    Risk Factors Of Preeclampsia Among Pregnant Women In Indonesia

    Preeclampsia is the second highest cause of maternal death in Indonesia. The incidence of preeclampsia in Indonesia is very high at 24%. West Java is a province in Indonesia with a high preeclampsia rate of 25%. Predisposing factors include preeclampsia including age. Health Research Data (2010) shows that the percentage of early marriage aged <20 years is still high at 46.7% and marriage age> 35 years at 0.6%. Objectives This study aims to determine the relationship between age and the incidence of preeclampsia at Dustira Hospital in Cimahi City. This study was conducted using the correlation with the case control approach. The population in this study was 130 postpartum mothers treated at Dustira Level II Hospital. The sampling technique used purposive sampling with a total of 24 for the preeclampsia group and 24 for the non-preeclampsia group. The data collected is secondary data collected by observing the medical record book. The research instrument uses a checklist sheet. The study was conducted in June 2019 at Dustira City II Kindergarten Hospital Cimahi. Univariate analysis uses frequency distribution and bivariate uses chi-square. Univariate test results showed that the incidence of preeclampsia was mostly experienced by pregnant women with age at risk of 58.3%. Bivariate test results showed a p value of 0.007 (α <0.05) and OR 7 (95% CI: 1.822-26.887) meaning that there was a relationship between age and the incidence of preeclampsia, and mothers with age at risk had 7 times greater occurrence of preeclampsia compared to age is not at risk. Pregnant women aged less than 20 years or more than 35 years are at high risk of experiencing preeclampsia. One of the efforts to prevent the occurrence of preeclampsia is through health promotion about the age of mothers who are safe to reproduce. It is expected that health workers, especially maternity nurses, can provide health services to the public regarding the ideal age for pregnancy or childbirth as well as the age at risk of preeclampsia.   Keywords: Indonesia, Maternal age pregnancies, Preeclampsi

    Penelitian ini bertujuan untuk mengetahui penerapan Total Quality Manajemen dan Kepuasan Konsumen di PT. Kharisma Siliwangi Bandung, serta pengaruh implementasi total quality manajemen terhadap kepuasan konsumen PT. Kharisma Siliwangi Bandung. Metode penelitian yang digunakan adalah metode deskriptif analisis dengan pendekatan kuantitatif. Sampel pada penelitian ini adalah 32 karyawan. Teknik sampling yang digunakan adalah simple random sampling. Data diperoleh dari responden dengan menggunakan alat pengumpul data berupa kuesioner. Analisis data yang digunakan adalah analisis regresi linier sederhana dengan menggunakan bantuan komputer program SPSS 17.0 for windows. Berdasarkan hasil analisis data dengan menggunakan metode statistik yaitu regresi linier sederhana, diperoleh nilai koefisien determinan (KD) sebesar 0,304. Hal ini menunjukan pengertian kepuasan konsumen (Y) dipengaruhi sebesar 30,40% oleh total quality manageman (X), sedangkansisanya (100% - 30,40% = 10,60%) dipengaruhi oleh faktor-faktor lai

    Non-alcoholic Fatty Liver Disease Related to Metabolic Syndrome: a Case-control Study

    Background: Non-alcoholic fatty liver disease (NAFLD) is a benign condition, but it can go for years and progress to liver cirrhosis or eventually to liver cancer. Metabolic syndrome (MS) is a condition associated with NAFLD. This study was aimed to know the risk factors of NAFLD related to metabolic syndrome. Method: A case-control study was performed in NAFLD patients with or without MS and healthy individuals. All subjects were recruited from population that underwent routine medical check-up at Sardjito Hospital, Jogjakarta, during March 2007–August 2008. Diagnosis of NAFLD is defined based on clinical and liver ultrasound findings. Diagnosis of MS is defined by International Diabetes Federation on criteria for the diagnosis of MS. Data were analyzed by using T-test, ANOVA and linear regression. Odds ratio (OR) (95% CI and p < 0.05) was calculated by cross-tab analysis. Results: There were 84 patients enrolled in the study (group I = 30 NAFLD + MS subjects; group II = 26 NAFLD patients; group III = 28 healthy). The data showed statistically significant Results in waist circumference, systole blood pressure, fasting glucose, triglyceride, high density lipoprotein (HDL) cholesterol level, homeostasis models assessment index ratio (HOMA-IR), free fatty acid (FFA), and adiponectin. The ANOVA and linear regression test among NAFLD groups showed significant difference only on HDL-cholesterol and FFA level. The lowest OR was 1.674 for HDL-cholesterol and highest OR was 13.571 for triglyceride. Conclusion: The independent factors of NAFLD related to metabolic syndrome are FFA and HDL- cholesterol level, even though a decreasing of HDL-cholesterol level has a lowest risk of NAFLD

    Moving from Ethnic Exclusions to Cultural Safety: How is athlete ethnicity discussed in research on menstrual health in sports? A Scoping Review.

    Objective: To investigate how athlete ethnicity is discussed in the inclusion and exclusion criteria, methodology, findings, and conclusions of research focused on menstrual health in sports science and medicine. Design: A scoping review of sports-based research conducted on athletes related to (1) menstrual health and ethnicity, (2) how researchers include/exclude participants based on ethnicity, and (3) how ethnicity is discussed. Data Sources: Electronic search of PubMed, and ProQuest. Eligibility criteria: Articles were included if they met the following criteria: (1) published before September 2023, (2) published in peer-reviewed journals, (3) participants were women athletes, (4) published in English, and (5) relating to menstrual health. Articles were assessed as good, fair, or poor quality using the Inclusion of Participant Ethnicity Quality Assessment Criteria. Results: From the 1089 studies available from the initial database search, 55 studies considered ethnicity. Nine studies met the inclusion criteria and were assessed as either good (22%), fair (44%), or poor (33%) in quality in their consideration of athlete ethnicity. 81% of research articles on menstrual health in sports do not consider athlete ethnicity, and when ethnicity is discussed, it rarely meets the criteria for cultural safety in the research process. Most studies did not factor ethnicity into the analysis and lacked cultural considerations in the research design and interventions. Conclusion: More careful inclusion of ethnicity in sports menstrual health-related research and recognition of social and cultural influences on health and research outcomes for indigenous and other ethnic minority groups is needed. Such research is required to support coaches, medical personnel, and support staff in designing culturally safe environments for sportswomen from diverse cultural and ethnic backgrounds

    The glial growth factors deficiency and synaptic destabilization hypothesis of schizophrenia

    BACKGROUND: A systems approach to understanding the etiology of schizophrenia requires a theory which is able to integrate genetic as well as neurodevelopmental factors. PRESENTATION OF THE HYPOTHESIS: Based on a co-localization of loci approach and a large amount of circumstantial evidence, we here propose that a functional deficiency of glial growth factors and of growth factors produced by glial cells are among the distal causes in the genotype-to-phenotype chain leading to the development of schizophrenia. These factors include neuregulin, insulin-like growth factor I, insulin, epidermal growth factor, neurotrophic growth factors, erbB receptors, phosphatidylinositol-3 kinase, growth arrest specific genes, neuritin, tumor necrosis factor alpha, glutamate, NMDA and cholinergic receptors. A genetically and epigenetically determined low baseline of glial growth factor signaling and synaptic strength is expected to increase the vulnerability for additional reductions (e.g., by viruses such as HHV-6 and JC virus infecting glial cells). This should lead to a weakening of the positive feedback loop between the presynaptic neuron and its targets, and below a certain threshold to synaptic destabilization and schizophrenia. TESTING THE HYPOTHESIS: Supported by informed conjectures and empirical facts, the hypothesis makes an attractive case for a large number of further investigations. IMPLICATIONS OF THE HYPOTHESIS: The hypothesis suggests glial cells as the locus of the genes-environment interactions in schizophrenia, with glial asthenia as an important factor for the genetic liability to the disorder, and an increase of prolactin and/or insulin as possible working mechanisms of traditional and atypical neuroleptic treatments

There is a constant need to decrease risk and increase patient safety in the hospital environment. The purpose of this article is to review the laboratory testing procedures for parathyroid hormone and adrenocorticotropic hormone (which are characterized by short half-lives) and to track failure modes and risks, and offer solutions to prevent them. During a routine quality improvement review at the Endocrine Laboratory in Tel Hashomer Hospital, we discovered these tests are frequently repeated unnecessarily due to multiple failures. The repetition of the tests inconveniences patients and leads to extra work for the laboratory and logistics personnel as well as the nurses and doctors who have to perform many tasks with limited resources.Methods: A team of eight staff members accompanied by the Head of the Endocrine Laboratory formed the team for analysis. The failure mode and effects analysis model (FMEA) was used to analyze the laboratory testing procedure and was designed to simplify the process steps and indicate and rank possible failures.Results: A total of 23 failure modes were found within the process, 19 of which were ranked by level of severity. The FMEA model prioritizes failures by their risk priority number (RPN). For example, the most serious failure was the delay after the samples were collected from the department (RPN =226.1).Conclusion: This model helped us to visualize the process in a simple way. After analyzing the information, solutions were proposed to prevent failures, and a method to completely avoid the top four problems was also developed.
