An analytical cost estimation approach for generic sheet metal 3D models
This paper defines a systematic workflow for production cost estimation of sheet metal stamped components. The approach represents a solution toward the adoption of Design to Cost methods during early product design. It consists in a sequence of steps that, starting from a 3D CAD model with annotations (material, roughness and tolerances) and production information (batch and production volume) leads to the manufacturing cost through an analytic cost breakdown (raw material, stamping and accessory processes, setup and tooling). The calculation process mainly consists in a first step where geometric algorithms calculate the sheet metal blank (dimensions, shape, thickness) and specific product features (e.g. flanges, louvers, embossing, etc.). The following steps allow to calculate the raw material, the stamping process and the process-related parameters, which are the manufacturing cost drivers (e.g. press, stamping rate/sequence/force and die dimensions/weight). The manufacturing cost is the sum of the previous calculated items. Testing the approach for three different components, the average absolute deviation measured between the estimated and actual cost was less than 10% and such a result looks promising for adopting this method for evaluating alternative design solutions
Experimental and numerical analysis of supersonic blade profiles developed for highly loaded impulse type steam turbine stages
The paper describes the results of a numerical and experimental research program addressing the aerodynamic investigation on the performance of blade profiles specifically developed for application in highly loaded impulse type turbine stages. The industrial requirements driving toward the adoption of highly loaded stage solutions are presented, along with an estimation of the profiles operating parameters. Two stator vanes and one rotor blade profile have been developed and extensively tested by means of flow field measurements and schlieren visualization in a transonic blow-down wind tunnel for linear cascades. Experimental results for the relevant operating conditions are presented, providing validation data for the CFD model used for blade design and evidencing that the main goals of the design optimization procedure have been achieved
Identification of MOR-positive B cell as possible innovative biomarker (Mu lympho-marker) for chronic pain diagnosis in patients with fibromyalgia and osteoarthritis diseases
Fibromyalgia (FM) diagnosis follows the American College of Rheumatology (ACR) criteria, based on clinical evaluation and written questionnaires without any objective diagnostic tool. The lack of specific biomarkers is a tragic aspect for FM and chronic pain diseases in general. Interestingly, the endogenous opioid system is close to the immune one because of the expression of opioid receptors on lymphocytes membrane. Here we analyzed the role of the Mu opioid receptor on B lymphocytes as a specific biomarker for FM and osteoarthritis (OA) patients. We enrolled three groups of females: FM patients, OA patients (chronic pain control group) and healthy subjects (pain-free negative control group). We collected blood samples to apply immunophenotyping analysis. Written tests were administrated for psychological analysis. Data were statistically analyzed. Final results showed that the percentage of Mu-positive B cells were statistically lower in FM and OA patients than in pain-free subjects. A low expression of Mu-positive B cell was not associated with the psychological characteristics investigated. In conclusion, here we propose the percentage of Mu-positive B cells as a biological marker for an objective diagnosis of chronic pain suffering patients, also contributing to the legitimacy of FM as a truly painful disease

A young infant with transient severe hypertriglyceridemia temporarily associated with meropenem administration : A case report and review of the literature
Background: Slight changes in the lipid profile can be observed over the acute phase of infectious diseases. Moreover, some antiinfective drugs can modify serum lipid concentrations, although antibiotics do not seem to have a relevant, direct, or acute effect on the lipid profile. Methods:A 75-day-old breastfed Caucasian female, born at term after a regular pregnancy, was hospitalized for osteomyelitis. She was immediately treated with intravenous meropenem and vancomycin. Therapy was effective, but after 22 days of treatment, her blood was found to be viscous with a purple shade. Results: A fasting blood sample showed serum triglycerides of 966mg/dL, total cholesterol of 258mg/dL, and high-density lipoprotein cholesterol of 15mg/dL. Secondary causes of hyperlipidemia and primary hereditary disorders were ruled out. Thereafter, the possibility that antibiotics may have had a role in the hypertriglyceridemia was considered, and meropenem was discontinued. After 72 hours of meropenem discontinuation, a sharp modification of lipid variables was observed, and further testing showed a complete normalization of the lipid profile. Conclusion: In this child with osteomyelitis, the increase in serum triglycerides appeared suddenly after 3 weeks of meropenem treatment and resolved quickly after meropenem discontinuation, thus highlighting the possible association between meropenem and lipid profile alterations. Monitoring the lipid profile should be considered in cases of long-term treatment with meropenem, and further studies on meropenem safety should include evaluation of the lipid profile
Cyclic nucleotide-dependent relaxation in human umbilical vessels
Umbilical vessels have a low sensitivity to dilate, and this property is speculated to have physiological implications. We aimed to investigate the different relaxing responses of human umbilical arteries (HUAs) and veins (HUVs) to agonists acting through the cAMP and cGMP pathways. Vascular rings were suspended in organ baths for isometric force measurement. Following precontraction with the thromboxane prostanoid (TP) receptor agonist U44069, concentration-response curves to the nitric oxide (NO) donor sodium nitroprusside (SNP), the soluble guanylate cyclase (sGC) stimulator BAY 41-2272, the adenylate cyclase (AC) activator forskolin, the \u3b2-adrenergic receptor agonists isoproterenol (ADRB1), salmeterol (ADRB2), and BRL37344 (ADRB3), and the phosphodiesterase (PDE) inhibitors milrinone (PDE3), rolipram (PDE4), and sildenafil (PDE5) were performed. None of the tested drugs induced a relaxation higher than 30% of the U44069-induced tone. Rings from HUAs and HUVs showed a similar relaxation to forskolin, SNP, PDE inhibitors, and ADRB agonists. BAY 41-2272 was significantly more efficient in relaxing veins than arteries. ADRB agonists evoked weak relaxations (< 20%), which were impaired in endothelium-removed vessels or in the presence of the NO synthase inhibitor L-NAME, sGC inhibitor ODQ. PKA and PKG inhibitors impaired ADBR1-mediated relaxation but did not affect ADRB2-mediated relaxation. ADRB3-mediated relaxation was impaired by PKG inhibition in HUAs and by PKA inhibition in HUVs. Although HUA and HUV rings were relaxed by BRL37344, immunohistochemistry and RT-qPCR analysis showed that, compared to ADRB1 and ADRB2, ADRB3 receptors are weakly or not expressed in umbilical vessels. In conclusion, our study confirmed the low relaxing capacity of HUAs and HUVs from term infants. ADRB-induced relaxation is partially mediated by endothelium-derived NO pathway in human umbilical vessels
Prevalencia y estacionalidad de la Pediculosis capitis en la población infante-juvenil de la region sanitaria, Buenos Aires, Argentina
The prevalence of the parasitic disease Pediculosis capitis was studied over a period of one year from 1-8-1992 to 31-7-1993, in Sanitary Region XI of the Buenos Aires province, Argentina which is composed of 15 districts located on the ENE, and in other areas under the influence of the Hospital Interzonal de Agudos, Especialidad PediatrÃa "Sup. Sor MarÃa Ludovica" de Plata, center of this Sanitary Region. A total number of 552 individuals of from medium to medium low social level of the child population of from 0 to 16 years of age were sampled at random over 52 consecutive weeks. All individuals who actually had the parasite at the time of examination were considered as positive for this disease. The results are expressed in terms of prevalence and its monthly and seasonal variations analyzed. Prevalence showed high values during all months, the lowest being in February (12%) and the highest in August (56.8%), the annual mean being of 38.04% (+/- 4.05%). Seasonally, its lowest value occurred in the summer (16.8%), the values for the remaining seasons being very similar to one another, but always higher than 38%.Se estudió la prevalencia de la Pediculosis capitis como enfermedad parasitaria durante el año calendario 1-8-1992 al 31-7-1993, en la Región Sanitaria XI de la provincia de Buenos Aires, Argentina, que comprende 15 partidos del Este Noreste de la provincia de Buenos Aires y otras zonas de influencia del Hospital Interzonal de Agudos, Especialidad PediatrÃa "Sor MarÃa Ludovica", cabecera de dicha Región Sanitaria. Se examinaron un total de 552 individuos de nivel social medio a medio bajo de la población infanto-juvenil de O a 16 años, tomados al azar en 52 muestreos semanales consecutivos, considerándose como positivos para esta enfermedad a aquellos parasitados al momento del diagnóstico. Los resultados se expresan en términos de prevalencia analizándose su variación mensual y estacional. La prevalencia fue relativamente alta en todos los meses, con valores mÃnimos en febrero (12%) y máximos en agosto (56,8%), siendo la media anual de 38,04% (+/- 4,05). Estacionalmente, su menor valor corresponde al Verano (16,8%), siendo muy semejantes los valores para las restantes estaciones, manteniéndose siempre superiores al 38%
Overnight Supervision of Alzheimer's Disease Patients in Nursing Homes - System Development and Field Trial
The number of patients affected by Alzheimer's disease among the population is currently growing, while the availability of resources for their assistance is decreasing. A solution for this problem is provided by the use of Ambient Assisted Living technologies, with the objectives to prolong the independent living of patients at home, to relieve assistance burden on caregivers, and to improve care effectiveness in nursing homes. This paper describes an integrated system designed to support the work of nurses during the night, to ensure comfort and safety of Alzheimer's disease patients in nursing homes. The project started from a similar solution designed for home use, suitably re-engineered for adoption in nursing homes. The system has been designed according to nurses' requirements and expectations, both by revising some existing functionalities, and by developing new components. The results gained from an experimental trial are also presented and discussed
Fever and pain management in childhood : Healthcare providers’ and parents’ adherence to current recommendations
In order to evaluate the adherence of healthcare providers and parents to the current recommendations concerning fever and pain management, randomized samples of 500 healthcare providers caring for children and 500 families were asked to complete an anonymous questionnaire. The 378 health care providers (HCPs) responding to the survey (75.6%) included 144 primary care pediatricians (38.1%), 98 hospital pediatricians (25.9%), 62 pediatric residents (16.4%), and 71 pediatric nurses (19.6%); the 464 responding parents (92.8%) included 175 whose youngest (or only) child was 645 years old (37.7%), 175 whose youngest (or only) child was aged 6-10 years (37.7%), and 114 whose youngest (or only) child was aged 11-14 years (24.6%). There were gaps in the knowledge of both healthcare providers and parents. Global adherence to the guidelines was lower among the pediatric nurses than the other healthcare providers (odds ratio 0.875; 95% confidence interval 0.795-0.964). Among the parents, those of children aged 6-10 and 11-14 years old, those who were older, and those without a degree answered the questions correctly significantly less frequently than the others. These findings suggest that there is an urgent need to improve the dissemination of the current recommendations concerning fever and pain management among healthcare providers and parents in order to avoid mistaken and sometimes risky attitudes, common therapeutic errors, and the unnecessary overloading of emergency department resources. Pediatric nurses and parents with older children, those who are older, and those with a lower educational level should be the priority targets of educational programmes
