Ultra Low Momentum Neutron Catalyzed Nuclear Reactions on Metallic Hydride Surfaces
Ultra low momentum neutron catalyzed nuclear reactions in metallic hydride
system surfaces are discussed. Weak interaction catalysis initially occurs when
neutrons (along with neutrinos) are produced from the protons which capture
``heavy'' electrons. Surface electron masses are shifted upwards by localized
condensed matter electromagnetic fields. Condensed matter quantum
electrodynamic processes may also shift the densities of final states allowing
an appreciable production of extremely low momentum neutrons which are thereby
efficiently absorbed by nearby nuclei. No Coulomb barriers exist for the weak

Paradoxical movement of the lower ribcage at rest and during exercise in COPD patients
Paradoxical inward displacement of the costal margin during inspiration is observed in many chronic obstructive pulmonary disease patients at rest but its importance is unclear.The current authors studied 20 patients (forced expiratory volume in one second 32.6 +/- 11.7, functional residual capacity 186 +/- 32% predicted) and 10 healthy controls at rest and during symptom-limited incremental exercise. With optoelectronic plethysmography, the phase shift between pulmonary and abdominal ribcage volumes and the percentage of inspiratory time the ribcage compartments moved in opposite directions were quantified, using control data to define the normal range of movement.Eight patients showed lower ribcage inspiratory paradox at rest (P+), while 12 patients did not (P-). This was unrelated to resting lung function or exercise tolerance. Total end-expiratory chest wall volume (EEVcw) increased immediately when exercise began in P+ patients, but later in exercise in P- patients. This difference in EEVcw was mainly due to a greater increase of end-expiratory pulmonary ribcage volume in P+ patients. During exercise, dyspnoea increased similarly in the two groups, while leg effort increased more markedly in the patients without paradox.In conclusion, lower ribcage paradox at rest is reproducible and associated with early-onset hyperinflation of the chest wall and predominant dyspnoea at end-exercise. COVID-19: what happened to all of the otolaryngology emergencies?

COVID-19 is a global pandemic caused by the novel coronavirus SARS-CoV-2, and Italy was the first European country to experience a large-scale outbreak. With this, every aspect of healthcare, the economy, and society has been deeply affected in Italy, so much so that fundamental changes are expected in the post-COVID-19 era. One aspect is that access to emergency care in Otolaryngology (ORL) patients has already changed in the month following the onset of the COVID-19 outbreak, and, seemingly, many non-COVID illnesses have been disappearing.
Although the government did not enact any limitations or prohibitions to emergency access for patients afflicted by acute ORL pathologies, the number of daily urgent consultations to the ORL Departments in Bari and Foggia University Hospitals has significantly reduced and, on some days, was even zero.
One possible reason for this change may be patients’ fear of acquiring a nosocomial COVID-19 infection. Even so, it is difficult to believe that some acute diseases, such as massive epistaxis, otitis, or pharyngotonsillitis, could have occurred so rarely or not at all or, alternately, could have been treated at home by patients or their families. Rather, most certainly, primary care physicians, who have been performing telehealth consultations in accordance with the guidelines from the Higher Institute of Health (ISS), have been providing an invaluable service [1]; it is posited that patients who received primary care services this way may have been able to delay or even forego specialist evaluation. This telehealth treatment has not only helped to keep patients at home but has provided other benefits and should be considered in the future.
Table Table11 shows data regarding the number of emergency department consultations by diagnosis in the 30 days preceding (from February 8th to March 8th, 2020) and 30 days following (from March 9th to April 9th, 2020) the onset of the COVID-19 outbreak at the two main tertiary referral centers in the Puglia region, as well as the percentage reduction for each diagnosis. The data revealed an overall decrease in the number of ORL emergency consultations by 80.8%. Of note, reductions were noted across all subspecialties.
Although some illnesses, such as rhinitis, sinusitis, uvular edema, and tinnitus, do not usually require immediate attention, it is more difficult to explain the large reduction, ranging from 80.2% to 88.9%, of other diagnoses, such as vertigo, epistaxis, sudden hearing loss, otitis media, and peritonsillar abscesses, which often merit urgent ORL consultations. The reduction of some diagnoses may be the consequence of national lockdown orders. For example, the 84.6% reduction in the number of nasal bone fractures may be related to the near-absence of motor vehicle accidents reported during this time. In contrast, it is more difficult to explain the reduction of other acute diseases such as sudden hearing loss, whose frequency decreased 83.3%, especially when compared to non-disease conditions, such as cerumen impaction, whose reduction in presentation, interestingly, was slightly more modest at 73.7%.
A reduction in the number of emergency ORL consultations by 80.8% during the COVID-19 pandemic creates new paradigms for the future. Over the past few decades, the workload performed by healthcare personnel in emergency departments has increased, and some have even noted a reduction in morale and job satisfaction, as their responsibilities include intervening in not only true emergencies but also managing many patients with non-acute complaints. One possible exacerbating factor is the length of waitlists to access outpatient care in the Italian National Healthcare System (NHS), as some patients may have to wait for months to see an otolaryngologist. This situation has motivated many to seek care in the emergency department to skip wait times and receive specialty evaluation and treatment without charge.
Decreasing the utilization of emergency department services by improving access to primary care services has been a long-term aim, articulated since at least 1958, when physicians working at Hartford Hospital published a pioneering piece in The New England Journal of Medicine [2] that showed that only a minimum number of patients who visited the emergency department required inpatient care. At that time, proposed solutions were to increase utilization of outpatient care and to educate patients to seek a preliminary triage consultation with their family physicians; it is possible that the COVID-19 outbreak is pushing our current society in a similar direction.
Certainly, the priority now is to fight the pandemic, but in time, many aspects of healthcare should be reviewed and better organization through telemedicine sought [3]. Hopefully, patients will have learnt to more appropriately utilize the numerous resources offered by the Italian NHS for all of its patients
Envisaging the Intrinsic Departure from Zipf’s Law as an Indicator of Economic Concentration along Urban–Rural Gradients
A rank-size rule following Zipf’s law was tested along a complete urban–rural hierarchy in Greece using 2021 census data released at different administrative levels. Testing five econometric specifications (linear, quadratic, and cubic forms, together with refined logistic and Gompertz forms) on log-transformed population numbers, deviations from the rank-size rule were assumed as an indicator of economic concentration (considering settlements, population, and activities jointly) along the density gradient in Greece. This hypothesis was verified using progressively disaggregated population numbers at (i) regional units (n = 75), (ii) ‘Kallikratis’ municipalities (n = 333), (iii) ‘Kapodistrian’ municipalities (n = 1037), and (iv) local communities (n = 6126). Econometric results were stable across geographical levels and indicate a relatively poor fit of linear specifications, the classical formulation of Zipf’s law. Quadratic specifications displayed a good fit for all territorial levels outperforming cubic specifications. Gompertz specifications outperformed logistic specifications under aggregate partitions (e.g., regional units and ‘Kallikratis’ municipalities). Quadratic specifications outperformed both logistic and Gompertz specifications under disaggregated levels of investigation (‘Kapodistrian’ municipalities and local communities). Altogether, these findings indicate the persistence of non-linear rank-size relationships estimated over a cross-section of population data at progressively detailed observational units. Such evidence enriches the recent literature on Zipf’s law, demonstrating the inherent complexity of rank-size rules tested on real data along the whole density gradient in a given country
Working in (Slow) Progress: Socio-Environmental and Economic Dynamics in the Forestry Sector and the Contribution to Sustainable Development in Europe
Forest ecosystems are increasingly subject to disturbances, such as extreme (climate) events, fires and pathological outbreaks, which exert significant (and still poorly quantified) economic impacts, despite their intrinsic resilience. How forest management addresses these challenges will have profound effects on human health, environmental diversity, (ecological and economic) productivity and the ability of forest ecosystems to recovery from exogenous shocks. Assuming forests as ensuring ecosystem services that are vital to society and human well-being, in addition to providing wood material, a better knowledge of forest ecosystems appears a key requirement to delineate a developmental strategy that guarantees environmental protection objectives and achievement of climate and energy targets. In the context of Sustainable Development Goals of the United Nations 2030 Agenda, the information available on forest management practices in Europe still seems not completely suitable to provide a comprehensive overview of ecosystem conditions, their economic value, and their biodiversity. Based on a literature review, the present contribution focuses on recent dynamics of the forestry sector in Europe, delineating the inherent complexity in the environmental-economic nexus. Additionally, the extensive use of a key natural resource such as wood exerts social implications for local districts adapting to ecological change-from climate warming to landscape transformations. In line with earlier studies, our paper confirms the role of the technical-economic dimension of forestry in sustainable development paths of districts and regions, affecting together (i) short-term economic dynamics, (ii) the growth prospects of the sector, (iii) the organization of the supply chain, (iv) the interconnections between companies, and (v) investment strategies at large. One of the major issues is the low reliability of official statistics to provide a coherent picture of the actual status of forest resources. At the same time, forestry practices are pivotal in maintaining habitats and species while increasing the timber production that remains (too) sustainable. This is the policy direction constantly given by the European Commission to stimulate over time public bodies and private (in whose hands is about 60% of European forests) to start virtuous paths in the circular economy with considerable repercussions in terms of additional jobs, recycled materials, mt of CO2 equivalent avoided, and increase in community added value. In this context, forestry should assure a more 'holistic' contribution to sustainable development paths at various spatial scales. This means concentrating on both economic and environmental targets based on the identification of significant (ecological-economic) dimensions that may delineate future lines of investigation and policy intervention, while increasing between member countries and regional authorities
Zika virus induces FOXG1 nuclear displacement and downregulation in human neural progenitors
Congenital alterations in the levels of the transcription factor Forkhead box g1 (FOXG1) coding gene trigger "FOXG1 syndrome," a spectrum that recapitulates birth defects found in the "congenital Zika syndrome," such as microcephaly and other neurodevelopmental conditions. Here, we report that Zika virus (ZIKV) infection alters FOXG1 nuclear localization and causes its downregulation, thus impairing expression of genes involved in cell replication and apoptosis in several cell models, including human neural progenitor cells. Growth factors, such as EGF and FGF2, and Thr271 residue located in FOXG1 AKT domain, take part in the nuclear displacement and apoptosis protection, respectively. Finally, by progressive deletion of FOXG1 sequence, we identify the C-terminus and the residues 428-481 as critical domains. Collectively, our data suggest a causal mechanism by which ZIKV affects FOXG1, its target genes, cell cycle progression, and survival of human neural progenitors, thus contributing to microcephaly
