    Bilateral anterior sternothoracotomy (clamshell incision): a suitable alternative for bilateral lung sarcoma metastasis in children

    BACKGROUND: The aim of our study was to assess the postoperative course of bilateral anterior sternothoracotomy (BAT) in children with sarcoma metastases, in a curative care perspective. METHODS: We reviewed the records of seven patients younger than 18 years old, who underwent surgical procedures for sarcoma metastasis to the lung between 2000 and 2012. We compared the postoperative course of the BAT group with that of patients who underwent unilateral posterolateral thoracotomies (PLTs) for the same etiology. RESULTS: Of 17 surgical procedures, there were seven BAT and 10 unilateral PLT. Mean ages at the time of the procedures were 12.9 ± 5.4 years old for BAT, and 17.4 ± 1.9 years old for PLT. Mean operative time was 173 ± 37 minutes in the BAT group, and 145 ± 39 minutes in the PLT group (P = 0.19). Patients received epidural analgesia in all cases; this was for a mean time of 3.8 ± 1.3 days in the BAT group, and 3.21 ± 4 days in the PLT group (P = 0.36). Chest tubes were removed after 3.6 ± 1.3 days in the BAT group, and 3 ± 1.2 days in the PLT group (P = 0.69). Total hospital stay was 7.7 ± 6.6 days in the BAT group, and 7 ± 1.2 days in the PLT group (P = 0.72). CONCLUSION: In our experience, BAT seems suitable and shows outcomes similar to those of PLT for sarcoma metastasis resection. The BAT procedure allows the manual exploration of both lungs during a single surgical intervention, and so reduces the delay of further therapies

    Effect of reducing barley grain degradability on lambs weight, colostrum, milk production, their composition and some biochemical blood trats in Awassi ewes

    This study was conducted in research department of animal resource / Al-Rashidiya, by using 21 of Awassi ewes in the last two months of gestation, ewes were divided according to their body weight, age and milk production during the previous season into three groups, daily feed intake in all groups was restricted with 1.5 kg / ewe on rations consist of barley, wheat bran, soybean meal, urea and straw. the first group was fed on ration contained barley untreated with formaldehyde (T1), while barley grain treated with formaldehyde in the ration of second group (T2), the third group fed on formaldehyde treated barley in late gestation and switch directly after lambing to the untreated barley (T3). Results indicated that differences was non-significant in colostrum yield 138±16.41, 163±19.26 and 127±19.43 g/ hr and in its components, milk yield was increased (P<0.05) in T2 1411±99.04 g/ day as compared with T1 and T2 1080±77.02 and 940±34.54 g/ day respectively, while no significant differences was noted in milk composition. Efficiency of protein and energy intake for milk production significantly higher (P<0.01) in T2 which were 26.42±2.01 and 42.24±3.55% compared to T1 (20.56±1.59 and 34.31±2.99%) and only in energy efficiency as compared with T3 which were (17.70±0.78 and 30.62±1.34%). Result of blood parameters after four week postpartum was indicated a significant (P<0.05) increase in cholesterol concentration in T1 69.60±4.85 mg/ dl compared to T2 and T3 56.55±3.67 and 54.73±3.99 mg/ dl, blood creatinine concentration decreased (P<0.05) in T3 5.03±0.53 mg/ dl as compared to T1 and T2 which were 9.60±0.77 and 9.35±0.53 mg/ dl respectively. In conclusion, reducing barley grain degradability improve feed efficiency and ewes performance, while the directly change of the feeding after lambing from low degraded barley to the normal barley grain had a negative effect in performance

    Management of Polypoid Gallbladder Lesions in Children: A Multicenter Study

    INTRODUCTION:  Polypoid lesions of the gallbladder (PLG) are relatively common in adults, while they are very rare in children. The use of high-quality ultrasonography leads to increased detection of PLG, although less than 20 pediatric cases of primary PLG have been reported in the literature. The aim of this study was to address the experience of PLG management in children. MATERIALS AND METHODS:  A retrospective multicenter review of children with ultrasonographically defined PLG between 2006 and 2016 was performed. The data from 12 pediatric surgery centers were compiled for this purpose. RESULTS:  Eighteen patients (mean age: 10.4 ± 4.1 years) were included and managed according to each center\u27s protocols. Cholecystectomy was performed for nine symptomatic patients. Histopathology conclusively revealed four tubular and five papillary adenomas, with a median size of 12 mm (ranging from 3 to 35 mm). Nine asymptomatic children were monitored by sonography over a 24-month (ranging from 12 to 66 months) follow-up period. The median PLG size was 7 mm (ranging from 3 to 9 mm). Two lesions disappeared after 1 and 2 years of follow-up, respectively. None of the patients developed symptoms or malignant transformation. CONCLUSION:  This report confirms appropriate use of a conservative approach with annual clinical and ultrasound follow-up for small-sized and isolated PLG in children given the absence of malignancy and potential vanishing entities. Surgical treatment should be considered in case of lesions larger than 10 mm in size or when they are associated with symptoms

    Origins of the Ambient Solar Wind: Implications for Space Weather

    The Sun's outer atmosphere is heated to temperatures of millions of degrees, and solar plasma flows out into interplanetary space at supersonic speeds. This paper reviews our current understanding of these interrelated problems: coronal heating and the acceleration of the ambient solar wind. We also discuss where the community stands in its ability to forecast how variations in the solar wind (i.e., fast and slow wind streams) impact the Earth. Although the last few decades have seen significant progress in observations and modeling, we still do not have a complete understanding of the relevant physical processes, nor do we have a quantitatively precise census of which coronal structures contribute to specific types of solar wind. Fast streams are known to be connected to the central regions of large coronal holes. Slow streams, however, appear to come from a wide range of sources, including streamers, pseudostreamers, coronal loops, active regions, and coronal hole boundaries. Complicating our understanding even more is the fact that processes such as turbulence, stream-stream interactions, and Coulomb collisions can make it difficult to unambiguously map a parcel measured at 1 AU back down to its coronal source. We also review recent progress -- in theoretical modeling, observational data analysis, and forecasting techniques that sit at the interface between data and theory -- that gives us hope that the above problems are indeed solvable.Comment: Accepted for publication in Space Science Reviews.     A high-latitude coronal mass ejection observed by a constellation of coronagraphs: Solar Orbiter/Metis, STEREO-A/COR2, and SOHO/LASCO

    Context. A few days before the first perihelion of the Solar Orbiter nominal mission, which occurred on 2022 March 26, the Metis coronagraph on board Solar Orbiter detected a coronal mass ejection (CME) that was moving away from the far side of the Sun (with respect to Solar Orbiter) at high northern latitudes. The eruption was also seen by other spacecraft, in particular, by STEREO-A, which was in quadrature configuration with Solar Orbiter. Aims. We analyse the different views of the CME by a constellation of spacecraft with the purpose to determine the speed and acceleration of the CME, and to identify the source region of the CME. Methods. Considering the positions of various spacecraft on 2022 March 22, this CME happened to be within the field of view of STEREO-A/SECCHI, and it was visible over the limb from SOHO/LASCO. We present the results of the 3D reconstruction of the CME based on the graduated cylindrical shell model and of the identification of the possible origin of the CME using extreme-ultraviolet (EUV) observations by Solar Orbiter/EUI, STEREO-A/EUVI, and SDO/AIA. The observations in EUV are compared with the coronal magnetic structure obtained by the potential field source surface method. Results. The 3D reconstruction of the CME derives a central latitude of 29 N, a Stonyhurst longitude of 125, and an average radial speed at the apex of 322 ± 33 km s1 between 4 and 13 RȮ, which is probably not high enough to generate a shock wave. The estimated average acceleration of the CME is 16 ± 11 m s2 in the same range of distances from the Sun. This CME may be associated with the disappearance of a coronal cloud prominence, which is seen in the EUV by STEREO-A/EUVI and SDO/AIA, and is also associated with rapidly evolving emerging magnetic flux

    Solar magnetism eXplorer (SolmeX)

    The magnetic field plays a pivotal role in many fields of Astrophysics. This is especially true for the physics of the solar atmosphere. Measuring the magnetic field in the upper solar atmosphere is crucial to understand the nature of the underlying physical processes that drive the violent dynamics of the solar corona—that can also affect life on Earth. SolmeX, a fully equipped solar space observatory for remote-sensing observations, will provide the first comprehensive measurements of the strength and direction of the magnetic field in the upper solar atmosphere. The mission consists of two spacecraft, one carrying the instruments, and another one in formation flight at a distance of about 200 m carrying the occulter to provide an artificial total solar eclipse. This will ensure high-quality coronagraphic observations above the solar limb. SolmeX integrates two spectro-polarimetric coronagraphs for off-limb observations, one in the EUV and one in the IR, and three instruments for observations on the disk. The latter comprises one imaging polarimeter in the EUV for coronal studies, a spectro-polarimeter in the EUV to investigate the low corona, and an imaging spectro-polarimeter in the UV for chromospheric studies. SOHO and other existing missions have investigated the emission of the upper atmosphere in detail (not considering polarization), and as this will be the case also for missions planned for the near future. Therefore it is timely that SolmeX provides the final piece of the observational quest by measuring the magnetic field in the upper atmosphere through polarimetric observations

    Reticulated origin of domesticated emmer wheat supports a dynamic model for the emergence of agriculture in the fertile crescent

    We used supernetworks with datasets of nuclear gene sequences and novel markers detecting retrotransposon insertions in ribosomal DNA loci to reassess the evolutionary relationships among tetraploid wheats. We show that domesticated emmer has a reticulated genetic ancestry, sharing phylogenetic signals with wild populations from all parts of the wild range. The extent of the genetic reticulation cannot be explained by post-domestication gene flow between cultivated emmer and wild plants, and the phylogenetic relationships among tetraploid wheats are incompatible with simple linear descent of the domesticates from a single wild population. A more parsimonious explanation of the data is that domesticated emmer originates from a hybridized population of different wild lineages. The observed diversity and reticulation patterns indicate that wild emmer evolved in the southern Levant, and that the wild emmer populations in south-eastern Turkey and the Zagros Mountains are relatively recent reticulate descendants of a subset of the Levantine wild populations. Based on our results we propose a new model for the emergence of domesticated emmer. During a pre-domestication period, diverse wild populations were collected from a large area west of the Euphrates and cultivated in mixed stands. Within these cultivated stands, hybridization gave rise to lineages displaying reticulated genealogical relationships with their ancestral populations. Gradual movement of early farmers out of the Levant introduced the pre-domesticated reticulated lineages to the northern and eastern parts of the Fertile Crescent, giving rise to the local wild populations but also facilitating fixation of domestication traits. Our model is consistent with the protracted and dispersed transition to agriculture indicated by the archaeobotanical evidence, and also with previous genetic data affiliating domesticated emmer with the wild populations in southeast Turkey.
