    Intracellular symbiosis is very common in the insect world. For the aphid Cinara cedri, we have identified by electron microscopy three symbiotic bacteria that can be characterized by their different sizes, morphologies, and electrodensities. PCR amplification and sequencing of the 16S ribosomal DNA (rDNA) genes showed that, in addition to harboring Buchnera aphidicola, the primary endosymbiont of aphids, C. cedri harbors a secondary symbiont (S symbiont) that was previously found to be associated with aphids (PASS, or R type) and an alpha-proteobacterium that belongs to the Wolbachia genus. Using in situ hybridization with specific bacterial probes designed for symbiont 16S rDNA sequences, we have shown that Wolbachia was represented by only a few minute bacteria surrounding the S symbionts. Moreover, the observed B. aphidicola and the S symbionts had similar sizes and were housed in separate specific bacterial cells, the bacteriocytes. Interestingly, in contrast to the case for all aphids examined thus far, the S symbionts were shown to occupy a similarly sized or even larger bacteriocyte space than B. aphidicola. These findings, along with the facts that C. cedri harbors the B. aphidicola strain with the smallest bacterial genome and that the S symbionts infect all Cinara spp. analyzed so far, suggest the possibility of bacterial replacement in these species

    Bisphosphonate-associated osteonecrosis of the jaw : a proposal for conservative treatment

    The use of bisphosphonates (BPs) has proven effective in the treatment of bone-related diseases, despite the potential risk of developing osteonecrosis of the jaw (ONJ). So far, results for the treatment of ONJ have not been satisfactory. In our study, we have treated two patients diagnosed with ONJ. In addition to local treatments a hydrogel was applied, with promising results. The fistulae disappeared 2-3 weeks into the treatment. After a six-month follow-up period there has been no sign of recurrence. The extent of maxillary bone exposure has diminished notably, although not entirely disappeared. In all cases, the patients exhibit no other symptoms (they suffer from no pain or swelling or functional impotence). For this reason we believe this protocol might be useful in the case of patients who suffer from pain and fistulation secondary to BP-associated ONJ to improve the state of their lesions until definitive treatment can be undertaken. Although these findings are not conclusive, given that we are reporting data on two patients only, we believe that this might be an alternative treatment in refractory cases where other therapies are counter-indicated. A controlled randomized and prospective study would be required to confirm our findings

Para su realización, se han seleccionado 32 pacientes (26 mujeres y 6 hombres) diagnosticados de cervicoartrosis. Se han valorado factores como el dolor y el balance articular.     Familial pulmonary fibrosis is the strongest risk factor for idiopathic pulmonary fibrosis

    SummaryIdiopathic pulmonary fibrosis (IPF) is a lethal lung disorder of unknown etiology. The disease is likely the result of complex interactions between genetic and environmental factors. Evidence suggests that certain environmental factors, such as cigarette smoking and metal dust exposures, or comorbidities like gastroesophageal reflux, and type 2 diabetes mellitus (DM2) may increase risk to develop IPF. Substantial uncertainty remains, however, regarding these and other putative risk factors for IPF. In this study we performed a case–control analysis including 100 patients with IPF and 263 controls matched for age sex and place of residence. We used a structured questionnaire to identify potential risk factors for IPF, including environmental and occupational exposures as well as the relevance of family history of pulmonary fibrosis. The multivariate analysis revealed that family history of pulmonary fibrosis [OR = 6.1, CI95% 2.3–15.9; p < 0.0001] was strongly associated with increased risk of IPF. Actually, 20% of the cases reported a parent or sibling with pulmonary fibrosis. Gastroesophageal reflux [OR = 2.9, CI: 1.3–6.6; p = 0.007], former cigarette smoking [OR = 2.5, CI: 1.4–4.6, p = 0.003], and past or current occupational exposure to dusts, smokes, gases or chemicals [OR = 2.8, CI: 1.5–5.5; p = 0.002] were also associated with the disease. Despite being a significant risk factor on univariate analysis DM2 was not significant in multivariate analysis. These findings indicate that family history of pulmonary fibrosis is a strong risk factor for IPF. Also, we confirmed that occupational exposures, gastroesophageal reflux and former smoking increase the risk for this disease

    The efficacy of acupuncture and decompression splints in the treatment of temporomandibular joint pain-dysfunction syndrome

    Objectives: The goal of the present study was to evaluate the results of applying acupuncture or occlusal decompression splints in the treatment of patients diagnosed with the temporomandibular joint pain-dysfunction syndrome. Design of the study: We conducted a randomized clinical trial including 20 patients to whom the mentioned treatments were applied. Results were evaluated through an analogue pain scale, measurements of mouth opening and jaw lateral deviation in millimetres, and assessment of sensitivity to pressure on different points: preauricular, masseter muscle, temporal muscle and trapezius. Parameters were evaluated before and 30 days after the treatment. For standardized pressure, we used a pressure algometer. Results: Patients treated with decompression splints showed reductions in subjective pain and pain upon pressure on temporal, masseter and trapezius muscles, as well as increased mouth opening after the treatment. Patients treated with acupuncture showed pain reduction in the short term and improvements in all of the evaluated para- meters (stronger pressure was required to produce pain; mouth opening was improved). Conclusion: Acupuncture was an effective complement and/or an acceptable alternative to decompression splints in the treatment of myofascial pain and temporomandibular joint pain-dysfunction syndrome

    Abscess of the pontine dorsolateral tegmentum due to E. coli and influenza A(H1N1)pdm09 virus co-infection

    Los abscesos del tronco del encéfalo son entidades raras que suelen afectar a la protuberancia. Los microorganismos implicados son muy variados en función del foco de diseminación. Presentamos el caso de una paciente pluripatológica que desarrolló una neumonía multilobar secundaria a virus de la gripe A (H1N1)pdm09 en el postoperatorio de una colecistectomía urgente, requiriendo ventilación mecánica. Tras mejorar clínicamente y proceder a la desintubación, la paciente no recuperó el nivel de conciencia. Posteriormente, mediante pruebas de imagen se halló un absceso en el tegmento dorsolateral pontino izquierdo, región recientemente asociada al coma. Pese a administrar cobertura antibiótica amplia, la paciente falleció cinco días después. Este caso ilustra la importancia de considerar el absceso pontino como causa de mala evolución neurológica en pacientes críticos y la necesidad de realizar pruebas de imagen para descartar lesiones intracraneales, especialmente en regiones asociadas a coma.Brain stem abscesses are rare entities that predominantly affect the pons. A wide variety of microorganisms may be implied, depending on the focus of dissemination. We present the case of a female patient with multiple comorbidities who developed multi-lobar pneumonia due to influenza A(H1N1)pdm09 virus during the postoperative period after an emergent cholecystectomy, requiring mechanical ventilation (MV). Following clinical improvement and withdrawal of MV, the patient did not recover consciousness. Forty-eight hours later, imaging exams showed an abscess in the left pontine dorsolateral tegmentum –a region recently associated with coma-. Despite the administration of broad-spectrum antibiotics, the patient died five days later. This case illustrates the importance of considering pontine abscesses as a cause of poor neurological course in critically ill patients, as well as the need for doing imaging exams to rule out intracranial lesions, particularly in coma-associated areas
