    Moving singing for lung health online in response to COVID-19: experience from a randomised controlled trial

    Introduction Singing for Lung Health (SLH) is a popular arts-in-health activity for people with long-term respiratory conditions. Participants report biopsychosocial benefits, however research on impact is limited. The ‘SHIELD trial’, a randomised controlled, single (assessor) blind, trial of 12 weeks SLH vs usual care for people with Chronic Obstructive Pulmonary Disease (COPD) (n=120) was set-up to help to address this. The first group (n=18, 9 singing and 9 controls) started face-to-face (5 sessions) before changing to online delivery (7 sessions) due to COVID-19 related physical distancing measures. As such, the experience of this group is here reported as a pilot study to inform further research in this area. Methods We conducted semi-structured interviews and thematic analysis regarding barriers, facilitators and key considerations for transitioning from face-to-face to online delivery. Pilot quantitative outcomes include attendance, pre and post measures of quality of life and disease impact (SF-36, CAT score), breathlessness (MRC breathlessness scale, Dyspnoea-12), depression (PHQ9), anxiety (GAD-7), balance confidence (ABC scale) and physical activity (clinical visit PROactive physical activity in COPD tool, combining subjective rating and actigraphy). Results Attendance was 69% overall, (90% of the face-to-face sessions, 53% online sessions). Analysis of semi-structured interviews identified three themes regarding participation in SLH delivered face-to-face and online, these where 1) perceived benefits; 2) digital barriers (online); 3) digital facilitators (online). Findings were summarised into key considerations for optimising transitioning singing groups from face-to-face to online delivery. Pilot quantitative data suggested possible improvements in depression (treatment effect -4.78 PHQ9 points, p< 0.05, MCID 5) and balance confidence (treatment effect +17.21 ABC Scale points, p=0.04, MCID 14.2). Discussion This study identifies key considerations regarding the adaptation of SLH from face-to-face to online delivery. Pilot data suggest online group singing for people with COPD may deliver benefits related to reducing depression and improved balance confidence

    Volumetric, relaxometric and diffusometric correlates of psychotic experiences in a non-clinical sample of young adults

    BackgroundGrey matter (GM) abnormalities are robust features of schizophrenia and of people at ultra high-risk for psychosis. However the extent to which neuroanatomical alterations are evident in non-clinical subjects with isolated psychotic experiences is less clear.MethodsIndividuals (mean age 20 years) with (n = 123) or without (n = 125) psychotic experiences (PEs) were identified from a population-based cohort. All underwent T1-weighted structural, diffusion and quantitative T1 relaxometry MRI, to characterise GM macrostructure, microstructure and myelination respectively. Differences in quantitative GM structure were assessed using voxel-based morphometry (VBM). Binary and ordinal models of PEs were tested. Correlations between socioeconomic and other risk factors for psychosis with cortical GM measures were also computed.ResultsGM volume in the left supra-marginal gyrus was reduced in individuals with PEs relative to those with no PEs. The greater the severity of PEs, the greater the reduction in T1 relaxation rate (R1) across left temporoparietal and right pre-frontal cortices. In these regions, R1 was positively correlated with maternal education and inversely correlated with general psychopathology.ConclusionsPEs in non-clinical subjects were associated with regional reductions in grey-matter volume reduction and T1 relaxation rate. The alterations in T1 relaxation rate were also linked to the level of general psychopathology. Follow up of these subjects should clarify whether these alterations predict the later development of an ultra high-risk state or a psychotic disorder

    Oakleaf: an S locus-linked mutation of Primula vulgaris that affects leaf and flower development

    •In Primula vulgaris outcrossing is promoted through reciprocal herkogamy with insect-mediated cross-pollination between pin and thrum form flowers. Development of heteromorphic flowers is coordinated by genes at the S locus. To underpin construction of a genetic map facilitating isolation of these S locus genes, we have characterised Oakleaf, a novel S locus-linked mutant phenotype. •We combine phenotypic observation of flower and leaf development, with classical genetic analysis and next-generation sequencing to address the molecular basis of Oakleaf. •Oakleaf is a dominant mutation that affects both leaf and flower development; plants produce distinctive lobed leaves, with occasional ectopic meristems on the veins. This phenotype is reminiscent of overexpression of Class I KNOX-homeodomain transcription factors. We describe the structure and expression of all eight P. vulgaris PvKNOX genes in both wild-type and Oakleaf plants, and present comparative transcriptome analysis of leaves and flowers from Oakleaf and wild-type plants. •Oakleaf provides a new phenotypic marker for genetic analysis of the Primula S locus. We show that none of the Class I PvKNOX genes are strongly upregulated in Oakleaf leaves and flowers, and identify cohorts of 507 upregulated and 314 downregulated genes in the Oakleaf mutant

    Nerve Agent Hydrolysis Activity Designed into a Human Drug Metabolism Enzyme

    Organophosphorus (OP) nerve agents are potent suicide inhibitors of the essential neurotransmitter-regulating enzyme acetylcholinesterase. Due to their acute toxicity, there is significant interest in developing effective countermeasures to OP poisoning. Here we impart nerve agent hydrolysis activity into the human drug metabolism enzyme carboxylesterase 1. Using crystal structures of the target enzyme in complex with nerve agent as a guide, a pair of histidine and glutamic acid residues were designed proximal to the enzyme's native catalytic triad. The resultant variant protein demonstrated significantly increased rates of reactivation following exposure to sarin, soman, and cyclosarin. Importantly, the addition of these residues did not alter the high affinity binding of nerve agents to this protein. Thus, using two amino acid substitutions, a novel enzyme was created that efficiently converted a group of hemisubstrates, compounds that can start but not complete a reaction cycle, into bona fide substrates. Such approaches may lead to novel countermeasures for nerve agent poisoning

The structures of the new compounds Cs7Sm11[TeO3]12Cl16 (I) and Rb7Nd11[TeO3]12Br16 (II) (both tetragonal, space group I4/mcm) correspond to the sequence of [MLn11(TeO3)12] and [M6×16] layers and bear very strong similarities to those of known selenite analogs.     High Broadband Light Transmission for Solar Fuels Production Using Dielectric Optical Waveguides in TiO₂ Nanocone Arrays

    We describe the fabrication and use of arrays of TiO₂ nanocones to yield high optical transmission into semiconductor photoelectrodes covered with high surface loadings of light-absorbing electrocatalysts. Covering over 50% of the surface of a light absorber with an array of high-refractive-index TiO₂ nanocones imparted antireflective behavior ( 85% transmission of broadband light to the underlying Si, even when thick metal contacts or opaque catalyst coatings were deposited on areas of the light-facing surface that were not directly beneath a nanocone. Three-dimensional full-field electromagnetic simulations for the 400 – 1100 nm spectral range showed that incident broadband illumination couples to multiple waveguide modes in the TiO₂ nanocones, reducing interactions of the light with the metal layer. A proof-of-concept experimental demonstration of light-driven water oxidation was performed using a p⁺n-Si photoanode decorated with an array of TiO₂ nanocones additionally having a Ni catalyst layer electrodeposited onto the areas of the p⁺n-Si surface left uncovered by the TiO₂ nanocones. This photoanode produced a light-limited photocurrent density of ~ 28 mA cm⁻² under 100 mW cm⁻² of simulated Air Mass 1.5 illumination, equivalent to the photocurrent density expected for a bare planar Si surface even though 54% of the front surface of the Si was covered by an ~ 70 nm thick Ni metal layer

Work at St Andrews was supported by the Leverhulme Trust (RPG-2013-343).We report the crystal structures and magnetic properties of two pseudopolymorphs of the S=1/2 Ti3+ coordination framework, KTi(C2O4)2⋅xH2O. Single-crystal x-ray and powder neutron diffraction measurements on α−KTi(C2O4)2⋅xH2O confirm its structure in the tetragonal I4/mcm space group with a square planar arrangement of Ti3+ ions. Magnetometry and specific heat measurements reveal weak antiferromagnetic interactions, with J1≈7 K and J2/J1=0.11 indicating a slight frustration of nearest- and next-nearest-neighbor interactions. Below 1.8 K, α−KTi(C2O4)2⋅xH2O undergoes a transition to G-type antiferromagnetic order with magnetic moments aligned along the c axis of the tetragonal structure. The estimated ordered moment of Ti3+ in α−KTi(C2O4)2⋅xH2O is suppressed from its spin-only value to 0.62(3) μB, thus verifying the two-dimensional nature of the magnetic interactions within the system. β−KTi(C2O4)2⋅2H2O, on the other hand, realizes a three-dimensional diamondlike magnetic network of Ti3+ moments within a hexagonal P6222 structure. An antiferromagnetic exchange coupling of J≈54 K—an order of magnitude larger than in α−KTi(C2O4)2⋅xH2O—is extracted from magnetometry and specific heat data. β−KTi(C2O4)2⋅2H2O undergoes Néel ordering at TN=28 K, with the magnetic moments aligned within the ab plane and a slightly reduced ordered moment of 0.79 μB per Ti3+. Through density-functional theory calculations, we address the origin of the large difference in the exchange parameters between the α and β pseudopolymorphs. Given their observed magnetic behaviors, we propose α−KTi(C2O4)2⋅xH2O and β−KTi(C2O4)2⋅2H2O as close to ideal model S =1/2 Heisenberg square and diamond lattice antiferromagnets, respectively.PostprintPeer reviewe

    An online breathing and wellbeing programme (ENO Breathe) for people with persistent symptoms following COVID-19: a parallel-group, single-blind, randomised controlled trial

    BACKGROUND: There are few evidence-based interventions for long COVID; however, holistic approaches supporting recovery are advocated. We assessed whether an online breathing and wellbeing programme improves health related quality-of-life (HRQoL) in people with persisting breathlessness following COVID-19. METHODS: We conducted a parallel-group, single-blind, randomised controlled trial in patients who had been referred from one of 51 UK-based collaborating long COVID clinics. Eligible participants were aged 18 years or older; were recovering from COVID-19 with ongoing breathlessness, with or without anxiety, at least 4 weeks after symptom onset; had internet access with an appropriate device; and were deemed clinically suitable for participation by one of the collaborating COVID-19 clinics. Following clinical assessment, potential participants were given a unique online portal code. Participants were randomly assigned (1:1) to either immediate participation in the English National Opera (ENO) Breathe programme or to usual care. Randomisation was done by the research team using computer-generated block randomisation lists, with block size 10. The researcher responsible for randomisation was masked to responses. Participants in the ENO Breathe group participated in a 6-week online breathing and wellbeing programme, developed for people with long COVID experiencing breathlessness, focusing on breathing retraining using singing techniques. Those in the deferred group received usual care until they exited the trial. The primary outcome, assessed in the intention-to-treat population, was change in HRQoL, assessed using the RAND 36-item short form survey instrument mental health composite (MHC) and physical health composite (PHC) scores. Secondary outcome measures were the chronic obstructive pulmonary disease assessment test score, visual analogue scales (VAS) for breathlessness, and scores on the dyspnoea-12, the generalised anxiety disorder 7-item scale, and the short form-6D. A thematic analysis exploring participant experience was also conducted using qualitative data from focus groups, survey responses, and email correspondence. This trial is registered with ClinicalTrials.gov, NCT04830033. FINDINGS: Between April 22 and May 25, 2021, 158 participants were recruited and randomly assigned. Of these, eight (5%) individuals were excluded and 150 participants were allocated to a treatment group (74 in the ENO Breathe group and 76 in the usual care group). Compared with usual care, ENO Breathe was associated with an improvement in MHC score (regression coefficient 2·42 [95% CI 0·03 to 4·80]; p=0·047), but not PHC score (0·60 [-1·33 to 2·52]; p=0·54). VAS for breathlessness (running) favoured ENO Breathe participation (-10·48 [-17·23 to -3·73]; p=0·0026). No other statistically significant between-group differences in secondary outcomes were observed. One minor self-limiting adverse event was reported by a participant in the ENO Breathe group who felt dizzy using a computer for extended periods. Thematic analysis of ENO Breathe participant experience identified three key themes: (1) improvements in symptoms; (2) feeling that the programme was complementary to standard care; and (3) the particular suitability of singing and music to address their needs. INTERPRETATION: Our findings suggest that an online breathing and wellbeing programme can improve the mental component of HRQoL and elements of breathlessness in people with persisting symptoms after COVID-19. Mind-body and music-based approaches, including practical, enjoyable, symptom-management techniques might have a role supporting recovery. FUNDING: Imperial College London
