Espondilodiscitis por Fusobacterium necrophorum: a propósito de un caso
Se presenta un caso de espondilodiscitis por Fusobacterium necrophorum
que es un germen gramnegativo. El paciente evolucionó de forma satisfactoria, a pesar de
que no siguió correctamente las indicaciones terapéuticas. A propósito del caso, se hacen
algunas reflexiones sobre el diagnóstico y el tratamiento de las infecciones raquídeasThe authors report a case of spondylodiscitis due to Fusobacterium necrophorum,
an anaerobic gram-negative bacillus. The patient's recovery was satisfactory
although he did not comply properly with the required treatment. Some considerations

Compresión del nervio cubital por quiste sinovial en el canal epitrócleo-olecraneano
Se presenta un caso de compresión del nervio cubital por un quiste sinovial a
nivel del canal epitrócleo-olecraneano. El tratamiento fue quirúrgico y consistió en la resección
del ganglión asociada a la exoneurolisis y transposición anterior del nervio. La evolución
fue satisfactoria, produciéndose una mejoría progresiva de sus síntomas preoperatorios.A rare case of cubital nerve compression due to a synovial cyst in the epitrochlear
groove is presented. It was treated surgically by cyst resection and exoneurolisis together
with anterior transposition of the nerve. The follow-up was satisfactory, with gradual
improvement of the previous symptoms
Complicaciones torácicas en un paciente con luxación subclavicular del hombro
Se presenta una paciente de 88 años de edad con una luxación subclavicular de
hombro derecho y un traumatismo torácico asociado que incluía fractura de las tres primeras
costillas y hemotórax. El traumatismo torácico parecía estar muy estrechamente relacionado
con la luxación glenohumeral. El tratamiento de la luxación se efectuó mediante reducción cerrada
e inmovilización con vendaje de Gilchrist. El traumatismo del hemitórax derecho requirió
toracocentesis en 2 ocasiones e ingreso en la unidad de cuidados intensivos por
insuficiencia respiratoria. La evolución fue satisfactoria, estando la paciente asintomática a los
3 meses del traumatismo, con movilidad del hombro prácticamente completa.The case of a 88-year-old women with a subclavicular right shoulder dislocation
and concomitant thoracic trauma involving fracture of the three first ribs and hemothorax
is reported. The thoracic trauma appeared to be closely related to the shoulder dislocation.
Close reduction and Gilchrist's immovilization was the treatment applied to the shoulder dislocation.
Right thoracic trauma required evacuation of the hemothorax in two ocasions and inward
in the Intensive Care Unit. Three months after trauma, the outcome was satisfactory with
almost complete recovery of the shoulder range of motion
Luxación aislada de la articulación radiocubital distal : Un nuevo caso
Se presenta 1 caso de luxación dorsal aislada de la articulación radiocubital distal
en un paciente varón de 18 años que sufrió una caída sobre la mano izquierda en posición de pronación
del antebrazo. Fue tratado mediante reducción cerrada bajo anestesia local y posterior
inmovilización en un yeso braquioantcbraquial con el codo en 90° de flexión y el antebrazo en supinación
durante 6 semanas. El resultado tras el tratamiento rehabilitador posterior a la retirada
del yeso fue satisfactorio, sin dolor ni alteraciones estéticas ni funcionales en la extremidad.A case of isolated dislocation of the distal radioulnar joint is presented. A 18-yearold

and cast inmobilization of the upper extremity with the elbow in 90° of flexion and the forearm

Los corticoides epidurales en el tratamiento del síndrome del canal lumbar estrecho
Se presenta una serie de 67 pacientes diagnosticados de síndrome del canal lumbar
estrecho, tratados todos ellos con infíltraciones epidurales de corticosteroides, asociadas a un
programa de rehabilitación realizado a continuación de las inyecciones. Tras el análisis de los datos
se ha observado un porcentaje de buenos resultados cercano al 80%. Asimismo, el estudio ha
puesto en evidencia la influencia clara del factor rehabilitación en la calidad del resultado final
de los pacientes.The authors present a study on 67 patients with degenerative lumbar spinal stenosis.
The treatment consisted in epidural injections with corticosteroids and a program of physical
therapy. There was 80% of good results. A clear influence of the physical therapy program on the
quality of the final result, has been proved
Los corticoides epidurales en el tratamiento de la hernia discal
Se presenta una serie de 233 pacientes diagnosticados de hernia discal lumbar
y tratados mediante infiltraciones epidurales de corticoides, asociadas a un programa
de rehabilitación realizado a continuación de las inyecciones. Los resultados con dicho
tratamiento fueron satisfactorios en un 75% de los casos. Se observó una mejor evolución
en pacientes varones, en pacientes cuya profesión conlleva esfuerzo físico, en hernias
discales L5-S1, y en aquellos que han seguido un programa de rehabilitación tras las
infiltraciones.An study on 233 patients with lumbar disc herniation, treated by means of epidural injections of steroids, added to a physical therapy programme is presented. The results obtained were fully satisfactory in 75% of the cases. Better results were observed in male patients, in patients subject to heavy physical activity in their laboral life, in disc herniations at L5-S1 level, and finally in those patients following a physical therapy programme after injections
Epidemiological trends of HIV/HCV coinfection in Spain, 2015-2019
Altres ajuts: Spanish AIDS Research Network; European Funding for Regional Development (FEDER).Objectives: We assessed the prevalence of anti-hepatitis C virus (HCV) antibodies and active HCV infection (HCV-RNA-positive) in people living with HIV (PLWH) in Spain in 2019 and compared the results with those of four similar studies performed during 2015-2018. Methods: The study was performed in 41 centres. Sample size was estimated for an accuracy of 1%. Patients were selected by random sampling with proportional allocation. Results: The reference population comprised 41 973 PLWH, and the sample size was 1325. HCV serostatus was known in 1316 PLWH (99.3%), of whom 376 (28.6%) were HCV antibody (Ab)-positive (78.7% were prior injection drug users); 29 were HCV-RNA-positive (2.2%). Of the 29 HCV-RNA-positive PLWH, infection was chronic in 24, it was acute/recent in one, and it was of unknown duration in four. Cirrhosis was present in 71 (5.4%) PLWH overall, three (10.3%) HCV-RNA-positive patients and 68 (23.4%) of those who cleared HCV after anti-HCV therapy (p = 0.04). The prevalence of anti-HCV antibodies decreased steadily from 37.7% in 2015 to 28.6% in 2019 (p < 0.001); the prevalence of active HCV infection decreased from 22.1% in 2015 to 2.2% in 2019 (p < 0.001). Uptake of anti-HCV treatment increased from 53.9% in 2015 to 95.0% in 2019 (p < 0.001). Conclusions: In Spain, the prevalence of active HCV infection among PLWH at the end of 2019 was 2.2%, i.e. 90.0% lower than in 2015. Increased exposure to DAAs was probably the main reason for this sharp reduction. Despite the high coverage of treatment with direct-acting antiviral agents, HCV-related cirrhosis remains significant in this population
