A cross sectional study was carried out in the District of Kuala Langat, Selangor in February 1997. The intention is to determine the relationship between effective communication of individuals towards patient satisfaction and factors that influence effective communication. The study was carried out in outpatient clinics of six health clinics and a district hospital which involves 336 patients selected using universal sampling. The data was obtained using guided questionnaires. The outcome of the study shows significant relationship between effective communication and patient satisfaction (t = 3.06, p < 0.05). Communication media, verbal communication and non verbal communication are also the factors that shows significant relationship towards effective communication. Other factors studied such as patient age, sex and ethnic group, doctor's attitude, sex of the doctor and doctor's communication skill do not show any significant relationship towards effective communication. Multiple logistic regression analysis was carried out between media factor, verbal communication and non-verbal communication against effective communication. The result shows that a doctor using media during interaction with a patient, communicates 2.8 times more effective than a doctor who does not utilise the media. For a doctor using verbal communication with easily understood language, the communication is 3.4 times more effective than a doctor who does not use an easily understood language. Meanwhile, a doctor who use non-verbal communication, produce 2.3 times more effective communication as compared to a doctor who did otherwise. By controlling all the confounding factors at the same time, the three factors above still shows significant relationship. This indicates that the factors are the predictive factors towards effective communication. Since effective communication by doctors is an important factor in ensuring patient satisfaction, therefore a comprehensive study should be carried out by taking into consideration other factors which would contribute to effective communication and patient satisfactio

    Get PDF
The pineapple industry produces a substantial amount of solid waste like peels, cores, stems, crowns and pulp. Pineapple waste disposal can cause to microbial spoilage and environmental problems due to the waste material containing high moisture and sugar content. Utilization of pineapple waste, focusing on the peels, to produce a high value added product of essential oil is a good option. However, up to date, there are only a very limited studies specifically aimed on the extraction methods of essential oil from pineapple peels. Therefore, the aim of this study was to demonstrate a feasible method for extraction of essential oil from pineapple peels. Three methods used in the study were (1) hydro- distillation (HD), (2) hydro-distillation with enzyme-assisted pretreatment (HDEA) and (3) supercritical fluid extraction (SFE). Among the methods used, only SFE method resulted in the formation of essential oil with 0.17% (w/w) yield, whereas HD and HDEA methods only produced the hydrosol. The microscopic observation using scanning electron microscope of the sample’s cell wall substantiated that only SFE method resulted in the rupture the essential oil gland after the extraction. The GC-MS analysis showed that volatile compounds mainly identified in the essential oil produced through SFE method were propanoic acid ethyl ester (40.25%), lactic acid ethyl ester (19.35%), 2-Heptanol (15.02%), 3-Hexanone (2.60%) and butanoic acid ethyl ester (1.58%). The analysis results show that pineapple peels contained of important volatile compounds, thus indicating its’ potential to be used as substrate for the aromatic essence production

Cyclodextrin glycosyltransferase (CGTase) (EC 2.4.1.19) represents one of the most important groups of microbial amylolytic enzymes that degrade starch which synthesize cyclodextrin through cyclization reactions. The cyclodextrins able to form inclusion complexes with variety of organic and inorganic guest molecules, thus bring to wide range of applications in food, pharmaceutical, chemical industries, agriculture and environmental engineering. However, the major concern in the CGTase production is the lower enzyme productivities and longer incubation time for maximum enzyme production by wild type bacteria. Besides that, the mixtures of α-, β- and γ-cyclodextrins produced in different ratios contributed to the high purification cost. In this study, the cgt-BS gene from Bacillus sp. NR5 UPM which consists of putative promoter region is used to enhance the recombinant β-CGTase production, in terms of enzyme yield and stability, as well as to reduce the cultivation time. Furthermore, the addition of glycine as an inducer is shown able to enhance the extracellular secretion of β-CGTase up to 1.5-fold by increasing the permeability of the membrane. Therefore, the heterologous expression of β-CGTase followed by optimization of glycine supplementation could be used to enhance the extracellular β-CGTase production, hence could improve the production of cyclodextrin

Cyclodextrin glycosyltransferase (CGTase) is one of the most important groups of microbial amylolytic enzymes that have been used for degradation of starch to yield cyclodextrin (CD) via cyclization reaction. The increasing demand for CD in industrial application has led to an extensive study about CGTase which begin with screening, isolation and characterization of CGTase-producing bacteria. The identification of CGTase producer involves the use of solid media containing phenolphthalein and methyl orange as indicators that was detected by the colour changes. The formations of clearance zone around the bacterial colony in the starch-containing medium were observed and the diameters were measured to gauge the hydrolytic efficiency of the bacteria. Out of 65 soil bacterial samples screened, Bacillus sp. NR5 UPM was identified as the most prolific CGTase producer, which produced highest CGTase activity (11.709 U/ml) and highest β-CD concentration (2.504 mg/ml) with α-CD:β-CD:γ-CD ratio was 0.5:91.1:8.4 when using raw soluble starch as a substrate. It also showed as the best CGTase producer when using sago starch as a substrate (15.514 U/ml). This isolate was known as a raw starchdegrading enzyme producer since it has the capability to use raw starch as a substrate. Thus, in the future, this new isolate perhaps can share the biggest market in industrial application.Key words: Cyclodextrin, cyclodextrin glycosyltransferase, starch

Enzymatic characterization of heterologously expressed fungal β-glucosidase BGLII in Escherichia coli

Saprophytic filamentous fungi such as Trichoderma sp. are capable of producing three categories of cellulase for cellulose degradation into glucose; endoglucanase, cellobiohydrolase and β-glucosidase. Trichoderma reesei in particular has an extensively studied cellulase enzyme system, establishing it as a model organism for enzymatic production. Studies on β-glucosidases produced by Trichoderma sp. have elucidated two variants, which have been classified into glycosyl hydrolase families 1 and 3, both including retaining enzymes capable of substrate hydrolysis via double-displacement method with retention of anomeric carbon. Extensive studies on BglI enzyme isolated has been compared and found to have sequence similarity to other known glycosyl hydrolase family 3 β-glucosidases, an enzyme that is cell wall bound or expressed extracellularly, affects rate of reducing sugar formation from cellulase and involved in synthesis of soluble inducer of cellulolytic enzymes. Comparatively, enzyme characterization of the second β-glucosidase, BglII, has been relatively recent. Nevertheless, it has been identified to be expressed intracellularly, has high affinity and catalyzes transglycosylation reactions on cellobiose. Of particular interest, BglII has shown lower sensitivity to glucose inhibition, an appealing character for bioprocess development for efficient lignocellulosic biomass saccharification. As such, using local Trichoderma sp., this study has sought to characterize the BglII gene isolated and following heterologous expression in Escherichia coli, characterize the recombinant enzyme for enzyme activity, glucose tolerance and product synthesis
