The production of metal artefacts in Southern Etruria (Central Italy): case studies from copper to Iron Age

An analytical study is presented, aimed to determine the elemental composition of copper-based artefacts dated back from Copper Age to Early Iron Age (mid-fourth millennium to the VIIIth century B.C.), found on the Tyrrhenian side of the peninsula, corresponding to the Lazio region. The objects belong to different archaeological contexts and had various functions. They were analysed by the X-ray fluorescence technique. The results highlight the experimental character of Copper Age metallurgy, which will later evolve in the established use of copper-tin alloys. Regarding the Bronze Age, despite the typological and functional heterogeneity of the artefacts and the wide chronological range, the alloys are relatively homogeneous in composition, with regular changes that appear related to chronology, according to what is already known for the Italian peninsula. Such changes are supposedly due to variations in the availability of tin, which was not locally mined. Early Iron Age metallurgy is represented by the Selvicciola Hoard solely, which restricts the possibility of generalizing the conclusions. A striking feature of the alloys is the great compositional difference between the complete and the fragmented artefacts. The formers are made of tin bronze, whereas in the latter tin is replaced by antimony and/or lead. The use of such unusual alloys is unlikely due to lack of metallurgical knowledge. Considering the urbanized communities that arose in the Middle-Tyrrhenian area during the Early Iron Age, we suppose that such variability in a single context might be related to a production system capable of using alloys of different quality and value to satisfy a diversified demand

Pulmonary Artery Catheter Monitoring in Patients with Cardiogenic Shock: Time for a Reappraisal?

Cardiogenic shock represents one of the most dramatic scenarios to deal with in intensive cardiology care and is burdened by substantial short-term mortality. An integrated approach, including timely diagnosis and phenotyping, along with a well-established shock team and management protocol, may improve survival. The use of the Swan-Ganz catheter could play a pivotal role in various phases of cardiogenic shock management, encompassing diagnosis and haemodynamic characterisation to treatment selection, titration and weaning. Moreover, it is essential in the evaluation of patients who might be candidates for long-term heart-replacement strategies. This review provides a historical background on the use of the Swan-Ganz catheter in the intensive care unit and an analysis of the available evidence in terms of potential prognostic implications in this setting

Human impact at Lago di Mezzano (central Italy) during the Bronze Age: a multidisciplinary approach

The purpose of this contribution is to disentangle climate forcing and human influence in the catchment of Lago di Mezzano through the interfingering of data obtained by means of archaeological, geomorphological, sedimentological and palynological approaches. A systematic archaeological survey has been undertaken and three submerged settlement areas with piles, pottery and metal tools have so far been found. The artifacts indicate that the site was inhabited, although not continuously, during the Bronze Age. Geomorphological investigations and observations in some trenches dug out on the lake shore indicate that great variations in the lake's size and strong changes in the catchment/lake surface area ratio occurred during the second half of the Holocene. Palynological, micro-charcoal, sedimentological and geochemical analyses carried out on long cores taken from the lake centre have indicated environmental changes due to either climatic influence or human impact. Even if human presence in the area has been detected during the whole Bronze Age period, the human populations caused a strong impact only in two periods centred around 3600 and 3200 cal. years BP when they settled along the lake shore, and around 3400 cal. years BP when they caused an increase in erosion leading to the beginning of the deposition of turbidites. Le punte di lancia del lago di Mezzano (Valentano, VT). Uno sguardo d’insieme

Nel corso dell’estate del 2017, a seguito di sopralluoghi mirati a causa della forte siccità, la Soprintendenza Archeologia, Belle Arti e Paesaggio per l’Area Metropolitana di Roma, la Provincia di Viterbo e l’Etruria Meridionale ha segnalato alla Regione Lazio e al Comune di Valentano lo stato di grave crisi idrica del Lago di Mezzano. A seguito di tale segnalazione la Soprintendenza, la Regione Lazio attraverso la Direzione della Riserva Naturale Selva del Lamone (Farnese) ed il Museo della Preistoria della Tuscia e della Rocca Farnese di Valentano hanno effettuato sia nel mese di agosto che successivamente sopralluoghi, rilievi topografici e riprese fotografiche e filmate; inoltre sono state effettuate anche immersioni per verificare la situazione subacquea. Nel corso dei sopralluoghi subacquei sono stati recuperati alcuni materiali archeologici dei quali in questa sede si presenta soltanto una punta di lancia in bronzo, raccolta presso il palo 302 di M17; lo studio del reperto ha imposto un approfondimento sulla prima lancia rinvenuta nel lago nell’area M28 e da tempo edita (Pellegrini 1993, n. 9, p. 84, tav. 5

La spada dell’ipogeo R. Analisi archeometriche

Il n. 20 della collana Quaderni del Sistema museale del Lago di Bolsena è il terzo volume pubblicato in questa serie dal Museo della preistoria della Tuscia e della Rocca Farnese di Valentano. Il Naviglione di Farnese. Dall’Età del Rame all’Età etrusco-arcaica è frutto di un lavoro corale di ricerca che, sotto il coordinamento di Patrizia Petitti, Carlo Persiani e Fabio Rossi, ha coinvolto numerosi specialisti. Non deve stupire che il sito oggetto di questa pubblicazione sia posto al di fuori dei confini del Comune di Valentano. Questo per due motivi. Da una parte il Museo della preistoria, per sua stessa denominazione e missione, si pone come centro di riferimento per la preistoria di tutta la Tuscia. Inoltre, Farnese è uno dei comuni che risulta tra quelli fondatori del Sistema museale. L’incursione del Museo della preistoria di Valentano in territorio farnesano, pertanto, va letta non soltanto come legittima, ma come esempio di vera e propria buona pratica di natura sistemica. Ciascun museo del Sistema infatti, caratterizzato da una propria tematica e da una propria chiave di lettura legata alla disciplina della quale è espressione, oltre a svolgere la funzione di centro interpretativo in relazione alle emergenze patrimoniali di più immediata pertinenza, può e deve dare un proprio contributo alla lettura del territorio abbracciato dall’intera rete. Agli occhi di un non-addetto-ai-lavori, di un lettore cioè che può soltanto in parte comprendere ed apprezzare lo sforzo analitico e il rigore interpretativo riversati nella scrittura dei contributi che costituiscono il presente lavoro, il Quaderno 20 ha un suo specifico fascino. Fascino legato all’idea di scavo, e all’immaginario che questa pratica è in grado di attivare, di solleticare. Lo scavo inaugurale di cui fu protagonista Rittatore Vonwiller, a cui si deve l’individuazione della Necropoli del Naviglione sul finire degli anni Sessanta del Novecento. Quelli realizzati un ventennio dopo, con la ripresa dell’interesse nei confronti del sito. Quello che i curatori hanno promosso tra le carte ed i documenti prodotti in vari decenni di studi, di relazioni, di documenti prodotti dalle competenti autorità preposte alla conoscenza ed alla tutela del patrimonio archeologico. Rimane il rammarico, questo sì, per l’interruzione – dopo il 1997 – degli scavi, interruzione dovuta al taglio, operato a livello centrale, dei fondi necessari a questo tipo di impresa. Nonché per il danno apportato al sito dagli scavi clandestini. Ma tutto ciò non può far perdere di vista il grande merito che va attribuito a questa pubblicazione, quello di operare un primo importante passo verso la pubblica restituzione delle conoscenze circa questo importante capitolo della storia e dell’archeologia del paesaggio del Sistema museale del Lago di Bolsena

Prevalence and clinical characteristics of refractoriness to optimal proton pump inhibitor therapy in non-erosive reflux disease

Background: The real size of the gastro-oesophageal reflux disease (GERD) population not responding to proton pump inhibitor (PPI) therapy has still not been fully elucidated. Causes of PPI refractoriness include incorrect diagnosis and lack of adherence to therapy, in terms of incorrect dosage and timing. Aims: To evaluate the prevalence of refractoriness to optimal PPI therapy and the contribution of non-erosive reflux disease (NERD), reflux hypersensitivity, and functional heartburn, to PPI refractoriness. The association of functional GI symptoms in non-responders was evaluated. Methods: Frequency and severity of GERD symptoms (heartburn, regurgitation, chest pain), dysphagia, belching, epigastric pain, postprandial distress, irritable bowel syndrome (IBS), globus, and ear nose and throat (ENT) symptoms were evaluated in patients previously classified as non-responders. Patients with at least one of the oesophageal symptoms with a frequency 653 /week were treated with esomeprazole 40 mg once daily for 8 weeks and then re-evaluated. Non-responders (patients with oesophageal symptoms 653 times per week) underwent 24 hour multichannel intraluminal impedance-pH monitoring. Results: Of 573 consecutive patients, 92 with oesophageal symptoms and classified as PPI-refractory underwent the esomeprazole trial; 60 did not respond. IBS, epigastric pain, and post-prandial distress episodes were associated with a poor response on multivariate analysis. NERD, reflux hypersensitivity, and functional heartburn patients constituted 32%, 42%, and 26%, respectively of the PPI-refractory group. Conclusions: True refractoriness in patients with GERD symptoms attending a secondary care setting is lower than previously reported. Following a careful history and optimal PPI dosing, the rate of refractoriness was 20%. True NERD constitutes only a third of the PPI-refractory group

Detection and Clinical Significance of Circulating Tumor Cells in Patients Undergoing Radical Cystectomy for Urothelial Bladder Cancer

Introduction: Estimation of prognosis is patients undergoing radical cystectomy is often unreliable, as occult disease remains undetected by conventional diagnostic tools. The purpose of this study was to evaluate the feasibility and the clinical significance of a polymerase chain reaction assay to detect cytokeratin 7 (CK7) mRNA expression in peripheral blood cells of patients undergoing radical cystectomy for clinically nonmetastatic bladder cancer. Patients and Methods: From 2005 to 2009, 59 patients undergoing radical cystectomy and pelvic lymph node dissection were prospectively investigated. Peripheral blood was collected prior to surgery, and a nested polymerase chain reaction assay was developed to identify patients with circulating cells expressing CK7 mRNA. Preoperative, histopathologic data and clinical outcome were compared with CK7 findings. Results: CK7 expression was detected in 23 (38.9%) of 59 patients and correlated to T stage and lymph node status. After a median follow-up of 42 months, 29 patients experienced a recurrence, whereas 36 died. The presence of CK7-positive cells was significantly associated with an increased risk for recurrence and decreased survival as compared with patients who were CK7-negative (P < .001 and . P < .001, respectively; hazard ratios of 8.77 and 5.2 for recurrence and overall death, respectively). The detection of CK7-positive cells was an independent predictor of recurrence and death in a multivariable analysis. Conclusion: The detection of CK7 mRNA in the circulating cells of patients undergoing radical cystectomy for urothelial cancer identifies those with significantly increased risk of cancer recurrence and death

Appropriateness of antiplatelet therapy for primary and secondary cardio- and cerebrovascular prevention in acutely hospitalized older people

Aims: Antiplatelet therapy is recommended for the secondary prevention of cardio- and cerebrovascular disease, but for primary prevention it is advised only in patients at very high risk. With this background, this study aims to assess the appropriateness of antiplatelet therapy in acutely hospitalized older people according to their risk profile. Methods: Data were obtained from the REPOSI register held in Italian and Spanish internal medicine and geriatric wards in 2012 and 2014. Hospitalized patients aged ≥65 assessable at discharge were selected. Appropriateness of the antiplatelet therapy was evaluated according to their primary or secondary cardiovascular prevention profiles. Results: Of 2535 enrolled patients, 2199 were assessable at discharge. Overall 959 (43.6%, 95% CI 41.5–45.7) were prescribed an antiplatelet drug, aspirin being the most frequently chosen. Among patients prescribed for primary prevention, just over half were inappropriately prescribed (52.1%), being mainly overprescribed (155/209 patients, 74.2%). On the other hand, there was also a high rate of inappropriate underprescription in the context of secondary prevention (222/726 patients, 30.6%, 95% CI 27.3–34.0%). Conclusions: This study carried out in acutely hospitalized older people shows a high degree of inappropriate prescription among patients prescribed with antiplatelets for primary prevention, mainly due to overprescription. Further, a large proportion of patients who had had overt cardio- or cerebrovascular disease were underprescribed, in spite of the established benefits of antiplatelet drugs in the context of secondary prevention

Italian pediatric nutrition survey

Introduction the prevalence of malnutrition in children and its impact on clinical outcomes is underrecognized by clinicians in Italy as well as worldwide. A novel definition of pediatric malnutrition has been recently proposed by a working group of the Academy of Nutrition and Dietetics and American Society for Parenteral and Enteral Nutrition (A.S.P.E.N.), based on the correlation between illness and the use of zscores of anthropometric measurements. Aim to investigate the prevalence of malnutrition and related nutritional support among hospitalized children in Italy, in a nationwide survey performed in a single day (16/4/2015). Methods an open access website (http://nday.biomedia.net) was used to collected data from 73 hospitals and 101 wards in 14 Italian regions (1994 patients). Anonymous information was collected on hospitals' characteristics, patient's anthropometry, admission diagnosis, presence of chronic diseases and use of nutritional support: oral nutritional supplements (ONS), enteral nutrition (EN) or parenteral nutrition (PN). Z-scores of anthropometric measurements, calculated with Epi Info 7.1.5, defined nutritional status: wasting was identified by BMI or Weight-for-Length z-score (<â\u88\u921 mild, <â\u88\u922 moderate, <â\u88\u923 severe), stunting by Height-for-Age Z-score <â\u88\u922. WHO 2006 and CDC 2000 growth charts were used respectively for children younger and older than 2 years old. Results 1790 complete records were obtained for hospitalized patients aged 0â\u80\u9320 years, with median age 6.16 (0.1â\u80\u9320 years and 53.3% males). 52.9% were aged 0â\u80\u936 years and 58.8% of children suffered from chronic diseases. Wasting was detected in 28.7% of the total sample with higher occurrence observed in age ranges 0â\u80\u936 and 14â\u80\u9320 years, while 17.3% of patients showed stunting; surprisingly almost 27% of them were aged 0â\u80\u932. A ranking of the admission diagnosis with the highest rate of malnutrition was complied. The prevalence of wasting was significantly (p < 0.005) higher amongst children with chronic diseases (34.1% vs. 27.1%); stunting prevalence tripled in patients with chronic disease (24.5% vs. 8.3%). Only 23.5% of malnourished children (17%, 25.6% and 36.7%, respectively mild, moderate and severe malnutrition) received nutritional support: 11.7% received oral nutrition supplements (ONS, modular or complete), 11.5% enteral nutrition (EN, 6.4% via nasogastric tube, 5.1% via gastrostomy) and 6.8 % received parenteral nutrition (PN); in some patients a combination of two. Nutritional support is more commonly used among stunting patients, 39.5% of children under treatment. Conclusion Malnutrition of any grade was observed in nearly 1/3 and stunting in 17% of the reported hospitalized children, and it is likely to be underrecognized as the nutritional support reached only a small part of the malnourished children

Hospital Care of Older Patients With COPD: Adherence to International Guidelines for Use of Inhaled Bronchodilators and Corticosteroids

313noObjectives: We aimed to analyze the prevalence and impact of COPD in older patients hospitalized in internal medicine or geriatric wards, and to investigate adherence to the Global Initiative for Chronic Obstructive Lung Disease (GOLD) guidelines, associated clinical factors, and outcomes. Design: Data were obtained from REgistro POliterapie SIMI (REPOSI), a prospective multicenter observational registry that enrolls inpatients aged 65 years. Setting and Participants: Older hospitalized patients enrolled from 2008 to 2016 with a diagnosis of COPD. Measures: We evaluated adherence to the 2018 GOLD guidelines at admission and discharge, by examining the prescription of inhaled bronchodilators and corticosteroids in COPD patients. We also evaluated the occurrence of outcomes and its association with COPD and guideline adherence. Results: At hospital admission, COPD was diagnosed in 1302 (21.5%) of 6046 registered patients. COPD patients were older, with more impaired clinical and functional status and multiple comorbidities. Overall, 34.3% of COPD patients at admission and 35.6% at discharge were adherent to the GOLD guidelines. Polypharmacy (5 drugs) at admission [odds ratio (OR): 3.28, 95% confidence interval (CI): 2.24-4.81], a history of acute COPD exacerbation (OR: 2.65, 95% CI: 1.44-4.88) at admission, smoking habit (OR: 1.45, 95% CI: 1.08-1.94), and polypharmacy at discharge (OR: 6.76, 95% CI: 4.15-11.0) were associated with adherence to guidelines. COPD was independently associated with the risk of cardiovascular and respiratory death and rehospitalization occurrence compared to patients without COPD during follow-up. Adherence to guidelines was inversely associated with the occurrence of death from all causes (OR: 0.12, 95% CI: 0.02-0.90). Conclusions/Implications: COPD was common in older patients acutely hospitalized, showing an impaired functional and clinical status. Prescriptions for older COPD patients were often not adherent to GOLD guidelines. Poor adherence to guidelines was associated with a worse clinical status.
