    Management of Road Infrastructure Safety

    Road Infrastructure Safety Management (RISM) refers to a set of procedures that support a road authority in decision making related to the improvement of safety on a road network. Some of these procedures can be applied to existing infrastructure, thus enabling a reactive approach; and other procedures are used in early stages of a project's life-cycle allowing a proactive approach. The objective of this paper is to provide an overview of the most well-known procedures and present a series of recommendations for successful road infrastructure safety management. The work described in the paper was completed by the IRTAD sub-working group on Road Infrastructure Safety Management and presented in detail in the respective Report. The methodology followed on this purpose included the description of the most consolidated RISM procedures, the analysis of the use of RISM procedures worldwide and the identification of possible weaknesses and barriers to their implementation, the provision of good practice examples and the contribution to the scientific assessment of procedures. Specifically, the following RISM procedures were considered: Road Safety Impact Assessment (RIA), Efficiency Assessment Tools (EAT), Road Safety Audit (RSA), Network Operation (NO), Road Infrastructure Safety Performance Indicators (SPI), Network Safety Ranking (NSR), Road Assessment Programs (RAP), Road Safety Inspection (RSI), High Risk Sites (HRS) and In-depth Investigation. Each procedure was described along with tools and data needed for its implementation as well as relevant common practices worldwide. A synthesis summarizing the key information for each procedure was also drafted. Based on a survey on 23 IRTAD member countries from worldwide, the lack of resources or tools is the most commonly stated reason for not applying a RISM procedure. This has been frequently found mainly in European countries. Another common reason is the absence of recommendations/guidelines, especially for SPI, RAP, RSI and RSA. This highlights the importance of the presence of some legislation regulating the application of the procedures. Lack of data was found important mainly for SPI, HRS and EAT. Good practices of road infrastructure safety management have been explored in order to find solutions to the issues highlighted by the survey and provide examples about how these issues have been overcome in some countries. Specifically, issues related to data, legal framework, funding, knowledge, tools and dealing with more RISM procedures were addressed. Finally, nine key messages and six recommendations for better Road Infrastructure Safety Management were developed based on the conclusions made

    Acute abdomen as a consequence of an unusual suicide attempt: intra-abdominal injection of sulfuric acid

    Caustic ingestion is a common cause of life-threatening upper gastrointestinal tract injuries. It mostly happens in children as accidental exposure, but may occur in adults as a result of suicide attempt. We present a case of an acute abdomen that occurred after a peculiar way of self-administration of sulfuric acid as a suicide attempt in an adult psychiatric male patient, already known for self-harm with caustic agents in the previous years. In a few hours, the patient developed diffuse peritonitis, pneumoperitoneum, and a rapid hemodynamic deterioration, as a consequence of ileum and sigmoid necrosis, requiring an emergency surgery with the application of a damage control strategy. The patient was then transferred to intensive care unit for hemodynamic stabilization, and definitive surgical correction of the abdominal lesions was performed after 3 days with Hartmann procedure. Thirty-nine days after hospital admission, the patient was discharged. In conclusion, to our knowledge, never has been reported in the literature a case of intra-abdominal self-administration of caustic substance causing a rapid evolution of clinical conditions and requiring the application of damage control strategy

    MUNDS: a new approach to evaluating safety technologies

    Real-world evaluations of the safety benefits of new integrated safety technologies are hampered by the lack of sufficient data to assess early reliable benefits. To address this, a new approach was developed using a case-control, meta-analysis of coordinated national police data from Australia, Finland, Italy, New Zealand, Sweden and the UK, in assessing the benefits of Electronic Stability Control (ESC). The results showed that singlevehicle injury crash reductions varied between 21% and 54%, dependent on the speed zone of the crash and the road condition (significantly more effective in wet/icy road conditions than dry roads). For injury crashes involving more than one vehicle, ESC was twice as effective preventing crashes in high speed than lower speed zones. The findings using this new approach were consistent with those published by various equivalent individual studies, bearing in mind their wider international scope in terms of driving conditions and vehicle fleets studied. It was concluded that this new approach using a “prospective” meta-analysis method has the potential to expedite the process of evaluating emerging vehicle safety technologies that would otherwise be subject to much greater delays before sufficient evidence could be collected

    Evaluation of the benefits of vehicle safety technology: the MUNDS study

    Real-world retrospective evaluation of the safety benefits of new integrated safety technologies is hampered by the lack of sufficient data to assess early reliable benefits. This MUNDS study set out to examine if a “prospective” case-control meta-analysis had the potential to provide more rapid and rigorous analyses of vehicle and infrastructure safety improvements. To examine the validity of the approach, an analysis of the effectiveness of ESC using a consistent analytic strategy across 6 European and Australasian databases was undertaken. It was hypothesised that the approach would be valid if the results of the MUNDS analysis were consistent with those published earlier (this would confirm the suitability of the MUNDS approach). The findings confirm the hypothesis and also found stronger and more robust findings across the range of crash-types, road conditions, vehicle sizes and speed zones than previous. The study recommends that while a number of limitations were identified with the findings that need be addressed in future research, the MUNDS approach nevertheless should be adopted widely for the benefit of all vehicle occupants

    Partial growth hormone insensitivity and dysregulatory immune disease associated with de novo germline activating STAT3 mutations

    Germinal heterozygous activating STAT3 mutations represent a novel monogenic defect associated with multi-organ autoimmune disease and, in some cases, severe growth retardation. By using whole-exome sequencing, we identified two novel STAT3 mutations, p.E616del and p.C426R, in two unrelated pediatric patients with IGF-I deficiency and immune dysregulation. The functional analyses showed that both variants were gain-of-function (GOF), although they were not constitutively phosphorylated. They presented differences in their dephosphorylation kinetics and transcriptional activities under interleukin-6 stimulation. Both variants increased their transcriptional activities in response to growth hormone (GH) treatment. Nonetheless, STAT5b transcriptional activity was diminished in the presence of STAT3 GOF variants, suggesting a disruptive role of STAT3 GOF variants in the GH signaling pathway. This study highlights the broad clinical spectrum of patients presenting activating STAT3 mutations and explores the underlying molecular pathway responsible for this condition, suggesting that different mutations may drive increased activity by slightly different mechanisms.Fil: Gutiérrez, Mariana Lilián.     Effectiveness of low speed autonomous emergency braking in real-world rear-end crashes

    This study set out to evaluate the effectiveness of low speed autonomous emergency braking (AEB) technology in current model passenger vehicles, based on real-world crash experience. The Validating Vehicle Safety through Meta-Analysis (VVSMA) group comprising a collaboration of government, industry consumer organisations and researchers, pooled data from a number of countries using a standard analysis format and the established MUND approach. Induced exposure methods were adopted to control for any extraneous effects. The findings showed a 38 percent overall reduction in rear-end crashes for vehicles fitted with AEB compared to a comparison sample of similar vehicles. There was no statistical evidence of any difference in effect between urban (≤60km/h) and rural (>60km/h) speed zones. Areas requiring further research were identified and widespread fitment through the vehicle fleet is recommended

    Congenital myopathies: Clinical phenotypes and new diagnostic tools

    Congenital myopathies are a group of genetic muscle disorders characterized clinically by hypotonia and weakness, usually from birth, and a static or slowly progressive clinical course. Historically, congenital myopathies have been classified on the basis of major morphological features seen on muscle biopsy. However, different genes have now been identified as associated with the various phenotypic and histological expressions of these disorders, and in recent years, because of their unexpectedly wide genetic and clinical heterogeneity, next-generation sequencing has increasingly been used for their diagnosis. We reviewed clinical and genetic forms of congenital myopathy and defined possible strategies to improve cost-effectiveness in histological and imaging diagnosis

    Biosecurity and Yield Improvement Technologies Are Strategic Complements in the Fight against Food Insecurity

    The delivery of food security via continued crop yield improvement alone is not an effective food security strategy, and must be supported by pre- and post-border biosecurity policies to guard against perverse outcomes. In the wake of the green revolution, yield gains have been in steady decline, while post-harvest crop losses have increased as a result of insufficiently resourced and uncoordinated efforts to control spoilage throughout global transport and storage networks. This paper focuses on the role that biosecurity is set to play in future food security by preventing both pre- and post-harvest losses, thereby protecting crop yield. We model biosecurity as a food security technology that may complement conventional yield improvement policies if the gains in global farm profits are sufficient to offset the costs of implementation and maintenance. Using phytosanitary measures that slow global spread of the Ug99 strain of wheat stem rust as an example of pre-border biosecurity risk mitigation and combining it with post-border surveillance and invasive alien species control efforts, we estimate global farm profitability may be improved by over US$4.5 billion per annum

    Development of improved pink tomato (Solanum lycopersicum L.) lines for the creation of remarkable hybrids supported by gene expression analysis

    The "pink" color fruit trait in tomato is associated with a recessive monogenic y locus on chromosome 1. The pink skin phenotype is due to the lack of naringenin chalcone (NarCh), the predominant yellow pigment that accumulates during ripening. SlMYB12 is the transcription factor involved in the regulation of the NarCh biosynthesis, and its suppression causes the pink phenotype. This project aims to improve the ISI Sementi pink germplasm by generating new tomato fixed lines. During 2020, different heterozygous F1 generations with red phenotype were obtained by crossing a pink-fruited line (ISI 1) with three different red-fruited lines (ISI 2, ISI 3, ISI 4). These hybrids were then used to make a first backcross with ISI 1 and other 12 parental lines with similar characteristics. More than 20 BC1F1 lines were obtained. Expression analysis of the MYB12 gene was carried out by quantitative Real-Time PCR on 3 BC1F1lines, also considering a hybrid and a control red-fruited line. In the pink-fruited lines, MYB12 expression was almost completely abolished. Since various sequence modifications localized in MYB12 of different commercial pink lines have been reported in literature, a 591 bp portion of MYB12 cDNA was amplified and sequenced. The results showed no differences in the sequence between all the germplasm tested and the accessions of the MYB12 genes deposited in the database; therefore, the reduction in gene expression in the lines generated by ISI Sementi could be attributable to modifications present in other points of the gene. Currently the project is still in progress: we are developing BC2F1 generations and will continue with the stabilization of future parental lines useful for the development of new commercial hybrid
