A dynamic multiple agent model is constructed and the controllability principle is compared with the agency model's solution.     Withdrawal from treatment as an outcome in the Isolde study of COPD

    Objectives: To investigate the determinants of patient withdrawal from our study, and the effect of these withdrawals on the outcome of treatment with inhaled corticosteroids in patients with COPD. Design: A double-blind, placebo-controlled, randomized trial. Setting: Eighteen outpatient centers in the United Kingdom. Participants: Seven hundred fifty-one patients with stable COPD defined clinically as baseline postbronchodilator FEV1 > 0.8 L and < 85% predicted, FEV1/FVC ratio < 70%, and FEV1 change after albuterol < 10% of predicted. Intervention: Random assignment of either 500 micrograms bid of inhaled fluticasone propionate (FP)using a spacer device or an identical placebo inhaler. Treatment was continued for 3 years or until patients withdrew from follow-up. Measurements and results: Postbronchodilator FEV1 was measured on three occasions before randomization and every 3 months thereafter. Health status was assessed by the disease-specific St. George Respiratory Questionnaire (SGRQ) and the modified short-form 36 questionnaire (SF-36) at baseline and every 6 months. Three hundred thirty-nine patients withdrew, of whom 156 patients received FP. Prescription of frequent courses of oral prednisolone was the most common reason for withdrawing as specified in the protocol (69 patients in the FP group withdrew due to respiratory symptoms, compared with 93 patients in the placebo group). This explained the significantly greater dropout of placebo-treated patients that was most evident when FEV1 was < 50% predicted. Patients withdrawing had a significantly more rapid decline in health status, measured by both the SGRQ and the SF-36 (p < 0.001). Those withdrawing from the placebo group had a more rapid decline in FEV1 and more exacerbations than the FP-treated groups. Baseline FEV1 was lower in dropouts than in patients completing the study receiving placebo, but there was no difference between the respective groups receiving FP. Conclusions: Patients who withdrew from follow-up were those with the most rapidly deteriorating health status and lung function. Losing these patients from the final analysis can reduce the power of a study to achieve its primary end point

    New insight into the formation of structural defects in poly(vinyl chloride)

    The monomer conversion dependence of the formation of the various types of defect structures in radical suspension polymerization of vinyl chloride was examined via both H-1 and C-13 NMR spectrometry. The rate coefficients for model propagation and intra- and intermolecular hydrogen abstraction reactions were obtained via high-level ab initio molecular orbital calculations. An enormous increase in the formation of both branched and internal unsaturated structures was observed at conversions above 85%, and this is mirrored by a sudden decrease in stability of the resulting PVC polymer. Above this threshold-conversion, the monomer is depleted from the polymer-rich phase, and the propagation rate is thus substantially reduced, thereby allowing the chain-transfer processes to compete more effectively. In contrast to the other defects, the chloroallylic end groups were found to decrease at high conversions. On the basis of the theoretical and experimental data obtained in this study, this decrease was attributed to copolymerization and abstraction reactions that are expected to be favored at high monomer conversions. Finally, a surprising increase in the concentration of the methyl branches was reported. Although a definitive explanation for this behavior is yet to be obtained, the involvement of transfer reactions of an intra- or intermolecular nature seems likely, and (in the latter case) these could lead to the presence of tertiary chlorine in these defects

    The Abbey of the Dunes (Koksijde) and Noordduinen: the environmental link restored

    The Abbey of the Dunes (Ten Duinen) at Koksijde, founded in the 12th century and abandoned around 1600, forms the subject of a land use planning project realized by the Flemish Land Agency (Vlaamse Landmaatschappij). Initially the project focussed on the museum and the integration of the abbey ruins in an open-air museum. However the site presented an opportunity to involve the Noordduinen, bordering on the abbey park, in a nature restoration project. The key intention was to restore the landscape connection between the abbey and the dunes, as it was in the Middle Ages. The proposed actions consist of breaking up a road, deviation of the traffic, upgrading the abbey park and start of a nature restoration project in the Noordduinen. The main bottlenecks were deviation of the traffic, excavation and preservation of archaeological finds and intensive recreational use of the Noordduinen. Continuous consultation and deliberation with all involved authorities and with the local inhabitants of Koksijde resulted in a plan supported by the majority of the community

    The ALTO project at IPN Orsay

    In order to probe neutron rich radioactive noble gases produced by photo-fission, a PARRNe1 experiment (Production d'Atomes Radioactifs Riches en Neutrons) has been carried out at CERN. The incident electron beam of 50 MeV was delivered by the LIL machine: LEP Injector Linac. The experiment allowed to compare under the same conditions two production methods of radioactive noble gases: fission induced by fast neutrons and photo-fission. The obtained results show that the use of the electrons is a promising mode to get intense neutron rich ion beams. Thereafter, the success of this photo-fission experiment, a conceptual design for the installation at IPN Orsay of a 50 MeV electron accelerator close to the PARRNe-2 device has been worked out: ALTO Project.     The triple neurokinin-receptor antagonist CS-003 inhibits neurokinin A-induced bronchoconstriction in patients with asthma

    peer reviewedNeurokinin A (NKA) causes bronchoconstriction in asthmatic patients. In vitro both NK1 and NK2 receptors can mediate airway contraction. Moreover in guinea pigs, NK3 receptors facilitate cholinergic neurotransmission. Dual tachykinin NK1/NK2 receptor antagonism results in prevention of NKA-induced bronchoconstriction. We have now examined the effect of a single dose of the triple tachykinin receptor antagonist CS-003 on NKA-induced bronchoconstriction in asthmatics. A double blind, crossover, placebo-controlled trial in 16 mild asthmatics was performed. One single dose of CS-003 (200 mg, solution in distilled water) or matched placebo was given orally on the assessment days. NKA-provocation tests were performed pre-dose and 1, 8 and 24h after dosing. There was a significant shift to the right of the dose-response curve at 1 and 8 h after intake of CS-003. PC20 was not reached in 12/16 patients at 1 h post-dose and in 5/16 patients at 8h post-dose. This did not occur under placebo treatment. A single dose of 200 mg CS-003 protected significantly against NKA-induced bronchoconstriction at 1 and 8 h post-dose in mild asthmatics. (c) 2005 Elsevier Ltd. All rights reserved
