    Participation of bone marrow-derived cells in hippocampal vascularization after status epilepticus

    Purpose: Diseases such as temporal lobe epilepsy, brain trauma and stroke can induce endothelial cell proliferation and angiogenesis in specific brain areas. During status epilepticus (SE), bone marrow-derived cells are able to infiltrate and proliferate, dramatically increasing at the site of injury. However, it is still unclear whether these cells directly participate in vascular changes induced by SE.Method: To investigate the possible role of bone marrow-derived cells in angiogenesis after seizures, we induced SE by pilocarpine injection in previously prepared chimeric mice. Mice were euthanized at 8 h, 7 d or 15 d after SE onset.Results: Our results indicated that SE modified hippocampal vascularization and induced angiogenesis. Further, bone marrow-derived GFP(+) cells penetrated through the parenchyma and participated in the formation of new vessels after SE. We detected bone marrow-derived cells closely associated with vessels in the hippocampus, increasing the density of blood vessels that had decreased immediately after pilocarpine-induced SE.Conclusion: We conclude that epileptic seizures directly affect vascularization in the hippocampus mediated by bone marrow-derived cells in a time-dependent manner.     Síndrome pós-Covid-19: breve revisão sistemática / Long-Covid: brief systematic review

    A COVID-19 é uma infecção respiratória aguda causada pelo Coronavírus-2 da Síndrome Respiratória Aguda Grave (SARS-COV-2). Sua transmissibilidade, através do contato direto ou com gotículas de pessoas infectadas, permitiu-o se alastrar da China para o mundo, resultando em uma pandemia global com milhões de mortes. Dentre as vítimas infectadas que sobreviveram à patologia, há indivíduos que continuam manifestando sintomas remanescentes após a fase aguda, sendo tais sintomas concernentes à Síndrome Pós-COVID. Este estudo objetiva averiguar o conteúdo bibliográfico disponível acerca das manifestações da Síndrome pós-COVID. Trata-se de uma revisão sistemática, com buscas nas bases de dados PUBMED, SciELO e LILACS, utilizando os operadores booleanos: (LONG-COVID) OR (CHRONIC COVID SYNDROME) OR (LONG-HAUL COVID) OR (POST-ACUTE COVID-19 SYNDROME). Excluiu-se artigos que não se enquadravam na temática proposta, que estavam indisponíveis, que eram duplicatas ou que não fossem em língua inglesa, portuguesa ou espanhola. Foram encontrados 579 artigos e, desses, 20 estudos foram incluídos. Dentre os sintomas manifestados na Síndrome Pós-COVID, constata-se que fadiga, mialgia, dispneia, cefaleia, anosmia, ageusia e artralgia são os sintomas mais relatados pelos estudos. As inflamações no sistema nervoso acarretam sintomas como déficit de atenção, “brain fog”, desordem de memória e distúrbios do sono. Disfunções psicológicas estão presentes, tais quais depressão, anedonia, ansiedade, estresse pós-traumático, letargia e humor triste. Esses sintomas prejudicam a qualidade de vida do indivíduo ao dificultar a realização de atividades habituais. Enfatiza-se a necessidade de acompanhamento multiprofissional nos pacientes convalescentes da COVID-19, para mitigar suas complicações e sequelas

    PesquisarCOM: efeitos de uma oficina de experimentação corporal com pessoas cegas e com baixa visão

    A pesquisa Perceber sem Ver realiza-se no Instituto Benjamin Constant (IBC) e conta com um dispositivo-intervenção, as Oficinas de Experimentação Corporal, oferecidas às pessoas cegas e com baixa visão. Nas oficinas, exploramos o encontro entre corpos-músicas-bexigas-sons e investigamos aquilo que o corpo pode vir a criar. Definimos corpo como corporeidade, existência que se realiza na prática de experimentar-se. A perda da visão exige uma (re)eleboração da relação entre cada corpo singular e o mundo. Perguntamos: quais os efeitos de uma oficina de experimentação corporal com pessoas cegas e com baixa visão? Lançando mão do método pesquisarCOM, afirmamos que o trabalho corporal coloca em cena um eu-corpo sabido de si mesmo, que no caminho de experimentar-se, apreende de si e partilha essas descobertas em conjunto. O corporeisar-se, dado nas oficinas, passa corporeisar a cegueira, passa também pelo corporeisar nossos próprios corpos de pesquisadoras, nossos próprios referenciais visuocêntricos.Palavras-chave: Corporeidade; PesquisarCOM; Deficiência Visual

    Sylvia Caiuby Novaes é professora do Departamento de Antropologia da Universidade de São Paulo (USP) e dedica-se há cerca de 50 anos à pesquisa e ao ensino em antropologia. Entre outras realizações, ela é uma das pioneiras da antropologia visual no Brasil, é fundadora do Laboratório de Imagem e Som da Antropologia (LISA) e editora responsável pela revista Gesto, Imagem e Som. Revista de Antropologia (GIS). Nesta entrevista, realizada por mais de 30 orientandos de diferentes gerações, Sylvia fala sobre sua trajetória, projetos, visão de mundo, suas diversas viagens, o fascínio pelas pesquisas de campo e a universidade.     ANÁLISE DA INFLUÊNCIA DA HIPERTENSÃO ARTERIAL SISTÊMICA E DA INSUFICIÊNCIA CARDÍACA NO AGRAVO DO QUADRO CLÍNICO DE PACIENTES COM DOENÇA RENAL CRÔNICA: uma revisão de literatura

    Introduction: systemic arterial hypertension (SAH) and heart failure, epidemiologically, are diseases that model consequences for other systems of the human body, for example chronic kidney disease (CKD). The development of this appears to be a social consequence of lack of knowledge, as its secondary outcomes are controllable and treatable. Countries like Brazil have exorbitant expenses when it comes to financing dialysis and transplant procedures, with an increase in these numbers, especially in young patients decompensated for their underlying diseases. The objective of this work is to observe the incidence in the literature of SAH and heart failure in patients related to the worsening of CKD. Methodology: descriptive study in narrative review, which seeks to answer the PICO acromion “What is the influence of systemic arterial hypertension and heart failure on the worsening of the clinical condition of patients with chronic kidney disease? ”. Discussion: CKD's pathophysiology is the loss of kidney function, where they lose functionality and destroy their specific cells, resulting in the inability to maintain metabolic balance. It proves to be a problem of public responsibility, where more and more deaths in the population are reported. The main risk factors for CKD are highly prevalent chronic diseases such as hypertension and heart failure, the first being the most described in the literature as a triggering factor. Thus resulting in worsening of renal function laboratory results, resulting in chronic kidney injury (CRF). Results: Analyzing the databases, articles in the last 10 years were observed, where 38.6% had the descriptors systemic arterial hypertension and heart failure, describing them as their main secondary outcome. Conclusion: to the scientific society, it contributes summarized and updated indexes reporting the relationship between these precursor pathologies. To society, it informs the problem and a way to inform the patient about their health condition and better understanding.Introdução: hipertensão arterial sistêmica (HAS) e Insuficiência cardíaca, epidemiologicamente são doenças modeladoras de consequências a outros sistemas do corpo humano, por exemplo a doença renal crônica (DRC). O desenvolvimento desta mostra-se como consequência social a falta de conhecimento, pois seus desfechos secundários são controláveis e tratáveis. Países como o Brasil, possuem gastos exorbitantes quando ao custeio de procedimentos de diálise e transplante, sendo observado uma crescente nestes números, principalmente em pacientes jovens descompensados das doenças de base. O objetivo deste trabalho são observar a incidência na literatura, sobre a HAS e insuficiência cardíaca em pacientes relacionadas ao agravo da DRC. Metodologia: estudo descritivo em revisão de narrativa, que procura responder ao acrômio PICO “Qual é a influência da hipertensão arterial sistêmica e da insuficiência cardíaca no agravo do quadro clínico de pacientes com doença renal crônica? ”. Discussão: DRC tem como fisiopatologia a perda da função renal, onde estes perdem a funcionalidade e destroem suas células especificas, resultando na incapacidade em manter o equilíbrio metabólico. Mostra-se uma mazela de responsabilidade pública, onde cada vez mais relados de morte na população são relatados. Os principais fatores de risco para a DRC são doenças crônicas de alta prevalência como HAS e insuficiência cardíaca, sendo a primeira a mais descrita na literatura como fator desencadeante. Assim resultando na piora dos resultados laboratoriais de função renal, resultando em uma injúria renal crônica (IRC). Resultados: Analisando as bases de dados, foi observado artigos nos últimos 10 anos, onde 38,6% tinham os descritores hipertensão Arterial sistêmica e insuficiência cardíaca, descrevendo como seu principal desfecho secundário. Conclusão: à sociedade científica, contribui com índices resumidos e atualizados relatando a relação entre estas patologias precursoras. À sociedade, informa sua problemática e uma maneira em informar o paciente sobre a sua condição de saúde e melhor compreensão

    Global disparities in surgeons’ workloads, academic engagement and rest periods: the on-calL shIft fOr geNEral SurgeonS (LIONESS) study

    : The workload of general surgeons is multifaceted, encompassing not only surgical procedures but also a myriad of other responsibilities. From April to May 2023, we conducted a CHERRIES-compliant internet-based survey analyzing clinical practice, academic engagement, and post-on-call rest. The questionnaire featured six sections with 35 questions. Statistical analysis used Chi-square tests, ANOVA, and logistic regression (SPSS® v. 28). The survey received a total of 1.046 responses (65.4%). Over 78.0% of responders came from Europe, 65.1% came from a general surgery unit; 92.8% of European and 87.5% of North American respondents were involved in research, compared to 71.7% in Africa. Europe led in publishing research studies (6.6 ± 8.6 yearly). Teaching involvement was high in North America (100%) and Africa (91.7%). Surgeons reported an average of 6.7 ± 4.9 on-call shifts per month, with European and North American surgeons experiencing 6.5 ± 4.9 and 7.8 ± 4.1 on-calls monthly, respectively. African surgeons had the highest on-call frequency (8.7 ± 6.1). Post-on-call, only 35.1% of respondents received a day off. Europeans were most likely (40%) to have a day off, while African surgeons were least likely (6.7%). On the adjusted multivariable analysis HDI (Human Development Index) (aOR 1.993) hospital capacity > 400 beds (aOR 2.423), working in a specialty surgery unit (aOR 2.087), and making the on-call in-house (aOR 5.446), significantly predicted the likelihood of having a day off after an on-call shift. Our study revealed critical insights into the disparities in workload, access to research, and professional opportunities for surgeons across different continents, underscored by the HDI

    Global Retinoblastoma Presentation and Analysis by National Income Level.

    Importance: Early diagnosis of retinoblastoma, the most common intraocular cancer, can save both a child's life and vision. However, anecdotal evidence suggests that many children across the world are diagnosed late. To our knowledge, the clinical presentation of retinoblastoma has never been assessed on a global scale. Objectives: To report the retinoblastoma stage at diagnosis in patients across the world during a single year, to investigate associations between clinical variables and national income level, and to investigate risk factors for advanced disease at diagnosis. Design, Setting, and Participants: A total of 278 retinoblastoma treatment centers were recruited from June 2017 through December 2018 to participate in a cross-sectional analysis of treatment-naive patients with retinoblastoma who were diagnosed in 2017. Main Outcomes and Measures: Age at presentation, proportion of familial history of retinoblastoma, and tumor stage and metastasis. Results: The cohort included 4351 new patients from 153 countries; the median age at diagnosis was 30.5 (interquartile range, 18.3-45.9) months, and 1976 patients (45.4%) were female. Most patients (n = 3685 [84.7%]) were from low- and middle-income countries (LMICs). Globally, the most common indication for referral was leukocoria (n = 2638 [62.8%]), followed by strabismus (n = 429 [10.2%]) and proptosis (n = 309 [7.4%]). Patients from high-income countries (HICs) were diagnosed at a median age of 14.1 months, with 656 of 666 (98.5%) patients having intraocular retinoblastoma and 2 (0.3%) having metastasis. Patients from low-income countries were diagnosed at a median age of 30.5 months, with 256 of 521 (49.1%) having extraocular retinoblastoma and 94 of 498 (18.9%) having metastasis. Lower national income level was associated with older presentation age, higher proportion of locally advanced disease and distant metastasis, and smaller proportion of familial history of retinoblastoma. Advanced disease at diagnosis was more common in LMICs even after adjusting for age (odds ratio for low-income countries vs upper-middle-income countries and HICs, 17.92 [95% CI, 12.94-24.80], and for lower-middle-income countries vs upper-middle-income countries and HICs, 5.74 [95% CI, 4.30-7.68]). Conclusions and Relevance: This study is estimated to have included more than half of all new retinoblastoma cases worldwide in 2017. Children from LMICs, where the main global retinoblastoma burden lies, presented at an older age with more advanced disease and demonstrated a smaller proportion of familial history of retinoblastoma, likely because many do not reach a childbearing age. Given that retinoblastoma is curable, these data are concerning and mandate intervention at national and international levels. Further studies are needed to investigate factors, other than age at presentation, that may be associated with advanced disease in LMICs

    Worldwide trends in underweight and obesity from 1990 to 2022: a pooled analysis of 3663 population-representative studies with 222 million children, adolescents, and adults

    Background Underweight and obesity are associated with adverse health outcomes throughout the life course. We estimated the individual and combined prevalence of underweight or thinness and obesity, and their changes, from 1990 to 2022 for adults and school-aged children and adolescents in 200 countries and territories. Methods We used data from 3663 population-based studies with 222 million participants that measured height and weight in representative samples of the general population. We used a Bayesian hierarchical model to estimate trends in the prevalence of different BMI categories, separately for adults (age ≥20 years) and school-aged children and adolescents (age 5–19 years), from 1990 to 2022 for 200 countries and territories. For adults, we report the individual and combined prevalence of underweight (BMI <18·5 kg/m2) and obesity (BMI ≥30 kg/m2). For schoolaged children and adolescents, we report thinness (BMI <2 SD below the median of the WHO growth reference) and obesity (BMI >2 SD above the median). Findings From 1990 to 2022, the combined prevalence of underweight and obesity in adults decreased in 11 countries (6%) for women and 17 (9%) for men with a posterior probability of at least 0·80 that the observed changes were true decreases. The combined prevalence increased in 162 countries (81%) for women and 140 countries (70%) for men with a posterior probability of at least 0·80. In 2022, the combined prevalence of underweight and obesity was highest in island nations in the Caribbean and Polynesia and Micronesia, and countries in the Middle East and north Africa. Obesity prevalence was higher than underweight with posterior probability of at least 0·80 in 177 countries (89%) for women and 145 (73%) for men in 2022, whereas the converse was true in 16 countries (8%) for women, and 39 (20%) for men. From 1990 to 2022, the combined prevalence of thinness and obesity decreased among girls in five countries (3%) and among boys in 15 countries (8%) with a posterior probability of at least 0·80, and increased among girls in 140 countries (70%) and boys in 137 countries (69%) with a posterior probability of at least 0·80. The countries with highest combined prevalence of thinness and obesity in school-aged children and adolescents in 2022 were in Polynesia and Micronesia and the Caribbean for both sexes, and Chile and Qatar for boys. Combined prevalence was also high in some countries in south Asia, such as India and Pakistan, where thinness remained prevalent despite having declined. In 2022, obesity in school-aged children and adolescents was more prevalent than thinness with a posterior probability of at least 0·80 among girls in 133 countries (67%) and boys in 125 countries (63%), whereas the converse was true in 35 countries (18%) and 42 countries (21%), respectively. In almost all countries for both adults and school-aged children and adolescents, the increases in double burden were driven by increases in obesity, and decreases in double burden by declining underweight or thinness. Interpretation The combined burden of underweight and obesity has increased in most countries, driven by an increase in obesity, while underweight and thinness remain prevalent in south Asia and parts of Africa. A healthy nutrition transition that enhances access to nutritious foods is needed to address the remaining burden of underweight while curbing and reversing the increase in obesit
