    Symptomatic Hypoglycemia Related to Inappropriately High IGF-II Serum Levels in a Patient with Desmoplastic Small Round Cell Tumor

    A 45-year old man was diagnosed with desmoplastic small round cell tumor (DSRCT) with involvement of the peritoneum and pelvis. Disease progression was observed despite systemic chemotherapy. Six months after diagnosis, he developed severe hypoglycemia presented with seizures. He received intravenous glucose infusion and hydrocortisone with poor glycemic control, but with seizures resolution. The investigation excluded insulinoma, adrenal, liver and GH deficiencies. Laboratory showed slight rise of IGF-II and significant increase of the ratio IGF-II : IGF-I, which is pathognomonic of non-islet cell tumor hypoglycemia (NICTH). He received the diagnoses of NICTH related to IGF-II inappropriate production by DSRCT. Despite the attempt to control tumor mass and hypoglycemia, the patient died 9 months after diagnosis. NICTH related to inappropriate IGF-II secretion should be investigated in all cancer patients with refractory hypoglycemia whom insulinoma and other metabolic abnormalities were excluded from

    The recent discovery that the major histocompatibility complex of class I (MHC I) expression has a role in the synaptic elimination process, represented an insight into understanding the cross talk between neurons. In the present study, the possibility that glatiramer acetate (GA) treatment influences the MHC class I expression and the synaptic plasticity process in the spinal cord during the course of EAE was investigated. C57BL/6J mice were induced to EAE and submitted to treatment either with a placebo solution or with GA (0.05 mg/animal, subcutaneously, on a daily basis). All the animals were sacrificed at the peak disease (14 days after induction) or at the point of recovery of the clinical signs (21 days after induction). The spinal cords were removed and submitted to immunohistochemical examination, Western blotting and transmission electron microscopy analysis. The results showed that GA treatment was able to decrease synaptic loss during the course of EAE, which correlates with the downregulation of the MHC I complex. The present results reinforce the neuroprotective role of GA treatment, by reducing synaptic loss during the course of the disease. Such action may be associated with the recently described role of MHC I regulation during the synaptic plasticity process.71188-20

    Periodontal condition and recurrence of periodontitis associated with alcohol consumption in periodontal maintenance therapy

    This study followed individuals in periodontal maintenance therapy (PMT) over 6 years and longitudinally evaluated the effects of the frequency of alcohol consumption on the recurrence of periodontitis (RP). From a 6-year follow-up cohort study with 268 individuals under PMT, 142 patients who attended at least one PMT visit within 12 months were determined to be eligible. Based on their alcohol consumption, participants were categorized into 3 groups: none or occasional alcohol use (NA; n=88), moderate alcohol use (MA; n=26) and intense alcohol use (IA; n=24). Complete periodontal examination and alcohol consumption were evaluated at 2 times, T1 (after active periodontal therapy) and T2 (6 years). The frequencies of RP in the NA, MA and IA groups were 46.5%, 57.6%, and 79.1%, respectively. The following variables were significantly associated with RP in final multivariate logistic regression model: age >50 years old (OR = 1.79; 95%CI 1.42-2.91; p=0.002), current smoking (OR = 2.42; 95%CI 1.33-4.31; p=0.001), and intensive alcohol use (OR = 1.96; 95%CI: 1.37-2.64; p=0.024). Interaction between intensive alcohol use and smoking showed a high OR estimate of 3.15 (95%CI 1.29-6.32) for RP. IA individuals undergoing PMT presented worse periodontal condition, higher rates of RP and tooth loss when compared to NA individuals. Additionally, the interaction between intensive alcohol use and smoking significantly increased the risk for RP

Work on novel cassava biomaterials, which are low-cost, unexploited and with zero competition for food supply, are included. Using an integrated sustainable process, it shows how to indirectly reduce waste streams, through their effective use, guaranteeing zero carbon footprints and acting as a non-traditional strategy for equilibrium atmosphere and active packaging systems.     Obesity and low stature: nutritional status of individuals belonging to the same family

    Chronic undernourishment in infancy and obesity associated with short stature in the adult phase may have common determining biological and socio-environmental factors. This study with a view to contributing to the understanding of this relationship has as objective the verification of how children and parents distribution of nutritional conditions in pauperized families takes place and what the factors of a social, environmental and biological nature involvedare. It was studied 214 children aged 6 months to 5 years and 293 children and adolescents aged 6 to 18 years, belonging to pauper families earning monthly incomes less than US 70,living in two rural municipalities of the State of São Paulo, Brazil. Maternal schooling, number of rooms in the dwelling, age bellow 24 months, problems at birth and low stature of the mother especially when associated with obesity and, were all associated with the height deficitin children up to five years of age. The association of fathers height was weak. Conversely to the children of less then five years of age, the children of six year or over and adolescents were more clearly subject to the adverse socio-environmental and family conditions. Type of dwelling,number of individuals by dwelling, fathers absence and alcohol abuser in the family, lowstature of the mother associated with obesity were strongly related to the height deficit.     Alternative sanitizers to paraformaldehyde for incubation of fertile eggs

    The aim of this study was to evaluate an ethanolic extract of propolis and clove essential oil as a substitute for paraformaldehyde for the sanitation of fertile eggs. In total, 1,800 hatching eggs (from 40-week-old CPK [Pesadão Vermelho] breeder hens) were randomly distributed among the treatments (grain alcohol, clove essential oil, ethanolic extract of propolis, and paraformaldehyde). Spraying was the application method for all treatments except for paraformaldehyde, for which fumigation was used. The experimental design was a randomized block design with 4 treatments. Analysis of the incubation parameters was based on 6 replications per treatment. The egg weight loss was lower in the eggs treated with ethanolic extract of propolis (8.59 ± 3.34%) than in the eggs treated with grain alcohol (13.40 ± 2.87%), clove essential oil (12.96 ± 3.33%), and paraformaldehyde (13.05 ± 3.24%). The hatchability of the fertile eggs (51.39 ± 5.81%) and the hatchability of the set eggs (44.74 ± 6.79%) were negatively affected by the application of ethanolic extract of propolis. Late mortality of eggs treated was higher than early mortality in the grain alcohol (12.14 ± 4.72%; 2.86 ± 3.30%), clove essential oil (4.60 ± 5.95%; 3.03 ± 3.50%), and ethanolic extract of propolis (36.63 ± 6.60%, 11.98 ± 4.30%) treatments. The eggs treated with clove essential oil (67.90 ± 1.87%), paraformaldehyde (67.80 ± 1.85%), or grain alcohol (67.50 ± 1.92%) presented chick yields as expected. However, due to the high yield of eggs treated with ethanolic extract of propolis (69.25 ± 1.68%), its application at the concentration used in the present research is not recommended.     The partial inhibition of hypothalamic IRX3 exacerbates obesity

    The Iroquois homeobox 3 (Irx3) gene has been identified as a functional long-range target of obesity-associated variants within the fat mass and obesity-associated protein (FTO) gene. It is highly expressed in the hypothalamus, and both whole-body knockout and hypothalamic restricted abrogation of its expression results in a lean phenotype, which is mostly explained by the resulting increased energy expenditure in the brown adipose tissue. Because of its potential implication in the pathogenesis of obesity, we evaluated the hypothalamic cell distribution of Irx3 and the outcomes of inhibiting its expression in a rodent model of diet-induced obesity. Methods: Bioinformatics tools were used to evaluate the correlations between hypothalamic Irx3 and neurotransmitters, markers of thermogenesis and obesity related phenotypes. Droplet-sequencing analysis in >20,000 hypothalamic cells was used to explore the types of hypothalamic cells expressing Irx3. Lentivirus was used to inhibit hypothalamic Irx3 and the resulting phenotype was studied. Findings: IRX3 is expressed predominantly in POMC neurons. Its expression is inhibited during prolonged fasting, as well as when mice are fed a high-fat diet. The partial inhibition of hypothalamic Irx3 using a lentivirus resulted in increased diet-induced body mass gain and adiposity due to increased caloric intake and reduced energy expenditure. Interpretation: Contrary to the results obtained when lean mice are submitted to complete inhibition of Irx3, partial inhibition of hypothalamic Irx3 in obese mice causes an exacerbation of the obese phenotype.
