Autoimmune pancreatitis in patients with inflammatory bowel disease: A real-world multicentre collaborative ECCO CONFER study

The long-term outcomes of AIP and IBD in patients with coexisting AIP–IBD and predictors of complicated AIP course have rarely been reported. Methods: An ECCO COllaborative Network For Exceptionally Rare case reports project [ECCO-CONFER] collected cases of AIP diagnosed in patients with IBD. Complicated AIP was defined as a composite of endocrine and/or exocrine pancreatic insufficiency, and/or pancreatic cancer. We explored factors associated with complicated AIP in IBD. Results: We included 96 patients [53% males, 79% ulcerative colitis, 72% type 2 AIP, age at AIP diagnosis 35 ± 16 years]. The majority of
Crohn’s disease [CD] cases [78%] had colonic/ileocolonic involvement. In 59%, IBD preceded AIP diagnosis, whereas 18% were diagnosed simultaneously. Advanced therapy to control IBD was used in 61% and 17% underwent IBD-related surgery. In total, 82% of patients were treated with steroids for AIP, the majority of whom [91%] responded to a single course of treatment. During a mean follow-up of 7 years, AIP complications occurred in 25/96 [26%] individuals. In a multivariate model, older age at AIP diagnosis was associated with a complicated AIP course (odds ratio [OR] = 1.05, p = 0.008), whereas family history of IBD [OR = 0.1, p = 0.03], and CD diagnosis [OR = 0.2, p = 0.04] decreased
the risk of AIP complications. No IBD- or AIP-related deaths occurred. Conclusions: In this large international cohort of patients with concomitant AIP–IBD, most patients have type 2 AIP and colonic IBD. Ferromagnetism in the Kondo-lattice
We discuss the ferromagnetic phase of the planar Kondo lattice model using the bond fermion technique. For sufficiently small Kondo exchange we find a weakly ferromagnetic phase already for the weakly doped Kondo insulator. Choroba nowotworowa u pacjenta z nieswoistą chorobą zapalną jelit — zasady postępowania
Częstość występowania zarówno nieswoistych chorób zapalnych jelit (NChZJ), jak i nowotworów rośnie. Wyniki licznych badań wskazują także, że NChZJ oraz niektóre leki stosowane w ich terapii mogą sprzyjać rozwojowi powikłań onkologicznych. W niniejszym opracowaniu przedstawiono aktualny stan wiedzy, dotyczący czynników ryzyka zachorowania na nowotwory złośliwe w NChZJ, związane z przewlekłym zapaleniem w przewodzie pokarmowym, jak i z potencjalnym wpływem onkogennym wybranych leków immunosupresyjnych i biologicznych. Omówiono także zasady postępowania terapeutycznego w przypadku chorego z NChZJ, który chorował w przeszłości lub choruje aktualnie na nowotwór złośliwy. Dodatkowo zaprezentowano dane na temat wpływu leczenia onkologicznego na przebieg NChZJ, z uwzględnieniem roli nowych cząsteczek stosowanych w ramach immunoterapii schorzeń nowotworowych.

Diagnostyka nieswoistych chorób zapalnych jelit (NChZJ) jest złożonym procesem obejmującym zarówno badania laboratoryjne, obrazowe, jak i endoskopowe. W ostatnich latach nastąpił istotny postęp zwłaszcza w zakresie możliwości nieinwazyjnego diagnozowania i monitorowania aktywności NChZJ za pomocą wielu biomarkerów. Obok parametrów powszechnie znanych, takich jak białko C-reaktywne, istnieje także grupa tak zwanych kopromarkerów, których najważniejszy przedstawiciel to kalprotektyna. W niniejszym opracowaniu przedstawiono charakterystykę wybranych biomarkerów, które znajdują zastosowanie w codziennej praktyce klinicznej. Szczególną uwagę poświęcono najbardziej przydatnemu biomarkerowi — kalprotektynie, omawiając jednak także pewne ograniczenia tego badania laboratoryjnego

Optical conductivity due to orbital polarons in systems with orbital degeneracy
We consider the impact of orbital polarons in doped orbitally ordered systems
on optical conductivity using the simplest generic model capturing the
directional nature of either (or ) orbital states in certain
transition metal oxides, or orbital states of cold atoms in optical
lattices. The origin of the optical transitions is analyzed in detail and we
demonstrate that the optical spectra: (i) are determined by the string picture,
i.e., flipped orbitals along the hole hopping path, and (ii) consist of three
narrow peaks which stem from distinct excitations. They occur within the
Mott-Hubbard gap similar to the superconducting cuprates but indicate hole
confinement, in contrast to the spin t-J model. Finally, we point out how to

Tofacitinib - nowy lek w terapii nieswoistych chorób zapalnych jelit
Nieswoiste choroby zapalne jelit, do których zaliczamy wrzodziejące zapalenie jelita grubego oraz chorobę Leśniowskiego-Crohna, należą do nieuleczalnych, przewlekłych zapalnych chorób o nieznanej etiologii. Dotyczą one głównie młodych ludzi. Mogą prowadzić do nieodwracalnego uszkodzenia przewodu pokarmowego i niepełnosprawności. W ostatnich latach dokonał się istotny postęp w terapii nieswoistych chorób zapalnych jelit. Dostępne są owoczesne leki biologiczne o zróżnicowanych mechanizmach działania.
Najnowszym osiągnięciem jest pojawienie się tak zwanych leków małocząsteczkowych, których głównym przedstawicielem jest tofacitinib, będący inhibitorem kinaz typu JAK. Lek ten, znany już od dłuższego czasu w reumatologii, został w 2018 roku zarejestrowany do stosowania we wrzodziejącym zapaleniu jelita grubego. W niniejszym opracowaniu przedstawiono aktualny stan wiedzy na temat mechanizmów działania oraz zasad prowadzenia terapii tym lekiem z perspektywy gastroenterologicznej
Anti-TNF antibodies do not induce the apoptosis of lamina propria mononuclear cells in uninflamed intestinal tissue in patients with Crohn’s disease
It is not known if anti-tumor necrosis factor (anti-TNF) agents provoke only apoptosis of lamina propria mononuclear cells (LPMC) engaged in inflammatory processes or whether it’s a general phenomenon concerning all LPMC. In this study we carried out an immunohistochemical analysis of the expression of several apoptosis-related proteins (active caspase-3, Bax, Bcl-2, Fas, TNFR1, CD4, and CD8) in uninflamed mucosa in Crohn’s disease (CD) patients treated with anti-TNF agents. 16 CD patients (mean age 34 ± 11, mean disease duration 7 ± 5 years) were included in the study. 10 patients were treated with infliximab and 6 — with adalimumab. The expression of active caspase 3, Bax, Bcl-2, Fas, TNFR1 and CD8 in LPMC did not change significantly after the therapy. We concluded that anti-TNF antibodies did not promote LPMC apoptosis in uninflamed tissues. This is in contrast to the phenomena observed in inflamed tissues. These data show that anti-TNF antibodies rather restore the susceptibility to apoptosis of LPMC in inflamed areas of the gut in CD, than directly induce LPMC apoptosis; otherwise the anti-TNF antibodies should have also induced apoptosis in the uninflamed mucosa
How could nanobiotechnology improve treatment outcomes of anti-TNF-α therapy in infammatory bowel disease? Current knowledge, future directions
Despite significant advances in therapeutic possibilities for the treatment of inflammatory bowel disease (IBD) in recent years, there is still a big room for improvement. In particular, biological treatment can induce not only clinical remission but also mucosal healing of the gastrointestinal tract. Among these therapeutic molecules, anti-tumor necrosis factor-alpha (anti-TNF-) antibodies were the first to revolutionize treatment algorithms in IBD. However, due to the parenteral route of administration and systemic mode of action, TNF- blockers are characterised by high rates of immunogenicity-related loss of response and serious adverse events. Moreover, intravenous or subcutaneous therapy is not considered patient-friendly and requires occasional, direct contact with healthcare centres. To overcome these limitations, several attempts have been made to design oral pharmaceutical formulations of these molecules. It is hypothesized that oral anti-TNF- antibodies therapy can directly provide a targeted and potent anti-inflammatory effect in the inflamed gastrointestinal tissues without significant systemic exposure, improving long-term treatment outcomes and safety. In this review, we discuss the current knowledge and future perspectives regarding different approaches made towards entering a new era of oral anti-TNF- therapy, namely, the tailoring of biocompatible nanoparticles with anti-TNF- antibodies for site-specific targeting to IBD. The influence of anti-TNF therapy on CD31 and VEGF expression in colonic mucosa of Crohn’s disease patients in relation to mucosal healing
Introduction. Immune-mediated angiogenesis may play an important role in the pathogenesis of inflammatory lesions in Crohn’s disease (CD). The study aimed to assess the influence of anti-tumour necrosis factor (anti-TNF) therapy on the angiogenesis in relation to microscopic and endoscopic healing in CD patients.
Material and methods. Colonic tissue samples from 17 CD patients were taken during colonoscopy before and after anti-TNF therapy. Endoscopic and microscopic severities were estimated using validated scores. Immunohistochemical expression of CD31 and vascular endothelial growth factor (VEGF) were assessed in parallel.
Results. The expression of CD31 and VEGF decreased significantly after the anti-TNF therapy in parallel to endoscopic improvement; however, the microscopic activity did not change significantly. There was a correlation between the change in CD31 and VEGF expression (p = 0.01; r = 0.6), as well as endoscopic healing (p = 0.04; r = 0.4). CD31 immunoexpression correlated with the number of poly- and mononuclear cells in the infiltrates in the mucosal lamina propria before the therapy (p = 0.02; r = 0.5).
Conclusions. We suggest that modulation of vascular proliferation can be a novel option to increase the efficacy of biological therapy in CD.
