexan parasite Neospora caninum is an important causative agent of congenital neosporosis, resulting in abortion, birth of weak offspring and neuromuscular disorders in cattle, sheep, and many other species. Among several compound classes that are currently being developed, two have been reported to limit the effects of congenital neosporosis: (i) bumped kinase inhibitors (BKIs) target calcium dependent protein kinase 1 (CDPK1), an enzyme that is encoded by an apicoplast-derived gene and found only in apicomplexans and plants. CDPK1 is essential for host cell invasion and egress; (ii) endochin-like quinolones (ELQs) are inhibitors of the cytochrome bc1 complex of the mitochondrial electron transport chain and thus inhibit oxidative phosphorylation. We here report on the in vitro and in vivo activities of BKI-1748, and of ELQ-316 and its respective prodrugs ELQ-334 and ELQ-422, applied either as single-compounds or ELQ-BKI-combinations. In vitro, BKI-1748 and ELQ-316, as well as BKI-1748 and ELQ-334, acted synergistically, while this was not observed for the BKI-1748/ELQ-422 combination treatment. In a N. caninum-infected pregnant BALB/c mouse model, the synergistic effects observed in vitro were not entirely reproduced, but 100% postnatal survival and 100% inhibition of vertical transmission was noted in the group treated with the BKI-1748/ELQ-334 combination. In addition, the combined drug applications resulted in lower neonatal mortality compared to treatments with single drugs

    From DNA sequence to application: possibilities and complications

    The development of sophisticated genetic tools during the past 15 years have facilitated a tremendous increase of fundamental and application-oriented knowledge of lactic acid bacteria (LAB) and their bacteriophages. This knowledge relates both to the assignments of open reading frames (ORF’s) and the function of non-coding DNA sequences. Comparison of the complete nucleotide sequences of several LAB bacteriophages has revealed that their chromosomes have a fixed, modular structure, each module having a set of genes involved in a specific phase of the bacteriophage life cycle. LAB bacteriophage genes and DNA sequences have been used for the construction of temperature-inducible gene expression systems, gene-integration systems, and bacteriophage defence systems. The function of several LAB open reading frames and transcriptional units have been identified and characterized in detail. Many of these could find practical applications, such as induced lysis of LAB to enhance cheese ripening and re-routing of carbon fluxes for the production of a specific amino acid enantiomer. More knowledge has also become available concerning the function and structure of non-coding DNA positioned at or in the vicinity of promoters. In several cases the mRNA produced from this DNA contains a transcriptional terminator-antiterminator pair, in which the antiterminator can be stabilized either by uncharged tRNA or by interaction with a regulatory protein, thus preventing formation of the terminator so that mRNA elongation can proceed. Evidence has accumulated showing that also in LAB carbon catabolite repression in LAB is mediated by specific DNA elements in the vicinity of promoters governing the transcription of catabolic operons. Although some biological barriers have yet to be solved, the vast body of scientific information presently available allows the construction of tailor-made genetically modified LAB. Today, it appears that societal constraints rather than biological hurdles impede the use of genetically modified LAB.

    Electrochemical disinfection of toilet wastewater using wastewater electrolysis cell

    The paucity of proper sanitation facilities has contributed to the spread of waterborne diseases in many developing countries. The primary goal of this study was to demonstrate the feasibility of using a wastewater electrolysis cell (WEC) for toilet wastewater disinfection. The treated wastewater was designed to reuse for toilet flushing and agricultural irrigation. Laboratory-scale electrochemical (EC) disinfection experiments were performed to investigate the disinfection efficiency of the WEC with four seeded microorganisms (Escherichia coli, Enterococcus, recombinant adenovirus serotype 5, and bacteriophage MS2). In addition, the formation of organic disinfection byproducts (DBPs) trihalomethanes (THMs) and haloacetic acids (HAA_5) at the end of the EC treatment was also investigated. The results showed that at an applied cell voltage of +4 V, the WEC achieved 5-log_(10) reductions of all four seeded microorganisms in real toilet wastewater within 60 min. In contrast, chemical chlorination (CC) disinfection using hypochlorite [NaClO] was only effective for the inactivation of bacteria. Due to the rapid formation of chloramines, less than 0.5-log_(10) reduction of MS2 was observed in toilet wastewater even at the highest [NaClO] dosage (36 mg/L, as Cl_2) over a 1 h reaction. Experiments using laboratory model waters showed that free reactive chlorine generated in situ during EC disinfection process was the main disinfectant responsible for the inactivation of microorganisms. However, the production of hydroxyl radicals [ OH], and other reactive oxygen species by the active bismuth-doped TiO_2 anode were negligible under the same electrolytic conditions. The formation of THMs and HAA_5 were found to increase with higher applied cell voltage. Based on the energy consumption estimates, the WEC system can be operated using solar energy stored in a DC battery as the sole power source
