    ХРОНИЧЕСКАЯ БОЛЕЗНЬ ПОЧЕК И ПОЧЕЧНАЯ НЕДОСТАТОЧНОСТЬ У БОЛЬНЫХ СПИДОМ

    Цель исследования заключалась в изучении развития хронической болезни почек с формированием почечной недостаточности у ВИЧ-инфицированных больных в стадии СПИДа. Нами проанализированы основные показатели крови, клинико-лабораторные данные, состояние азотемии и скорость клубочковой фильтрации (СКФ) у 119 больных, находившихся на стационарном лечении Одесского областного Центра по профилактике и борьбе со СПИДом. Наличие признаков мочевого синдрома отмечалось в 82% случаев, при этом повышение уровня креатинина крови регистрировалось у 32,77% больных. Снижение СКФ менее 60 мл/мин имело место у 57,14% больных с гиперкреатининемией. Таким образом, у большинства больных СПИДом отмечается формирование ХБП. При этом, чем длительнее болеют пациенты, тем выше азотемия и тем ниже СКФ и, как следствие, быстрее развивается почечная недостаточность, причем ее развитие наступает в среднем за 7,94±0,92 лет

    The use of foreign materials in modern herniology, on the one hand, reduces the number of recurrences, and on the other - increases the risk of several complications: profuse exudation and possible infection in the postoperative wound, extensive cicatricial deformation of a zone of operations, persistent pain syndrome and possible reduced fertility in men. In the experiment 30 rats were studied influence of drugs «mesogel®» and «iodopiron» in the course of wound process during operations using mesh explant. Histological methods were evaluated the usefulness of the formation of connective tissue, degree of involvement in cicatricial process is located in the area of operation of tissues and structures. Compared with the control group of animals is set to a relatively low density and fewer collagen fibers.     Colorectal cancer ranks third after breast cancer in terms of incidence and second after lung cancer in terms of mortality.Management of rectal cancer requires a multidisciplinary approach, with the surgical management playing the main role. There are currently three resective techniques that complement the traditional open surgery: laparoscopic surgery, robotic surgery, and transanal total mesorectal excision.Rectal cancer in elderly patients is particularly hard to diagnose and treat surgically due to multiple comorbidities and limited functional reserve.     Addition of Ag2O in Er3+doped oxyfluorophosphate glass to allow the drawing of optical fibers

    Here, Ag2O containing glasses in the NaPO3–CaF2 glass network were prepared using standard melting process. The addition of Ag2O was found to increase the thermal stability of the glass due to the decrease in the Q2 units at the expense of Q1 units, to decrease the intensity of the upconversion under 980 nm pumping and to have a small impact on the nucleation and growth mechanism. Due to the thermal stability against crystallization of the glass prepared with 4 mol % of Ag2O, we demonstrate that fiber can be drawn from this glass. Despite the formation of Ag nanoparticles at the surface of the fiber although the drawing is a fast process, light can still be confined in the fiber. The fiber exhibits a large emission band centered at 1.5 μm under 980 nm pumping.Peer reviewe

    Abnormal Distracter Processing in Adults with Attention-Deficit-Hyperactivity Disorder

    Background: Subjects with Attention-Deficit Hyperactivity Disorder (ADHD) are overdistractible by stimuli out of the intended focus of attention. This control deficit could be due to primarily reduced attentional capacities or, e. g., to overshooting orienting to unexpected events. Here, we aimed at identifying disease-related abnormalities of novelty processing and, therefore, studied event-related potentials (ERP) to respective stimuli in adult ADHD patients compared to healthy subjects. Methods: Fifteen unmedicated subjects with ADHD and fifteen matched controls engaged in a visual oddball task (OT) under simultaneous EEG recordings. A target stimulus, upon which a motor response was required, and non-target stimuli, which did not demand a specific reaction, were presented in random order. Target and most non-target stimuli were presented repeatedly, but some non-target stimuli occurred only once (‘novels’). These unique stimuli were either ‘relative novels ’ with which a meaning could be associated, or ‘complete novels’, if no association was available. Results: In frontal recordings, a positive component with a peak latency of some 400 ms became maximal after novels. In healthy subjects, this novelty-P3 (or ‘orienting response’) was of higher magnitude after complete than after relative novels, in contrast to the patients with an undifferentially high frontal responsivity. Instead, ADHD patients tended to smaller centro-parietal P3 responses after target signals and, on a behavioural level, responded slower than controls

    The modulation of auditory novelty processing by working memory load in school age children and adults: a combined behavioral and event-related potential study

    <p>Abstract</p> <p>Background</p> <p>We investigated the processing of task-irrelevant and unexpected novel sounds and its modulation by working-memory load in children aged 9-10 and in adults. Environmental sounds (novels) were embedded amongst frequently presented standard sounds in an auditory-visual distraction paradigm. Each sound was followed by a visual target. In two conditions, participants evaluated the position of a visual stimulus (0-back, low load) or compared the position of the current stimulus with the one two trials before (2-back, high load). Processing of novel sounds were measured with reaction times, hit rates and the auditory event-related brain potentials (ERPs) Mismatch Negativity (MMN), P3a, Reorienting Negativity (RON) and visual P3b.</p> <p>Results</p> <p>In both memory load conditions novels impaired task performance in adults whereas they improved performance in children. Auditory ERPs reflect age-related differences in the time-window of the MMN as children showed a positive ERP deflection to novels whereas adults lack an MMN. The attention switch towards the task irrelevant novel (reflected by P3a) was comparable between the age groups. Adults showed more efficient reallocation of attention (reflected by RON) under load condition than children. Finally, the P3b elicited by the visual target stimuli was reduced in both age groups when the preceding sound was a novel.</p> <p>Conclusion</p> <p>Our results give new insights in the development of novelty processing as they (1) reveal that task-irrelevant novel sounds can result in contrary effects on the performance in a visual primary task in children and adults, (2) show a positive ERP deflection to novels rather than an MMN in children, and (3) reveal effects of auditory novels on visual target processing.</p

    Electrophysiological correlates of selective attention: A lifespan comparison

    <p>Abstract</p> <p>Background</p> <p>To study how event-related brain potentials (ERPs) and underlying cortical mechanisms of selective attention change from childhood to old age, we investigated lifespan age differences in ERPs during an auditory oddball task in four age groups including 24 younger children (9–10 years), 28 older children (11–12 years), 31 younger adults (18–25), and 28 older adults (63–74 years). In the Unattend condition, participants were asked to simply listen to the tones. In the Attend condition, participants were asked to count the deviant stimuli. Five primary ERP components (N1, P2, N2, P3 and N3) were extracted for deviant stimuli under Attend conditions for lifespan comparison. Furthermore, Mismatch Negativity (MMN) and Late Discriminative Negativity (LDN) were computed as difference waves between deviant and standard tones, whereas Early and Late Processing Negativity (EPN and LPN) were calculated as difference waves between tones processed under Attend and Unattend conditions. These four secondary ERP-derived measures were taken as indicators for change detection (MMN and LDN) and selective attention (EPN and LPN), respectively. To examine lifespan age differences, the derived difference-wave components for attended (MMN and LDN) and deviant (EPN and LPN) stimuli were specifically compared across the four age groups.</p> <p>Results</p> <p>Both primary and secondary ERP components showed age-related differences in peak amplitude, peak latency, and topological distribution. The P2 amplitude was higher in adults compared to children, whereas N2 showed the opposite effect. P3 peak amplitude was higher in older children and younger adults than in older adults. The amplitudes of N3, LDN, and LPN were higher in older children compared with both of the adult groups. In addition, both P3 and N3 peak latencies were significantly longer in older than in younger adults. Interestingly, in the young adult sample P3 peak amplitude correlated positively and P3 peak latency correlated negatively with performance in the Identical Picture test, a marker measure of fluid intelligence.</p> <p>Conclusion</p> <p>The present findings suggest that patterns of event-related brain potentials are highly malleable within individuals and undergo profound reorganization from childhood to adulthood and old age.</p
