    Prophylaxis and treatment of invasive fungal infections in hematological patients

    The evidence from the literature strongly support antifungal prophylaxis in high risk haematological patients, such as patients with AML during remission induction chemotherapy and alloHSCT patients. Current antifungal prophylaxis guidelines for high risk patients recommend azoles (fluconazole, posaconazole, voriconazole) and echinocandins (micafungin) with the strongest level of evidence. In terms of treatment, the choice between empiric therapy (or fever driven) and pre-emptive therapy (or diagnostic driven) is still debated. Not a single therapeutic strategy is appropriate in every patients, in particular empirical antifungal therapy may be recommended in patients at very high risk, while a pre-emptive approach may be advised for those at standard risk. In order to exploit the synergistic and/or additive effect of two antifungal drugs it's possible to combine two agents that work with different mechanisms of action (e.g. echinocandins + azoles or polyenes). Once the treatment has been initiated we should consider the therapeutic drug monitoring (TDM) of the drugs, especially when the pharmacokinetic variability is high and the dose-concentration effect relationships is not predictable (e.g. for itraconazole, voriconazole and posaconazole)

    Effect of Sibutramine on Weight Management and Metabolic Control in Type 2 Diabetes

    OBJECTIVE—The aim of this study was to provide a comprehensive meta-analysis of randomized controlled clinical studies on the effects of sibutramine on weight loss and glycemic control in obese subjects with type 2 diabetes. RESEARCH DESIGN AND METHODS—Controlled clinical trials assessing the effect sizes of sibutramine on weight loss effects on glycemia in obese subjects with type 2 diabetes were identified and reviewed using the Cochrane Library, Medline, EMBASE, and a manual search. RESULTS—Eight placebo-controlled, double-blind, randomized trials of sibutramine were included. After sibutramine treatment, the decrease in body weight and waist circumference was significantly greater than in the placebo group. Fasting blood glucose and HbA1c significantly decreased after sibutramine treatment. Treatment benefits were seen in plasma triglycerides and HDL, without significant variations in serum total and LDL cholesterol. No differences in systolic blood pressure between the sibutramine and the placebo groups were seen, while recording of diastolic blood pressure and heart rate showed that sibutramine produced a small increase relative to placebo. CONCLUSIONS—A pharmacological approach in a weight management program for patients with type 2 diabetes may be helpful in glycemic control and in the management of other risk factors. Sibutramine may help improve glucose control because it is conducive to weight loss. The reviewed data on the effect of sibutramine further enforce the recommendations that weight management may be the most important therapeutic task for most obese subjects with type 2 diabetes

    HOG-Dot: A Parallel Kernel-Based Gradient Extraction for Embedded Image Processing

    International audienceIn this paper we propose HOG-Dot, a method for the direct computation of the polar image gradients coordinates from the pixels values. The proposed algorithm, to be used as the first step of the Histogram of Oriented Gradient (HOG) pipeline, approximates the exact gradient with its projection onto a versor chosen among the projection plane set. Instead of non linear computations, the HOG-Dot method exploits linear operations while introducing a bounded approximation error with respect to other HOG approaches, thus resulting a more suitable solution for embedded devices. Concerning the state of the art, it also achieves improved accuracy with the mathematical spatial gradient formulation

    Parallel Image Gradient Extraction Core For FPGA-based Smart Cameras

    International audienceOne of the biggest efforts in designing pervasive Smart Camera Networks (SCNs) is the implementation of complex and computationally intensive computer vision algorithms on resource constrained embedded devices. For low-level processing FPGA devices are excellent candidates because they support massive and fine grain data parallelism with high data throughput. However, if FPGAs offers a way to meet the stringent constraints of real-time execution, their exploitation often require significant algorithmic reformulations. In this paper, we propose a reformulation of a kernel-based gradient computation module specially suited to FPGA implementations. This resulting algorithm operates on-the-fly, without the need of video buffers and delivers a constant throughput. It has been tested and used as the first stage of an application performing extraction of Histograms of Oriented Gradients (HOG). Evaluation shows that its performance and low memory requirement perfectly matches low cost and memory constrained embedded devices

    Scanning electron microscopic analysis of the efficacy of acid etching on cat enamel

    The effect of etching on cat enamel made with 40% orthophosphoric acid for different times was evaluated. Twenty-one cat teeth were selected and randomly divided into three groups of seven teeth each. They were subjected to etching on a circular area of the coronal enamel (diameter = 2 mm) for 30 s (group A), 45 s (group B) and 60 s (group C). The samples obtained were observed by a scanning electron microscope focusing on the border area between etched and unetched enamel, to highlight the differences. The micrographs were subjected to blind assessment of three experienced operators. The groups were statistically assessed with the Wilcoxon test. At 30, 45 and 60 s, the acid attack results only in the formation of an irregular enamel surface and without uncovering and attack of the prismatic organisation. Prismatic areas with preferential interprismatic action could be detected in few samples etched for 60 s. Analysis with ImageJ was also used to quantify the efficacy of acid etching in the conditions used for human enamel, by an evaluation of grey levels. In cat enamel the etching times considered are not as effective as in human enamel for the purpose of adhesion and the presence of a thick prismless layer could explain this result

    Neurodegeneration in Alzheimer Disease: Role of Amyloid Precursor Protein and Presenilin 1 Intracellular Signaling

    Alzheimer disease (AD) is a heterogeneous neurodegenerative disorder characterized by (1) progressive loss of synapses and neurons, (2) intracellular neurofibrillary tangles, composed of hyperphosphorylated Tau protein, and (3) amyloid plaques. Genetically, AD is linked to mutations in few proteins amyloid precursor protein (APP) and presenilin 1 and 2 (PS1 and PS2). The molecular mechanisms underlying neurodegeneration in AD as well as the physiological function of APP are not yet known. A recent theory has proposed that APP and PS1 modulate intracellular signals to induce cell-cycle abnormalities responsible for neuronal death and possibly amyloid deposition. This hypothesis is supported by the presence of a complex network of proteins, clearly involved in the regulation of signal transduction mechanisms that interact with both APP and PS1. In this review we discuss the significance of novel finding related to cell-signaling events modulated by APP and PS1 in the development of neurodegeneration

    Error Resilient Video Streaming with BCH Code Protection in Wireless Sensor Networks

    Video streaming in Wireless Sensor Networks (WSNs) is a promising and challenging application for enabling high-value services. In such a context, the reduced amount ofavailable bandwidth, as well as the low-computational power available for acquiring and processing video frames, imposes the transmission of low resolution images at a low frame rate. Considering the aforementioned limitations, the amount of information carried by each video frame must be considered of utmost importance and preserved, as much as possible, against network losses that could introduce possible artifacts in the reconstructed dynamics of the scene.In this paper we first evaluate the impact of the bit error rate on the quality of the received video stream in a real scenario, then we propose a forward error correction technique based on the use of BCH codes with the aim of preserving the video quality. The proposed technique, against already proposed techniques in the WSN research field, has been specially designed to maintain a full back-compatibility with the IEEE802.15.4 standard in order to create a suitable solution aiming at accomplishing the Internet of Things (IoT) vision. Performance results evaluated in terms of Peak Signal-to-Noise Ratio (PSNR) show that the proposed solution reaches a PSNR improvement of 4.16 dB with respect to an unprotected transmission, while requiring an additional overhead equal to 22.51% in number of transmitted bits, and minimal impact on frame rate reduction and energy consumption. When higher protection levels have been imposed, bigger PSNR values have been experienced at the cost of an increased additional overhead, lower frame rates, and bigger energy consumption values
