AbstractBackgroundThe application of mesh-reinforced hiatal closure has resulted in a significant reduction in recurrence rates in comparison with primary suture repair. One of the most debated issues is the risk of complications related to the use of the prosthesis, such as esophageal erosion and postoperative dysphagia. The aim of this study is to present our short-terms results in the treatment of laparoscopic paraesophageal hiatal hernia (LPHH) with a synthetic polyglycolic acid:trimethylene carbonate mesh (Gore Bio A®).MethodsFrom January 2011 to December 2012, 10 patients with large paraesophageal hiatal hernias and hiatal defect over 5 cm were included. Primary simple suture of the crura and additional reinforcement with a Gore Bio A® mesh was performed. Hiatal hernia or gastroesophageal reflux disease (GERD) symptoms recurrence, dysphagia and mesh-related complications were investigated.ResultsOf the 10 patients undergoing mesh repair, there were 7 women and 3 men with a mean age of 65.5 years. All operations were completed laparoscopically. Median postoperative stay was 3 days. After a median follow-up of 20.3 months, one patient developed a recurrent hiatal hernia (10%). There were no mesh-related complications.ConclusionsThe use of Gore Bio A® mesh for the laparoscopic repair of large paraesophageal hiatal hernias is safe and with a reasonably low recurrence rate in this short-term study. Additional long-term studies with ample numbers carried out for years will be necessary to see if this synthetic mesh is not only safe but also successful in the prevention of recurrences

Arrested dynamics of the dipolar hard-sphere model

We report the combined results of molecular dynamics simulations and theoretical calculations concerning various dynamical arrest transitions in a model system representing a dipolar fluid, namely, N (softcore) rigid spheres interacting through a truncated dipole-dipole potential. By exploring different regimes of concentration and temperature, we find three distinct scenarios for the slowing down of the dynamics of the translational and orientational degrees of freedom: At low ({η\eta} = 0.2) and intermediate (η{\eta} = 0.4) volume fractions, both dynamics are strongly coupled and become simultaneously arrested upon cooling. At high concentrations ({η\eta} <\lt 0.6), the translational dynamics shows the features of an ordinary glass transition, either by compressing or cooling down the system, but with the orientations remaining ergodic, thus indicating the existence of partially arrested states. In this density regime, but at lower temperatures, the relaxation of the orientational dynamics also freezes. The physical scenario provided by the simulations is discussed and compared against results obtained with the self-consistent generalized Langevin equation theory, and both provide a consistent description of the dynamical arrest transitions in the system. Our results are summarized in an arrested states diagram which qualitatively organizes the simulation data and provides a generic picture of the glass transitions of a dipolar fluid

Columnar spaces in Valencian Gothic Architecture. The cloister of the Carthusian Monastery of Porta Coeli

[EN] The Cartuja of Porta Coeli preserves a Gothic cloister which, despite its small size and apparent simplicity, stands out for its compositional beauty and sophisticated stonework. It is, moreover, a particularly singular work in the context of Valencian medieval architecture. The eremitic nature of the Carthusian order, far from any contact with the outside world, has kept this exquisite architectural example, where the tradition of stonework and structural masonry coexist, practically intact until the present day. This text shows the results of the graphic and documentary analysis carried out, restoring, by means of the survey, the compositional laws and the trace as a graphic design method. The article, therefore, presents an initial study of this architectural space, placing it in the context of Valencian stonemasonry, at the time of the great master stonemasons, and, furthermore, it aims to provide a well-founded approach to the discovery of its authorship.[ES] La Cartuja de Porta Coeli conserva un claustro gótico que, pese a sus reducidas dimensiones y aparente sencillez, destaca por su belleza compositiva y sofisticada manufactura de cantería. Navarro Camallonga, P.; Verdejo Gimeno, P.; Priego De Los Santos, JE.; López Patiño, MG. Cultural Heritage Restoration of a Hemispherical Vault by 3D Modelling and Projection of Video Images with Unknown Parameters and from Unknown Locations

[EN] Reverse engineering applied to architectural restoration for the reconstruction of structural surfaces depends on metric precision. Sometimes there are elements on these surfaces whose value is even higher than the building itself. This is the case for many churches whose ceilings have pictorial works of art. Reconstruction requires the existence of some identifiable remainder and/or a surface geometry that enables mathematical development. In our case, the vault has an irregular hemispherical geometry (without possible mathematical development), and there are no significant remains of the painting (which was destroyed by a fire). Through the 3D modelling of the irregular vault and two historic frames with a camera of unknown geometry, an inverse methodology is designed to project the original painting without metric deformations. For this, a new methodology to locate the camera positions is developed. After, a 3D virtual mathematical model of the complete image on the vault is calculated, and from it, partial 3D virtual images are automatically calculated depending on the variable unknown positions of the video cannons (distributed along the upper corridor of the apse) that will project them (visually forming a perfect complete 3D image)Herráez Boquera, J.; Denia Rios, JL.; Priego De Los Santos, E.; Navarro Esteve, PJ.; Martín Sánchez, MT.; Rodríguez Pereña, J. Spontaneous functional recovery after focal damage in neuronal cultures

    Get PDF
Damage in biological neuronal networks triggers a complex functional reorganization whose mechanisms are still poorly understood. To delineate this reorganization process, here we investigate the functional alterations of in vitro rat cortical circuits following localized laser ablation. The analysis of the functional network configuration before and after ablation allowed us to quantify the extent of functional alterations and the characteristic spatial and temporal scales along recovery. We observed that damage precipitated a fast rerouting of information flow that restored network's communicability in about 15 min. Functional restoration was led by the immediate neighbors around trauma but was orchestrated by the entire network. Our in vitro setup exposes the ability of neuronal circuits to articulate fast responses to acute damage, and may serve as a proxy to devise recovery strategies in actual brain circuits. Moreover, this biological setup can become a benchmark to empirically test network theories about the spontaneous recovery in dynamical networks

Metabolomics analysis of type 2 diabetes remission identifies 12 metabolites with predictive capacity: a CORDIOPREV clinical trial study.

Type 2 diabetes mellitus (T2DM) is one of the most widely spread diseases, affecting around 90% of the patients with diabetes. Metabolomics has proven useful in diabetes research discovering new biomarkers to assist in therapeutical studies and elucidating pathways of interest. However, this technique has not yet been applied to a cohort of patients that have remitted from T2DM. All patients with a newly diagnosed T2DM at baseline (n = 190) were included. An untargeted metabolomics approach was employed to identify metabolic differences between individuals who remitted (RE), and those who did not (non-RE) from T2DM, during a 5-year study of dietary intervention. The biostatistical pipeline consisted of an orthogonal projection on the latent structure discriminant analysis (O-PLS DA), a generalized linear model (GLM), a receiver operating characteristic (ROC), a DeLong test, a Cox regression, and pathway analyses. The model identified a significant increase in 12 metabolites in the non-RE group compared to the RE group. Cox proportional hazard models, calculated using these 12 metabolites, showed that patients in the high-score tercile had significantly (p-value < 0.001) higher remission probabilities (Hazard Ratio, HR, high versus low = 2.70) than those in the lowest tercile. The predictive power of these metabolites was further studied using GLMs and ROCs. The area under the curve (AUC) of the clinical variables alone is 0.61, but this increases up to 0.72 if the 12 metabolites are considered. A DeLong test shows that this difference is statistically significant (p-value = 0.01). Our study identified 12 endogenous metabolites with the potential to predict T2DM remission following a dietary intervention. These metabolites, combined with clinical variables, can be used to provide, in clinical practice, a more precise therapy. Siglo Cero. 43 (243): 7-28.)
