Comparison between Central and Brachial Blood Pressure in Hypertensive Elderly Women and Men
Aim. To compare the values of central and brachial systemic blood pressure (SBP) between women and men over 60 years of age with systemic arterial hypertension. Methods. This study was a quantitative, descriptive, cross-sectional study with elderly patients admitted to and selected from spontaneous and scheduled demand at basic health units in Uberlândia, Minas Gerais, Brazil, between March 2013 and March 2014. We included 69 study participants and compared central and brachial SBP using a Sphygmocor® XCEL device (AtCor Medical, Sydney, Australia). Results. Significant differences were found in the blood pressure values of the whole population in the central versus brachial systolic blood pressure (SP) [140(21) versus 153(23) mmHg] and in the central versus brachial pulse pressure (PP) [55(18) versus 70(18) mmHg]. Additionally, females exhibited higher blood pressure levels than males [central SP 144(23) versus 134(16) mmHg and brachial SP 161(26) versus 148(18) mmHg and central PP 62(17) versus 45(14) mmHg and brachial PP 80(21) versus 63(15) mmHg, resp.]. Internações por condições sensÃveis à atenção primária: um estudo ecológico
OBJETIVO: Avaliar a tendência das internações por condições sensÃveis à atenção primária e sua relação com a cobertura da Estratégia Saúde da FamÃlia. MÉTODOS: Estudo ecológico de séries temporais utilizando registros do Sistema de Informação Hospitalar referentes ao perÃodo de 2005 a 2015, com dados para o estado de Goiás. As análises de tendência foram realizadas por regressão linear generalizada pelo método de Prais-Winsten com variância robusta, que permitiu verificar se a tendência de internações por condições sensÃveis à atenção primária foi estacionária (p > 0,05), declinante (p < 0,05 e coeficiente de regressão negativo) ou ascendente (p < 0,05 e coeficiente de regressão positivo) em cada região de Goiás e por grupo diagnóstico estratificado por sexo. A correlação de Pearson foi utilizada para verificar o grau de associação entre cobertura da Estratégia Saúde da FamÃlia e a taxa de internações por condições sensÃveis à atenção primária. RESULTADOS: As internações por condições sensÃveis à atenção primária foram responsáveis por 1.092.070 (30,0%) internações em Goiás. A taxa média de internações por condições sensÃveis à atenção primária foi estaticamente menor que a taxa por outras condições no perÃodo analisado (167,6% versus 386,2%; t = -13,18; p < 0,001). Verificou-se uma tendência decrescente das taxas de internações por condições sensÃveis à atenção primária em Goiás e na maioria das regiões de saúde. As tendências variaram entre os sexos nos grupos de causas. Observou-se correlação negativa entre a cobertura da Estratégia Saúde da FamÃlia e a taxa de internações por condições sensÃveis à atenção primária no estado e também na maioria das regiões de saúde. CONCLUSÕES: As internações por condições sensÃveis à atenção primária tiveram tendência de redução significativa ao longo do perÃodo analisado. Apesar dessa diminuição progressiva, essa taxa permanece elevada e a tendência de redução não foi linear para todas as causas. Estes resultados possibilitam direcionar polÃticas públicas, ao desenhar um panorama geral das internações por condições sensÃveis à atenção primária por sexo e região no estado.OBJECTIVE: To evaluate the trend of hospitalizations due to primary care sensitive conditions and its relationship with the Family Health Strategy coverage. METHODS: Ecological study of time series using the records from the Hospital Information System, from 2005 to 2015, with data for the state of Goiás, Brazil. Trend analyses were performed by the generalized linear regression method of Prais-Winsten with robust variance, which allowed to verify if the trend of hospitalizations due to primary care sensitive conditions was stationary (p > 0.05), declining (p < 0.05 and negative regression coefficient), or ascending (p < 0.05 and positive regression coefficient) in each region of Goiás and for each diagnosis group, stratified by sex. Pearson correlation was used to verify the degree of association between the Family Health Strategy coverage and the rate of hospitalizations due to primary care sensitive conditions. RESULTS: Hospitalizations due to primary care sensitive conditions accounted for 1,092,070 (30.0%) of hospitalizations in Goiás. The average hospitalizations rate due to primary care sensitive conditions was statically less than the rate for other conditions in the analyzed period (167.6% against 386.2%; t = -13.18; p < 0.001). There has been a downward trend in hospitalizations trend due to primary care sensitive conditions in Goiás and in most health regions. The trends varied between sexes in the groups of causes. We observed a negative correlation between the Family Health Strategy coverage and the hospitalizations trend due to primary care sensitive conditions in the state and also in most health regions. CONCLUSIONS: Hospitalizations due to primary care sensitive conditions had a significant reduction trend over the analyzed period. Despite this progressive decrease, this rate remains high and the reduction trend was not linear for all causes. These results allow for directing public policies, while drawing a general overview of hospitalizations due to primary care sensitive conditions by sex and region in the state
Serious acute respiratory syndrome: a case series in a municipality region of central Brazil
The influenza B virus is more stable than influenza A, with less antigenic drift and consequent immunologic stability, and does not undergo the process of antigenic shift, its participation in epidemics is minimal, being of lesser academic interest. The aim of this work was to describe the occurrence of a series of SARS cases in a municipality in the Central region of Brazil. This is a case series study with a descriptive and quantitative approach of Serious Acute Respiratory Syndrome (SARS) in institutionalized individuals and a health professional from a long-term institution in the municipality where the study was conducted. The variables studied were: age, comorbidities, vaccination status, date of symptoms onset, symptoms, occurrence of death, information regarding provided care (hospital care, exams, and medications). The study was approved by the Research Ethics Committee of the Federal University of Goiás (UFG), under opinion number 2.167.287. Case 1 was diagnosed with influenza B, treated with antibiotic therapy, with no antiviral drugs administered, and culminated in death. Cases 2 and 3 were confirmed as influenza B, being treated with antiviral drugs and discharged due to full recovery. Case 4 was confirmed as influenza B virus by epidemiological link, treated with antiviral drugs and discharged due to full recovery. An early diagnosis, adequate clinical management, transmissible disease research based on the 11 health promotion steps and actions can promote the reduction of morbimortality by influenza type B
Nas mãos de quem está a gestão do SUS? Percepções dos gestores municipais sobre gerenciamento
This study had as goal to analyze the perceptions of the municipal health managers of a regional interagency committee of the State of Goiás, on the instruments of management of the Unified Health System (UHS). This is a descriptive study, with qualitative approach, developed in a health region and the sample was composed by nine managers. The study shows the female predominance in UHS management. Regarding the level of education, one note that 78% of the managers have higher education. The data showed that 100% of the managers said assume the position of the health municipal system management by means of political statement. Thus, the municipal management of the Unified Health System is often carried out by individuals, although most of them has higher education in health, with weak and inefficient knowledge regarding the design of UHS and basic management tools.Este estudo teve como objetivo analisar as percepções dos gestores municipais de saúde de uma Comissão Intergestora Regional do Estado de Goiás, sobre os instrumentos de gestão do Sistema Único de Saúde (SUS). Trata-se de uma pesquisa descritiva, com abordagem qualitativa, desenvolvida em uma região de saúde, a amostra foi constituÃda por nove gestores. Os resultados apontam predominância feminina na gestão do SUS. Quanto ao grau de escolaridade nota-se que 78% dos gestores possuem curso superior. A admissão ao cargo de gestor do sistema municipal de saúde ocorreu por meio de indicação polÃtica para 100% dos entrevistados. Os dados apresentados mostram que a gestão municipal do SUS é realizada, por indivÃduos que embora a maioria tenhaformação superior na área da saúde , o conhecimento ainda é incipiente e ineficiente, no que se refere à concepção do SUS e instrumentos básicos de gestão.Descritores: Sistema Único de Saúde; Planejamento em Saúde; Gestão em Saúde
Acidente de trabalho com material biológico na enfermagem / Work accident with biological material in nursing
Objetivo: apresentar análise epidemiológica dos acidentes com material biológico ocorrido entre profissionais de enfermagem de 2015 a 2017, notificados ao Sistema de Informação de Agravos de Notificação.  Método: pesquisa descritiva retrospectiva, quantitativa, com levantamento no banco de dados do sistema do SINAN NET do Centro de Referência da Superintendência Regional de Saúde. Resultados: a faixa etária mais acometida 30 a 49 anos, a categoria com maior número de casos, técnicos de enfermagem, a administração de medicação por via endovenosa e o descarte inadequado foram circunstâncias do acidente mais evidente. Tipo de exposição: percutânea, agente causador: agulha com lúmen e o Equipamento de Proteção Individual menos usado foi o óculos; ocorrendo a emissão da Categoria de Acidente de Trabalho. Conclusões: o uso incorreto de Equipamentos de Proteção Individual evidencia a importância da promoção de ações educativas sobre riscos biológicos, embora os resultados do uso fossem satisfatórios.Â
Multiresistant microorganism infection in newborns at an intermediate neonatal care unit and intensive care unit of reference: cross-sectional study
The objective was to describe the infection profile of multidrug-resistant organisms of newborns (NB), admitted to the neonatal intermediate care unit and the intensive care unit. It was a descriptive cross-sectional study of epidemiological nature, composed of 931 medical records and/or Hospital Infection Control Committee notification records, during the period of June to September 2012. Eight hundred and seventy newborns (NB) were admitted to the neonatal intensive care units and to intermediate care, with a final sample of 45 NB presenting bloodstream infections (BSI) caused by multiresistant microorganisms. From the analyses, the following results were highlighted: gestational age < 37 weeks in 42 (93.3%); low weight at birth between < 750g to 1.499g. The profile for resistance to enterobacteriaceae was 100% for cefepime and ceftazidime concerning non-fermenting gram-negative bacteria, it was found that (100%) of P.aeruginosa was resistant to aztreonam, (100%) of S. maltophilia resistant to ceftazidime and to gentamycin, however, (1.1%) of S. aureus isolates were resistant to oxacillin and (12.5%) of S. haemolyticus demonstrated a confirmed resistance to vancomycin. These results are worrying and express the importance of correct monitoring, and the need for producing a propaedeutic protocol in consensus with the multiprofessional team through the Hospital Infection Control Committee and managers, which is then implemented and regularly assessed by the service
Cuidadores informais de portadores de doenças crônicas: investigação convergente assistencial / Informal caregivers of patients with chronic diseases: convergent assistential research
Objetivo: Avaliar os anseios e expectativas de cuidadores informais de indivÃduos adultos com doenças crônicas de um hospital em relação à implantação de um projeto de educação em saúde no contexto da pesquisa convergente assistencial. Metodologia: Trata-se de pesquisa qualitativa, do tipo pesquisa convergente-assistencial (PCA), realizada no perÃodo de agosto a setembro de 2017, com cuidadores informais de indivÃduos adultos, portadores de doenças crônica, internados em uma clÃnica medica de um hospital universitário, de uma capital da região Centro-Oeste do Brasil. Dados colhidos por observação não sistemática participante, entrevistas semiestruturadas e grupo de convergência, e submetidos à análise de conteúdo. Resultados: emergiram as categorias: Vivências de satisfação com o grupo de apoio ao cuidador: saindo da invisibilidade; Vivências de satisfação com o grupo de apoio ao cuidador: muito além de palestras, e espiritualidade como forma de cuidar: ação de enfermagem por meio do método convergente-assistência. Conclusão: demonstrou que os cuidadores requerem um cuidado particular e qualificado por parte da equipe de enfermagem. Além de, evidenciar que as atividades voltadas para os cuidadores devem levar em consideração suas necessidades em pontuar seus problemas e de serem ouvidos. O estudo permitiu ao pesquisador se envolver com atividades de pesquisa e assistência, adotando postura compatÃvel com o modelo psicossocial de educação em saúde, diferentemente do modelo biomédico e tecnicista ainda prevalente em nossa sociedade. Comparison between the values of central and brachial function in elderly women and men with arterial hypertension systemic
Background: Systemic arterial pressure (SBP) changes with aging. Central SBP has
a stronger correlation with cardiovascular risk factors than brachial SBP. In the
elderly, progressive arterial stiffness and the early reflection wave develop to the
amplification of the pulse pressure (PP), as a consequence of the elevation of the
systolic blood pressure (SP). Mortality due to cardiovascular diseases in
postmenopausal women has a prevalence equal to or higher than men due to low
levels of estrogen plasma.
Objective: To compare the values of central and brachial systemic arterial pressure
among women and men over 60 years of age with systemic arterial hypertension.
Methods: A quantitative, descriptive, cross-sectional study with elderly patients

in the central SP vs the brachial SP 140 (21) vs 153 (23), mmHg and in the central
PP vs brachial 55 (18) vs 70 (18), mmHg. Furthermore, females presented higher
blood pressure levels than males, central SP 144 (23) vs 134 (16), mmHg and
brachial 161 (26) vs 148 (18), mmHg, central PP 62 (17) vs 45 (14), mmHg and
brachial 80 (21) vs 63 (15), mmHg, and this was significant.
Conclusion: There were therefore significant differences in SP and PP pressure,
both at the central and brachial levels in hypertensive elderly, observed in the total
study population and between women and men.Tese (Doutorado)Fundamentos: A pressão arterial sistêmica (PAS) se modifica com o
envelhecimento. A PAS central apresenta correlação mais forte com os fatores de
riscos cardiovasculares do que a PAS braquial. Nos idosos, o enrijecimento arterial
progressivo e a onda de reflexão precoce, desenvolvem à amplificação da pressão
de pulso (PP), em consequência da elevação da pressão arterial sistólica (PS). A
mortalidade por doenças cardiovasculares em mulheres na pós-menopausa,
apresenta prevalência igual ou superior aos homens, devido aos baixos nÃveis de
estrogênio plasmático.
Objetivo: Comparar os valores da pressão arterial sistêmica central e braquial entre
mulheres e homens acima de 60 anos de idade portadores (as) de hipertensão
arterial sistêmica.
Métodos: Estudo quantitativo, descritivo, transversal, com pacientes idosos
admitidos e selecionados a partir da demanda espontânea e programada nas
unidades básicas de saúde de Uberlândia-MG, entre março de 2013 a março de
2014. Foram incluÃdos 69 participantes da pesquisa e comparamos a PAS central e
braquial através do equipamento Sphygmocor® XCEL (AtCor Medical, Sydney,
Austrália).
Resultados: Houveram diferenças significantes nos valores pressóricos de toda a
população tanto na PS central vs braquial 140(21) vs 153(23), mmHg quanto na PP
central vs braquial 55(18) vs 70(18), mmHg. Além disso, o sexo feminino apresentou
nÃveis pressóricos maiores do que no masculino, PS central 144(23) vs 134(16),
mmHg e braquial 161(26) vs 148(18), mmHg, PP central 62(17) vs 45(14), mmHg e
braquial 80(21) vs 63(15), mmHg, e isso foi significante.
Conclusão: Portanto, houve diferenças significantes nos valores pressóricos da PS
e PP, tanto a nÃvel central quanto braquial em idosos hipertensos, observados na
população total do estudo e entre mulheres e homens
Comparison between the value of blood pressure central and brachial in patients with systemic arterial hypertension
Introduction: Systolic blood pressure (SP) and pulse pressure (PP) rise gradually
during the aging process as a consequence of a reduction in arterial elasticity. The
measure of systemic arterial pressure (SAP) taken at the root of the aorta has been
considered an independent determinant of cardiovascular mortality superior to the
values of brachial SAP. Aim: To compare the values of SAP central to those of
braquial SAP in patients of different age brackets who have systemic hypertension.
Method: We evaluated the central SAP at the root of the aorta and the brachial SAP
in the left arm using the ocillometric method 244 hypertensive patients who had been
submitted to cineangiocoronarography. Five groups of patients were constituted:
Group I, 39-49, n=36, Group II, 50-59, n=67, Group III, 60-69, n=69, Group IV, 70-79,
n=46, Group V ≥ 80 y.o, n=26. Results: When central SP was compared to brachial
SP, it was possible to find significance in patients who were 50 y.o and upwards. It
was not possible to find a statistical difference between central diastolic pressure and
brachial except in patients between the ages of 60-69. When comparing central to
brachial PP, we observed that central PP was significantly greater (between 11 and
15 mmHg) in all patient above the age of 50. Conclusion: In older people, the values
of SP and PP, taken directly at the root of the aorta, are superior to those obtained by
indirect means from the brachial artery. These differences are significant from the

cardiovascular superior aos valores PAS braquial. Objetivo: Comparar os valores da
PAS central e PAS braquial em portadores de hipertensão arterial nas diversas
faixas etárias. Método: Avaliamos a PAS central na raiz da aorta e a PAS braquial no
braço esquerdo pelo método oscilométrico em 244 pacientes com hipertensão
submetidos à cineangiocoronariografia. Foram constituÃdos cinco grupos de
pacientes: Grupo I, 39-49 anos, n= 36, Grupo II, 50-59 anos, n= 67, Grupo III, 60-69
anos, n= 69, Grupo IV, 70-79 anos, n= 46 e o Grupo V ≥ 80 anos, n= 26. Resultados:
Ao comparar a PS central versus PS braquial foi possÃvel encontrar significância a
partir dos 50 anos de vida. Não encontramos diferença estatÃstica entre a pressão
diastólica central versus diastólica braquial, exceto nos pacientes com idade entre
60-69 anos. Na comparação entre a PP central e PP braquial observamos que a PP
central foi significativamente maior (entre 11 a 15 mmHg) em todos os pacientes com
idade superior a 50 anos. Conclusão: Com o envelhecimento, os valores das PS e
de PP, aferidas diretamente na raiz da aorta, são superiores àqueles obtidos por
método indireto na artéria braquial. Essas diferenças são significantes a partir dos 50
anos de idade
