    Amplification of High-Order Rainbows of a Cylinder with an Elliptical Cross Section

    The intensity of high-order rainbows for normally incident light and certain rotation angles of a cylinder with an elliptical cross section is greatly amplified with respect to the intensity for a circular cross-sectional cylinder. The amplification is due to a number of the internal reflections occurring past the critical angle for total internal reflection, and the effect is especially strong for odd-order rainbows, beginning with the third order. Experimentally, the fourth-and the fifth-order rainbows of a nearly elliptical cross-sectional glass rod were observed and analyzed. (C) 1998 Optical Society of America

    Low uptake of palliative care for COPD patients within primary care in the UK

    Mortality and symptom burden from chronic obstructive pulmonary disease (COPD) and lung cancer are similar but there is thought to be an inequality in palliative care support (PCS) between diseases. This nationally representative study assessed PCS for COPD patients within primary care in the UK.This was a cohort study using electronic healthcare records (2004-2015). Factors associated with receiving PCS were assessed using logistic regression for the whole cohort and deceased patients.There were 92 365 eligible COPD patients, of which 26 135 died. Only 7.8% of the whole cohort and 21.4% of deceased patients received PCS. Lung cancer had a strong association with PCS compared with other patient characteristics, including Global Initiative for Chronic Obstructive Lung Disease stage and Medical Research Council Dyspnoea score (whole cohort, lung cancer: OR 14.1, 95% CI 13.1-15; deceased patients, lung cancer: OR 6.5, 95% CI 6-7). Only 16.7% of deceased COPD patients without lung cancer received PCS compared with 56.5% of deceased patients with lung cancer. In patients that received PCS, lung cancer co-diagnosis significantly increased the chances of receiving PCS before the last month of life (1-6 versus ≤1 month pre-death: risk ratio 1.4, 95% CI 1.3-1.7).Provision of PCS for COPD patients in the UK is inadequate.     Langerhans Cell Sarcoma: A Case Report Demonstrating Morphological and Immunophenotypical Variability within a Single Lesion.

    Langerhans cells are antigen presenting dendritic cells and tumours arising from these are rare. The tumours arising from these dendritic cells are divided into two categories according to a WHO classification: Langerhans cell histiocytosis and Langerhans cell sarcoma. It is the degree of atypia and clinical aggressiveness that distinguishes the two subtypes. Langerhans cell sarcoma (LCS) is a neoplastic proliferation of Langerhans cells which can occur in skin, bone marrow, lymph nodes, spleen, liver, and lung. LCS can present with multiple cutaneous and systemic lesions. We present a case of Langerhans cell sarcoma with a unique morphological appearance and variable immunohistochemical profile within a single cutaneous lesion. LCS is a rare malignancy and its diagnosis is based on morphology, immunophenotypical examination, and ultrastructural analysis by electron microscopy. Our case highlights a unique morphological description of LCS wherein the pleomorphic neoplastic cells show epidermotropism and are surrounded by a variable amount of inflammatory infiltrate within a single cutaneous lesion. A single cutaneous lesion of Langerhans cell sarcoma with variable immunohistochemical profile has not been described so far.This article is freely available via Open Access.     Entangled-Photon Imaging of a Pure Phase Object

    We demonstrate experimentally and theoretically that a coherent image of a pure phase object may be obtained by use of a spatially incoherent illumination beam. This is accomplished by employing a two-beam source of entangled photons generated by spontaneous parametric down-conversion. Though each of the beams is, in and of itself, spatially incoherent, the pair of beams exhibits higher-order inter-beam coherence. One of the beams probes the phase object while the other is scanned. The image is recorded by measuring the photon coincidence rate using a photon-counting detector in each beam. Using a reflection configuration, we successfully imaged a phase object implemented by a MEMS micro-mirror array.     Artificial Intelligence for the Electron Ion Collider (AI4EIC)

    The Electron-Ion Collider (EIC), a state-of-the-art facility for studying the strong force, is expected to begin commissioning its first experiments in 2028. This is an opportune time for artificial intelligence (AI) to be included from the start at this facility and in all phases that lead up to the experiments. The second annual workshop organized by the AI4EIC working group, which recently took place, centered on exploring all current and prospective application areas of AI for the EIC. This workshop is not only beneficial for the EIC, but also provides valuable insights for the newly established ePIC collaboration at EIC. This paper summarizes the different activities and R&D projects covered across the sessions of the workshop and provides an overview of the goals, approaches and strategies regarding AI/ML in the EIC community, as well as cutting-edge techniques currently studied in other experiments

    Plaque excision with the Silverhawk catheter: Early results in patients with claudication or critical limb ischemia

    ObjectiveThis study was conducted to detail the early experience after infrainguinal atherectomy using the Silverhawk plaque excision catheter for the treatment of symptomatic peripheral vascular disease.MethodsA prospective database was established in August 2004 in which data for operations, outcomes, and follow-up were recorded for patients undergoing percutaneous plaque excision for peripheral arterial occlusive disease. Society for Vascular Surgery (SVS) ischemia scores and femoropopliteal TransAtlantic Inter-Society Consensus (TASC) criteria were assigned. A follow-up protocol included duplex ultrasound surveillance at 1, 3, and 6 months and then yearly thereafter. Standard statistical analyses were performed.ResultsDuring a 17-month period, 66 limbs of 60 patients (37 men [61.7%]) underwent 70 plaque excisions (four repeat procedures). Indications included tissue loss based on SVS ischemia at grades 5 and 6 (25/70), rest pain at grade 4 (22/70), and claudication at grades 2 to 3 (23/70). The mean lesion length was 8.8 ± 0.7 cm. The technical success rate was 87.1% (61/70). Adjunctive treatment was required in 17 procedures (24.3%), consisting of 14 balloon angioplasties and three stents. Femoropopliteal TASC criteria included 5 TASC A lesions, 14 TASC B lesions, 32 TASC C lesions, and 19 TASC D lesions. Although 17 plaque excisions included a tibial vessel, no patient underwent isolated tibial atherectomy. The mean increase in ankle-brachial index was 0.27 ± 0.04 and in toe pressure, 20.3 ± 6.9 mm Hg. Mean duplex ultrasound follow-up was 5.2 months (range, 1 to 17 months). One-year primary, primary assisted, and secondary patency was 61.7%, 64.1%, and 76.4%, respectively. Restenosis or occlusion developed in 12 patients (16.7%) and was detected at a mean of 2.8 ± 0.7 months. Restenosis or occlusion was significantly more common (P < .05) in patients with TASC C and D lesions compared with patients with TASC A and B lesions. Six (8.3%) of 12 patients underwent reintervention on the basis of duplex ultrasound surveillance results. Four (33.3%) of 12 patients experienced reocclusion during the same hospitalization, and amputation and open revascularization were required in two patients each.ConclusionsPercutaneous plaque excision is a viable treatment option for lower extremity revascularization. Outcomes are related to ischemia and lesion severity. Patency and limb salvage rates are equivalent to other endovascular modalities

    Risk Assessment of Erosion from Concentrated Flow on Rangelands Using Overland Flow Distribution and Shear Stress Partitioning

    Erosion rates of overland flow on rangelands tend to be relatively low, but under certain conditions where flow is concentrated, soil loss can be significant. Therefore, a rangeland site can be highly vulnerable to soil erosion where overland flow is likely to concentrate and exert high shear stress on soil grains. This concept is commonly applied in cropland and wildland soil erosion modeling using predictions of flow effective shear stress (shear stress applied on soil grains). However, historical approaches to partition shear stress in erosion models are computationally complex and require extensive parameterization. Furthermore, most models are not capable of predicting the conditions in which concentrated flow occurs on rangelands. In this study, we investigated the rangelands conditions at which overland flow is more likely to become concentrated and developed equations for partitioning the shear stress of concentrated flow on rangelands. A logistic equation was developed to estimate the probability of overland flow to become concentrated. Total shear stress of rangeland overland flow was partitioned into components exerted on soil, vegetation, and rock cover using field experimental data. In addition, we investigated the vegetation cover limit at which the effective shear stress component is substantially reduced, limiting the erosion rate. The results from the partitioning equations show that shear stress exerted on soil grains was relatively small in sheet flow. Shear stress exerted on soil grains in concentrated flow was significantly higher when bare soil exceeded 60% of the total surface area but decreased significantly when the bare soil area was less than 25% or when the plant base cover exceeded 20%. These percentages could be used as relative measures of hydrologic recovery for disturbed rangelands or as triggers that indicate a site is crossing a threshold beyond which soil erosion might accelerate due to the high effective shear stress
