    The relationship between medical innovation and health expenditures

    Onur Başer (MEF Author)Objectives: It is widely accepted that medical innovation includes many costly activities, and that it is a key driver of rising health care expenditures. Understanding the relationship between medical innovation and health care expenditures is critical for health policy makers to effectively make resource allocation decisions. This study seeks to investigate the relationship between medical innovation and health care expenditures. Methods: We assessed data from the World Intellectual Property Organization and World Bank statistics for the year 2014, which included data from 72 countries. The number of patent publications in the categories of medical technology, biotechnology, and pharmaceuticals were included as medical innovation indicators; public heath, health care costs per capita, and total health care costs (percentage of gross domestic product [GDP]) were included as indicators of health care-related expenditures. A canonic correlation analysis (CCA) was performed to examine the degree of association between the sets of medical innovation and health care expenditure variables. Results: Study results indicate that there is a strong positive correlation between medical innovation and health care expenditure variables (rc= 0.68, p<0.001). Conclusions: In light of this study, health policy makers should manage the relationship between medical innovation and health care expenditures with a focus on accessibility. Improved communication channels in the social system, increased international cooperation, and the determination of a proper balance between the benefits and costs of innovation may help to continue improving medical innovation and enhance health care accessibility.     Geometrical optimization of spin clusters for the preservation of quantum coherence

    We investigate the influence of geometry on the preservation of quantum coherence in spin clusters subjected to a thermal environment. Assuming weak inter-spin coupling, we explore the various buffer network configurations that can be embedded in a plane. Our findings reveal that the connectivity of the buffer network is crucial in determining the preservation duration of quantum coherence in an individual central spin. Specifically, we observe that the maximal planar graph yields the longest preservation time for a given number of buffer spins. Interestingly, our results demonstrate that the preservation time does not consistently increase with an increasing number of buffer spins. Employing a quantum master equation in our simulations, we further demonstrate that a tetrahedral geometry comprising a four-spin buffer network provides optimal protection against environmental effects

    Two-photon coincident-frequency-entanglement via extended phase matching

    We demonstrate a new class of frequency-entangled states generated via spontaneous parametric down-conversion under extended phase matching conditions. Biphoton entanglement with coincident signal and idler frequencies is observed over a broad bandwidth in periodically poled KTiOPO4_4. We demonstrate high visibility in Hong-Ou-Mandel interferometric measurements under pulsed pumping without spectral filtering, which indicates excellent frequency indistinguishability between the down-converted photons.     Economic impact and complications of treated and untreated hepatitis C virus patients in Turkey

    Onur Başer (MEF Author)Background: According to the Turkish Ministry of Health’s guidelines,standard double therapy, a combination of pegylated interferon-alphaand ribavirin, was the only treatment option for patients withhepatitis C virus (HCV) infection until the end of 2011. Objective: Theprimary objective was to compare risk-adjusted clinical and economicoutcomes between treated and untreated patients with HCV infection.Methods: Patients with HCV infection were identified from theTurkish National Health Insurance Database (2009–2011) using International Classification of Diseases, 10th Revision, Clinical Modification codes.The first prescription date was designated as the index date. Mortalityand hepatocellular carcinoma (HCC) rates and health care costs oftreated and untreated patients were compared using propensity scorematching. Baseline demographic and clinical factors were controlledin the models. Subgroup analysis was conducted for patient groupswith and without a cirrhosis diagnosis. Results: Out of 12,990 patientsincluded in the study, 1,583 were treated for HCV infection. Out of2,467 patients who had a cirrhosis diagnosis, 231 were treated,whereas out of 10,523 patients without cirrhosis, 1,352 patients weretreated. Treated patients were younger, less likely to be diagnosedwith comorbid conditions, and less likely to reside in Central orEastern Anatolia. After adjusting for baseline demographic andclinical factors, mortality (2.27% vs. 5.31%; P o 0.001) and HCC rates(0.69% vs. 1.96%; P o 0.001) were found to be lower for treatedpatients. Differences were more significant among patients diagnosedwith cirrhosis. Treated patients incurred higher risk-adjusted annualcosts (€6172 vs. €1680; P o 0.001), mainly because of pharmaceuticalcosts (€4918 vs. €583; P o 0.001).     The effect of solvent and pressure on polycaprolactone solutions for particle and fibre formation

    Polycaprolactone (PCL) is a widely used material in many applications to tackle health problems worldwide. Formed micro- or nanosized PCL particles and fibres benefit from a higher surface area to volume ratio and are valuable in those applications, thus there is always a push to achieve smaller diameters. Electrohydrodynamic (EHD) technologies have been at the forefront in the production of polymeric biomaterials, and pressurised gyration (PG) has also enhanced possibilities by its ability to spin comparable fibres at rapid speeds. In this work, PCL microparticles and fibres were separately produced by changing key operating parameters of EHD and PG systems and PCL solution properties. Initially, PCL microparticles were formed by electrospraying with different binary solvent systems, followed by pressurised gyration fibre production with various singular solvents and a pre-optimised binary solvent system. As anticipated, the use of binary systems altered particle morphologies and diameters, while increased pressure and the use of different solvents greatly affected the characteristics of resulting fibres. The morphology of PCL was found to be highly dependent on the solvents and operating parameters of the technology used

    Providers' assessment of a novel interactive health information technology in a pediatric intensive care unit

    Objective: To explore perceptions of critical care providers about a novel collaborative inpatient health information technology (HIT) in a pediatric intensive care unit (PICU) setting. Methods: This cross-sectional, concurrent mixed methods study was conducted in the PICU of a large midwestern children's hospital. The technology, the Large Customizable Interactive Monitor (LCIM), is a flat panel touch screen monitor that displays validated patient information from the electronic health record. It does not require a password to login and is available in each patient's room for viewing and interactive use by physicians, nurses, and families. Quantitative data were collected via self-administered, standardized surveys, and qualitative data via in-person, semistructured interviews between January and April 2015. Data were analyzed using descriptive statistics and inductive thematic analysis. Results: The qualitative analysis showed positive impacts of the LCIM on providers' workflow, team interactions, and interactions with families. Providers reported concerns regarding perceived patient information overload and associated anxiety and burden for families. Sixty percent of providers thought that LCIM was useful for their jobs at different levels, and almost 70% of providers reported that LCIM improved information sharing and communication with families. The average overall satisfaction score was 3.4 on a 0 to 6 scale, between "a moderate amount" and "pretty much." Discussion and Conclusion: This study provides new insight into collaborative HIT in the inpatient pediatric setting and demonstrates that using such technology has the potential to improve providers' experiences with families and just-in-time access to EHR information in a format more easily shared with families

    Automated discrimination of psychotropic drugs in mice via computer vision-based analysis

    Cataloged from PDF version of article.We developed an inexpensive computer vision-based method utilizing an algorithm which differentiates drug-induced behavioral alterations. The mice were observed in an open-field arena and their activity was recorded for 100 min. For each animal the first 50 min of observation were regarded as the drug-free period. Each animal was exposed to only one drug and they were injected (i.p.) with either amphetamine or cocaine as the stimulant drugs or morphine or diazepam as the inhibitory agents. The software divided the arena into virtual grids and calculated the number of visits (sojourn counts) to the grids and instantaneous speeds within these grids by analyzing video data. These spatial distributions of sojourn counts and instantaneous speeds were used to construct feature vectors which were fed to the classifier algorithms for the final step of matching the animals and the drugs. The software decided which of the animals were drug-treated at a rate of 96%. The algorithm achieved 92% accuracy in sorting the data according to the increased or decreased activity and then determined which drug was delivered. The method differentiated the type of psychostimulant or inhibitory drugs with a success ratio of 70% and 80%, respectively.
