    Increasing late stage colorectal cancer and rectal cancer mortality demonstrates the need for screening: a population based study in Ireland, 1994-2010

    BACKGROUND: This paper describes trends in colorectal cancer incidence, survival and mortality from 1994 to 2010 in Ireland prior to the introduction of population-based screening. METHODS: We examined incidence (National Cancer Registry Ireland (NCRI) and mortality (Central Statistics Office) from 1994 to 2010. Age standardised rates (ASR) for incidence and mortality have been calculated, weighted by the European standard population. Annual percentage change was calculated in addition to testing for linear trends in treatment and case fraction of early and late stage disease. Relative survival was calculated considering deaths from all causes. RESULTS: The colorectal cancer ASR was 63.7 per 100,000 in males and 38.7 per 100,000 in females in 2010. There was little change in the ASR over time in either sex, or when colon and rectal cancers were considered separately; however the number of incident cancers increased significantly during 1994-2010 (1752 to 2298). The case fractions of late stage (III/IV) colon and rectal cancers rose significantly over time. One and 5 year relative survival improved for both sexes between the periods 1994-2008. Colorectal cancer mortality ASRs decreased annually from 1994-2009 by 1.8% (95% CI -2.2, -1.4). Rectal cancer mortality ASRs rose annually by 2.4% (95% CI 1.1, 3.6) and 2.8% (95% CI 1.2, 4.4) in males and females respectively. CONCLUSIONS: Increases in late-stage disease and rectal cancer mortality demonstrate an urgent need for colorectal cancer screening. However, the narrow age range at which screening is initially being rolled-out in Ireland means that the full potential for reductions in late-stage cancers and incidence and mortality are unlikely to be achieved. While it is possible that the observed increase in rectal cancer mortality may be partly an artefact of cause of death misclassification, it could also be explained by variations in treatment and adherence to best practice guidelines; further investigation is warranted

The following article looks closer at the NAICS industry sectors that make up the 2008 Fortune 500 list and the performance level of each sector represented using multiple dimensions of analysis. This research investigates the 2008 Fortune 500 companies in terms of their NAICS industry sector, geography, revenue generation, and profitability. The study further explores employment levels and labor productivity within and across sectors. Additionally, the education levels attained by the top executive are revealed. Finally, the report compares industry sectors in an effort to better understand the composition of the 2008 Fortune 500 list and its contribution to the American economy as a whole

    National College Health Risk Behavior Survey of Western Washington University Students, Spring, 1995

    In May, 1995, the National College Health Risk Behavior Survey-developed by the Centers for Disease Control and Prevention (CDC)-was mailed to 1,000 randomly selected Western Washington University students. The final tally of completed forms was 482, a 48.2% survey response rate. The National College Health Risk Behavior Survey was established by the CDC to monitor the prevalence of behaviors most influencing the health of college students. In addition to Western students, the survey was administered to over 10,000 representative college students nationwide. (Baseline data from that administration was not available at the time of the publication of this report). The National College Health Risk Behavior Survey gathers data in six behavioral categories contributing to the leading causes of mortality and morbidity among college students: 1) behaviors that result in unintentional or intentional injuries; 2) use of tobacco; 3) use of alcohol and other drugs 4) sexual behaviors that contribute to unintended pregnancy and sexually transmitted diseases, including HIV infection; 5)body weight/nutrition; and 6) physical activity. The primary purpose of administering the Health Risk Behavior Survey at Western was to produce baseline data on the status of college student health, especially as it relates to the 29 national health objectives described in Healthy People 2000: National Health Promotion and Disease Prevention Objectives. (U.S. Department of Health and Human Services #91-50212, 1991.) To date no such comprehensive assessment of the overall health of Western students exists. While student utilization rates of health services have been rigorously recorded, and though utilization rates are important for assuring adequate delivery of such services, they do not accurately reflect the health profile of the student body as a whole. Data from the Health Risk Behavior Survey completes the picture of student health at Western and is invaluable to strategic planning processes targeting the most compelling student needs

    Recent genome-wide studies conducted in European Whites have identified novel susceptibility genes for childhood acute lymphoblastic leukemia (ALL). We sought to examine whether these loci are susceptibility genes among Hispanics, whose reported incidence of childhood ALL is the highest of all ethnic groups in California, and whether their effects differ between Hispanics and non-Hispanic Whites (NHWs). We genotyped 13 variants in these genes among 706 Hispanic (300 cases, 406 controls) and 594 NHW (225 cases, 369 controls) participants in a matched population-based case-control study in California. We found significant associations for the five studied ARID5B variants in both Hispanics (p values of 1.0 Ă— 10(-9) to 0.004) and NHWs (p values of 2.2 Ă— 10(-6) to 0.018). Risk estimates were in the same direction in both groups (ORs of 1.53-1.99 and 1.37-1.84, respectively) and strengthened when restricted to B-cell precursor high-hyperdiploid ALL (>50 chromosomes; ORs of 2.21-3.22 and 1.67-2.71, respectively). Similar results were observed for the single CEBPE variant. Hispanics and NHWs exhibited different susceptibility loci at CDKN2A. Although IKZF1 loci showed significant susceptibility effects among NHWs (p < 1 Ă— 10(-5)), their effects among Hispanics were in the same direction but nonsignificant, despite similar minor allele frequencies. Future studies should examine whether the observed effects vary by environmental, immunological, or lifestyle factors

    An exploration of heat tolerance in mice utilizing mRNA and microRNA expression analysis

    Background Individuals who rapidly develop hyperthermia during heat exposure (heat-intolerant) are vulnerable to heat associated illness and injury. We recently reported that heat intolerant mice exhibit complex alterations in stress proteins in response to heat exposure. In the present study, we further explored the role of genes and molecular networks associated with heat tolerance in mice. Methodology Heat-induced physiological and biochemical changes were assessed to determine heat tolerance levels in mice. We performed RNA and microRNA expression profiling on mouse gastrocnemius muscle tissue samples to determine novel biological pathways associated with heat tolerance. Principal Findings Mice (n = 18) were assigned to heat-tolerant (TOL) and heat-intolerant (INT) groups based on peak core temperatures during heat exposures. This was followed by biochemical assessments (Hsp40, Hsp72, Hsp90 and Hsf1 protein levels). Microarray analysis identified a total of 3,081 mRNA transcripts that were significantly misregulated in INT compared to TOL mice (p\u3c0.05). Among them, Hspa1a, Dnajb1 and Hspb7 were differentially expressed by more than two-fold under these conditions. Furthermore, we identified 61 distinct microRNA (miRNA) sequences significantly associated with TOL compared to INT mice; eight miRNAs corresponded to target sites in seven genes identified as being associated with heat tolerance pathways (Hspa1a, Dnajb1, Dnajb4, Dnajb6, Hspa2, Hspb3 and Hspb7). Conclusions The combination of mRNA and miRNA data from the skeletal muscle of adult mice following heat stress provides new insights into the pathophysiology of thermoregulatory disturbances of heat intolerance

    Reliability of Peak Treadmill Exercise Tests in Mild Alzheimer Disease

This, along with an unprecedented growth in the elderly population, is leading to dramatic increases in the incidence of AD. Thus, effective strategies for promoting healthy brain aging and preventing AD are increasingly important. One strategy that appears promising in promoting healthy brain aging is exercise and physical activity. Evidence is accumulating that endurance exercise is beneficial to brain health (Laurin, Verreault, Lindsay, MacPherson, & Rockwood, 2001), and increased cardiorespiratory fitness is associated with increased brain volume in subjects with very mild to mild AD (Burns et al., 2008). While enhancing cardiorespiratory fitness may be a strategy for preventing cognitive decline in AD, there is limited information available on the validity and reliability of cardiorespiratory fitness measures in this population. The gold standard measure of cardiorespiratory fitness is maximum oxygen consumption (VO2max) (Frankin, 2001), the highest rate of oxygen uptake attainable during maximal or exhaustive exercise (American College of Sports Medicine, 2005). If the subject becomes exhausted and ends the test prior to reaching the physiologic VO2max, the end of the test is called peak oxygen consumption (VO2peak). It is unknown if advanced age and cognitive difficulties in people with AD would limit their ability to fully participate in a standard graded exercise test to reliably assess VO2max or VO2peak. Treadmill exercise testing has been found to be reliable in subjects with traumatic brain injury and mental retardation, although these subjects were very young (Fernhall, Millar, Tymeson, & Burkett, 1990; Mossberg & Greene, 2005). Traumatic brain injury and mental retardation are different disease processes than AD and would be expected to result in static rather than progressive cognitive symptoms. With AD, memory is impaired as is the ability to follow commands, however patients in the earliest stages of AD would be expected to respond to prompting and reminders to follow testing procedures. To our knowledge, no studies have assessed the reliability of peak treadmill exercise testing in subjects with AD. In our previous research on patients with very mild to mild AD (Burns, et al., 2008), we have found them to be capable of ambulating on a treadmill and completing peak treadmill exercise testing with 3 participants out of 74 (126 total peak exercise tests) identified as having EKG changes during testing. All 3 participants had negative follow-up testing in cardiology. The purpose of this study was to investigate the reliability of a graded peak treadmill exercise test in elderly people with early AD

    Hailing from Sweet Home, Oregon, where his father introduced him to the fascinations of pondwater (McCarthy 2018), Jim McCarthy graduated from Gonzaga University, and in the late 1960s joined the Food Chain Research Group at the Scripps Institution of Oceanography, where he received his doctorate in 1971. The Food Chain Research Group, which was becoming recognized as the premier research group on plankton, was at that time directed by such distinguished scientists as John Strickland and Dick Eppley, among others. The goal of the Food Chain Group was to understand plankton dynamics and trophodynamics, “to a degree that will enable man to exercise satisfactory control of the environment and make useful predictions” (Institute of Marine Resources annual report, 1968, cited in Shor 1978:143) and “to predict the formation and transfer of nutrients through the full cycle of life in the ocean” (Shor 1978:140). It was there that Jim became immersed in all aspects of nutrients, plankton, and the marine food web
