Analysis of the Economic Impact of Sorghum and Millet Research in Mali
Sorghum (Sorghum bicolor (L.) Moench) and pearl millet [Pennisetum glaucum] are very impor tant to the
economy and people of Mali. But, their productivity is low given the reliance on traditional, low-input
production practices. The Institut d'Economie Rurale (IER) was started soon after the country's independence
to find ways of improving the productivity of food crops in collaboration with regional and international
agricultural research institutes (e.g., IRAT, ICRISAT, CIRAD-CA) . A numbe r of improved seed-based
sorghum and millet technologies have since been developed and diffused. They were developed from two
approaches: (1) selection within local germplasm, which consisted of collecting, testing, purifying, and supplying
farmers with readily available materials. These are identified as Generation 1 materials; and (2) plant breeding,
which consisted of crossing with exotic germplasm, and pedigree selection. Outputs of this second approach are
identified as Generation 2 materials. This study evaluates the returns to sorghum and pearl millet research
investments in Mali by combining farm-level survey information from 1990 to 1995 with that from research and
extension in an economic surplus framework. The results indicate that by 1995, 30% of the sorghum and 3 7%
of the millet areas were sown to improved varieties. Th e estimated benefits from research and extension efforts
range from US 25 million (for pearl millet). These represent internal rates of
returns of 69% and 50%, respectively. A disaggregated analysis indicates higher yield gains and higher returns
to Generation 2 materials than to Generation 1 materials for bot h sorghum and pearl millet. Unit costs were also
much lower for Generation 2 materials. The major constraints cited by farmers as limiting their ability to adopt
improved materials include lack of information, lack of improved seeds, and low soil fertility. The study concludes
that the breeding philosophy should be diversified to respond to the need of the changing socioeconomic
environment with the recent devaluation of the CFA. It also recommends that efforts be made to improve the
economic farming environment to enable farmers to adopt mor e productive agricultural technologies which are
necessary for rural poverty alleviation and improvement in national food security
Les Occlusions Intestinales Par NĆud Ileo-SigmoĂŻdien : Aspects Diagnostiques, ThĂ©rapeutiques Et Pronostiques
Introduction: Ileosigmoid knot (ISK), also known as compound volvulus or double volvulus, is a rare disease and affects mostly male subjects in their fourth decade. Delayed diagnosis often leads to complications with a high incidence of digestive necrosis. Materials and methods: This was a 10-year retrospective, descriptive and analytical study from 1 January 2007 to 31 December 2016, which covered all patients operated emergently for ISK in the surgical and surgical specialty departments at the Niamey National Hospital (NNH). Results: The series involved 8 cases of ISK, representing 0.82% of intestinal obstructions. There were 7 men and 1 woman. The average age of patients was 44.33 years with extremes ranging from 25 to 60 years. The main clinical signs were: abdominal pain (100%), inability to pass gas and stool (100%), vomiting (87.5%) and abdominal meteorism (87.5%). Four patients (50%) had a clear obstructive syndrome and the other 4 patients (50%) had associated signs of peritoneal irritation. Paraclinical diagnosis was difficult because of atypical clinico-radiological signs, but in all cases an abdominal x-ray was performed and showed a double loop of dilated sigmoid shadow in 87.5% of the cases. Laparotomy was the first approach used in all patients and led to a 50% necrosis rate. Hartmannâs colectomy and ideal colectomy were used in identical proportions, with 50% each. Ideal colectomy was characterized by a morbidity rate of 50% and a mortality rate of 25% compared to 0% for Hartmannâs procedure. The overall morbidity was 25% and the overall mortality was 12.5%. The average hospital stay was 65.87 days (range 17 to 128 days). The average time before reestablishment of digestive continuity was 50.37 days (range 31-128 days). Conclusion: Ileosigmoid knot is a rare condition at Niamey national hospital where the diagnostic approach is similar to that of occlusions in general, but remains difficult due to its uncommonness and atypical clinico-radiological signs. Necrosis rates and postoperative complications remain high

Introduction: Acute intussusception is a rare clinical entity in adults where it accounts for only 1-2% of intestinal obstructions. The authors wanted to report cases of acute intussusception in adult patients, their diagnostic aspects, their management, and their etiologies. Patients and Methods: This study is a retrospective study of the medical files of patients of both sexes. They include adults over 15 years of age, operated between January 2010 and December 2014, who were diagnosed with obstruction due to acute intestinal intussusception. Results: Six cases of adult intestinal intussusception were collected. The average age was 26.5 years. The sex ratio was 1 and there were so many men as women. Five out of six patients were unstable on admission. The diagnosis was made preoperative in two cases out of six, 33.3%. Ultrasound revealed a target sign in two cases. The ileo-ileal form was the most frequent (5 cases out of 6) or 83.3%. Also, there were two cases of intestinal necrosis out of six. Intestinal resection was performed in five cases or 83.33%. The cause of intussusception was found in four cases out of six or 66.7%. Here, a tumor was the cause in half of the cases. Immediate surgical follow-up was uncomplicated in all patients. Conclusion: Acute intestinal intussusception of the adult is a very rare condition. The preoperative diagnosis of acute intussusception of the adult remains delicate. The ileo-ileal form is more frequent than the ileo-colic form in adults. Treatment is always surgical in adults
Is parasite clearance clinically important after malaria treatment in a high transmission area? A 3-month follow-up of home-based management with herbal medicine or ACT
Argemone mexicana (AM), a validated herbal medicine for uncomplicated malaria, seems to prevent severe malaria without completely clearing parasites in most patients. This study, in a high transmission area of South Mali, explores whether residual parasitaemia at day 28 was associated with subsequent malaria episodes and/or anaemia. Three hundred and one patients were randomly assigned to AM or artesunate/amodiaquine as first line treatment, of whom 294 were followed up beyond the standard 28 days, to 84 days. From day 29 to day 84, there were no significant differences between treatment groups in new clinical episodes of uncomplicated malaria (0.33 vs 0.31 episodes/patient), severe malaria (< 6% per month of patients aged †5 years) or moderate anaemia (hematocrit < 24%: 1.1% in both groups at day 84). Total parasite clearance at day 28 was not correlated with incidence of uncomplicated or severe malaria or of moderate anaemia over the subsequent two months. Total parasite clearance at day 28 was not clinically important in the context of high transmission. If this finding can be confirmed, some antimalarials which are clinically effective but do not completely clear parasites could nevertheless be appropriate in high transmission areas. Such a policy could be tested as a way to delay resistance to artemisinin combination therapies
Prise En Charge Des Anévrismes Artériels Dans Un Centre Africain Non Spécialisée
Introduction: Arterial aneurysms affect 7 to 8% of people over 65 in the West and are the 2nd leading cause of death in these countries. In Africa this frequency is poorly evaluated. The objective of this work is to report the management of arterial aneurysms at the National Hospital of Niamey (HNN). Patients and methods: This was a retrospective, descriptive study over a period of eight (8) years from January 2009 to December 2016, performed in the surgical departments of the National Hospital of Niamey. Included in the study were patients of both sexes, hospitalized and / or operated for arterial aneurysm. Not included were patients treated for arterial aneurysm with incomplete records or those concerning the neurosurgical sphere. Results: During the study period, 17,748 patients were hospitalized in the general surgery departments, including 16 patients for arterial aneurysm, or 0.09% of surgical pathologies. There were 13 men (81.25%) and 3 women (18.75%), or a sex ratio of 4.33. The average age was 55.75 years with extremes ranging from 25 years old to 90 years old. The circumstances of discovery of the aneurysms were swelling of the antero-internal aspect of the thigh in 7 cases (43.75%), abdominal mass 6 cases (37.50%), then 2 cases (12.50%) of chest pain and incidental discovery in 1cas (6.25%). The most common risk factor was high blood pressure with 43.75% (7 cases). The aneurysm sat on the femoral artery in 43.75% (n = 7), of which 6 on the deep femoral and 1 on the superficial femoral, on the infrarenal aorta in 31.25% (n = 5), on thoracic aorta 12.50% (n = 2), on the iliac artery 12.50% (n = 2). For the diagnosis the angioscanner is realized in all the patients and in addition Doppler ultrasound in 43, 75% of cases. Twelve (12) patients benefited from curative surgical intervention by prosthetic graft by PTFE in 58.33% of cases and by Dacron in 41.66%. The average stay was 29.75 days and the immediate operative followup was complicated by thrombosis in 12.5% and parietal suppurations in 6.25%. We recorded two (2) deaths, ie 12.5% among non-operated patients. Conclusion: Arterial aneurysms are rare diseases at the HNN. Conventional surgery is the treatment performed in our patients. The postoperative course was simple in most cases
Multi-scale governance in agriculture systems: Interplay between national and local institutions around the production dimension of food security in Mali
Enforcement of rules and laws designed at the national level is still one of the dominant institutional mechanisms for effective multiscale governance in most countries. At times, such blanket regulations are not only unable to meet practical needs at local levels, but they may conflict with local institutional logics, thereby creating new challenges. This study looks at three institutional arrangements in the agriculture and food security sector in the district of Koutiala, Mali to analyse the institutional variety across scale and the underlying institutional logics. On one side, the Cooperative Law as well as the Seed Law both designed at national level to enable famersâ access to agriculture services and improved seeds have yielded mixed results with regard to anticipated outcomes. The cooperative law is believed to degrade the social cohesion and the mutual support on which vulnerable farmers rely when facing climatic and non-climatic risks whereas the new seed system is found onerous and unaffordable for farmers. On the other side, the local convention for the management of natural resources established as part of ongoing decentralised governance policy seems to resonate with local culture but challenged by other stakeholders. Through exploring these cases, this paper tests bricolage as an analytical framework for doing an institutional diagnostic. It aims at contributing to methodological and theoretical insights on the way sustainable institutions can be generated in conflicting institutional logics in the context of multi-scale governance
Impact of Crop Diversification on Household Food and Nutrition Security in Southern and Central Mali
Many African countries, including Mali, depend on the production of a single or a limited range of crops for national food security. In Mali, this heavy reliance on a range of basic commodities or staple crops, or even just one, exacerbates multiple risks to agricultural production, rural livelihoods, and nutrition. With this in mind, the smart food campaign was initiated to strengthen the resilience and nutritional situation of households and peasant communities where the diet is mainly cereal-based and remains very undiversified and poor in essential micronutrients. As part of the campaign, our study aims to analyze the impact of agricultural diversification on food consumption and household nutritional security. The analysis uses survey data from 332 individuals randomly selected. Multinomial logistic regression and the Simpson diversity index were used to determine the index and estimate the determinants of crop diversification. The consumption score index weighted by consumption frequency and anthropometric
indices (for children) were used to assess the nutritional status of households. The results show four types of strategies of diversification: 7.55% are cereals only, 5.66% combine milletâsorghumâgroundnut, 41.51% combine milletâsorghumâgroundnutâcowpea, and 45.28% combine milletâsorghumâgroundnutâcowpeaâmaize. The estimation of the
regression model shows that socioeconomic factors have a positive influence. With a consumption score index of 34 in the villages and 40.5 in Bamako, based on eight food groups, we find that the quality of food is insufficient in rural areas, but it is acceptable in the urban center of Bamako. Analysis of the nutritional status of children aged 6â48
months reveals that 30% of the surveyed population is in a situation of nutritional insecurity (all forms combined). To help improve crop diversification and the nutritional
quality of foods, we suggest, among other things, subsidies and public spending tofacilitate access to inputs that allow the acquisition of a wider range of inputs and services, intensification of nutrition awareness, and education programs to maximize the incentive to consume nutritious foods from self-production and market purchases. Finally, we propose to facilitate access to technologies promoting food diversification and improving food and nutritional security, particularly in rural areas

Low-cost adaptation options to support green growth in agriculture, water resources, and coastal zones
The regional climate as it is now and in the future will put pressure on investments in sub-Saharan Africa in water resource management, fisheries, and other crop and livestock production systems. Changes in oceanic characteristics across the Atlantic Ocean will result in remarkable vulnerability of coastal ecology, littorals, and mangroves in the middle of the twenty-first century and beyond. In line with the countries' objectives of creating a green economy that allows reduced greenhouse gas emissions, improved resource efficiency, and prevention of biodiversity loss, we identify the most pressing needs for adaptation and the best adaptation choices that are also clean and affordable. According to empirical data from the field and customized model simulation designs, the cost of these adaptation measures will likely decrease and benefit sustainable green growth in agriculture, water resource management, and coastal ecosystems, as hydroclimatic hazards such as pluviometric and thermal extremes become more common in West Africa. Most of these adaptation options are local and need to be scaled up and operationalized for sustainable development. Governmental sovereign wealth funds, investments from the private sector, and funding from global climate funds can be used to operationalize these adaptation measures. Effective legislation, knowledge transfer, and pertinent collaborations are necessary for their success
