    Conservation agriculture in the dry Mediterranean climate

    The objective of this article is to review: (a) the concepts and principles that underpin Conservation Agriculture (CA) ecologically and operationally; (b) the potential benefits that can be harnessed through CA systems in the dry Mediterranean climates; (c) current status of adoption and spread of CA in the dry Mediterranean climate countries; and (d) opportunities for CA in the Central and West Asia and North Africa (CWANA) region. CA, comprising minimum mechanical soil disturbance and no-tillage seeding, organic mulch cover, and crop diversification is now practised on some 125 million ha, corresponding to about 9% of the global arable cropped land. Globally, the area under CA is spread across all continents and all agro-ecologies, including the dryland climates in the Mediterranean basin region as well as in the Mediterranean climates elsewhere in the world. Worldwide empirical and scientific evidence is available to show that significant productivity, economic, social and environmental benefits exist that can be harnessed through the adoption of CA principles for sustainable production intensification in the dry Mediterranean climates, including those in the CWANA region. The benefits include: fundamental change for the better in the sustainability of production systems and ecosystem services; higher stable yields and incomes; climate change adaptation and reduced vulnerability to the highly erratic rainfall distribution; and reduced greenhouse gas emissions. CA has taken off globally and is now spreading in several Mediterranean climates outside the Mediterranean basin particularly in South America, South Africa and Australia. In the dry Mediterranean climates in the CWANA region, CA is perceived to be a powerful tool of land management but CA has not yet taken off. Research on CA in the CWANA region has shown that there are opportunities for CA adoption in rainfed and irrigated farming systems involving arable and perennial crops as well as livestock

    Spatial and temporal variability of CO2 emisions in soils under conventional tillage and no-till farming

    Agricultural soils can act as a carbon sink depending on the soil management practices employed. As a result of this functional duality, soil management systems are present in international documents relating to climate change mitigation. Agricultural practices are responsible for 14% of total greenhouse gas emissions (GHG’s) (MMA, 2009)(1). Conservation agriculture (CA) is one of the most effective agricultural systems for reducing CO2 emissions, as it increases the sequestration of atmospheric carbon in the soil. In order to assess the performance of CA in terms of CO2 emissions, a field trial was conducted comparing soil derived CO2 fluxes under No-till (NT) farming and under conventional tillage. Three pilot farms were selected in the cereal-growing area of southern Spain, located in Las Cabezas de San Juan (Seville), Carmona (Seville) and Cordoba. Each pilot farm comprises six experimental plots with an approximate area of five hectares; three of the six plots implement CA practices, while the other three use conventional tillage techniques. The subdivision of each tillage system into 3 plots allowed the simultaneous cropping of the three crops of the wheat-sunflower-legume rotation each year. Results showed that carbon dioxide emissions were 31 to 91% higher in tilled soils than in untilled soils, and that there was a great seasonal variability of CO2 emissions, as weather conditions also differed considerably for the different sampling periods. In all cases, the CO2 fluxes emitted into the atmosphere were always higher when soil was subject to conventional tillage

    Electronic Patient Reporting of Adverse Events and Quality of Life: A Prospective Feasibility Study in General Oncology

    PURPOSE: Adverse event (AE) reporting is essential in clinical trials. Clinician interpretation can result in under-reporting; therefore, the value of patient self-reporting has been recognized. The National Cancer Institute has developed a Patient-Reported Outcomes version of the Common Terminology Criteria for Adverse Events (PRO-CTCAE) for direct patient AE reporting. A nonrandomized prospective cohort feasibility study aimed to explore the compliance and acceptability of an electronic (Internet or telephone) system for collecting patient self-reported AEs and quality of life (QOL). METHODS: Oncology patients undergoing treatment (chemotherapy, targeted agents, hormone therapy, radiotherapy, and/or surgery) at 2 hospitals were sent automated weekly reminders to complete PRO-CTCAE once a week and QOL (for a maximum of 12 weeks). Patients had to speak/understand English and have access to the Internet or a touch-tone telephone. Primary outcome was compliance (proportion of expected questionnaires), and recruitment rate, attrition, and patient/staff feedback were also explored. RESULTS: Of 520 patients, 249 consented (47.9%)—mean age was 62 years, 51% were male, and 70% were married—and 230 remained on the study at week 12. PRO-CTCAE was completed at 2,301 (74.9%) of 3,074 timepoints and QOL at 749 (79.1%) of 947 timepoints. Individual weekly/once every 4 weeks compliance reduced over time but was more than 60% throughout. Of 230 patients, 106 (46.1%) completed 13 or more PRO-CTCAE, and 136 (59.1%) of 230 patients completed 4 QOL questionnaires. Most were completed on the Internet (82.3%; mean age, 60.8 years), which was quicker, but older patients preferred the telephone option (mean age, 70.0 years). Positive feedback was received from patients and staff. CONCLUSION: Self-reporting of AEs and QOL using an electronic home-based system is feasible and acceptable. Implementation of this approach in cancer clinical trials may improve the precision and accuracy of AE reporting

    Combined plasmonic and dielectric rear reflectors for enhanced photocurrent in solar cells

    A doubling of the photocurrent due to light trapping is demonstrated by the combination of silvernanoparticles with a highly reflective back scatterer fabricated by Snow Globe Coating on the rear of a 2 μm polycrystalline silicon thin film solar cell. The binder free high refractive index titania particles can overcome light losses due to transmission. Modelling indicates that adding plasmonicnanoparticles to the back scatterer widens the angular distribution of scattered light such that over 80% of long wavelength light is scattered outside the Si/air loss cone and trapped in the cell, compared to 30% for the titania alone.This project was funded by the Austrian Science Fund (FWF): J-2979, the Australian Research Council and the Australian Solar Institute

    Effect of agronomic and environmental factors on CO2 emissions on a dryland rotation

    Agriculture is a substantial source of greenhouse gas emissions (GHG) in many countries. Conservation agriculture includes soil management systems that help to reduce CO2 emission levels. However, there are many factors involved in the production of these emissions such as soil management type and time at which the agriculture operations are performed, crop phenological state, the weather, and handling of the residue amongst others. In the long term, the relationships that exist between these factors seem to determine the balance of these emissions. In this study, we analyzed the influence of the soil management system as well as the climatology of the different seasons studied and the phenological state of the different crops implanted. For this purpose a field trial was conducted in Las Cabezas de San Juán (Seville). This pilot farm consisted of six experimental plots with an approximate area of 5 ha; conservation agriculture practices were employed in three of the six plots while traditional tillage management was used in the other three. Within these plots the three crops of the wheat-sunflower-legume rotation were tested simultaneously. The study was conducted over four agricultural seasons - 2009/10, 2010/11, 2011/12 and 2012/13. Each of these cropping seasons were characterised by very different rainfall amounts, registering a total of 814.4, 721.6, 268.2 and 676.4 l/m2, respectively. When we studied the evolution of emissions over four seasons, an increase could be observed for both management systems during the time in which the crops were established due to the roots respiration processes. These increases were heavily influenced by the rainfall recorded during the time in which the crop was in place. In the case of wheat, higher emissions were produced during the cultivation time of the first and fourth season during which 84% and 60% of the total rainfall of each season was recorded. These emissions were 9 and 5 kg CO2/ha for conventional tillage and no tillage, respectively for the 2009/10 season and 11.7 and 6.8 kg CO2/ha, respectively in the 2012/13 season. Conversely during the 2011/12 season, a season in which lower precipitation was registered, the higher emissions were comparatively minor with respect to the previous values, specifically 3.7 and 1.9 kg CO2/ha for non-tillage and conventional tillage

    Electronic patient self-assessment and management (SAM): a novel framework for cancer survivorship

    <p>Abstract</p> <p>Background</p> <p>We propose a novel framework for management of cancer survivorship: electronic patient Self-Assessment and Management (SAM). SAM is a framework for transfer of information to and from patients in such a way as to increase both the patient's and the health care provider's understanding of the patient's progress, and to help ensure that patient care follows best practice.</p> <p>Methods</p> <p>Patients who participate in the SAM system are contacted by email at regular intervals and asked to complete validated questionnaires online. Patient responses on these questionnaires are then analyzed in order to provide patients with real-time, online information about their progress and to provide them with tailored and standardized medical advice. Patient-level data from the questionnaires are ported in real time to the patient's health care provider to be uploaded to clinic notes. An initial version of SAM has been developed at Memorial Sloan-Kettering Cancer Center (MSKCC) and the University of California, San Francisco (UCSF) for aiding the clinical management of patients after surgery for prostate cancer.</p> <p>Results</p> <p>Pilot testing at MSKCC and UCSF suggests that implementation of SAM systems are feasible, with no major problems with compliance (> 70% response rate) or security.</p> <p>Conclusion</p> <p>SAM is a conceptually simple framework for passing information to and from patients in such a way as to increase both the patient's and the health care provider's understanding of the patient's progress, and to help ensure that patient care follows best practice.</p

    The Effect of Conservation Agriculture and Environmental Factors on CO2 Emissions in a Rainfed Crop Rotation

    There are many factors involved in the release of CO2 emissions from the soil, such as the type of soil management, the soil organic matter, the soil temperature and moisture conditions, crop phenological stage, weather conditions, residue management, among others. This study aimed to analyse the influence of these factors and their interactions to determine the emissions by evaluating the environmental cost expressed as the kg of CO2 emitted per kg of production in each of the crops and seasons studied. For this purpose, a field trial was conducted on a farm in Seville (Spain). The study compared Conservation Agriculture, including its three principles (no-tillage, permanent soil cover, and crop rotations), with conventional tillage. Carbon dioxide emissions measured across the four seasons of the experiment showed an increase strongly influenced by rainfall during the vegetative period, in both soil management systems. The results of this study confirm that extreme events of precipitation away from the normal means, result in episodes of high CO2 emissions into the atmosphere. This is very important because one of the consequences for future scenarios of climate change is precisely the increase of extreme episodes of precipitation and periods extremely dry, depending on the area considered. The total of emission values of the different plots of the study show how the soils under the conventional system (tillage) have been emitting 67% more than soils under the conventional agriculture system during the 2010/11 campaign and 25% for the last campaign where the most appreciable differences are observed

    Pharmaceutical assistance programs for cancer patients in the era of orally administered chemotherapeutics

    Introduction: The rising cost of cancer drugs may make treatment unaffordable for some patients. Patients often rely on drug manufacturer-administered Pharmaceutical Assistance Programs (PAPs) to obtain drugs and reduced or no cost. The overall usage of PAPs within cancer care delivery is unknown. Methods: We included all cancer patients across an academically affiliated, integrated health system in North Carolina during 2014 (N = 8591). We identified the subset of patients receiving PAP assistance to afford one or more cancer drugs, in order to calculate the proportion of patients receiving PAP assistance, and the retail value of the assistance. Results: Among 8591 cancer patients, 215 unique patients submitted a total of 478 successful PAP requests for cancer drugs. 40% of PAP-utilizing patients were uninsured, 23% had Medicaid coverage, 20% had Medicare coverage, 2% were dual Medicare/Medicaid eligible, and 14% were commercially insured. Among all cancer patients who received medical treatment, 6.0% required PAP assistance, whereas 10.6% receiving an oral agent required PAP assistance. The proportion receiving PAP assistance varied substantially by drug, ranging from <1% of patients (e.g. carboplatin, methotrexate) to 50% of patients (e.g. ponatinib, temsirolimus). The majority of the retail value obtained was for oral agents, including 1,556,575ofimatiniband1,556,575 of imatinib and 1,449,633 of dasatinib, which were the two drugs with the highest aggregate retail value. Conclusions: A substantial proportion of cancer patients receive private charitable assistance to obtain standard-of-care treatments. This includes patients with federal and private insurance, suggesting an inability of patients to meet cost-sharing requirements
