    Supine sleep and positional sleep apnea after acute ischemic stroke and intracerebral hemorrhage

    OBJECTIVE: Obstructive sleep apnea is frequent during the acute phase of stroke, and it is associated with poorer outcomes. A well-established relationship between supine sleep and obstructive sleep apnea severity exists in non-stroke patients. This study investigated the frequency of supine sleep and positional obstructive sleep apnea in patients with ischemic or hemorrhagic stroke. METHODS: Patients who suffered their first acute stroke, either ischemic or hemorrhagic, were subjected to a full polysomnography, including the continuous monitoring of sleep positions, during the first night after symptom onset. Obstructive sleep apnea severity was measured using the apnea-hypopnea index, and the NIHSS measured stroke severity. RESULTS: We prospectively studied 66 stroke patients. The mean age was 57.6±11.5 years, and the mean body mass index was 26.5±4.9. Obstructive sleep apnea (apnea-hypopnea index >5) was present in 78.8% of patients, and the mean apnea-hypopnea index was 29.7±26.6. The majority of subjects (66.7%) spent the entire sleep time in a supine position, and positional obstructive sleep apnea was clearly present in the other 23.1% of cases.     Relative expression of CRy10 in GM cotton lines resistant to boll weevil.

    Boll weevil (Anthonomus grandis) is a serious pest of cotton crop in several parts of world. The control is done by synthetic pesticides that increase the management costs. Embrapa coordinates an important research involving development of GM cotton resistant to boll weevil, by genetic transformation. Seven line (T0-8H) containing a Btcry 10 have been identified by feeding bioassays and ELISA, which showed reasonable toxicity against adult boll weevils. In this work, these lines were used in order to estimate the presence and relative expression of cry 10 by PCR and RT-qPCR, respectively, from young buds of 50d-plants. RT-qPCR assays were performed, by using Syber Green Rox Plus Master Mix 2X (LGC). Three endogenous cotton gene (GhACT, GhUBQ14 and GhPP2A) were used as a standard control. At first, 95°C/15 min and 40 cycles of 95°C/ 20 sec, 60°C/ 20 sec and 72°C/ 20 sec. Then, a curve of denaturation (melting curve) was performed after conclusion of amplification, at 95°C/15 sec, 60°C/15 sec, rising 2°/min until reaching 95°C. All reactions were carried out with experimental and biological replications. The threshold cycle (Ct) and PCR efficiency was estimated by Real-time PCR Miner program. The analyses were performed by using qBASEPlus program. Graphics, Cqs and Melt curve were automatically generated, based on the normalization method with a reference gene, ??Cq. Varied levels of expression were found in GM lines, too low in 8H-269 and 8H-357, mid in 8H-282 and 8H-346 and high in 8H- 336 (14x). These data agreed with previous results obtained by ELISA assays. Eleven lines derivate from 8H-336 (T1) were analyzed by PCR assays with genomic DNA, using 2 primer combinations. More than 50% showed amplicons confirming the presence of gene in selected lines. Taking in account that a reasonable level of resistance should overcoming 2 ug of protein/g tissue, we suggested that 8H-366 is the best genotype for control the cotton boll weevil. This material will be further advanced for entomological assays with larvae and adults of boll weevil

    Lenalidomide Maintenance and Measurable Residual Disease in a Real-World Multiple Myeloma Transplanted Population Receiving Different Treatment Strategies Guided by Access to Novel Drugs in Brazil

    Despite recent advances in multiple myeloma (MM), the incorporation of novel agents and measurable residual disease (MRD) monitoring in low-income countries remains a challenge. Although lenalidomide maintenance (M-Len) after autologous stem cell transplantation (ASCT) has been associated with improved outcomes and MRD has refined the prognosis of complete response (CR) cases, until now, there have been no data on the benefits of these approaches in Latin America. Here, we evaluate the benefits of M-Len and MRD using next-generation flow cytometry (NGF-MRD) at Day + 100 post-ASCT (n = 53). After ASCT, responses were evaluated based on the International Myeloma Working Group criteria and NGF-MRD. MRD was positive in 60% of patients with a median progression-free survival (PFS) of 31 months vs. not reached (NR) for MRD-negative cases (p = 0.05). The patients who received M-Len continuously had a significantly better PFS and overall survival (OS) than those without M-Len (median PFS: NR vs. 29 months, p = 0.007), with progression in 11% vs. 54% of cases after a median follow-up of 34 months, respectively. In a multivariate analysis, MRD status and M-Len therapy emerged as independent predictors of PFS (median PFS of M-Len/MRD− vs. no M-Len/MRD+ of NR vs. 35 months, respectively; p = 0.01). In summary, M-Len was associated with improved survival outcomes in our real-world MM cohort in Brazil, with MRD emerging as a useful reproducible tool to identify patients at an earlier risk of relapse.     Agronomic biofortification with selenium impacts storage proteins in grains of upland rice

    © 2019 Society of Chemical Industry BACKGROUND: Selenium (Se) is an essential element for humans and animals. Rice is one of the most commonly consumed cereals in the world, so the agronomic biofortification of cereals with Se may be a good strategy to increase the levels of daily intake of Se by the population. This study evaluated the agronomic biofortification of rice genotypes with Se and its effects on grain nutritional quality. Five rates of Se (0, 10, 25, 50, and 100 g ha−1) were applied as selenate via the soil to three rice genotypes under field conditions. RESULTS: Selenium concentrations in the leaves and polished grains increased linearly in response to Se application rates. A highly significant correlation was observed between the Se rates and the Se concentration in the leaves and grains, indicating high translocation of Se. The application of Se also increased the concentration of albumin, globulin, prolamin, and glutelin in polished grains. CONCLUSION: Biofortifying rice genotypes using 25 g Se ha−1 could increase the average daily Se intake from 4.64 to 66 μg day−1. Considering that the recommended daily intake of Se by adults is 55 μg day−1, this agronomic strategy could contribute to alleviating widespread Se malnutrition. © 2019 Society of Chemical Industry

    Órteses para o paciente com osteoartrite do polegar: o que os terapeutas ocupacionais no Brasil indicam?

    Órteses são consideradas a primeira linha de atuação para o tratamento de disfunções causadas pela osteoartrite (OA) da articulação carpometacárpica do polegar. Entretanto, não existem estudos que investiguem as preferências e indicações de órteses por profissionais brasileiros. Objetivo: Identificar os modelos de órteses indicados por terapeutas ocupacionais para pacientes com OA do polegar, bem como as barreiras encontradas para o uso deste recurso. Métodos: Estudo exploratório transversal, realizado através de questionário eletrônico com terapeutas ocupacionais brasileiros. Os dados obtidos foram analisados por meio da distribuição, frequência e porcentagem das respostas. Resultados: Participaram da pesquisa 52 terapeutas ocupacionais de todo o Brasil, atuando junto a pacientes reumáticos. A maioria das indicações foi de órteses sob medida (81%), envolvendo mais de uma articulação (83,7%). Políticas institucionais e ausência de materiais foram as principais dificuldades para 48,1% dos participantes. Conclusão: A indicação de órteses para pacientes com OA se faz presente na prática clínica de terapeutas ocupacionais e torna-se importante o incentivo a estratégias que permitam o aperfeiçoamento e difusão de ações de reabilitação junto a esta população.Orthoses are the first line of action for the treatment of dysfunctions caused by osteoarthritis (OA) of the carpometacarpal joint of the thumb. However, there are no studies that investigate the preferences and orthotics prescriptions made by Brazilian health professionals. Objective: To identify orthotic designs indicated by occupational therapists for patients with thumb OA and the barriers encountered to use these devices. Methods: Cross-sectional, exploratory study conducted by electronic questionnaire with Brazilian occupational therapists. Data were analyzed using the distribution, frequency and percentage of responses. Results: 52 Brazilian occupational therapists who treated patients with rheumatic diseases participated in this study. Most of the participants indicated custom-made models (81%), involving more than one joint (83.7%). Institutional policies and lack of materials were the main barriers for 48.1% of the participants. Conclusion: The prescription of orthoses for patients with OA is present in the clinical practice of occupational therapists, and it is important to encourage strategies to the development and dissemination of rehabilitation actions for this population
