    Fracture of abutment screw supporting a cemented implant-retained prosthesis with external hexagon connection: a case report with sem evaluation

    One of the causes of implant failures in cemented implant-retained prostheses is the fracture of abutment screw or UCLA abutment. This article reports a case of simultaneous fracture of two UCLA abutments screws occurring in an implant-supported prosthesis placed in the mandibular molar region. The fractured structures were examined under scanning electron microscopy to investigate the probable causes of the failure, which were not related to failures on materials or fabrication of the screws, but rather were due to shear forces. The misfit in cemented prostheses may be the most likely cause of shear force generation

    Aripiprazol no tratamento do transtorno de estresse pós-traumático: um ensaio clínico aberto

    OBJECTIVE: Post traumatic stress disorder is frequent in the general population (7.8%-lifetime-USA). The selective serotonin reuptake inhibitors are the first choice of treatment but result in low remission rates. This study aims to evaluate the effect of aripiprazole monotherapy for the treatment of post traumatic stress disorder. METHOD: Thirty-two patients diagnosed with post traumatic stress disorder were included in a 16-week open label trial of aripiprazole. They were evaluated at baseline, week 8, and 16 with the Clinician-Administered PTSD Scale, Beck Depression Inventory, Beck Anxiety Inventory, Medical Outcome Study Short Form 36, and Social Adjustment Scale. Statistical analysis were performed with an intention-to-treat approach and last observation carried forward. A general linear model for repeated measures comparing the factor with 3 continuous measures from baseline, 8 and 16 weeks was used. A between-subject factor was included RESULTS: Nine patients discontinued the treatment. The mean aripiprazole dose was 9.6 (± 4.3) mg/day. The mean scores at baseline and endpoint for all measures were: Clinician-Administered PTSD Scale - 82.7 (± 23.1) and 51.4 (± 31.4) (F = 11.247, p = 0.001); Beck Anxiety Inventory - 31.7 (± 13.4) and 25.4 (± 18.2) (F = 8.931, p = 0.011); Social Adjustment Scale - 2.4 (± 0.45) and 2.27 (± 0.57) (F = 8.633, p = 0.012); Medical Outcome Study Short Form 36 - 76.6 (± 14.11) and 94.01 (± 25.06) (F = 10.127 p = 0.007); and Beck Depression Inventory - 26.06 (± 11.6) and 21.35 (± 12.6) (F = 1.580, p = 0.042). In all measurements, the differences were statistically significant. CONCLUSIONS: Patients achieved a good response to treatment with aripiprazole, but placebo-controlled studies are needed for more accurate results.OBJETIVO: O transtorno de estresse pós-traumático é um quadro prevalente (7,8%-lifetime-EUA) que provoca grande prejuízo aos pacientes. Os inibidores seletivos de recaptação de serotonina, medicação de primeira escolha para o tratamento, mostram baixos índices de remissão. Este estudo pretende apresentar uma diferente escolha de medicamento para tratar o transtorno de estresse pós-traumático. MÉTODO: Trinta e dois pacientes com transtorno de estresse pós-traumático receberam aripiprazol por 16 semanas. Foram submetidos na entrada, 8 e 16 semanas às escalas Clinician-Administered PTSD Scale, Beck Depression Inventory, Beck Anxiety Inventory, Medical Outcome Study Short Form 36 e Social Adjustment Scale. Foi usado o modelo linear generalizado para medidas repetidas comparando o fator com as três medidas contínuas nos três pontos de avaliação. Foi feita uma comparação entre sujeitos (grupo tratamento) usando modelo linear generalizado univariado. Usamos a intenção de tratamento e a estratégia da última observação com endpoint (Last Observation Carried Forward). RESULTADOS: Nove pacientes descontinuaram antes da segunda avaliação. A dose média foi 9,6 (± 4,3) mg/dia. As medidas na entrada e no final do tratamento foram: Clinician-Administered PTSD Scale - 82,7 (± 23,1) e 51,4 (± 31,4) (F = 11,247, p = 0,001); Beck Anxiety Inventory - 31,7 (± 13,4) e 25,4 (± 18,2) (F = 8,931, p = 0,011); Social Adjustment Scale - 2,4 (± 0,45) e 2,27 (± 0,57) (F = 8,633, p = 0,012); Medical Outcome Study Short Form 36 - 76,6 (± 14,11) e 94,01 (± 25,06) (F = 10,127 p = 0,007); e Beck Depression Inventory - 26,06 (± 11,6) e 21,35 (± 12,6) (F = 1,580, p = 0,042).     Early Trauma Inventory (ETI): cross-cultural adaptation and internal consistency

    Early life stress is a strong predictor of future psychopathology during adulthood. The Early Trauma Inventory (ETI) was developed to detect the presence and impact of traumatic experiences that occurred up to 18 years of age. The ETI was translated and cross-culturally adapted and had its consistency evaluated. Victims of violence that met the inclusion and exclusion criteria were submitted to SCID-I and ETI. Ninety-one patients with post-traumatic stress disorder (PTSD) were included. Cronbach's alpha in the different domains varied from 0.595 to 0.793, and the total score was 0.878. Except for emotional abuse, most of the various domains displayed inter-item correlation rates of 0.51 to 0.99. The adapted version was useful for clinical and research purposes and showed good internal consistency and inter-item correlation. The ETI is a valid instrument with good consistency for evaluating history of childhood and adolescent trauma in adults.As experiências traumáticas precoces são um fator de risco preditivo de problemas psicopatológicos futuros. O Early Trauma Inventory (ETI) é um instrumento que avalia em indivíduos adultos experiências traumáticas ocorridas antes dos 18 anos de idade. Tal instrumento foi traduzido, transculturalmente adaptado e sua consistência interna foi avaliada. Vítimas de violência que preencheram os critérios de inclusão e exclusão foram submetidas a uma entrevista diagnóstica (SCID-I) e ao ETI. Foram incluídos 91 pacientes com o transtorno do estresse pós-traumático (TEPT). O alfa de Cronbach nos diferentes domínios variou de 0,595-0,793, e o escore total foi de 0,878. A maior parte dos itens nos vários domínios, com exceção do abuso emocional, apresentou índices de correlação interitem entre 0,51-0,99. A versão adaptada foi útil tanto na clínica quanto na pesquisa. Apresentou boa consistência interna e na correlação interitem.     DIAGNÓSTICO DO USO DO SOLO EM APP NA MICROBACIA DO CÓRREGO SANTO ANTONIO - SÃO MANUEL (SP) EM FUNÇÃO DA LEGISLAÇÃO AMBIENTAL

    O presente trabalho visou à utilização de geotecnologias no mapeamento de uso da terra em áreas de APP's, permitindo as suas delimitações e diagnóstico de possíveis intervenções antrópicas, contribuindo para futuras fiscalizações ambientais. A microbacia com 4286,15ha, situa-se entre as coordenadas geográficas 22o 31' 52" a 22o  38' 20" de latitude S e 48o 33' 40" a 48o 38' 47" de longitude WGr.  As bases cartográficas utilizadas foi a carta planialtimétrica para o georreferenciamento da imagem de satélite de 2011. O SIG-IDRISI Selva foi utilizado na classificação supervisionada e a determinação das áreas do mapa temático. O uso da terra mostrou que a cultura da cana-de-açúcar ocupa a maior parte da área (81,00%), mostrando com isso a predominância da ocupação agrícola na região. A imagem de satélite e a utilização do SIG mostraram-se importantes ferramentas, fornecendo resultados confiáveis num pequeno intervalo de tempo. Os dados obtidos auxiliarão nos futuros planejamentos de recuperação da área, visto que possibilitaram a verificação de que a área não vem sendo ambientalmente preservada, pelo fato de apresentar apenas 12,54% de matas, sendo que o mínimo exigido pela legislação do Código Florestal Brasileiro vigente é de 20%

    Symbiotic skin bacteria as a source for sex-specific scents in frogs

    Amphibians are known to possess a wide variety of compounds stored in their skin glands. While significant progress has been made in understanding the chemical diversity and biological relevance of alkaloids, amines, steroids, and peptides, most aspects of the odorous secretions are completely unknown. In this study, we examined sexual variations in the volatile profile from the skin of the tree frog Boana prasina and combined culture and culture-independent methods to investigate if microorganisms might be a source of these compounds. We found that sesquiterpenes, thioethers, and methoxypyrazines are major contributors to the observed sex differences. We also observed that each sex has a distinct profile of methoxypyrazines, and that the chemical origin of these compounds can be traced to a Pseudomonas sp. strain isolated from the frog´s skin. This symbiotic bacterium was present in almost all individuals examined from different sites and was maintained in captive conditions, supporting its significance as the source of methoxypyrazines in these frogs. Our results highlight the potential relevance of bacteria as a source of chemical signals in amphibians and contribute to increasing our understanding of the role that symbiotic associations have in animals.Fil: Brunetti, Andrés Eduardo.     Geoprocessamento na geração de cartas temáticas para fins de planejamento ambiental na microbacia do córrego Comur em Botucatu-SP

    O levantamento do uso da terra numa dada região é fundamental para a compreensão dos padrões de organização daquele determinado espaço. O presente trabalho teve como objetivo avaliar o geoprocessamento aplicado na geração de cartas temáticas para fins de planejamento ambiental, através do Sistema de Informações Geográficas Idrisi Andes 15.0. A microbacia do Córrego Comur está situada no município de Botucatu (SP) e entre as coordenadas geográficas: latitude 22º 44’ 42” a 22º 48’ 12” S e longitudes 48º 23’ 04” a 48º 25’ 54”  W Gr, com uma área de 1458,4ha. Os resultados permitiu inferir que as classes de declive de 0 a 3% e de 3 a 6% ocorrentes na microbacia representam mais de 67% da área e as de 0 a 20% predominaram em mais de 80% da microbacia. As áreas com relevo suavemente ondulado (3 a 6%) foram as mais representativas (36,92%). A unidade de solo Latossolo Vermelho Distroférrico ocorreu em mais de 45% da área. O SIG mostrou-se uma excelente ferramenta imprescindível, pois facilita e agiliza a obtenção dos dados, além de permitir seu armazenamento, que poderão ser utilizados para outras análises em futuros planejamentos geoambientais.The rising of the soil use in a certain area is fundamental for the understanding of the organization patterns of that determined space. The present work had as objective to evaluate the applied geoprocessing in the generation of thematic maps aiming at environmental planning, through the Geographical Information System Idrisi Andes 15.0. The Stream Comur watershed is placed in the municipality of Botucatu (SP) and among the geographical coordinates: latitude 22o 44’ 42” to 22o 48’ 12” S and longitudes 48o 23’ 04” to 48o 25’ 54” W Gr, with an area of 1458.4 ha. The results enabled to infer that the slope classes from 0 to 3% and from 3 to 6% which exist in the watershed represent more than 67% of the area and the one from 0 to 20% prevailed in more than 80% of the watershed. The areas with relief smoothly wavy (3 to 6%) they were the most representative (36.92%). the unit of soil Latossolo Vermelho Distroférrico happened in more than 45% of the area. The GIS was an excellent and indispensable tool, because it facilitates and it activates the obtaining of the data, besides enabling its storage, which can be used for other analyses in future geoenvironmental planning.

    Evaluation of Physical Activity Habits in Patients with Posttraumatic Stress Disorder

    OBJECTIVE: In this study, we present data from a survey that aimed to assess the physical activity habits of adult Brazilian patients with Posttraumatic Stress Disorder. METHOD: Fifty male and female patients with Posttraumatic Stress Disorder participated in this study. The mean age at onset was 37±12 years, and the mean time between diagnosis and follow-up was 3.6±4.2 years. RESULTS: Substantial changes in physical activity habits were observed following the onset of PTSD. While more than half of the patients participated in physical activities prior to Posttraumatic Stress Disorder onset, there was a significant reduction in their participation afterwards. The justifications for stopping physical activities or sport participation were lack of time and lack of motivation. DISCUSSION: Several studies have shown that physical exercise decreases reverts symptoms of psychiatric disorders such as depression, anxiety and social isolation. We could therefore hypothesize that patients with Posttraumatic Stress Disorder who exercise should experience the same benefits. CONCLUSION: Our findings demonstrated that patients with Posttraumatic Stress Disorder have low levels of participation in sports or physical activities

    A randomized, double-blind, placebo-controlled trial to assess the efficacy of topiramate in the treatment of post-traumatic stress disorder

    <p>Abstract</p> <p>Background</p> <p>Topiramate might be effective in the treatment of posttraumatic stress disorder (PTSD) because of its antikindling effect and its action in both inhibitory and excitatory neurotransmitters. Open-label studies and few controlled trials have suggested that this anticonvulsant may have therapeutic potential in PTSD. This 12-week randomized, double-blind, placebo-controlled clinical trial will compare the efficacy of topiramate with placebo and study the tolerability of topiramate in the treatment of PTSD.</p> <p>Methods and design</p> <p>Seventy-two adult outpatients with DSM-IV-diagnosed PTSD will be recruited from the violence program of Federal University of São Paulo Hospital (UNIFESP). After informed consent, screening, and a one week period of wash out, subjects will be randomized to either placebo or topiramate for 12 weeks. The primary efficacy endpoint will be the change in the Clinician-administered PTSD scale (CAPS) total score from baseline to the final visit at 12 weeks.</p> <p>Discussion</p> <p>The development of treatments for PTSD is challenging due to the complexity of the symptoms and psychiatric comorbidities. The selective serotonin reuptake inhibitors (SSRIs) are the mainstream treatment for PTSD, but many patients do not have a satisfactory response to antidepressants. Although there are limited clinical studies available to assess the efficacy of topiramate for PTSD, the findings of prior trials suggest this anticonvulsant may be promising in the management of these patients.</p> <p>Trial Registration</p> <p>NCT 00725920</p

    COVID-19 Severity and Survival over Time in Patients with Hematologic Malignancies: A Population-Based Registry Study

    Mortality rates for COVID-19 have declined over time in the general population, but data in patients with hematologic malignancies are contradictory. We identified independent prognostic factors for COVID-19 severity and survival in unvaccinated patients with hematologic malignancies, compared mortality rates over time and versus non-cancer inpatients, and investigated post COVID-19 condition. Data were analyzed from 1166 consecutive, eligible patients with hematologic malignancies from the population-based HEMATO-MADRID registry, Spain, with COVID-19 prior to vaccination roll-out, stratified into early (February–June 2020; n = 769 (66%)) and later (July 2020–February 2021; n = 397 (34%)) cohorts. Propensity-score matched non-cancer patients were identified from the SEMI-COVID registry. A lower proportion of patients were hospitalized in the later waves (54.2%) compared to the earlier (88.6%), OR 0.15, 95%CI 0.11–0.20. The proportion of hospitalized patients admitted to the ICU was higher in the later cohort (103/215, 47.9%) compared with the early cohort (170/681, 25.0%, 2.77; 2.01–3.82). The reduced 30-day mortality between early and later cohorts of non-cancer inpatients (29.6% vs. 12.6%, OR 0.34; 0.22–0.53) was not paralleled in inpatients with hematologic malignancies (32.3% vs. 34.8%, OR 1.12; 0.81–1.5). Among evaluable patients, 27.3% had post COVID-19 condition.
