    Aravind Adiga’s novel The White Tiger encapsulates the complexities of identity formation in a milieu effected by neo-capitalism. The novel, for many, is about a new identity made available to the hitherto marginalized in the form of opportunities unveiled by market forces. It is also perceived as a registration of the frustration and anger of the deprived that has become conscious of the new possibilities. Understandably, interpreting the novel on these lines leads to the identification of the protagonist Balram as a champion of the marginalized, settling scores with the oppressive system. However, there are far subtler notes in the protagonist’s attitude to these sweeping changes than the simple and one dimensional approach of an achiever who is able to break the “rooster coop” and revel at the reversal of fortune. Neither is it a representative voice of the suppressed class turning the table on its oppressor by using the opportunities offered by the global market. The “notes” of the voices that emerge as Balram, the boy from darkness, moves up the ladder to become Ashok Sharma, the entrepreneur hiding in light, not only lack symphony but also create a distinct dissonance. Clearly, the discord in the changing notes is brought about by the forces that changed the world he lives in-a neo-capitalist world.     Facial expression is a major area for non-verbal language in day to day life communication. As the statistical analysis shows only 7 percent of the message in communication was covered in verbal communication while 55 percent transmitted by facial expression. Emotional expression has been a research subject of physiology since Darwin’s work on emotional expression in the 19th century. According to Psychological theory the classification of human emotion is classified majorly into six emotions: happiness, fear, anger, surprise, disgust, and sadness. Facial expressions which involve the emotions and the nature of speech play a foremost role in expressing these emotions. Thereafter, researchers developed a system based on Anatomic of face named Facial Action Coding System (FACS) in 1970. Ever since the development of FACS there is a rapid progress of research in the domain of emotion recognition. This work is intended to give a thorough comparative analysis of the various techniques and methods that were applied to recognize and identify human emotions. This analysis results will help to identify the proper and suitable techniques, algorithms and the methodologies for future research directions. In this paper extensive analysis on the various recognition techniques used to identify the complexity in recognizing the facial expression is presented. This work will also help researchers and scholars to ease out the problem in choosing the techniques used in the identification of the facial expression domain

    Evaluation of efficacy and safety profile of tamsulosin in benign prostatic hyperplasia

    Background: To evaluate the efficacy and safety profile of alpha-1A receptor subtype specific antagonist-tamsulosin in benign prostatic hyperplasia.Methods: An open label, non-randomised, prospective, single centred study who were visiting urology department with confirmed diagnosis of benign prostatic hyperplasia (BPH) were included in the study. Thirty patients with BPH were prescribed tamsulosin and were followed up to three months. The initial or baseline data collected were identification of the patients such as international prostate symptom score (IPSS), prostate specific antigen, urine analysis, and ultrasonography of prostate including post void residual urine. The patients were followed up to 3 months to measure clinical and laboratory outcomes (IPSS score, post void residual urine, uroflow rates etc.).Results: In our study, patients receiving tamsulosin 0.4 mg once daily showed a significant improvement in post-void residual urine (PVRU), uroflow rates and IPSS score. The improvements in the above parameters were found to be statistically insignificant at first and third month of follow-up.Conclusions: In our study, tamsulosin an alpha-1A receptor subtype specific antagonist showed significant improvement in BPH symptoms and the drug was well tolerated

    pre-printInterbody fusion techniques have been promoted as an adjunct to lumbar fusion procedures in an effort to enhance fusion rates and potentially improve clinical outcome. The medical evidence continues to suggest that interbody techniques are associated with higher fusion rates compared with posterolateral lumbar fusion (PLF) in patients with degenerative spondylolisthesis who demonstrate preoperative instability. There is no conclusive evidence demonstrating improved clinical or radiographic outcomes based on the different interbody fusion techniques. The addition of a PLF when posterior or anterior interbody lumbar fusion is performed remains an option, although due to increased cost and complications, it is not recommended. No substantial clinical benefit has been demonstrated when a PLF is included with an interbody fusion. For lumbar degenerative disc disease without instability, there is moderate evidence that the standalone anterior lumbar interbody fusion (ALIF) has better clinical outcomes than the ALIF plus instrumented, open PLF. With regard to type of interbody spacer used, frozen allograft is associated with lower pseudarthrosis rates compared with freeze-dried allograft; however, this was not associated with a difference in clinical outcome

    A PROSPECTIVE OBSERVATIONAL STUDY ON SAFETY AND EFFICACY OF HYDROXYCHLOROQUINE AGAINST DICLOFENAC IN OSTEOARTHRITIS

    Objective: The main aim of this study is to compare the effectiveness of treatment of osteoarthritis (OA) with hydroxychloroquine against diclofenac and to compare the safety of the treatment of OA with hydroxychloroquine against diclofenac. The secondary objectives are the comparison of onset and duration of action of both the treatments.Methods: A prospective interventional study has been done in a tertiary care teaching hospital for 1 year from January 2016 to December 2016. The interventional model included an active control (diclofenac - nonsteroidal anti-inflammatory drugs). The control group received diclofenac 75 mg for 12 weeks, whereas the test group received hydroxychloroquine 200 mg for 3 months.Results: Pain control is significantly rapid in diclofenac group, whereas the duration of action is significantly increased in hydroxychloroquine. It can be clearly seen that the hydroxychloroquine group significantly increases the quality of life when compared to diclofenac group. It is clearly seen that the incidence of gastric ulcers is more common in diclofenac group when compared to hydroxychloroquine group.Conclusion: Hydroxychloroquine offers a newer spectrum in the management of OA. Although it is slow acting, it has significantly increased the quality of patient as it can have a prolonged action. Hence, hydroxychloroquine can be used as a newer entity in the management of OA

    Lumbar spondylosis, a degenerative condition of lumbar spine is affecting majority of adults presently. The treatment options available nowadays are diverse and often undependable ensuing rising costs and variability in management throughout the country. It may be considered as Kateegraha owing to the grasping pain or stiffness in the Katee Pradesha. Katee is one of the Vatasthaanas and pain/stiffness is caused by Vata, hence Kateegraha may be considered as Vatavyadhi. Vasthi therapy is the Agryachikitsa for Vata Dosha which can be done here. Madhughrita Yapanavasthi is Vatahara, Brumhana and Trikaprishta Soolahara. It would be especially good in degenerative conditions associated with pain affecting Trika region like Lumbar spondylosis. The study design was of interventional type. Twenty patients, satisfying the selection criteria were selected from the OPD of Govt. Ayurveda College Panchakarma Hospital, Thiruvananthapuram. They were given Deepana Pachana first, and then Snehasweda and finally Madhughrita Yapanavasthi for 8 days while the follow up period were for 16 days. The assessment was done before treatment, after treatment and after follow up. Changes in the following variables- pain, stiffness, tenderness, range of movements and muscle strength were analyzed. For all variables except the muscle strength p value were statistically significant at the level of 0.001. The p value for Muscle strength was significant at the level of 0.05 for after treatment - after follow up comparison. And it was significant at the level of 0.01 for before treatment - after follow up comparison. Madhughrita Yapanavasthi is thus effective in managing Lumbar spondylosis
