    Introduction: The Age-Well clinical trial is an ongoing monocentric, randomized, controlled trial aiming to assess an 18-month preventive meditation-based intervention directly targeting the attentional and emotional dimensions of aging to promote mental health and well-being in elderly people. Methods: One hundred thirty-seven cognitively unimpaired older adults are randomized to either an 18-month meditation-based intervention, a structurally matched foreign language training, or a passive control arm. The impact of the intervention and underlying mechanisms are assessed with detailed cognitive, behavioral, biological, neuroimaging and sleep examinations. Results: Recruitment began in late 2016 and ended in May 2018. The interventions are ongoing and will be completed by early 2020. Discussion: This is the first trial addressing the emotional and cognitive dimension of aging with a long-term nonpharmacological approach and using comprehensive assessments to investigate the mechanisms. Results are expected to foster the development of preventive strategies reducing the negative impact of mental conditions and disorders

    Exposure to negative socio-emotional events induces sustained alteration of resting-state brain networks in older adults

    Basic emotional functions seem well preserved in older adults. However, their reactivity to and recovery from socially negative events remain poorly characterized. To address this, we designed a ‘task–rest’ paradigm in which 182 participants from two independent experiments underwent functional magnetic resonance imaging while exposed to socio-emotional videos. Experiment 1 (N = 55) validated the task in young and older participants and unveiled age-dependent effects on brain activity and connectivity that predominated in resting periods after (rather than during) negative social scenes. Crucially, emotional elicitation potentiated subsequent resting-state connectivity between default mode network and amygdala exclusively in older adults. Experiment 2 replicated these results in a large older adult cohort (N = 127) and additionally showed that emotion-driven changes in posterior default mode network–amygdala connectivity were associated with anxiety, rumination and negative thoughts. These findings uncover the neural dynamics of empathy-related functions in older adults and help understand its relationship to poor social stress recovery

    Effect of an 18-Month Meditation Training on Regional Brain Volume and Perfusion in Older Adults: The Age-Well Randomized Clinical Trial.

Meditation practices recently emerged as a promising mental training exercise to foster brain health and reduce dementia risk. Objective: To investigate the effects of meditation training on brain integrity in older adults. Design, Setting, and Participants: Age-Well was a randomized, controlled superiority trial with blinded end point assessment. Community-dwelling cognitively unimpaired adults 65 years and older were enrolled between November 24, 2016, and March 5, 2018, in France. Participants were randomly assigned (1:1:1) to (1) an 18-month meditation-based training, (2) a structurally matched non-native language (English) training, or (3) no intervention arm. Analysis took place between December 2020 and October 2021. Interventions: Meditation and non-native language training included 2-hour weekly group sessions, practice of 20 minutes or longer daily at home, and 1-day intensive practices. Main Outcomes and Measures: Primary outcomes included volume and perfusion of anterior cingulate cortex (ACC) and insula. Main secondary outcomes included a global composite score capturing metacognitive, prosocial, and self-regulatory capacities and constituent subscores. Results: Among 137 participants (mean [SD] age, 69.4 [3.8] years; 83 [60.6%] female; 54 [39.4%] male) assigned to the meditation (n = 45), non-native language training (n = 46), or no intervention (n = 46) groups, all but 1 completed the trial. There were no differences in volume changes of ACC (0.01 [98.75% CI, -0.02 to 0.05]; P = .36) or insula (0.01 [98.75% CI, -0.02 to 0.03]; P = .58) between meditation and no intervention or non-native language training groups, respectively. Differences in perfusion changes did not reach statistical significance for meditation compared with no intervention in ACC (0.02 [98.75% CI, -0.01 to 0.05]; P = .06) or compared with non-native language training in insula (0.02 [98.75% CI, -0.01 to 0.05]; P = .09). Meditation was superior to non-native language training on 18-month changes in a global composite score capturing attention regulation, socioemotional, and self-knowledge capacities (Cohen d, 0.52 [95% CI, 0.19-0.85]; P = .002). Conclusions and Relevance: The study findings confirm the feasibility of meditation and non-native language training in elderly individuals, with high adherence and very low attrition. Findings also show positive behavioral effects of meditation that were not reflected on volume, and not significantly on perfusion, of target brain areas.     Worry and ruminative brooding: associations with cognitive and physical health in older adults

    IntroductionMental health conditions are associated with cognition and physical function in older adults. We examined whether worry and ruminative brooding, key symptoms of certain mental health conditions, are related to subjective and/or objective measures of cognitive and physical (cardiovascular) health.MethodsWe used baseline data from 282 participants from the SCD-Well and Age-Well trials (178 female; agemean = 71.1 years). We measured worry and ruminative brooding using the Penn State Worry Questionnaire and the Ruminative Response Scale-brooding subscale. We assessed subjective physical health using the WHOQOL-Bref physical subscale, and objective physical health via blood pressure and modified versions of the Framingham Risk Score and Charlson Comorbidity Index. With subjective and objective cognition, we utilized the Cognitive Difficulties Scale and a global composite (modified Preclinical Alzheimer’s Cognitive Composite, PACC5, with the Wechsler Adult Intelligence Scale-IV, category fluency, Mattis Dementia Rating Scale-2, and either the California Verbal Learning Test or the Rey Auditory Verbal Learning Test). We conducted linear regressions, adjusted for education, age, sex and cohort.ResultsWorry and ruminative brooding were negatively associated with subjective physical health (worry: β = −0.245, 95%CI −0.357 to −0.133, p < 0.001; ruminative brooding: β = −0.224, 95%CI −0.334 to −0.113, p < 0.001) and subjective cognitive difficulties (worry: β = 0.196, 95%CI 0.091 to 0.302, p < 0.001; ruminative brooding: β = 0.239, 95%CI 0.133 to 0.346, p < 0.001). We did not observe associations between worry or ruminative brooding and any measure of objective health.DiscussionWorry and ruminative brooding may be common mechanisms associated with subjective but not objective health. Alternatively, cognitively unimpaired older adults may become aware of subtle changes not captured by objective measures used in this study. Interventions reducing worry and ruminative brooding may promote subjective physical and cognitive health; however, more research is needed to determine causality of the relationships

    The impact of meditation on healthy ageing — the current state of knowledge and a roadmap to future directions

    International audienceThere is increasing evidence that meditation-based training promotes healthy ageing across many dimensions. This review summarizes the existing knowledge on the effects of meditation training on healthy ageing in the domains of emotions, cognition (with a special emphasis on attentional processes), and the preservation of related brain structures. Although evidence so far is promising, more rigorous randomized controlled studies with active control groups and long-term follow-up in older people are needed. We outline how these challenges can be addressed in future studies using the example of an ongoing project, Medit-Ageing (public name: Silver Santé Study), including two independent randomized controlled trials (RCT) as well as one cross-sectional study with meditation experts. With increases in life expectancy worldwide [1], promoting healthy ageing becomes increasingly important. Healthy ageing is not only crucial for maintaining the quality of life in older individuals, but also to enable elderly people to thrive in their role in society, be it in their profession or in their private lives (e.g. as grandparents or friends). In recent years, it has been recognized that meditation-based training offers a promising strategy to promote healthy ageing [2 ]. Indeed, the effects of meditation training are increasingly studied in young and middle-age adults. However, there is still relatively little research on the impact of meditation training on older adults. This review summarizes evidence on how meditation can contribute to healthy ageing and optimize cognitive and emotional processes impacted by ageing. It ends by outlining future research avenues. In order to investigate the effects of meditation training on cognition and emotion, usually two types of studies are employed: (i) cross-sectional studies comparing experienced meditators with non-meditators and (ii) longitudinal studies comparing meditation training to a control group. The most prevalent form of training employed in longitudinal studies is mindfulness meditation. Mindful-ness meditation consists of cultivating a vigilant awareness of one's own thoughts, actions, emotions, and motivations. In mindfulness meditation, one learns to intentionally attend to internal or external experiences in the present moment, without making any value judgment [3]. Meditation training may delay dementia and promote healthy ageing Dementia currently affects 50 million people worldwide [4], and is broadly defined by brain atrophy, significant decline in cognitive and/or behavioral functioning, and loss of ability to live independently [5]. In so far as meditation practice has been shown to improve cognition, wellbeing, and health in older age, it could potentially contribute to delay onset of dementia. Preliminary evidence suggests that meditation training preserves brain structure, glucose metabolism, and brain connectivity in older adults Cross-sectional evidence from several studies in expert meditators suggests that meditation may preserve brain structure and glucose metabolism. First studies in this domain showed a preservation of cortical thickness and grey matter volumes in young and middle-age meditation experts [6], as well as reduced age-related atrophy of brain grey matter volume in meditators compared to non-med-itators, particularly in hippocampus, frontal and temporal brain regions [7,8].     Whole blood serotonin levels in healthy elderly are negatively associated with the functional activity of emotion-related brain regions

    International audienceUnderstanding the role of neuromodulators of socio-affective processing is important to ensure psychological wellbeing during older years. Here, we investigated the link between blood serotonin levels and brain and behavioral responses to emotional information in healthy elderly. A priori regions of interest (ROI) were selected due to their role in emotion processing and their dense serotonergic innervation. Correlation analyses were performed between ROI-specific responses to emotional stimuli and whole blood serotonin levels. We found significant negative associations between serotonin and functional activity for the bilateral insula, dorsal anterior cingulate cortex and subgenual gyrus. No association with behavioral measures survived correction for multiple testing. Our results mirror prior pharmacological and genetic work on the link between serotonin and emotional brain reactivity in younger adults. Given the involvement of serotonin in several age-related changes, our study encourages future research to characterize the role of this neuromodulator in emotion processing across the lifespan

    Longitudinal Changes in Hippocampal Network Connectivity in Alzheimer's Disease

    International audienceObjective The hippocampus is connected to 2 distinct cortical brain networks, the posterior–medial and the anterior–temporal networks, involving different medial temporal lobe (MTL) subregions. The aim of this study was to assess the functional alterations of these 2 networks, their changes over time, and links to cognition in Alzheimer's disease. Methods We assessed MTL connectivity in 53 amyloid‐β–positive patients with mild cognitive impairment and AD dementia and 68 healthy elderly controls, using resting‐state functional magnetic resonance imaging, cross‐sectionally and longitudinally. First, we compared the functional connectivity of the posterior–medial and anterior–temporal networks within the control group to highlight their specificities. Second, we compared the connectivity of these networks between groups, and between baseline and 18‐month follow‐up in patients. Third, we assessed the association in the connectivity changes between the 2 networks, and with cognitive performance. Results We found decreased connectivity in patients specifically between the hippocampus and the posterior–medial network, together with increased connectivity between several MTL subregions and the anterior–temporal network. Moreover, changes in the posterior–medial and anterior–temporal networks were interrelated such that decreased MTL–posterior–medial connectivity was associated with increased MTL–anterior–temporal connectivity. Finally, both MTL–posterior–medial decrease and MTL–anterior–temporal increase predicted cognitive decline. Interpretation Our findings demonstrate that longitudinal connectivity changes in the posterior–medial and anterior–temporal hippocampal networks are linked together and that they both contribute to cognitive decline in Alzheimer's disease. These results shed light on the critical role of the posterior–medial and anterior–temporal networks in Alzheimer's disease pathophysiology and clinical symptoms. ANN NEUROL 2021;90:391–40
