    Tetramethylguanidine (TMG)-catalysed Synthesis of α-Aminophosphonates by a One-pot Reaction

    Aldimines (Schiff’s bases) undergo nucleophilic addition with diethyl/dimethyl/diphenylphosphite (Pudovik reaction) in the presence of a catalytic amount of tetramethylguanidine (TMG) at ambient temperature to afford the corresponding α-aminophosphonates in high yields. The Schiff’s bases were prepared by reacting cinnamaldehyde with substituted amines in refluxing absolute alcohol. The structures of the title compounds were established by elemental analysis and IR, 1H, 13C, 31PNMR and FAB mass spectral data.     Synthesis of Dinaphtho-dioxaphosphocin-8-oxides, Epoxides and Bisphosphonates

    Preparation of 8-substituted-16H-dinaphtho [2,1-d:1’,2’-g] [1,3,2] dioxaphosphocin 8-oxides (5a–g) with an eight-membered phosphorus heterocyclic system (2) and their epoxides and bisphosphonates was accomplished by reacting 8-bromo-dinaphthophosphocin (2) with different mono and bis Grignard reagents (3a–g and 6) followed by oxidation with H2O2. Their structures were confirmed by elemental and spectral (1H, 13C and 31P NMR) data analysis.     Agricultural Information Needs of Rainfed Farmers: A Study from Telangana

    A paradigm shift in rainfed agriculture can be obtained mainly through technological interventions. In this context, the role of extension and advisory services that are saddled with this responsibility should focus on novel approaches. Application of relevant information in agriculture sector brings positive impacts on farm productivity. Timely access to information on weather aspects, best farming practices, market information etc. help farmers to make correct decisions about what crops to plant, when to plant and where to sell their products. The best way to satisfy the rainfed farmers is to understand and consider their specific needs, constraints and capabilities properly, which helps to serve them in an efficient manner. A study has been undertaken to understand the different agricultural information needs of rainfed farmers in the Mahabubnagar district of Telangana state India. It is observed that farmers need various types of information related to agricultural activities and the major agricultural information needs of the farmers were found to be related to farm implements and machinery, water management, seed varieties, seed treatment, alternate crops and pest and disease management. In order to address these information needs, farmers were accessing a number of sources. It is important to enhance the information search capacity of the rainfed farmers to improve the farm productivity and income leading to enhanced livelihoods

    Global, regional, and national comparative risk assessment of 79 behavioural, environmental and occupational, and metabolic risks or clusters of risks, 1990-2015: a systematic analysis for the Global Burden of Disease Study 2015

    SummaryBackground The Global Burden of Diseases, Injuries, and Risk Factors Study 2015 provides an up-to-date synthesis of the evidence for risk factor exposure and the attributable burden of disease. By providing national and subnational assessments spanning the past 25 years, this study can inform debates on the importance of addressing risks in context. Methods We used the comparative risk assessment framework developed for previous iterations of the Global Burden of Disease Study to estimate attributable deaths, disability-adjusted life-years (DALYs), and trends in exposure by age group, sex, year, and geography for 79 behavioural, environmental and occupational, and metabolic risks or clusters of risks from 1990 to 2015. This study included 388 risk-outcome pairs that met World Cancer Research Fund-defined criteria for convincing or probable evidence. We extracted relative risk and exposure estimates from randomised controlled trials, cohorts, pooled cohorts, household surveys, census data, satellite data, and other sources. We used statistical models to pool data, adjust for bias, and incorporate covariates. We developed a metric that allows comparisons of exposure across risk factors—the summary exposure value. Using the counterfactual scenario of theoretical minimum risk level, we estimated the portion of deaths and DALYs that could be attributed to a given risk. We decomposed trends in attributable burden into contributions from population growth, population age structure, risk exposure, and risk-deleted cause-specific DALY rates. We characterised risk exposure in relation to a Socio-demographic Index (SDI). Findings Between 1990 and 2015, global exposure to unsafe sanitation, household air pollution, childhood underweight, childhood stunting, and smoking each decreased by more than 25%. Global exposure for several occupational risks, high body-mass index (BMI), and drug use increased by more than 25% over the same period. All risks jointly evaluated in 2015 accounted for 57·8% (95% CI 56·6–58·8) of global deaths and 41·2% (39·8–42·8) of DALYs. In 2015, the ten largest contributors to global DALYs among Level 3 risks were high systolic blood pressure (211·8 million [192·7 million to 231·1 million] global DALYs), smoking (148·6 million [134·2 million to 163·1 million]), high fasting plasma glucose (143·1 million [125·1 million to 163·5 million]), high BMI (120·1 million [83·8 million to 158·4 million]), childhood undernutrition (113·3 million [103·9 million to 123·4 million]), ambient particulate matter (103·1 million [90·8 million to 115·1 million]), high total cholesterol (88·7 million [74·6 million to 105·7 million]), household air pollution (85·6 million [66·7 million to 106·1 million]), alcohol use (85·0 million [77·2 million to 93·0 million]), and diets high in sodium (83·0 million [49·3 million to 127·5 million]). From 1990 to 2015, attributable DALYs declined for micronutrient deficiencies, childhood undernutrition, unsafe sanitation and water, and household air pollution; reductions in risk-deleted DALY rates rather than reductions in exposure drove these declines. Rising exposure contributed to notable increases in attributable DALYs from high BMI, high fasting plasma glucose, occupational carcinogens, and drug use. Environmental risks and childhood undernutrition declined steadily with SDI; low physical activity, high BMI, and high fasting plasma glucose increased with SDI. In 119 countries, metabolic risks, such as high BMI and fasting plasma glucose, contributed the most attributable DALYs in 2015. Regionally, smoking still ranked among the leading five risk factors for attributable DALYs in 109 countries; childhood underweight and unsafe sex remained primary drivers of early death and disability in much of sub-Saharan Africa. Interpretation Declines in some key environmental risks have contributed to declines in critical infectious diseases. Some risks appear to be invariant to SDI. Increasing risks, including high BMI, high fasting plasma glucose, drug use, and some occupational exposures, contribute to rising burden from some conditions, but also provide opportunities for intervention. Some highly preventable risks, such as smoking, remain major causes of attributable DALYs, even as exposure is declining.     Partition Functions of Holographic Minimal Models

    The partition function of the W_N minimal model CFT is computed in the large N 't Hooft limit and compared to the spectrum of the proposed holographic dual, a 3d higher spin gravity theory coupled to massive scalar fields. At finite N, the CFT contains additional light states that are not visible in the perturbative gravity theory. We carefully define the large N limit, and give evidence that, at N = infinity, the additional states become null and decouple from all correlation functions. The surviving states are shown to match precisely (for all values of the 't Hooft coupling) with the spectrum of the higher spin gravity theory. The agreement between bulk and boundary is partially explained by symmetry considerations involving the conjectured equivalence between the W_N algebra in the large N limit and the higher spin algebra of the Vasiliev theory.Comment: 56 page

    Using a modified Delphi methodology to gain consensus on the use of dressings in chronic wounds management

    Objective: Managing chronic wounds is associated with a burden to patients, caregivers, health services and society and there is a lack of clarity regarding the role of dressings in improving outcomes. This study aimed to provide understanding on a range of topics, including: the definition of chronicity in wounds, the burden of illness, clinical outcomes of reducing healing time and the impact of early interventions on clinical and economic outcomes and the role of matrix metalloproteinases (MMPs) in wound healing. Method: A systematic review of the literature was carried out on the role of dressings in diabetic foot ulcer (DFU), and venous leg ulcer (VLU) management strategies, their effectiveness, associated resource use/cost, and quality of life (QoL) impact on patients. From this evidence-base statements were written regarding chronicity in wounds, burden of illness, healing time, and the role of MMPs, early interventions and dressings. A modified Delphi methodology involving two iterations of email questionnaires followed by a face-to-face meeting was used to validate the statements, in order to arrive at a consensus for each. Clinical experts were selected, representing nurses, surgeons, podiatrists, academics, and policy experts. Results: In the first round, 38/47 statements reached or exceeded the consensus threshold of 80% and none were rejected. According to the protocol, any statement not confirmed or rejected had to be modified using the comments from participants and resubmitted. In the second round, 5/9 remaining statements were confirmed and none rejected, leaving 4 to discuss at the meeting. All final statements were confirmed with at least 80% consensus. Conclusion: This modified Delphi panel sought to gain clarity from clinical experts surrounding the use of dressings in the management of chronic wounds. A full consensus statement was developed to help clinicians and policy makers improve the management of patients with these conditions

    Taking stock of 10 years of published research on the ASHA programme: Examining India’s national community health worker programme from a health systems perspective

    Background: As India’s accredited social health activist (ASHA) community health worker (CHW) programme enters its second decade, we take stock of the research undertaken and whether it examines the health systems interfaces required to sustain the programme at scale. Methods: We systematically searched three databases for articles on ASHAs published between 2005 and 2016. Articles that met the inclusion criteria underwent analysis using an inductive CHW–health systems interface framework. Results: A total of 122 academic articles were identified (56 quantitative, 29 mixed methods, 28 qualitative, and 9 commentary or synthesis); 44 articles reported on special interventions and 78 on the routine ASHA program. Findings on special interventions were overwhelmingly positive, with few negative or mixed results. In contrast, 55% of articles on the routine ASHA programme showed mixed findings and 23% negative, with few indicating overall positive findings, reflecting broader system constraints. Over half the articles had a health system perspective, including almost all those on general ASHA work, but only a third of those with a health condition focus. The most extensively researched health systems topics were ASHA performance, training and capacity-building, with very little research done on programme financing and reporting, ASHA grievance redressal or peer communication. Research tended to be descriptive, with fewer influence, explanatory or exploratory articles, and no predictive or emancipatory studies. Indian institutions and authors led and partnered on most of the research, wrote all the critical commentaries, and published more studies with negative results. Conclusion: Published work on ASHAs highlights a range of small-scale innovations, but also showcases the challenges faced by a programme at massive scale, situated in the broader health system. As the programme continues to evolve, critical comparative research that constructively feeds back into programme reforms is needed, particularly related to governance, intersectoral linkages, ASHA solidarity, and community capacity to provide support and oversight

    Synthesis and Antimicrobial Activity of Some New α-Aminophosphonates

    Syntheses of new α-aminophosphonates 4a–l were achieved through a one-pot, three-component reaction process (the Kabachnik-Fields reaction). It involves the reaction with substituted salicylaldehydes, amines and dialkyl/diaryl phosphites in dry toluene at reflux temperature. The structures of the novel products were established by elemental analyses, IR, 1H, 13C and 31P NMR and mass spectroscopy. All the title compounds were screened for their antibacterial and antifungal activity.
