    Immunization of Children in a Rural Area of North Kashmir, India: A KAP Study.

    Background: Knowledge, attitude and practices about immunization among mothers of children aged 1-2 years was assessed. Method: 300 mothers were administered a semi-structured questionnaire at PHC Hajan from 1st march to 1st may 2011 to elicit the information about the knowledge, attitude and practices of the mothers regarding immunization. Results: 100% of mothers knew that vaccination is beneficial and protects their children from diseases. 39% knew OPV protects from polio while only 1% were aware of protective role of BCG. All mothers knew about immunization in pregnancy but 86% were unaware about its preventive role. 26% mothers believed that 3 doses of T.T (tetanus toxoid) are to be given during pregnancy. Whereas 98% of children were completely immunized, 93% completed on schedule. Eighty percent of mothers reported of fever following DPT. All mothers had received tetanus toxoid during pregnancy. Conclusion: Considering mothers' poor knowledge and good attitude, health education on immunization is emphasized to improve their practices

    Increased isolation of vibrio cholerae O1 serotype inaba over serotype ogawa in Pakistan

    Although the predominant Vibrio cholerae serotype in Pakistan is Ogawa and serotype Inaba is rare, there has been a significant increase in the isolation of Inaba in our referral laboratory in Karachi. This paper reports this observation and further analysis of previous cholera data from 1993 to 2005 to assess the trend of occurrence and resistance pattern of V. cholerae strains. From January to September 2005, 245/3292 (7.4%) specimens yielded growth of V. cholerae. Of these, 243 were serotype Inaba, outnumbering serotype Ogawa. This recent Inaba strain is 100% resistant to cotrimoxazole, 3% resistant to chloramphenicol and not resistant to ampicillin, tetracycline and ofloxacin. This sensitivity pattern is almost similar to that of the previous predominant serotype Ogawa

    Shock Propagation in Granular Flow Subjected to an External Impact

    We analyze a recent experiment [Phys. Rev. Lett., {\bf103}, 224501 (2009)] in which the shock, created by the impact of a steel ball on a flowing monolayer of glass beads, is quantitatively studied. We argue that radial momentum is conserved in the process, and hence show that in two dimensions the shock radius increases in time tt as a power law t1/3t^{1/3}. This is confirmed in event driven simulations of an inelastic hard sphere system. The experimental data are compared with the theoretical prediction, and is shown to compare well at intermediate times. At late times, the experimental data exhibit a crossover to a different scaling behavior. We attribute this to the problem becoming effectively three dimensional due to accumulation of particles at the shock front, and propose a simple hard sphere model which incorporates this effect. Simulations of this model capture the crossover seen in the experimental data.Comment: 8 pages, 8 figure

    Importance of clinical toxicology teaching and its impact in improving

    Objective: To assess the impact of a one-day clinical toxicology workshop in improving knowledge.Methods: A one-day clinical toxicology workshop was conducted as a pre-conference workshop of the Annual Emergency Medicine Conference at the Aga Khan University Hospital, Karachi, in April 2012. The course was composed of poisoning-related common clinical scenarios. The pre-test and post-test understanding was used to assess the impact of the course in improving knowledge. The participants also evaluated the workshop as a whole thorough written evaluation forms. SPSS 19 was ued for statistical analysis of the data.Result: There were 22 participants in the course. The pre-test mean score was 31.6±15.1% (95% CI; 24-40; n=19) compared to the post-test the mean score of 56.0±10.8% (95% CI; 47- 61; n=17).     Universal scaling dynamics in a perturbed granular gas

    We study the response of a granular system at rest to an instantaneous input of energy in a localised region. We present scaling arguments that show that, in dd dimensions, the radius of the resulting disturbance increases with time tt as tαt^{\alpha}, and the energy decreases as tαdt^{-\alpha d}, where the exponent α=1/(d+1)\alpha=1/(d+1) is independent of the coefficient of restitution. We support our arguments with an exact calculation in one dimension and event driven molecular dynamic simulations of hard sphere particles in two and three dimensions.Comment: 5 pages, 5 figure

    A comparison of WHO guidelines issued in 1997 and 2009 for dengue fever - single centre experience.

    Objective: To compare the original (1997) and revised (2009) versions of World Health Organization guidelines for dengue patientsMethods: Adult patients with a positive dengue Immunoglobulin M serology, and a diagnosis of dengue were included in the study at Aga Khan University Hospital during a three-year period from January 2005 to December 2007. Data related to these dengue patients was collected from their medical records. Guidelines were then applied by the research assistant and correlation among these guidelines was computed. SPSS 19 was used for statistical analysis.Results: A total of 612 patients were found with a diagnosis of dengue, but only 439 (71.73%) had a positive IgM. The median age of these 439 patients was 28 (interquartile range: 18) years and majority of them were males, 295 (67%). According to the 1997 guidelines, 383 (87%) patients were classified as having dengue, while according to the 2009 guidelines, all the 439 (100%) patients were classified with a dengue infection. Under WHO 1997, 21 (5.5%) cases were classified as dengue shock syndrome, while 2009 guidelines labelled 88 (20%) cases as severe dengue. There was a consensus on only 11 severe cases by both the guidelines, showing different results between the two.Conclusion: By using 2009 guidelines, a physician would classify more dengue patients as having severe disease

    CT scan in children with acute bacterial meningitis: experience from emergency department of a tertiary-care hospital in Karachi, Pakistan

    Objective: To determine the role of computed tomography scan in children presenting to emergency department with symptoms and signs of suspected acute bacterial meningitis. Methods: The retrospective analysis was done on children who were admitted through the Emergency Department at Aga Khan University Hospital, Karachi, from September 2009 to September 2011 with the diagnosis of acute bacterial meningitis. Information related to age, gender, presenting complaints, clinical signs and symptoms, computed tomography scan findings and final outcome of patients was gathered from the medical records. SPSS 19 was used for statistical analysis. Results: A total of 192 patients were admitted with the relevant diagnosis. The male-female ratio was 2.3:1. Computed tomography scan was done in 114 (59.4%) patients. The scan was reported normal in 90 (78.94%) patients. However, cerebral oedema was found in 16 (14.03%) patients, cerebral infarct in 6(5.26%) and hydrocephalus in 2 (1.75%) patients. Overall, there were 6 (3.1%) deaths. Conclusion: Comuted tomography scan may have a beneficial role in children with acute bacterial meningitis. However, further studies are required to use the scan as a routine investigation for such a diagnosis

    Search for the Higgs boson in lepton, tau and jets final states

    We present a search for the standard model Higgs boson in final states with an electron or muon and a hadronically decaying tau lepton in association with two or more jets using 9.7 fb^{-1} of Run II Fermilab Tevatron Collider data collected with the D0 detector. The analysis is sensitive to Higgs boson production via gluon fusion, associated vector boson production, and vector boson fusion, followed by the Higgs boson decay to tau lepton pairs or to W boson pairs. The ratios of 95% C.L. upper limits on the cross section times branching ratio to those predicted by the standard model are obtained for orthogonal subsamples that are enriched in either H -> tau tau decays or H -> WW decays, and for the combination of these subsample limits. The observed and expected limit ratios for the combined subsamples at a Higgs boson mass of 125 GeV are 11.3 and 9.0 respectively

    Search for a Narrow ttbar Resonance in ppbar Collisions at sqrt{s}=1.96 TeV

    We report a search for a narrow ttbar resonance that decays into a lepton+jets final state based on an integrated luminosity of 5.3/fb of proton-antiproton collisions at sqrt{s}=1.96 TeV collected by the D0 Collaboration at the Fermilab Tevatron Collider. We set upper limits on the production cross section of such a resonance multiplied by its branching fraction to ttbar which we compare to predictions for a leptophobic topcolor Z' boson. We exclude such a resonance at the 95% confidence level for masses below 835 GeV.Comment: 7 pages, 3 figures, submitted to Physical Review Letter
