    In Europe, pulmonary histoplasmosis is rarely diagnosed except in travelers. We report a probable autochthonous case of severe chronic pulmonary histoplasmosis in an immunocompetent man in Switzerland without travel history outside of Europe. Diagnosis was achieved by histopathology, fungal culture, and serology, but the source of the infection remains speculative

    <p>Abstract</p> <p>Background</p> <p>Angiogenesis is a common finding in chronic inflammatory diseases; however, its role in multiple sclerosis (MS) is unclear. Central nervous system lesions from both MS and experimental autoimmune encephalomyelitis (EAE), the animal model of MS, contain T cells, macrophages and activated glia, which can produce pro-angiogenic factors. Previous EAE studies have demonstrated an increase in blood vessels, but differences between the different phases of disease have not been reported. Therefore we examined angiogenic promoting factors in MS and EAE lesions to determine if there were changes in blood vessel density at different stages of EAE.</p> <p>Methods</p> <p>In this series of experiments we used a combination of vascular casting, VEGF ELISA and immunohistochemistry to examine angiogenesis in experimental autoimmune encephalomyelitis (EAE). Using immunohistochemistry we also examined chronic active MS lesions for angiogenic factors.</p> <p>Results</p> <p>Vascular casting and histological examination of the spinal cord and brain of rats with EAE demonstrated that the density of patent blood vessels increased in the lumbar spinal cord during the relapse phase of the disease (p < 0.05). We found an increased expression of VEGF by inflammatory cells and a decrease in the recently described angiogenesis inhibitor meteorin. Examination of chronic active human MS tissues demonstrated glial expression of VEGF and glial and blood vessel expression of the pro-angiogenic receptor VEGFR2. There was a decreased expression of VEGFR1 in the lesions compared to normal white matter.</p> <p>Conclusions</p> <p>These findings reveal that angiogenesis is intimately involved in the progression of EAE and may have a role in MS.</p

    How Technical Innovations May Help to Prevent Drug Shortages in Switzerland

    In this work, we investigated the technical feasibility of 'on-demand' production of selected drugs to cover their demand for a time window of 90 days. We focused on two sub-processes 'automated chemical synthesis' and 'formulation in micropellets'  to enable personalized dosing. The production of drugs 'on-demand' is challenging, important, but also attractive. Switzerland could thus gain access to an additional instrument for increasing resilience for supply-critical drugs. The biggest challenge in the case study presented here is the scalability of automated chemical synthesis and the application range of micropellet formulations

    Effect of feeding Alphitobius diaperinus meal on fattening performance and meat quality of growing-finishing pigs

    A total of 48 piglets with an average weight of 26 kg were allocated to 4 experimental groups of 12 animals, balanced according to litter, sex and weight, and fattened on feed containing 0, 3, 6, or 9% of Alphitobius diaperinus meal (ADM) replacing soybean meal (SOY) as protein source. The control feed contained 10.7% SOY while in the 9% ADM feed SOY was completely replaced. Feed was accessible ad libitum in transponder-controlled feeders. Feed consumption and fattening performance records started when the animals reached 35 kg. The 3-way crossbred animals (Landrace x Large White sows mated to Duroc, Pietrain, or Large White sire line bores) were slaughtered at a target carcass weight of 86 kg. No linear effect of ADM on daily gain and feed consumption was found. No effect on lean meat content nor on any of the meat quality traits was observed. The content of polyunsaturated fatty acids (PUFA) in the backfat increased with increasing amount of ADM in the feed. It is concluded that ADM may replace SOY in pig feed without exerting detrimental effects on growth performance, carcass composition and meat quality except for a higher PUFA-content in the adipose tissue

    Nanofiber Fabry-Perot microresonator for non-linear optics and cavity quantum electrodynamics

    We experimentally realize a Fabry-Perot-type optical microresonator near the cesium D2 line wavelength based on a tapered optical fiber, equipped with two fiber Bragg gratings which enclose a sub-wavelength diameter waist. Owing to the very low taper losses, the finesse of the resonator reaches F = 86 while the on-resonance transmission is T = 11 %. The characteristics of our resonator fulfill the requirements of non-linear optics and cavity quantum electrodynamics in the strong coupling regime. In combination with its demonstrated ease of use and its advantageous mode geometry, it thus opens a realm of applications.Comment: 4 pages, 3 figure

    All-optical switching and strong coupling using tunable whispering-gallery-mode microresonators

    We review our recent work on tunable, ultrahigh quality factor whispering-gallery-mode bottle microresonators and highlight their applications in nonlinear optics and in quantum optics experiments. Our resonators combine ultra-high quality factors of up to Q = 3.6 \times 10^8, a small mode volume, and near-lossless fiber coupling, with a simple and customizable mode structure enabling full tunability. We study, theoretically and experimentally, nonlinear all-optical switching via the Kerr effect when the resonator is operated in an add-drop configuration. This allows us to optically route a single-wavelength cw optical signal between two fiber ports with high efficiency. Finally, we report on progress towards strong coupling of single rubidium atoms to an ultra-high Q mode of an actively stabilized bottle microresonator.Comment: 20 pages, 24 figures. Verschiedene nationale und internationale Fachgesellschaften sowie die WHO haben Konsensuspapere oder Leitlinien zur Indikationsstellung und Befundung erstellt. Anhand des WHO-Guides sollen einige dieser Empfehlungen reflektiert und mit eigenen Erfahrungen kommentiert werden

    Multi effect plants and ionic liquids for improved absorption chillers

To improve this technology, different working pairs as well as plant designs are investigated using the simulation tool AspenPlus. The simulation is validated by comparing the results of single effect absorption chillers using the current commercially applied working pairs water/lithium bromide and ammonia/water with literature data. To increase the efficiency, double effect absorption chillers are implemented and analyzed. The performance of two kinds of double effect cycles, series and parallel, is compared using the working pair water/lithium bromide. In addition, ionic liquids (ILs) are investigated as a sorbent in order to improve the technology. So far, ILs have not been implemented in AspenPlus yet. Therefore, a guideline for the implementation of ILs in AspenPlus is outlined and the accordant phase equilibria results are validated with literature data. Simulations of single effect cycles using the ILs 1,3-dimethylimidazolium dimethylphosphate ([MMIM][DMP]) and 1-ethyl-3-methylimidazolium dimethylphosphate ([EMIM][DMP]) in combination with water as a refrigerant are performed and the results are compared to conventional working pairs.dc201

    A prognostic score for non-small cell lung cancer resected after neoadjuvant therapy in comparison with the tumor-node-metastases classification and major pathological response.

    Studies validating the prognostic accuracy of the tumor-node-metastases (TNM) classification in patients with lung cancer treated by neoadjuvant therapy are scarce. Tumor regression, particularly major pathological response (MPR), is an acknowledged prognostic factor in this setting. We aimed to validate a novel combined prognostic score. This retrospective single-center study was conducted on 117 consecutive patients with non-small cell lung cancer resected after neoadjuvant treatment at a Swiss University Cancer Center between 2000 and 2016. All cases were clinicopathologically re-evaluated. We assessed the prognostic performance of a novel prognostic score (PRSC) combining T-category, lymph node status, and MPR, in comparison with the eighth edition of the TNM classification (TNM8), the size adapted TNM8 as proposed by the International Association for the Study of Lung Cancer (IASLC) and MPR alone. The isolated ypT-category and the combined TNM8 stages accurately differentiated overall survival (OS, stage p = 0.004) and disease-free survival (DFS, stage p = 0.018). Tumor regression had a prognostic impact. Optimal cut-offs for MPR emerged as 65% for adenocarcinoma and 10% for non-adenocarcinoma and were statistically significant for survival (OS p = 0.006, DFS p &lt; 0.001). The PRSC differentiated between three prognostic groups (OS and DFS p &lt; 0.001), and was superior compared to the stratification using MPR alone or the TNM8 systems, visualized by lower Akaike (AIC) and Bayesian information criterion (BIC) values. In the multivariate analyses, stage III tumors (HR 4.956, p = 0.003), tumors without MPR (HR 2.432, p = 0.015), and PRSC high-risk tumors (HR 5.692, p &lt; 0.001) had significantly increased risks of occurring death. In conclusion, we support 65% as the optimal cut-off for MPR in adenocarcinomas. TNM8 and MPR were comparable regarding their prognostic significance.
