    Cranial ultrasound findings in preterm germinal matrix haemorrhage, sequelae and outcome

    Germinal matrix-intraventricular haemorrhage (GMH-IVH), periventricular haemorrhagic infarction (PHI) and its complication, post-haemorrhagic ventricular dilatation (PHVD), are still common neonatal morbidities in preterm infants that are highly associated with adverse neurodevelopmental outcome. Typical cranial ultrasound (CUS) findings of GMH-IVH, PHI and PHVD, their anatomical substrates and underlying mechanisms are discussed in this paper. Furthermore, we propose a detailed descriptive classification of GMH-IVH and PHI that may improve quality of CUS reporting and prediction of outcome in infants suffering from GMH-IVH/PHI

    Copper and Copper/Zn Ratio in a Series of Children with Chronic Diseases: A Cross-Sectional Study

    Copper is an essential micronutrient for humans. A cross-sectional and comparative study was done to assess serum Cu levels and serum copper/zinc (Cu/Zn) ratio and its association with nutritional indicators in a series of children and adolescents with chronic diseases. Anthropomet-ric, biochemical, dietary, body composition, and bone densitometry assessments were carried out. Serum Cu and Zn were measured by atomic absorption spectrophotometry. Seventy-eight patients (55% women) participated. The mean serum Cu in the entire series and by nutritional status through body mass index (BMI) was normal. Serum Cu decreased significantly with age and was meaning-fully higher in children than in adolescents. The risk of finding altered Cu levels in children and men was higher than in adolescents and women, respectively. Twenty-two per cent of patients had abnormal serum copper levels, 13 had hypercupremia, and four had hypocupremia. The Cu/Zn ratio was greater than 1.00 for 87% of the patients, which is an indicator of an inflammatory state. All patients with hypozincemia and hypocupremia had deficient Zn intake, but only 65% of the patients with hypercupremia had dietary Zn deficiency. Consequently, the Cu/Zn ratio could indicate an inflammatory state and a high risk of zinc deficiency in this specific child population

    Assessment of range uncertainty in lung-like tissue using a porcine lung phantom and proton radiography

    Thoracic tumours are increasingly considered indications for pencil beam scanned proton therapy (PBS-PT) treatments. Conservative robustness settings have been suggested due to potential range straggling effects caused by the lung micro-structure. Using proton radiography (PR) and a 4D porcine lung phantom, we experimentally assess range errors to be considered in robust treatment planning for thoracic indications. A human-chest-size 4D phantom hosting inflatable porcine lungs and corresponding 4D computed tomography (4DCT) were used. Five PR frames were planned to intersect the phantom at various positions. Integral depth-dose curves (IDDs) per proton spot were measured using a multi-layer ionisation chamber (MLIC). Each PR frame consisted of 81 spots with an assigned energy of 210 MeV (full width at half maximum (FWHM) 8.2 mm). Each frame was delivered five times while simultaneously acquiring the breathing signal of the 4D phantom, using an ANZAI load cell. The synchronised ANZAI and delivery log file information was used to retrospectively sort spots into their corresponding breathing phase. Based on this information, IDDs were simulated by the treatment planning system (TPS) Monte Carlo dose engine on a dose grid of 1 mm. In addition to the time-resolved TPS calculations on the 4DCT phases, IDDs were calculated on the average CT. Measured IDDs were compared with simulated ones, calculating the range error for each individual spot. In total, 2025 proton spots were individually measured and analysed. The range error of a specific spot is reported relative to its water equivalent path length (WEPL). The mean relative range error was 1.2% (1.5 SD 2.3 %) for the comparison with the time-resolved TPS calculations, and 1.0% (1.5 SD 2.2 %) when comparing to TPS calculations on the average CT. The determined mean relative range errors justify the use of 3% range uncertainty for robust treatment planning in a clinical setting for thoracic indications

    Real-World Apremilast Use for Treatment of Plaque Psoriasis in Italy: Patient Perspective, Characteristics, and Clinical Outcomes from the DARWIN Study

    Introduction: While several European studies have reported real-world apremilast use, patient-perceived benefits, and treatment satisfaction, local reimbursement criteria for apremilast vary and data from Italy are limited. Methods: The cross-sectional DARWIN study enrolled consecutive patients who had initiated apremilast for plaque psoriasis 6 (± 1) months prior to enrolment at a single visit across 24 Italian dermatological sites. Disease severity was assessed using body surface area (BSA) and Physician Global Assessment (PGA). Patient-reported outcomes assessed 6 (± 1) months after apremilast initiation were Dermatology Life Quality Index (DLQI), Patient Benefit Index (PBI), and 9-item Treatment Satisfaction Questionnaire for Medication (TSQM-9). Results: Of 184 patients enrolled between July 2019 and January 2021, 180 were included in the analysis. At apremilast initiation, median (25th-75th percentile) time since psoriasis diagnosis was 8.6 (3.2-22.2) years; median BSA, 10.0% (5.0-16.0); mean (standard seviation, SD) DLQI total score, 13.5 (8.0). Over half (54.9%) of patients with available data reported psoriasis had a very or extremely large effect on their quality of life (QoL); half reported itching (50.6%) and/or special areas involvement (50.0%). Most (73.9%) had comorbidities and were biologic-naïve (81.5%). The most common reasons for initiating apremilast were lack of efficacy of previous treatment (56.7%) and contraindications to other treatments (44.4%). At 6 (± 1) months, most patients were continuing apremilast and/or reported a Global PBI score ≥ 1 (minimum clinical benefit) (86.1% and 90.0%, respectively); approximately half achieved BSA ≤ 3% and/or DLQI total score ≤ 5 (47.1% and 48.5%); 18.8% achieved PGA = 0; mean (SD) TSQM-9 global treatment satisfaction score was 59.0 (24.8). Apremilast was well tolerated; no new safety signals were identified. Conclusions: Patients treated with apremilast for 6 months in Italian clinical practice reported improved QoL, clinically relevant improvements in symptoms, high treatment satisfaction, and high treatment persistence. Our data indicate apremilast is a valuable treatment option for moderate plaque psoriasis. Study registration: ClinicalTrials.gov identifier, NCT04031027

    El liderazgo está enfocado en la supervisión, organización y desempeño dentro de una empresa; por otro lado, la motivación está enfocada en el análisis de los trabajadores (necesidades, requerimientos, molestias, etc.) y, es necesaria para poder asegurar que estos se encuentren en óptimas condiciones para realizar su trabajo de manera eficiente. Se sabe que el liderazgo y la motivación se complementan; ello debido a que al realizar el análisis motivacional se puede identificar qué necesidades y factores de motivación necesitan los trabajadores; por lo que los líderes pueden dar respuesta a ello para lograr que los trabajadores adopten un estado de pertenencia con la empresa y deseen lograr los objetivos organizacionales tanto como los líderes de la misma. En este trabajo nos encargaremos de relacionar enfoques, tipos y modelos de liderazgo y motivación con la empresa Serprosa, así como el impacto que conllevan en su aplicación

    Relevamiento de arañas en la Unidad de Internación de personas privadas de libertad número 6 "Punta de Rieles": una experiencias educativa

    La experiencia educativa y de investigación se realizóen contexto de encierro con seis personas privadas delibertad y un monitor en territorio. Se relevaron arañas entres sitios de la Unidad penitenciaria: alojamiento deprivados de libertad, campo en abandono y huertaorgánica. Se manejó la hipótesis de que la diversidad severía influenciada por disturbios ambientales producidospor humanos. Los participantes fueron formados en losconceptos básicos del método científico, aplicado alestudio de la diversidad y composición de la araneofaunalocal. Se realizaron muestreos estacionales durante unaño, se recolectó y acondicionó el material. Seidentificaron 682 arañas pertenecientes a 18 familias. Seencontró mayor riqueza en campo en abandono y menoren vivienda, por tanto, se corroboró la hipótesis. Enverano se encontró la mayor abundancia de arañas. Lasfamilias más representadas fueron Lycosidae yLinyphiidae. El número de familias por estación fue similar,aunque varió en su composición. Se encontró actividadreproductiva en seis familias, la mayoría en primavera yverano. Este trabajo permitió un rico intercambio y fue undesafío educativo, debido a las restricciones deinfraestructura y materiales permitidos en la unidadpenitenciaria.     Immunoglobulin G Immune Complexes May Contribute to Neutrophil Activation in the Course of Severe Coronavirus Disease 2019

    Severe coronavirus disease 2019 (COVID-19) is associated with an overactive inflammatory response mediated by macrophages. Here, we analyzed the phenotype and function of neutrophils in patients with COVID-19. We found that neutrophils from patients with severe COVID-19 express high levels of CD11b and CD66b, spontaneously produce CXCL8 and CCL2, and show a strong association with platelets. Production of CXCL8 correlated with plasma concentrations of lactate dehydrogenase and D-dimer. Whole blood assays revealed that neutrophils from patients with severe COVID-19 show a clear association with immunoglobulin G (IgG) immune complexes. Moreover, we found that sera from patients with severe disease contain high levels of immune complexes and activate neutrophils through a mechanism partially dependent on FcγRII (CD32). Interestingly, when integrated in immune complexes, anti-severe acute respiratory syndrome coronavirus 2 IgG antibodies from patients with severe COVID-19 displayed a higher proinflammatory profile compared with antibodies from patients with mild disease. Our study suggests that IgG immune complexes might promote the acquisition of an inflammatory signature by neutrophils, worsening the course of COVID-19.Fil: Mazzitelli, Ignacio Gabriel.     Biophysical modeling and experimental validation of relative biological effectiveness (RBE) for 4He ion beam therapy

    Background: Helium (4He) ion beam therapy provides favorable biophysical characteristics compared to currently administered particle therapies, i.e., reduced lateral scattering and enhanced biological damage to deep-seated tumors like heavier ions, while simultaneously lessened particle fragmentation in distal healthy tissues as observed with lighter protons. Despite these biophysical advantages, raster-scanning 4He ion therapy remains poorly explored e.g., clinical translational is hampered by the lack of reliable and robust estimation of physical and radiobiological uncertainties. Therefore, prior to the upcoming 4He ion therapy program at the Heidelberg Ion-beam Therapy Center (HIT), we aimed to characterize the biophysical phenomena of 4He ion beams and various aspects of the associated models for clinical integration. Methods: Characterization of biological effect for 4He ion beams was performed in both homogenous and patient-like treatment scenarios using innovative models for estimation of relative biological effectiveness (RBE) in silico and their experimental validation using clonogenic cell survival as the gold-standard surrogate. Towards translation of RBE models in patients, the first GPU-based treatment planning system (non-commercial) for raster-scanning 4He ion beams was devised in-house (FRoG). Results: Our data indicate clinically relevant uncertainty of ±5–10% across different model simulations, highlighting their distinct biological and computational methodologies. The in vitro surrogate for highly radio-resistant tissues presented large RBE variability and uncertainty within the clinical dose range. Conclusions: Existing phenomenological and mechanistic/biophysical models were successfully integrated and validated in both Monte Carlo and GPU-accelerated analytical platforms against in vitro experiments, and tested using pristine peaks and clinical fields in highly radio-resistant tissues where models exhibit the greatest RBE uncertainty. Together, these efforts mark an important step towards clinical translation of raster-scanning 4He ion beam therapy to the clinic

    Application of Severe Weather Nowcasting to Case Studies in Air Traffic Management

    Effective and time-efficient aircraft assistance and guidance in severe weather environments remains a challenge for air traffic control. Air navigation service providers around the globe could greatly benefit from specific and adapted meteorological information for the controller position, helping to reduce the increased workload induced by adverse weather. The present work proposes a radar-based nowcasting algorithm providing compact meteorological information on convective weather near airports for introduction into the algorithms intended to assist in air-traffic management. The use of vertically integrated liquid density enables extremely rapid identification and short-term prediction of convective regions that should not be traversed by aircraft, which is an essential requirement for use in tactical controller support systems. The proposed tracking and nowcasting method facilitates the anticipation of the meteorological situation around an airport. Nowcasts of centroid locations of various approaching thunderstorms were compared with corresponding radar data, and centroid distances between nowcasted and observed storms were computed. The results were analyzed with Method for the Object-Based Evaluation from the Model Evaluation tools software (MET-10.0.1, Developmental Testbed Center, Boulder, CO, US) and later integrated into an assistance arrival manager software, showing the potential of this approach for automatic air traffic assistance in adverse weather scenarios

    Is an NWP-Based Nowcasting System Suitable for Aviation Operations?

    The growth of air transport demand expected over the next decades, along with the increasing frequency and intensity of extreme weather events, such as heavy rainfalls and severe storms due to climate change, will pose a tough challenge for air traffic management systems, with implications for flight safety, delays and passengers. In this context, the Satellite-borne and IN-situ Observations to Predict The Initiation of Convection for ATM (SINOPTICA) project has a dual aim, first to investigate if very short-range high-resolution weather forecast, including data assimilation, can improve the predictive capability of these events, and then to understand if such forecasts can be suitable for air traffic management purposes. The intense squall line that affected Malpensa, the major airport by passenger traffic in northern Italy, on 11 May 2019 is selected as a benchmark. Several numerical experiments are performed with a Weather Research and Forecasting (WRF) model using two assimilation techniques, 3D-Var in WRF Data Assimilation (WRFDA) system and a nudging scheme for lightning, in order to improve the forecast accuracy and to evaluate the impact of assimilated different datasets. To evaluate the numerical simulations performance, three different verification approaches, object-based, fuzzy and qualitative, are used. The results suggest that the assimilation of lightning data plays a key role in triggering the convective cells, improving both location and timing. Moreover, the numerical weather prediction (NWP)-based nowcasting system is able to produce reliable forecasts at high spatial and temporal resolution. The timing was found to be suitable for helping Air Traffic Management (ATM) operators to compute alternative landing trajectories
