    Effects of technology in nursing care

    La visión de la tecnología como herramienta de ayuda destinada a mejorar las condiciones de las personas influye en múltiples aspectos de la asistencia y del cuidado. La tecnología en enfermería y en los cuidados abarca; el papel de la enfermera como nexo de unión entre la tecnología y el paciente, la aplicación de una ética del cuidado, la capacitación del manejo de la tecnología y la educación en distintos ámbitos. La falta de manejo junto a problemas de funcionamiento pueden provocar ansiedad y estrés en las enfermeras, el uso de la tecnología dentro de la formación de profesionales de enfermería, refuerza las habilidades con las que se llegue a la práctica clínica o a las instituciones, favoreciendo la adaptación al entorno laboral. El innovar no solo depende de las instituciones de salud, la implementación de las nuevas tecnologías, busca que los profesionales vean una oportunidad para el crecimiento profesional y que se interesen por el desarrollo de proyectos que vayan encaminadas al uso y su aplicación. La tecnología es un elemento neutro, son las actuaciones de la enfermera lo que determina la presencia de aspecto positivos o negativos asociados a la aplicación de tecnología.The vision of technology as an aid to improve the conditions of those influences multiple aspects of health and care. Tecnologia em enfermagem e no cuidado engloba o papel do enfermeiro como elo de ligação entre a tecnologia e o paciente, a aplicação de uma ética do cuidado e a formação em gestão de tecnologia e educação em distintos campos. A inabilidade de gestão relacionada aos problemas de desempenho tecnológicos podem causar ansiedade e estresse nos enfermeiros. A utilização de tecnologia no âmbito da formação de enfermeiros reforça as habilidades com as quais o profissional irá deparar-se na prática clínica ou instituições, favorecendo uma melhor adaptação ao local de trabalho. O inovar depende não só nas instituições de saúde, mas também da implementação de novas tecnologias, da busca por parte dos profissionais que veem uma oportunidade de crescimento profissional e se interessem no desenvolvimento de projetos que visam a utilização e implementação das mesmas.     A experiencia das enfermeiras que trabalham com estudantes de enfermagem em um hospital: uma investigação fenomenológica

    Objective: this paper explores the experiences of registered nurses working with Spanish nursing students within the hospital. Methods: a qualitative phenomenological approach was followed. Purposeful sampling was employed. Twenty-one registered nurses, from a public hospital located in Spain, were included in the study. Data were collected by means of unstructured and semi-structured interviews and were analysed using Giorgi’s proposal. The Consolidated Criteria for Reporting Qualitative Research were followed. Results: three main themes described the experience of registered nurses: “The nurse’s relationship with nursing students”; most nurses emphasized the importance of the first contact with students and they considered students’ attitude to be key. “Defining the role of the student in clinical practice”; it is necessary to unify the nurse’s role and interventions to avoid misleading students and establish priorities in clinical practice. “Building bridges between clinical settings and the University”; the need to establish a common ground and connection between the university and hospital clinical settings was emphasized. Nurses felt that the training program should also be designed by the clinical settings themselves. Conclusions: understanding the meaning of nursing students with registered nurses might gain a deeper insight into their expectations.Objetivo: este artículo explora las experiencias de las enfermeras que trabajan con estudiantes de enfermería españolas en un hospital. Métodos: se utilizó un abordaje fenomenológico cualitativo y una muestra de conveniencia. Se incluyeron en el estudio veintidós enfermeras pertenecientes a un hospital público de España. Los datos fueron recogidos a través de entrevistas no estructuradas y semi-estructuradas, y analizadas usando la propuesta de Giorgi. Se siguieron los Criterios Consolidados para Relatos de Investigación Cualitativa. Resultados: tres temas principales describen la experiencia de las enfermeras: “La relación de la enfermera con las estudiantes de enfermería” la mayoría de las enfermeras dan énfasis a la importancia del primer contacto con las estudiantes y consideran que la actitud de las estudiantes es esencial. “La definición del rol de la estudiante en la práctica clínica”; es necesaria para unificar el rol y las intervenciones de la enfermera para evitar guiar en forma errónea a las estudiantes y establecer prioridades en la práctica clínica. “Construir puentes entre los ambientes clínicos y la Universidad”; se enfatizó la necesidad de establecer un terreno común y conexiones entre la universidad y los servicios clínicos del hospital. Las enfermeras piensan que el programa educativo debe ser también diseñado por los servicios clínicos. Conclusiones: comprender los significados de las estudiantes de enfermería con las enfermeras puede proveer una introspección más profunda de sus expectativas.Objetivo: este artigo explora as vivencias das enfermeiras que trabalham com estudantes de enfermagem espanholas em um hospital. Foram observados os Critérios Consolidados para Relatos de Pesquisa Qualitativa. Resultados: três temas principais descrevem a experiência das enfermeiras: “A relação da enfermeira com as estudantes de enfermagem”; a maioria das enfermeiras enfatizam a importância do primeiro contato com estudantes e consideram central a atitude dos estudantes. “A definição do papel da estudante na prática clínica”; é necessária para unificar o papel e as intervenções da enfermeira para evitar guiar erroneamente ao estudante e estabelecer prioridades na prática clínica. “Construir pontes entre os serviços clínicos e a Universidade”; enfatizou-se a necessidade de estabelecer um terreno comum e conexões entre a universidade e os serviços clínicos do hospital. As enfermeiras pensam que o programa educacional deve também ser desenhado pelos serviços clínicos. Conclusões: compreender os significados das estudantes de enfermagem com as enfermeiras graduadas pode permitir uma introspecção mais aprofundada nas suas expectativas

A qualitative phenomenological approach was conducted; a purposeful and theoretical sampling was implemented with supplemental nursing staff at Santa Barbara Hospital of Soria (Spain), to gain a more in-depth understanding of the Supplemental Nursing Staff ´s experience. Data were collected by in-depth interviews and through a field notebook. Data were analyzed using the Giorgi proposal. Twenty-one nurses with a mean age of 46 years were included. Three main topics emerged from the data analysis: building the first contact, carving out a niche and establishing interprofessional/interpersonal relationships. We conclude that the experience of hosting the supplemental nursing staff in changing clinical environments is conditioned by various factors. It is necessary for nurses and hospital managers to establish clear objectives with regard to the supplemental nursing staff´s role in the units.El propósito de este estudio es describir la experiencia de las enfermeras “de apoyo” en las diferentes unidades de hospitalización. Seguimos un enfoque fenomenológico cualitativo. Realizamos un muestreo por propósito y teórico de dicho personal del Hospital Santa Bárbara de Soria (España), con el fin de obtener una comprensión más profunda del fenómeno.     Exercises and Dry Needling for Subacromial Pain Syndrome: a Randomized Parallel- Group Trial.

    This randomized clinical trial investigated the effectiveness of exercise vs. exercise plus trigger point dry needling (TrP-DN) in subacromial pain syndrome. A randomized parallel-group trial, with 1-year follow-up was conducted. Fifty subjects with subacromial pain syndrome were randomly allocated to receive exercise alone or exercise +TrP-DN. Participants in both groups were asked to perform an exercise program of the rotator cuff muscles twice daily for 5 weeks. Further, patients allocated to the exercise +TrP-DN group also received dry needling to active TrPs in the muscles reproducing shoulder symptoms during the 2nd and 4th sessions. The primary outcome was pain-related disability assessed with the Disabilities of the Arm, Shoulder and Hand (DASH) questionnaire. Secondary outcomes included mean current pain and the worst pain experienced in the shoulder during the previous week. They were assessed at baseline, one week, and 3, 6, and 12 months after the end of treatment. Analysis was by intention to treat with mixed ANCOVA adjusted for baseline outcomes. At 12 months, 47 (94%) patients completed follow-up. Statistically larger improvements (all, P<0.01) in shoulder disability was found for the exercise +TrP-DN group at all follow up periods [post: Δ -20.6 (-23.8 to -17.4); 3 months: Δ -23.2 (-28.3 to -18.1); 6 months: Δ -23.6 (-28.9 to -18.3); 12 months: Δ -13.9 (-17.5 to -10.3). Both groups exhibited similar improvements in shoulder pain outcomes at all follow-up periods. The inclusion of TrP-DN to an exercise program was effective for improving disability in subacromial pain syndrome. No greater improvements in shoulder pain were observed.pre-print2407 K

    Identification of subgroups of patients with tension type headache with higher widespread pressure pain hyperalgesia

    Abstract Background Identification of subgroups of patients with different levels of sensitization and clinical features can help to identify groups at risk and the development of better therapeutic strategies. The aim of this study was to identify subgroups of patients with tension type headache (TTH) with different levels of sensitization, clinical pain features, and psychological outcomes. Methods A total of 197 individuals with TTH participated. Headache intensity, frequency, and duration and medication intake were collected with a 4-weeks diary. Pressure pain thresholds were assessed bilaterally over the temporalis muscle, C5-C6 joint, second metacarpal and tibialis anterior muscle to determine widespread pressure pain hyperalgesia. The Hospital Anxiety and Depression Scale assessed anxiety and depression. The State-Trait Anxiety Inventory evaluated the state and trait levels of anxiety. The Headache Disability Inventory evaluated the burden of headache. Health-related quality of life was determined with the SF-36 questionnaire. Groups were considered as positive (three or more criteria) or negative (less than three criteria) on a clinical prediction rule: headache duration <8.5 h/day; headache frequency <5.5 days/week; bodily pain <47 and vitality <47.5. Results The ANCOVA revealed that subjects in group 1 (positive rule, n = 89) exhibited longer headache history, shorter headache duration, lower headache frequency, higher widespread pressure hyperalgesia, higher anxiety trait levels, and lower quality of life (all, P < 0.01) than those subjects within group 2 (negative rule, n = 108). Differences were similar between men and women. Conclusions This study identified a subgroup of patients with TTH with higher sensitization, higher chronicity of headaches and worse quality of life but lower frequency and duration of headache episodes. This subgroup of individuals with TTH may need particular attention and specific therapeutic programs for avoiding potential chronification

    Challenges A ecting Access to Health and Social Care Resources and Time Management among Parents of Children with Rett Syndrome: A Qualitative Case Study.

    Rare diseases face serious sustainability challenges regarding the distribution of resources geared at health and social needs. Our aim was to describe the barriers experienced by parents of children with Rett Syndrome for accessing care resources. A qualitative case study was conducted among 31 parents of children with Rett syndrome. Data were collected through in-depth interviews, focus groups, researchers’ field notes and parents’ personal documents. A thematic analysis was performed and the Standards for Reporting Qualitative Research (SRQR) guidelines were followed. Three main themes emerged from the data: (a) essential health resources; (b) bureaucracy and social care; and (c) time management constraints. Parents have difficulties accessing appropriate health services for their children. Administrative obstacles exist for accessing public health services, forcing parents to bear the financial cost of specialized care. Time is an essential factor, which conditions the organization of activities for the entire family. Qualitative research offers insight into how parents of children with Rett syndrome experience access to resources and may help improve understanding of how Rett syndrome impacts the lives of both the children and their parentspost-print353 K
