CĂLICE: SILĂNCIO OU RESISTĂNCIA?
A canção CĂĄlice (1973) de Chico Buarque e Gilberto Gil Ă© um dos exemplos de como o sujeito compositor utiliza as metĂĄforas para dizer aquilo que nĂŁo pode dizer. A linguagem musical sempre apresentou uma relação com o silĂȘncio, pois este Ă© carregado de significação. Este trabalho pretende analisar como o nĂŁo-dito estĂĄ associado ao silĂȘncio, sendo trabalhado atravĂ©s do efeito metafĂłrico que o sujeito compositor utiliza para disfarçar o seu dizer, ou seja, os sentidos silenciados e que geram efeitos de sentido variados no interlocutor. O trabalho utiliza como fundamentação teĂłrica a AnĂĄlise do discurso de linha francesa, tal como delineada por PĂȘcheux e desenvolvida, no Brasil, por Orlandi e seguidores. Ao analisarmos a canção CĂĄlice, estamos interessados em observar o silĂȘncio no sentido do nĂŁo dito, mas que Ă© dito camuflado atravĂ©s das metĂĄforas
Small cell carcinoma of the anus: a case report
Small cell carcinoma of the anus is a very rare but aggressive tumour. We present a case of a 60-year old lady with small cell carcinoma of the anus. She had no metastatic disease on presentation. She had chemotherapy and radiotherapy but developed distant metastasis after completion of treatment. Immunohistochemistry is required to make a diagnosis. Chemotherapy remains the mainstay of treatment for small cell carcinoma of the anus with or without metastatic disease. Radiotherapy is for local control and relief of symptoms

Consensus on circulatory shock and hemodynamic monitoring. Task force of the European Society of Intensive Care Medicine.
OBJECTIVE: Circulatory shock is a life-threatening syndrome resulting in multiorgan failure and a high mortality rate. The aim of this consensus is to provide support to the bedside clinician regarding the diagnosis, management and monitoring of shock.
METHODS: The European Society of Intensive Care Medicine invited 12 experts to form a Task Force to update a previous consensus (Antonelli et al.: Intensive Care Med 33:575-590, 2007). The same five questions addressed in the earlier consensus were used as the outline for the literature search and review, with the aim of the Task Force to produce statements based on the available literature and evidence. These questions were: (1) What are the epidemiologic and pathophysiologic features of shock in the intensive care unit ? (2) Should we monitor preload and fluid responsiveness in shock ? (3) How and when should we monitor stroke volume or cardiac output in shock ? (4) What markers of the regional and microcirculation can be monitored, and how can cellular function be assessed in shock ? (5) What is the evidence for using hemodynamic monitoring to direct therapy in shock ? Four types of statements were used: definition, recommendation, best practice and statement of fact.
RESULTS: Forty-four statements were made. The main new statements include: (1) statements on individualizing blood pressure targets; (2) statements on the assessment and prediction of fluid responsiveness; (3) statements on the use of echocardiography and hemodynamic monitoring.
CONCLUSIONS: This consensus provides 44 statements that can be used at the bedside to diagnose, treat and monitor patients with shock
From theoretical concepts to policies and applied programmes: the landscape of integration of oral health in primary care
Background: Despite its importance, the integration of oral health into primary care is still an emerging practice in
the field of health care services. This scoping review aims to map the literature and provide a summary on the
conceptual frameworks, policies and programs related to this concept.
Methods: Using the Levac et al. six-stage framework, we performed a systematic search of electronic databases,
organizational websites and grey literature from 1978 to April 2016. All relevant original publications with a focus
on the integration of oral health into primary care were retrieved. Content analyses were performed to synthesize
the results.
Results: From a total of 1619 citations, 67 publications were included in the review. Two conceptual
frameworks were identified. Policies regarding oral heath integration into primary care were mostly oriented
toward common risk factors approach and care coordination processes. In general, oral health integrated care
programs were designed in the public health sector and based on partnerships with various private and
public health organizations, governmental bodies and academic institutions. These programmes used various
strategies to empower oral health integrated care, including building interdisciplinary networks, training nondental
care providers, oral health champion modelling, enabling care linkages and care coordinated process,
as well as the use of e-health technologies. The majority of studies on the programs outcomes were
descriptive in nature without reporting long-term outcomes.
Conclusions: This scoping review provided a comprehensive overview on the concept of integration of oral
health in primary care. The findings identified major gaps in reported programs outcomes mainly because of
the lack of related research. However, the results could be considered as a first step in the development of
health care policies that support collaborative practices and patient-centred care in the field of primary care
sector
Primary health care as assessed by health professionals: comparison of the traditional model versus the Family Health Strategy
First report on dung beetles in intra-Amazonian savannahs in Roraima, Brazil
This is the first study to address the dung beetle (Coleoptera: Scarabaeidae: Scarabaeinae) diversity in intra-Amazonian savannahs in the state of Roraima, Brazil. Our aim was to survey the dung beetle fauna associated with these savannahs (regionally called 'lavrado'), since little is known about the dung beetles from this environment. We conducted three field samples using pitfall traps baited with human dung in savannah areas near the city of Boa Vista during the rainy seasons of 1996, 1997, and 2008. We collected 383 individuals from ten species, wherein six have no previous record in intra-Amazonian savannahs. The most abundant species were Ontherus appendiculatus (Mannerheim, 1829), Canthidium aff. humerale (Germar, 1813), Dichotomius nisus (Olivier, 1789), and Pseudocanthon aff. xanthurus (Blanchard, 1846). We believe that knowing the dung beetles diversity associated with the intra-Amazonian savannahs is ideal for understanding the occurrence and distribution of these organisms in a highly threatened environment, it thus being the first step towards conservation strategy development
New insights into the distribution and conservation status of the Golden-White Tassel-Ear Marmoset Mico chrysoleucos (Primates, Callitrichidae)
Among the 13 Mico species recognized by the IUCN Red List of Threatened Species, six are listed as "Data Deficient". The geographic range of most of the Mico species has been estimated from only a few records. We report new localities and the geographic extension of Mico chrysoleucos. In addition, we confirmed the presence of the species in two distinct protected areas. We modeled the habitat suitability of M. chrysoleucos using the maximum entropy method and including new records obtained by the authors in the state of Amazonas, Brazil. From the total area of occurrence calculated for the species, 22.8% is covered by protected areas and indigenous lands. The annual mean deforestation rate estimated between 2000 and 2015 was 2.95%, and the total area deforested by 2015 was 3354Â km2 or 8.6% of the total distribution limits of the species. The habitat lost between 2000 and 2015 was 3.2% (1131Â km2 ) of the total potential distribution, while the habitat loss area legally protected was 31Â km2, and the habitat loss in settlements was equal to 691Â km2. Our results extend the geographic distribution of the species about 100Â km farther south, with the MaracanĂŁ River being a possible geographic barrier for the species. The significantly low rate of habitat loss inside protected areas and indigenous land, when compared to unprotected areas, points out the importance of these areas to M. chrysoleucos conservation. The species is relatively wide-ranging, legally protected, and resilient to regional anthropic threats. However, the hydroelectric schemes and the improvement of the road system in southern Amazonia pose an imminent threat to the species
