    Laryngeal Histoplasmosis in patients with Crohn’s Disease undergoing treatment with HUMIRA (adalimumab): A Case Series

    The FDA has advocated for increasing awareness of histoplasmosis for patients undergoing TNF blockers as delay in diagnosis can lead to a poor outcome.1 Laryngeal histoplasmosis is a rare entity that should be part of the differential diagnosis in patients receiving immune suppression for Crohn’s disease. Patients with laryngeal histoplasmosis often report hoarseness, mucosal ulcerations, dysphagia, and odynophagia. We present a series of two cases of laryngeal histoplasmosis in patients with Crohn’s. These cases illustrate the high index of suspicion required to make the diagnosis of laryngeal histoplasmosis, especially in the Midwestern United States. Diagnosis is made quickly and cost-effectively with proper staining. Delayed diagnosis may lead to dissemination, increased morbidity, and mortality

    Challenges Faced by Jesuit Worldwide Learning Students: Piloting a Mixed Methods Investigation

    In 2017 a Creighton University Global Initiative grant provided 20 scholarships in its online B.S. in Leadership Studies for Jesuit Worldwide Learning graduates of the Diploma in Liberal Studies. In 2018-19 the first cohort of scholarship students living in Afghanistan and Jordan were enrolled at Creighton University (CU). In spring 2019, three Creighton University students collaborated with two of the co-authors to develop and to implement a research project to explore barriers faced by Jesuit Worldwide Learning students enrolled in Creighton’s B.S. program. The purpose of this project was to test if Group Concept Mapping, a mixed methods research approach integrating qualitative and quantitative methods, combined with the analysis of narrative course evaluations and interview texts with students, faculty, and staff adopting the Grounded Theory approach can lead to insights that further our understanding of barriers and struggles faced by Jesuit Worldwide Learning students and their instructors, can help strengthen institutional gaps in international learning, and can be used for a future study. The process and the results strongly suggest that the methodology is indeed appropriate to systematically study this or a related research question

    Order \alpha_s^2 Contributions to charm production in charged-current deep-inelastic lepton-hadron scattering

    The most important part of the order αs2\alpha_s^2 corrections to the charm component of the charged-current structure functions F2(x,Q2)F_2(x,Q^2) and F3(x,Q2)F_3(x,Q^2) have been calculated. This calculation is based on the asymptotic form of the heavy-quark coefficient functions corresponding to the higher order corrections to the W-boson-gluon fusion process. These coefficient functions which are in principle only valid for Q2m2Q^2 \gg m^2 can be also used to estimate the order αs2\alpha_s^2 contributions at lower Q2Q^2 values provided x<0.1x < 0.1. It turns out that the above corrections are appreciable in the large Q2Q^2-region and they explain the discrepancy found for the structure functions between the fixed-flavour scheme (FFS) and the variable-flavour-number scheme (VFNS). These corrections also hamper the extraction of the strange-quark density from the data obtained for the charged-current and the electromagnetic-current processes.Comment: 45 pages LaTeX, 17 Postscript Figure

    The Strange Sea Density and Charm Production in Deep Inelastic Charged Current Processes

    Charm production as related to the determination of the strange sea density in deep inelastic charged current processes is studied predominantly in the framework of the MS\overline{{\rm{MS}}} fixed flavor factorization scheme. Perturbative stability within this formalism is demonstrated. The compatibility of recent next-to-leading order strange quark distributions with the available dimuon and F2νNF^{\nu N}_2 data is investigated. It is shown that final conclusions concerning these distributions afford further analyses of presently available and/or forthcoming neutrino data.Comment: 12 pages, LaTeX, 4 figure

    Second trimester inflammatory and metabolic markers in women delivering preterm with and without preeclampsia.

    ObjectiveInflammatory and metabolic pathways are implicated in preterm birth and preeclampsia. However, studies rarely compare second trimester inflammatory and metabolic markers between women who deliver preterm with and without preeclampsia.Study designA sample of 129 women (43 with preeclampsia) with preterm delivery was obtained from an existing population-based birth cohort. Banked second trimester serum samples were assayed for 267 inflammatory and metabolic markers. Backwards-stepwise logistic regression models were used to calculate odds ratios.ResultsHigher 5-α-pregnan-3β,20α-diol disulfate, and lower 1-linoleoylglycerophosphoethanolamine and octadecanedioate, predicted increased odds of preeclampsia.ConclusionsAmong women with preterm births, those who developed preeclampsia differed with respect metabolic markers. These findings point to potential etiologic underpinnings for preeclampsia as a precursor to preterm birth

    Evaluation of the zucker diabetic fatty (ZDF) rat as a model for human disease based on urinary peptidomic profiles

    Representative animal models for diabetes-associated vascular complications are extremely relevant in assessing potential therapeutic drugs. While several rodent models for type 2 diabetes (T2D) are available, their relevance in recapitulating renal and cardiovascular features of diabetes in man is not entirely clear. Here we evaluate at the molecular level the similarity between Zucker diabetic fatty (ZDF) rats, as a model of T2D-associated vascular complications, and human disease by urinary proteome analysis. Urine analysis of ZDF rats at early and late stages of disease compared to age- matched LEAN rats identified 180 peptides as potentially associated with diabetes complications. Overlaps with human chronic kidney disease (CKD) and cardiovascular disease (CVD) biomarkers were observed, corresponding to proteins marking kidney damage (eg albumin, alpha-1 antitrypsin) or related to disease development (collagen). Concordance in regulation of these peptides in rats versus humans was more pronounced in the CVD compared to the CKD panels. In addition, disease-associated predicted protease activities in ZDF rats showed higher similarities to the predicted activities in human CVD. Based on urinary peptidomic analysis, the ZDF rat model displays similarity to human CVD but might not be the most appropriate model to display human CKD on a molecular level

    We apply analytic perturbation theory to the Gross--Llewellyn Smith sum rule.     Prediction of preterm birth with and without preeclampsia using mid-pregnancy immune and growth-related molecular factors and maternal characteristics.

    OBJECTIVE:To evaluate if mid-pregnancy immune and growth-related molecular factors predict preterm birth (PTB) with and without (±) preeclampsia. STUDY DESIGN:Included were 400 women with singleton deliveries in California in 2009-2010 (200 PTB and 200 term) divided into training and testing samples at a 2:1 ratio. Sixty-three markers were tested in 15-20 serum samples using multiplex technology. Linear discriminate analysis was used to create a discriminate function. Model performance was assessed using area under the receiver operating characteristic curve (AUC). RESULTS:Twenty-five serum biomarkers along with maternal age &lt;34 years and poverty status identified &gt;80% of women with PTB ± preeclampsia with best performance in women with preterm preeclampsia (AUC = 0.889, 95% confidence interval (0.822-0.959) training; 0.883 (0.804-0.963) testing). CONCLUSION:Together with maternal age and poverty status, mid-pregnancy immune and growth factors reliably identified most women who went on to have a PTB ± preeclampsia

    Resolved Photon Processes

    We review the present level of knowledge of the hadronic structure of the photon, as revealed in interactions involving quarks and gluons ``in" the photon. The concept of photon structure functions is introduced in the description of deep--inelastic eγe \gamma scattering, and existing parametrizations of the parton densities in the photon are reviewed. We then turn to hard \gamp\ and \gaga\ collisions, where we treat the production of jets, heavy quarks, hard (direct) photons, \jpsi\ mesons, and lepton pairs. We also comment on issues that go beyond perturbation theory, including recent attempts at a comprehensive description of both hard and soft \gamp\ and \gaga\ interactions.     Determination of the Strange Quark Content of the Nucleon from a Next-to-Leading-Order QCD Analysis of Neutrino Charm Production

    We present the first next-to-leading-order QCD analysis of neutrino charm production, using a sample of 6090 νμ\nu_\mu- and νˉμ\bar\nu_\mu-induced opposite-sign dimuon events observed in the CCFR detector at the Fermilab Tevatron. We find that the nucleon strange quark content is suppressed with respect to the non-strange sea quarks by a factor \kappa = 0.477 \: ^{+\:0.063}_{-\:0.053}, where the error includes statistical, systematic and QCD scale uncertainties. In contrast to previous leading order analyses, we find that the strange sea xx-dependence is similar to that of the non-strange sea, and that the measured charm quark mass, mc=1.70±0.19GeV/c2m_c = 1.70 \pm 0.19 \:{\rm GeV/c}^2, is larger and consistent with that determined in other processes. Further analysis finds that the difference in xx-distributions between xs(x)xs(x) and xsˉ(x)x\bar s(x) is small. A measurement of the Cabibbo-Kobayashi-Maskawa matrix element Vcd=0.2320.020+0.018|V_{cd}|=0.232 ^{+\:0.018}_{-\:0.020} is also presented. uufile containing compressed postscript files of five Figures is appended at the end of the LaTeX source.Comment: Nevis R#150
