    The efficacy and predictability of maxillary first molar derotation with invisalign: a prospective clinical study in growing subjects

    To analyze the derotation of maxillary mesio-rotated first permanent molars in subjects with Class II edge-to-edge dental malocclusion in mixed dentition treated with Invisalign Clear Aligners (CA). In total, 36 patients (16 males, 20 females, 9.9 ± 1.9 years) treated with CA are enrolled from the Department of Orthodontics. Inclusion criteria are the following: Caucasian ancestry, mixed dentition, molar Class II edge-to-edge, no tooth/craniofacial anomalies, no caries/periodontal diseases. Pre-treatment (T1) and post-treatment (T2) digital casts, and final ClinCheck representations (T2ClinCheck) are acquired. The Henry’s angle (HA) is used to assess maxillary first molars rotation. The molars with an HA > 11° are taken (53 teeth). Five measurements are performed at T1, T2, and T2ClinCheck: Henry’s angle (HA), mesiobuccal-expansion (MBE), distobuccal-expansion (DBE), mesiobuccal-sagittal (MBS), and distobuccal-sagittal (DBS). A paired t-test was used to compare T2-T1 and T2ClinCheck-T2. The T2-T1 shows a distal-rotation (difference −6.3°) with an expansion of 2.2 mm for MBE and 1.5 mm for DBE. At T2, the mesiobuccal cusps show a distal movement of 1.0 mm and the distobuccal cusps of 0.9 mm. The HA’s T2ClinCheck-T2 difference is −4.2°. In the sagittal plane, the difference is 0.9 mm for the MBS and 0.7 mm for the DBS. The expansion showed the highest predictability (60% HA, 52.6% MBS, and 56.25% DBS). The CA effectively produces an arch expansion and upper molars’ distal rotation. Upper molar derotation provides a 1 mm of gain in arch perimeter and occlusal improvement

    Light-curing process for clear aligners’ attachment reproduction: comparison between two nanocomposites cured by the auxiliary of a new tool

    BACKGROUND: Attachments’ configuration play an important role during Clear Aligner Treatment (CAT) for aligner retention and control of movements planned. The aims were to compare the macroscopic morphology of attachments reproduced with flowable (FNC) and conventional (CNC) composites and the effects on them of two light-guide tips with different dimensions. METHODS: 4 resin casts derived from the initial scan of the same patient were obtained. 10 vestibular attachments were replaced on both upper and lower arches of each model with CNC (Models A, B) and FNC (Models C, D). Each composite was cured by means of the same LED lamp with both regular light-guide (Models A, B) and push and light toolÂź (Models C, D). The 80 attachments were qualitative analyzed by means of a digital stereo microscope. Surface roughness and waviness measurements were assessed by contact probe surface profiler (TalySurf CLI 2000; Taylor Hobson, Leicester, United Kingdom). Statistical analysis was performed with independent samples t-tests. Significance was established at the P < 0.05 level. RESULTS: Model A showed lower values of surface roughness (Ra − 1.41 ”m, Rt − 3.46 ”m) and waviness (Wa − 2.36 ”m, Wt − 10.95 ”m) when compared with Model C. Significant reduction of waviness (Wa − 3.85 ”m, Wt − 4.90 ”m) was observed on Model B when compared with Model D. Significant increase of roughness and waviness parameters (Ra 3.88 ”m, Rt 21.07, Wa 2.89 ”m, Wt 14.74 ”m) was found when CNC sample (Model A) was cured with regular light-guide tip. Higher values (Ra 2.33 ”m, Rt 24.07 ”m, Wa 1.67 ”m, Wt 20.79 ”m) were observed after regular light-guide tips curing on FNC sample (Model C). CONCLUSIONS: CNC resins determine more regular surfaces of attachments profiles. The additional use of a smaller light- guide of the LED push and light toolÂź allows to improve the macroscopic morphology of the attachments and to maximize light irradiance delivering by enhancing the polymerization process and the integrity of the features during the treatment

    Requalification of the river park: eco-houses in Elche

    The city of Elche, the second most important town of Alicante’s province, thanks to its 230,000 inhabitants, has a lot of heritage values such as the Misteri and the Palmeral. In its urban structure, the relation between the original urban plot and the modern one is made from the location of the river VinalopĂČ. Its canal is dry most of the year and its slopes are residual spaces that are not integrated with the modern and the ancient existing urban plot. The city of Elche relationship with the Palmeral does not give added value to the numerous urban parks that are in many parts of the city. This research could organize a new building typology of flats. It has tried to build a relationship with the environment that would bring benefits for the relationship between the urban plot and the river. In this way it would encourage a revitalization of the existing river park in support of architectural proposals such as an equipped park. In addition, these houses have bioclimatic technology of cooling evaporating using the natural airstream produced in the park which is induced through the building’s structural system. Introduced by the chimney effect, the air is cooled by vegetal masses. The orientation to the West of glazing, that looks at the river park, causes a lot of energy problems. This research analyses a wrapped and protective solar system based on a vertical rotating panel formed by low thickness and large format ceramic tiles in various sceneries. With modelling and the simulation through the software Design Builder, energetic demands for different sceneries are obtained which allow us to draw conclusions about the most efficient system in terms of energy consumption

    An Immunological Analysis of Dystroglycan Subunits: Lessons Learned from a Small Cohort of Non-Congenital Dystrophic Patients

    The dystroglycan (DG) expression pattern can be altered in severe muscular dystrophies. In fact, some congenital muscular dystrophies (CMDs) and limb-girdle muscular dystrophies (LGMDs) are caused by point mutations identified in six glycosyltransferase genes which are likely to target different steps along the posttranslational “O-glycosylation route” leading to a fully decorated and functional α-DG subunit. Indeed, hypoglycosylation of α-DG is thought to represent a major pathological event, in that it could reduce the DG’s ability to bind the basement membrane components, thus leading to sarcolemmal instability and necrosis. In order to set up an efficient standard immunological protocol, taking advantage of a wide panel of antibodies, we have analyzed the two DG subunits in a small cohort of adult dystrophic patients, whom an extensive medical examination had already clinically classified as affected by LGMD (5), Miyoshi (1) or distal (1) myopathy. Immunofluorescence analysis of skeletal muscle tissue sections revealed a proper sarcolemmal localization of the DG subunits in all the patients analyzed. However, Western blot analysis of lectin enriched skeletal muscle samples revealed an abnormal glycosylation of α-DG in two patients. Our work reinforces the notion that a careful immunological and biochemical analysis of the two DG subunits should be always considered as a prerequisite for the identification of new putative cases of dystroglycanopathy

    The Gulf of Trieste (Northern Adriatic Sea, Italy) is the coastal area of the Mediterranean Sea most highly contaminated by mercury (Hg) due to fluvial inputs from the Isonzo/Soča River system, draining over 500 years’ worth of cinnabar extraction activity from the Idrija mining district (Western Slovenia). The aim of this research is to investigate the concentration of Hg in hair samples taken from the general population of the Friuli Venezia Giulia (FVG) Region coastal area, as a marker of chronic exposure to Hg. Three hundred and one individuals — 119 males and 182 females — were recruited by convenience sampling in Trieste in September 2021. An amount of approximately 100 mg of hair was collected from the occipital scalp of each participant to measure the respective Hg concentrations (expressed as mg/kg). Moreover, participants completed a self-report questionnaire collecting extensive socio-demographic and life-style information. A multiple linear regression analysis was employed to investigate factors associated with increased levels of Hg concentration in hair. A mean Hg concentration in hair of 1.63 mg/kg was found, slightly above the 1.0 mg/kg threshold recommended by the WHO for pregnant women and children, although still well below the no observed adverse effects level (NOAEL) of 10 mg/kg. Among respondents, 55.6% showed a Hg concentration in hair > 1 mg/kg, 22.9% > 2 mg/kg, and 2 participants exhibited Hg levels > 10 mg/kg. The adjusted mean hair Hg level increased in those subjects who reported a preference for shellfish/crayfish/mollusks (RC = 0.35; 95%CI: 0.16; 0.55), whereas it decreased in those who reported a preference for frozen fish (RC = -0.23; 95%CI: − 0.39; − 0.06). Though a risk alert for Hg exposure for coastal residents from FVG is deemed unnecessary at this time, it is recommended that pregnant women limit their ingestion of locally caught fish to < 4 servings/month

    Portable X-ray Fluorescence (pXRF) as a Tool for Environmental Characterisation and Management of Mining Wastes: Benefits and Limits

    Portable X-ray fluorescence (pXRF) is one of the main geochemical techniques employed in multi-elemental analysis screening for contaminated sites management. As the confidence of pXRF analyses are matrix-specific, efforts are made to provide studies of pXRF quality on different geochemical datasets, focusing on less investigated elements such as mercury (Hg) and antimony (Sb), to help both new and experienced users. The analysis of environmental solid samples from two decommissioned mining sites in NE Italy, characterised by Pb-Zn and (Hg-rich) Cu-Sb ore deposits, were prepared with two different protocols and compared with traditional destructive analyses. Sample composition was found strictly dependent to the occurrence of false positives and overestimation at low concentrations. In contrast, milling the sample did not produce major variations in the overall quality. Lead (Pb), Sb, and Zn reached the definitive data quality in at least one of the two datasets. Consequently, as far as a thorough QA/QC protocol is followed, pXRF can rapidly produce chemical data that is as accurate as that produced by destructive standard laboratory techniques, thus allowing to identify potential sources of contamination that could be reprocessed for the extraction of valuable elements and mitigating the dispersion of contaminants and ecological or health risks
