    Problem gambling: a suitable case for social work?

    Problem gambling attracts little attention from health and social care agencies in the UK. Prevalence surveys suggest that 0.6% of the population are problem gamblers and it is suggested that for each of these individuals, 10–17 other people, including children and other family members, are affected. Problem gambling is linked to many individual and social problems including: depression, suicide, significant debt, bankruptcy, family conflict, domestic violence, neglect and maltreatment of children and offending. This makes the issue central to social work territory. Yet, the training of social workers in the UK has consistently neglected issues of addictive behaviour. Whilst some attention has been paid in recent years to substance abuse issues, there has remained a silence in relation to gambling problems. Social workers provide more help for problems relating to addictions than other helping professions. There is good evidence that treatment, and early intervention for gambling problems, including psycho-social and public health approaches, can be very effective. This paper argues that problem gambling should be moved onto the radar of the social work profession, via inclusion on qualifying and post-qualifying training programmes and via research and dissemination of good practice via institutions such as the Social Care Institute for Excellence (SCIE).     VHE Gamma Rays from PKS 2155-304

    The close X-ray selected BL Lac PKS 2155-304 has been observed using the University of Durham Mark 6 very high energy (VHE) gamma ray telescope during 1996 September/October/November and 1997 October/November. VHE gamma rays with energy > 300 GeV were detected from this object with a time-averaged integral flux of (4.2 +/- 0.7 (stat) +/- 2.0 (sys)) x 10^(-11) per cm2 per s. There is evidence for VHE gamma ray emission during our observations in 1996 September and 1997 October/November, with the strongest emission being detected in 1997 November, when the object was producing the largest flux ever recorded in high-energy X-rays and was detected in > 100 MeV gamma-rays. The VHE and X-ray fluxes show evidence of a correlation.Comment: 14 pages, 6 figures, accepted for publication in Ap.

    Beta-delayed-neutron studies of 135,136^{135,136}Sb and 140^{140}I performed with trapped ions

    Beta-delayed-neutron (β\betan) spectroscopy was performed using the Beta-decay Paul Trap and an array of radiation detectors. The β\betan branching ratios and energy spectra for 135,136^{135,136}Sb and 140^{140}I were obtained by measuring the time of flight of recoil ions emerging from the trapped ion cloud. These nuclei are located at the edge of an isotopic region identified as having β\betan branching ratios that impact the r-process abundance pattern around the A~130 peak. For 135,136^{135,136}Sb and 140^{140}I, β\betan branching ratios of 14.6(11)%, 17.6(28)%, and 7.6(28)% were determined, respectively. The β\betan energy spectra obtained for 135^{135}Sb and 140^{140}I are compared with results from direct neutron measurements, and the β\betan energy spectrum for 136^{136}Sb has been measured for the first time

    The extreme mobility of debris avalanches: A new model of transport mechanism

    International audienceLarge rockslide-debris avalanches, resulting from flank collapses that shape volcanoes and mountains on Earth and other object of the solar system, are rapid and dangerous gravity-driven granular flows that travel abnormal distances. During the last 50 years, numerous physical models have been put forward to explain their extreme mobility. The principal models are based on fluidization, lubrication, or dynamic disintegration. However, these processes remain poorly constrained. To identify precisely the transport mechanisms during debris avalanches, we examined morphometric (fractal dimension and circularity), grain size, and exoscopic characteristics of the various types of particles (clasts and matrix) from volcanic debris avalanche deposits of La Réunion Island (Indian Ocean). From these data we demonstrate for the first time that syn-transport dynamic disintegration continuously operates with the increasing runout distance from the source down to a grinding limit of 500 μm. Below this limit, the particle size reduction exclusively results from their attrition by frictional interactions. Consequently, the exceptional mobility of debris avalanches may be explained by the combined effect of elastic energy release during the dynamic disintegration of the larger clasts and frictional reduction within the matrix due to interactions between the finer particles

    Functional QTL mapping and genomic prediction of canopy height in wheat measured using a robotic field phenotyping platform

    Genetic studies increasingly rely on high-throughput phenotyping, but the resulting longitudinal data pose analytical challenges. We used canopy height data from an automated field phenotyping platform to compare several approaches to scanning for quantitative trait loci (QTLs) and performing genomic prediction in a wheat recombinant inbred line mapping population based on up to 26 sampled time points (TPs). We detected four persistent QTLs (i.e. expressed for most of the growing season), with both empirical and simulation analyses demonstrating superior statistical power of detecting such QTLs through functional mapping approaches compared with conventional individual TP analyses. In contrast, even very simple individual TP approaches (e.g. interval mapping) had superior detection power for transient QTLs (i.e. expressed during very short periods). Using spline-smoothed phenotypic data resulted in improved genomic predictive abilities (5–8% higher than individual TP prediction), while the effect of including significant QTLs in prediction models was relatively minor (<1–4% improvement). Finally, although QTL detection power and predictive ability generally increased with the number of TPs analysed, gains beyond five or 10 TPs chosen based on phenological information had little practical significance. These results will inform the development of an integrated, semi-automated analytical pipeline, which will be more broadly applicable to similar data sets in wheat and other crops

    Patient values informing medical treatment: a pilot community and advance care planning survey

    Medicine regards the prevention of death as an important priority. Yet patients may have a range of priorities of equal or greater importance. These other priorities are often not discussed or appreciated by treating doctors. OBJECTIVES: We sought to identify priorities of care for patients attending an advance care planning (ACP) clinic and among the general population, and to identify factors associated with priorities other than prolonging life. METHODS: We used a locally developed survey tool 'What Matters Most' to identify values. Choices presented were: maintaining dignity, avoiding pain and suffering, living as long as possible, and remaining independent. Participants rated the importance of each and then selected a main priority for their doctor. Participant groups were a purposive sample of 382 lay people from the general population and 100 attendees at an ACP clinic. RESULTS: Living as long as possible was considered to be less important than other values for ACP patients and for the general population. Only 4% of ACP patients surveyed and 2.6% of our general population sample selected 'living as long as possible' as their top priority for medical treatment. CONCLUSIONS: 'Living as long as possible' was not the most important value for ACP patients, or for a younger general population. Prioritisation of other goals appeared to be independent of extreme age or illness. When end of life treatment is being discussed with patients, priorities other than merely prolonging life should be considered

    Age of red blood cells and mortality in the critically ill

    INTRODUCTION: In critically ill patients, it is uncertain whether exposure to older red blood cells (RBCs) may contribute to mortality. We therefore aimed to evaluate the association between the age of RBCs and outcome in a large unselected cohort of critically ill patients in Australia and New Zealand. We hypothesized that exposure to even a single unit of older RBCs may be associated with an increased risk of death. METHODS: We conducted a prospective, multicenter observational study in 47 ICUs during a 5-week period between August 2008 and September 2008. We included 757 critically ill adult patients receiving at least one unit of RBCs. To test our hypothesis we compared hospital mortality according to quartiles of exposure to maximum age of RBCs without and with adjustment for possible confounding factors. RESULTS: Compared with other quartiles (mean maximum red cell age 22.7 days; mortality 121/568 (21.3%)), patients treated with exposure to the lowest quartile of oldest RBCs (mean maximum red cell age 7.7 days; hospital mortality 25/189 (13.2%)) had an unadjusted absolute risk reduction in hospital mortality of 8.1% (95% confidence interval = 2.2 to 14.0%). After adjustment for Acute Physiology and Chronic Health Evaluation III score, other blood component transfusions, number of RBC transfusions, pretransfusion hemoglobin concentration, and cardiac surgery, the odds ratio for hospital mortality for patients exposed to the older three quartiles compared with the lowest quartile was 2.01 (95% confidence interval = 1.07 to 3.77). CONCLUSIONS: In critically ill patients, in Australia and New Zealand, exposure to older RBCs is independently associated with an increased risk of death
