Reestructuraciones empresariales y medidas de flexibilidad interna: modificación sustancial e inaplicación de condiciones de trabajo
El presente trabajo tiene por objeto analizar con detalle la incidencia laboral que tiene la figura de la reestructuración empresarial. Esta institución se configura como una medida de cambio sustancial empresarial, que tiene una gran incidencia en las diferentes ramas del derecho. Dicho análisis se va a realizar partiendo de la vertiente laboral de la reestructuración empresarial y, más concretamente, desde dos figuras específicas del derecho laboral, que son dos ejemplos perfectos de reestructuración. Estas dos figuras son la modificación sustancial de condiciones de trabajo, del artículo 41 ET, y la inaplicación de las condiciones de trabajo, que encontramos en el artículo 82.3 ET. Se llevará a cabo un estudio sobre la configuración actual de ambas figuras y su incidencia en el ámbito laboral.This work is aimed at assessing in detail the labor incidence of the figure of the corporate restructuring. The corporate restructuring is configured as a measure of substantial business change, which has an impact in different ways of law. This assessment has been undertaken from the labor aspect of the corporate restructuring and, more specifically, from two concrete figures of labor law that are two perfect examples of restructuring. These two figures are the substantial modification of working conditions, article 41 ET, and the non-application of working conditions, which we find in article 82.3 ET. Filtros de luz azul en la práctica optométrica
Revisión bibliográfica sobre los efectos de la luz azul en las distintas estructuras oculares y sobre la visión y calidad de vida. Se han consultado diversas bases de datos y páginas oficiales para conocer la información publicada hasta la fecha sobre la luz azul y los límites de exposición recomendados y esclarecer qué papel tienen los dispositivos electrónicos actuales en la emisión de luz azul nociva.Se han analizado los estudios encontrados acerca de la eficacia de diferentes tipos de filtros de luz azul. Además, 374 sujetos han participado en una encuesta sobre el uso de diferentes ayudas ópticas contra la luz azul, lo que permitirá conocer la opinión de los usuarios sobre el efecto que producen en ellos las ayudas.Los estudios demuestran que la luz azul puede producir daño ocular fotoquímico, sin embargo, las emisiones de los dispositivos electrónicos no son suficientes para provocar daños. Se ha encontrado que la luz azul podría tener efectos beneficiosos en el control de miopía. Los filtros azules comerciales no evitan el daño, pero algunos parece que podrían ser eficaces en el control del ritmo circadiano.<br /
Regular consumption of LIPIGO® promotes the reduction of body weight and improves the rebound effect of obese people undergo a comprehensive weight loss program
Obesity is a global public health problem. Objective: To evaluate the effect of the regular consumption of the product Lipigo® on body weight and rebound effect on overweight/obese subjects undergoing a comprehensive weight loss program. Methods: A randomized, parallel, double-blind, placebo-controlled clinical trial was conducted with male and female subjects presenting a BMI 25–39.9 kg/m . All subjects underwent a comprehensive weight loss program (WLP) for 12 weeks, which included an individualized hypocaloric diet, physical activity recommendations, nutritional education seminars, and three times a day consumption of the product Lipigo® or Placebo. After-WLP, subjects continued the treatment for 9 months to assess rebound effect. Body weight (BW), BMI, and body composition were measured at the beginning and the end of the WLP, and in the follow-up. Results: A total of 120 subjects (85% women) 49.0 ± 9.5 years old and with a BW of 81.57 ± 13.26 kg (BMI 31.19 ± 3.44 kg/m ) were randomized and 73 subjects finished the study. At the end of the WLP, there was a tendency toward reduced BW (p = 0.093), BMI (p = 0.063), and WC (p = 0.059) in the treated group. However, subjects with obesity type 1 (OB1) from the treated group significantly reduced body weight (−5.27 ± 2.75 vs. −3.08 ± 1.73 kg; p = 0.017) and BMI (−1.99 ± 1.08 vs. −1.09 ± 0.55 kg/m ; p = 0.01) compared with placebo. They also presented a minor rebound effect after 9 months with product consumption (−4.19 ± 3.61 vs. −1.44 ± 2.51 kg; p = 0.026), minor BMI (−1.61 ± 1.43 vs. −0.52 ± 0.96 kg/m ; p = 0.025) and tended to have less fat-mass (−3.44 ± 2.46 vs. −1.44 ± 3.29 kg; p = 0.080) compared with placebo. Cadaveric and Ultrasound Validation of Percutaneous Electrolysis Approaches at the Arcade of Frohse: A Potential Treatment for Radial Tunnel Syndrome
Entrapment of the radial nerve at the arcade of Frohse could contribute to symptoms in patients with lateral epicondylalgia or radial tunnel syndrome. Our aim was to determine the validity of applying percutaneous electrolysis, targeting the supinator muscle at the Frohse’s arcade with ultrasound imaging and in a Thiel-embalmed cadaver model (not ultrasound-guiding). Percutaneous electrolysis targeting the supinator muscle was conducted in five healthy volunteers (ultrasound study) and three Thiel-embalmed cadaver forearms. Two approaches, one with the forearm supinated and other with the forearm pronated were conducted. The needle was inserted until the tip reached the interphase of both bellies of the supinator muscle. Accurate needle penetration of the supinator muscle was observed in 100% in both US-imaging and cadaveric studies. No neurovascular bundle of the radial-nerve deep branch was pierced in any insertion. The distance from the tip of the needle to the neurovascular bundle was 15.3 ± 0.6 mm with the forearm supinated, and 11.2 ± 0.6 mm with the forearm pronated. The results of the current study support that percutaneous electrolysis can properly target the supinator muscle with either the forearm in supination or in pronation. In fact, penetration of the neurovascular bundle was not observed in any approach when percutaneous needling electrolysis was performed by an experienced clinician
Dolor lumbar persistente tras la administración de [131I]Iodo-6-β-iodometil-19-norcolesterol: a propósito de un caso
The [131I]Iodine-6-β-iodomethyl-19-norcholesterol ([131I]Norcholesterol; NP-59), is indicated in the diagnosis of the functional state of adrenal cortical tissue, in the detection of functional tissue in hyperadrenocorticalism as well as in primary aldosteronism. We present the case of a 54-year-old woman, referred for evaluation of suspected aldosteronism, and who immediately after intravenous administration of [131I]Norcholesterol suffered a strong adverse drug reaction, with dizziness, flushing, increased blood pressure, chest tightness, low back pain and erythematous rash up to 9 days after injection. Symptoms resolved satisfactorily after administration of antihistamines and corticosteroids. This reaction was suspected to be related to the administration of [131I]Norcholesterol caused by a type I allergic reaction.El [131I]Iodo-6-β-iodometil-19-norcolesterol ([131I]Norcolesterol), radiofármaco indicado en el diagnóstico del estado funcional de tejido cortical suprarrenal, en la detección del tejido funcional en el hiperadrenocorticalismo así como en el aldosteronismo primario. Presentamos el caso de una mujer de 54 años de edad, remitida para la evaluación de una sospecha de aldosteronismo, y que inmediatamente después de la administración intravenosa de [131I]Norcolesterol sufrió una fuerte reacción adversa al medicamento, con mareo, rubor, aumento de la presión arterial, opresión en el pecho, dolor lumbar y sarpullido eritematoso hasta 9 días postinyección. Los síntomas se resolvieron satisfactoriamente tras la administración de antihistamínicos y corticoides. Se sospechó que esta reacción estuviera relacionada con la administración del [131I]Norcolesterol causada por una reacción alérgica de tipo I
Asthma outcomes improve with continuous positive airway pressure for obstructive sleep apnea
Continuous positive airway pressure () in asthma patients with concomitant obstructive sleep apnea syndrome () seems to have a favorable impact on asthma, but data are inconsistent due to methodological limitations of previous studies. Prospective, multicenter study. We examined asthma outcomes after 6 months of in 99 adult asthma patients (mean age 57 years) with (respiratory disturbance index ≥20). Asthma control and quality of life were assessed with the Asthma Control Questionnaire () and the Mini Asthma Quality of Life Questionnaire (Mini), respectively. Data were analyzed by intention-to-treat basis. The mean ± score of the decreased from 1.39 ± 0.91 at baseline to 1.0 ± 0.78 at 6 months (P = 0.003), the percentage of patients with uncontrolled asthma from 41.4% to 17.2% (P = 0.006), and the percentage of patients with asthma attacks in the 6 months before and after treatment from 35.4% to 17.2% (P = 0.015). The score of the increased from 5.12 ± 1.38 to 5.63 ± 1.17 (P = 0.009). There were also significant improvements in symptoms of gastroesophageal reflux and rhinitis, bronchial reversibility, and exhaled nitric oxide values (all P < 0.05). No significant changes were observed in drug therapy for asthma or their comorbidities nor in the patients' weight. Asthma control (both actual and future risk), quality of life, and lung function improved after starting continuous positive airway pressure in asthmatics with moderate to severe obstructive sleep apnea syndrome

Restoration of Vitamin D Levels Improves Endothelial Function and Increases TASK-Like K+ Currents in Pulmonary Arterial Hypertension Associated with Vitamin D Deficiency
Vitamin D (vitD) deficiency is highly prevalent in patients with pulmonary arterial hypertension (PAH). Moreover, PAH-patients with lower levels of vitD have worse prognosis. We hypothesize that recovering optimal levels of vitD in an animal model of PAH previously depleted of vitD improves the hemodynamics, the endothelial dysfunction and the ionic remodeling. Methods: Male Wistar rats were fed a vitD-free diet for five weeks and then received a single dose of Su5416 (20 mg/Kg) and were exposed to vitD-free diet and chronic hypoxia (10% O2) for three weeks to induce PAH. Following this, vitD deficient rats with PAH were housed in room air and randomly divided into two groups: (a) continued on vitD-free diet or (b) received an oral dose of 100,000 IU/Kg of vitD plus standard diet for three weeks. Hemodynamics, pulmonary vascular remodeling, pulmonary arterial contractility, and K+ currents were analyzed. Results: Recovering optimal levels of vitD improved endothelial function, measured by an increase in the endothelium-dependent vasodilator response to acetylcholine. It also increased the activity of TASK-1 potassium channels. However, vitD supplementation did not reduce pulmonary pressure and did not ameliorate pulmonary vascular remodeling and right ventricle hypertrophy. Conclusions: Altogether, these data suggest that in animals with PAH and severe deficit of vitD, restoring vitD levels to an optimal range partially improves some pathophysiological features of PAH
Heme oxygenase-1 and 2 common genetic variants and risk for restless legs syndrome
Varios neurotransmisores, neuropatológicos, neuroimagen, y los datos experimentales, sugieren que la deficiencia de hierro juega un papel importante en la fisiopatología del síndrome de piernas inquietas (RLS). HMOX (Hemeoxygenases) es un importante mecanismo de defensa contra el estrés oxidativo, principalmente a través de la degradación del hemo a biliverdin, libre de hierro, y monóxido de carbono. Hemos analizado si los genes HMOX1 y HMOX2 están relacionados con el riesgo de desarrollar el síndrome de piernas inquietas. Se analizó la distribución de genotipos y las frecuencias alélicas de los HMOX1 rs2071746, HMOX1 rs2071747, HMOX2 rs2270363 y rs1051308 HMOX2 SNPs, así como la presencia de variaciones de número de copia (CNVs) de estos genes en 205 sujetos RLS y 445 controles sanos. Las frecuencias de rs2071746 genotipo TT y R2071746T variante alélica fueron significativamente inferiores en los pacientes de SPI que en controles, aunque los otros 3 SNPs estudiados RLS no difirió entre pacientes y controles. Ninguno de los polimorfismos estudiados influyeron en el inicio de la enfermedad, la gravedad de la RLS, historia familiar de SPI, la ferritina sérica, o respuesta a agonistas dopaminérgicos, clonazepam o GABAergic drogas. El presente estudio sugiere una débil asociación entre el polimorfismo rs2071746 HMOX1 y el riesgo de desarrollar el SPI en la población española.Several neurochemical, neuropathological, neuroimaging, and experimental data, suggest that iron deficiency plays an important role in the pathophysiology of restless legs syndrome (RLS). Hemeoxygenases (HMOX) are an important defensive mechanism against oxidative stress, mainly through the degradation of heme to biliverdin, free iron, and carbon monoxide. We analyzed whether HMOX1 and HMOX2 genes are related with the risk to develop RLS. We analyzed the distribution of genotypes and allelic frequencies of the HMOX1 rs2071746, HMOX1 rs2071747, HMOX2 rs2270363, and HMOX2 rs1051308 SNPs, as well as the presence of Copy number variations (CNVs) of these genes in 205 subjects RLS and 445 healthy controls. The frequencies of rs2071746TT genotype and rs2071746T allelic variant were significantly lower in RLS patients than that in controls, although the other 3 studied SNPs did not differ between RLS patients and controls. None of the studied polymorphisms influenced the disease onset, severity of RLS, family history of RLS, serum ferritin levels, or response to dopaminergic agonist, clonazepam or GABAergic drugs. Association between vitamin D receptor rs731236 (Taq1) polymorphism and risk for restless legs syndrome in the Spanish caucasian population
Varios trabajos recientes sugieren un posible papel de la deficiencia de vitamina D en la etiología o el síndrome de las piernas inquietas (RLS). Hemos analizado la posible relación de 2 polimorfismos de un solo nucleótido (SNP) en el receptor de la vitamina D3 (GEN VDR) con el riesgo de SPI. Hemos estudiado la variante alélica genotipo y frecuencias de VDR rs2228570 y rs731236 VDR SNPs en 205 RLS pacientes y 445 controles sanos mediante un ensayo TaqMan. Las frecuencias de los rs731236AAgenotype y la variante alélica rs731236un SPI fue significativamente inferior en los pacientes que en los controles (P<0,005 y 0,01, respectivamente). El síndrome de las piernas inquietas pacientes portadoras de la variante alélica rs731236G había una edad temprana en el inicio, y los portadores del genotipo GG731236rs tuvieron mayor severidad de RLS, aunque estos datos desaparecieron después de los análisis multivariados. Ninguno de los SNPs estudiados estaba relacionada con la positividad de la historia familiar de SPI. Estos resultados sugieren una modesta, pero significativa asociación entre rs731236 VDR SNP y el riesgo de síndrome de piernas inquietas.Several recent works suggest a possible role of vitamin D deficiency in the etiology or restless legs syndrome (RLS). We analyzed the possible relationship of 2 common single nucleotide polymorphisms (SNPs) in the vitamin D3 receptor (VDR) gene with the risk for RLS. We studied the genotype and allelic variant frequencies of VDR rs2228570 and VDR rs731236 SNPs in 205 RLS patients and 445 healthy controls using a TaqMan essay. The frequencies of the rs731236AAgenotype and the allelic variant rs731236A were significantly lower in RLS patients than in controls (P<0.005 and<0.01, respectively). Restless legs syndrome patients carrying the allelic variant rs731236G had an earlier age at onset, and those carrying the rs731236GG genotype had higher severity of RLS, although these data disappeared after multivariate analyses. None of the SNPs studied was related with the positivity of family history of RLS.
