    Effects of polyphenol supplementation to in vitro antioxidant potential of citrus juices

    Food preparations containing substances with antioxidant activity have considerably aroused the interest of consumers, which has led to the marketing of a number of dietary supplements such as resveratrol, hesperidin or chlorogenic acid. On the other hand, natural antioxidants present in citrus fruit juices, together with dietary supplements, could contribute to an enhancement of their antioxidant properties and, in turn, alter their physiological impacts. The aim of the study was to evaluate the effect of supplementation of freshly squeezed citrus juices with resveratrol, hesperidin and chlorogenic acid on the in vitro antioxidant potential of the juices. The solutions of juices, selected polyphenols and the mixtures of solutions of juices and solution of polyphenols were analyzed to determine total polyphenol concentration using the Fast Blue method and total antioxidant potential by the ABTS method. As a result three different types of interactions were established: synergistic of chlorogenic acid which additionally increased the antioxidant potential by 35 - 38% over theoretical predictions, additive - after addition of resveratrol, and antagonistic - in the case of hesperidin. While the antioxidant potential was substantially higher in the mixture of mandarin juice with chlorogenic acid, the measured total polyphenol concentration decreased. Also a considerable decrease in the concentration of polyphenols was measured in the mixtures of mandarin and orange juices with resveratrol, all these possibly due to the sensitivity to other substances present in juices of the analytical method applied. A correlation between antioxidant potential of all examined solutions and total polyphenol concentration was found (r2 = 0.8879)

    WNT4 Expression in Primary and Secondary Kidney Diseases: Dependence on Staging

    Background/Aims: WNT4 protein is important for kidney development. Its expression was found to be altered in experimental models of chronic kidney disease (CKD). However, the expression of the WNT4 gene has yet not been studied in human renal biopsy samples from patients with broad spectrum of glomerular disease and at different stages of CKD. Thus, the aim of the study was to assess the WNT4 gene expression in renal biopsies of 98 patients using the real-time PCR technique. Materials: In order to assess the relative amounts of mRNA, in samples of patients with manifestation of different renal diseases and separately at different stages of CKD, by QPCR, total RNA was isolated from human kidney tissues collected during renal biopsies. Results of blood and urine samples assessment were used to calculate the correlations of biochemical parameters with WNT4 gene expression in both studied groups. Results: After pathomorphological evaluation, 49 patients were selected as presenting the most common cases in the studied group. Among the patients who developed focal segmental glomerulosclerosis (FSGS; n = 13), IgA nephropathy (IgAN; n = 10), IgAN with morphological presentation of focal segmental glomerulosclerosis (IgAN/FSGS; n = 8), membranous nephropathy (MN; n = 12), and lupus nephritis (LN; n = 6) were included in the analysis. We found that the level of WNT4 mRNA was higher in kidney specimens obtained from patients with MN as compared to those diagnosed with LN or IgAN. A correlation between WNT4 gene expression and serum albumin and cholesterol levels was observed in patients with FSGS, while WNT4 mRNA levels correlated with plasma sodium in patients diagnosed with LN. After consideration of 98 patients, based on the KDIGO classification of CKD, 20 patients were classified as CKD1 stage, 23 as stage 2, 13 as stage 3a, 11 as stage 3b, 13 as stage 4, and 18 as stage 5. WNT4 gene expression was lower in the CKD patients in stage 2 as compared to CKD 3a. Correlations of WNT4 mRNA level at different stages of CKD with indices of kidney function and lipid metabolism such as serum levels of HDL and LDL cholesterol, TG, urea, creatinine, sodium, and potassium were also found. Conclusions: Our results suggest that altered WNT4 gene expression in patients with different types of glomerular diseases and patients at different stages of CKD may play a role in kidney tissue disorganization as well as disease development and progression

    Comparison of the effects of TENS stimulation and water immersion on relieving labour pain suffered byprimiparas

    Objectives: The aim of this study was to compare pain suffered by primiparas when delivering a child in a traditional way with deliveries where either TENS stimulation or water immersion was used.Material and methods: Primiparas were divided into 3 groups. In group 1 there were 45 women for whom TENS stimulation was applied during delivery. Group 2 consisted of 38 women who remained in the water during the actual birth of the baby. Group 3 served as the control group and was composed of 32 women. The intensity of pain during delivery was assessed by means of a numerical scale. During the first delivery period, pain was assessed three times at cervical dilation of 2, 3 and 4 fingers.Results: The analysis of pain suffered by primiparas at 2-finger widening showed no statistically significant differences between the groups. However, the analysis of pain experienced at 3-finger opening showed significant differences between the group of women using TENS stimulation in comparison with the control group. When comparing pain at 4-finger opening, statistically significant differences were found between the group of women who delivered in water in comparison to both the control group and the group using TENS stimulation.Conclusions: TENS stimulation and water immersion are good methods to relieve labour pain; particularly helpful in the first period of labour. They are also safe, alternative, non-pharmacological methods of reducing labour pain

    Neonatal survival and kidney function after prenatal interventions for obstructive uropathies

    Objectives: Prenatal interventions in LUTO (lower urinary tract obstruction) usually are still question of a debate between gynaecologist and paediatric nephrologist. We aimed the study to assess the early survival rate and renal outcome in LUTO foetuses. Material and methods: The study was a prospective data analysis of 39 foetuses from singleton pregnancies. All pregnant women with LUTO in the foetus were qualified for VAS based on a local practice. The mean time of first urine analysis ranged between 13–30 weeks of pregnancy. Primary end-point analysis included live birth, 28d-survival, pulmonary and renal function assessment in neonatal period. Results: From initial number of 39, six patients miscarried before the procedure was performed. Overall, 33 VAS were performer at the mean 21 week of pregnancy (range 14–30 weeks). 25/39 foetuses survived until delivery. Three neonates died in first 3 days of life. In the first month 3 children required peritoneal dialysis, but at 28 day all children were dialysis-free. Overall survival rate at 28 day was 56%. Renal function preservation of the initial group (39) turned out to be low — 18% (7/39). Conclusions: Our study showed average survival curves and complications. LUTO in the foetus had mostly unfavourable outcome in the neonatal period. The prenatal intervention did not increase it significantly and did not guarantee the preservation of normal kidney function

    Epidural analgesia: effect on labor duration and delivery mode — a single-center cohort study

    Objectives: Parturients in labor experiencing severe pain may develop several complications, which could be avoided using various forms of labor analgesia. Researchers hold divergent opinions about the effect of epidural analgesia (EA) on labor duration and delivery mode. This paper aims to establish if EA affects the duration of the 1st and the 2nd phase of labor and the percentage of emergency Cesarean sections (CS) and instrumental delivery. Material and methods: The patients in this cohort study were recruited at St. Sophia’s Specialist Hospital in Warsaw, between 1/1/2020 and 6/1/2020. We used following inclusion criteria: patients aged 18–40 with singleton pregnancies and cephalic presentation of the fetus who gave live birth at a gestational age of 37–42 weeks to neonates with birthweight 2500–4250 g and received EA at the cervical dilation between three and six centimeters. The control group didn’t receive anesthesia. We excluded planned CS and vaginal births after CS. Data analysis was performed for all parturients and separately for multiparas and nulliparas. Results Out of 2550 deliveries, we included 1052 patients — 443 participants with EA and 609 in the control group. Patients with epidural analgesia experienced longer labor 415 vs 255 min (p < 0.01), longer 1st and 2nd stage (p < 0.01). They had a lower risk of emergency CS (OR = 0.56) (p < 0.01) but were more likely to have instrumental delivery. Conclusions: EA prolongs the first and the second stage of labor yet doesn’t affect neonatal outcomes. Moreover, the risk of emergency CS in nulliparas with EA is three times lower

    Variability in transport of terrigenous material on the shelves and the deep Arctic Ocean during the Holocene

    Arctic coastal zones serve as a sensitive filter for terrigenous matter input onto the shelves via river discharge and coastal erosion. This material is further distributed across the Arctic by ocean currents and sea ice. The coastal regions are particularly vulnerable to changes related to recent climate change. We compiled a pan-Arctic review that looks into the changing Holocene sources, transport processes and sinks of terrigenous sediment in the Arctic Ocean. Existing palaeoceanographic studies demonstrate how climate warming and the disappearance of ice sheets during the early Holocene initiated eustatic sea-level rise that greatly modified the physiography of the Arctic Ocean. Sedimentation rates over the shelves and slopes were much greater during periods of rapid sea-level rise in the early and middle Holocene, as a result of the relative distance to the terrestrial sediment sources. However, estimates of suspended sediment delivery through major Arctic rivers do not indicate enhanced delivery during this time, which suggests enhanced rates of coastal erosion. The increased supply of terrigenous material to the outer shelves and deep Arctic Ocean in the early and middle Holocene might serve as analogous to forecast changes in the future Arctic.Peer reviewe
