2015 Update on Acute Adverse Reactions to Gadolinium based Contrast Agents in Cardiovascular MR. Large Multi-National and Multi-Ethnical Population Experience With 37788 Patients From the EuroCMR Registry
Objectives: Specifically we aim to demonstrate that the results of our earlier safety data hold true in this much larger multi-national and multi-ethnical population. Background: We sought to re-evaluate the frequency, manifestations, and severity of acute adverse reactions associated with administration of several gadolinium- based contrast agents during routine CMR on a European level. Methods: Multi-centre, multi-national, and multi-ethnical registry with consecutive enrolment of patients in 57 European centres. Results: During the current observation 37788 doses of Gadolinium based contrast agent were administered to 37788 patients. The mean dose was 24.7 ml (range 5–80 ml), which is equivalent to 0.123 mmol/kg (range 0.01 - 0.3 mmol/kg). Forty-five acute adverse reactions due to contrast administration occurred (0.12 %). Most reactions were classified as mild (43 of 45) according to the American College of Radiology definition. The most frequent complaints following contrast administration were rashes and hives (15 of 45), followed by nausea (10 of 45) and flushes (10 of 45). The event rate ranged from 0.05 % (linear non-ionic agent gadodiamide) to 0.42 % (linear ionic agent gadobenate dimeglumine). Interestingly, we also found different event rates between the three main indications for CMR ranging from 0.05 % (risk stratification in suspected CAD) to 0.22 % (viability in known CAD). Conclusions: The current data indicate that the results of the earlier safety data hold true in this much larger multi-national and multi-ethnical population. Thus, the “off-label” use of Gadolinium based contrast in cardiovascular MR should be regarded as safe concerning the frequency, manifestation and severity of acute events
Intensity Distribution of Waves Transmitted Through a Multiple Scattering Medium
The distributions of the angular transmission coefficient and of the total
transmission are calculated for multiple scattered waves. The calculation is
based on a mapping to the distribution of eigenvalues of the transmission
matrix. The distributions depend on the profile of the incoming beam. The
distribution function of the angular transmission has a stretched exponential
decay. The total-transmission distribution grows log-normally whereas it decays
exponentially.Comment: 8 pages, revtex3.0, 3 postscript figures, NvR0

Random Walks for Spike-Timing Dependent Plasticity
Random walk methods are used to calculate the moments of negative image
equilibrium distributions in synaptic weight dynamics governed by spike-timing
dependent plasticity (STDP). The neural architecture of the model is based on
the electrosensory lateral line lobe (ELL) of mormyrid electric fish, which
forms a negative image of the reafferent signal from the fish's own electric
discharge to optimize detection of sensory electric fields. Of particular
behavioral importance to the fish is the variance of the equilibrium
postsynaptic potential in the presence of noise, which is determined by the
variance of the equilibrium weight distribution. Recurrence relations are
derived for the moments of the equilibrium weight distribution, for arbitrary
postsynaptic potential functions and arbitrary learning rules. For the case of
homogeneous network parameters, explicit closed form solutions are developed
for the covariances of the synaptic weight and postsynaptic potential
distributions.Comment: 18 pages, 8 figures, 15 subfigures; uses revtex4, subfigure, amsmat
CMR findings in patients with hypertrophic cardiomyopathy and atrial fibrillation
<p>Abstract</p> <p>Objectives</p> <p>We sought to evaluate the relation between atrial fibrillation (AF) and the extent of myocardial scarring together with left ventricular (LV) and atrial parameters assessed by late gadolinium-enhancement (LGE) cardiovascular magnetic resonance (CMR) in patients with hypertrophic cardiomyopathy (HCM).</p> <p>Background</p> <p>AF is the most common arrhythmia in HCM. Myocardial scarring is also identified frequently in HCM. However, the impact of myocardial scarring assessed by LGE CMR on the presence of AF has not been evaluated yet.</p> <p>Methods</p> <p>87 HCM patients underwent LGE CMR, echocardiography and regular ECG recordings. LV function, volumes, myocardial thickness, left atrial (LA) volume and the extent of LGE, were assessed using CMR and correlated to AF. Additionally, the presence of diastolic dysfunction and mitral regurgitation were obtained by echocardiography and also correlated to AF.</p> <p>Results</p> <p>Episodes of AF were documented in 37 patients (42%). Indexed LV volumes and mass were comparable between HCM patients with and without AF. However, indexed LA volume was significantly higher in HCM patients with AF than in HCM patients without AF (68 ± 24 ml·m-2 versus 46 ± 18 ml·m-2, p = 0.0002, respectively). The mean extent of LGE was higher in HCM patients with AF than those without AF (12.4 ± 14.5% versus 6.0 ± 8.6%, p = 0.02). When adjusting for age, gender and LV mass, LGE and indexed LA volume significantly correlated to AF (r = 0.34, p = 0.02 and r = 0.42, p < 0.001 respectively). By echocardiographic examination, LV diastolic dysfunction was evident in 35 (40%) patients. Mitral regurgitation greater than II was observed in 12 patients (14%). Multivariate analysis demonstrated that LA volume and presence of diastolic dysfunction were the only independent determinant of AF in HCM patients (p = 0.006, p = 0.01 respectively). Receiver operating characteristic curve analysis indicated good predictive performance of LA volume and LGE (AUC = 0.74 and 0.64 respectively) with respect to AF.</p> <p>Conclusion</p> <p>HCM patients with AF display significantly more LGE than HCM patients without AF. However, the extent of LGE is inferior to the LA size for predicting AF prevalence. LA dilation is the strongest determinant of AF in HCM patients, and is related to the extent of LGE in the LV, irrespective of LV mass.</p
Astrophysical Fluids of Novae: High Resolution Pre-decay X-ray spectrum of V4743 Sagittarii
Eight X-ray observations of V4743 Sgr (2002), observed with Chandra and
XMM-Newton are presented. The nova turned off some time between days 301.9 and
371, and the X-ray flux subsequently decreased from day 301.9 to 526 following
an exponential decline time scale of days. We use the absorption
lines present in the SSS spectrum for diagnostic purposes, and characterize the
physics and the dynamics of the expanding atmosphere during the explosion of
the nova. The information extracted from this first stage is then used as input
for computing full photoionization models of the ejecta in V4743 Sgr. The SSS
spectrum is modeled with a simple black-body and multiplicative Gaussian lines,
which provides us of a general kinematical picture of the system, before it
decays to its faint phase (Ness et al. 2003). In the grating spectra taken
between days 180.4 and 370, we can resolve the line profiles of absorption
lines arising from H-like and He-like C, N, and O, including transitions
involving higher principal quantum numbers. Except for a few interstellar
lines, all lines are significantly blue-shifted, yielding velocities between
1000 and 6000 km/s which implies an ongoing mass loss. It is shown that
significant expansion and mass loss occur during this phase of the explosion,
at a rate . Our measurements show that the efficiency of the amount of
energy used for the motion of the ejecta, defined as the ratio between the
kinetic luminosity and the radiated luminosity , is
of the order of one.Comment: 25 pages, 9 figures. Accepted in book: Recent Advances in Fluid

The organization of care in pediatric radiotherapy across SIOP Europe affiliated centers:A multicenter survey in the framework of the 'Joint Action on Rare Cancers' project
BACKGROUND/PURPOSE: To reduce inequalities among SIOPE-affiliated countries, standard and optional levels to deliver 'Good Clinical Practice' compliant treatment in pediatric radiation oncology have been published. The aim of this project was to map the availability of pediatric radiotherapy resources across SIOPE-affiliated radiotherapy departments.MATERIALS/METHODS: An online survey with 34 questions was distributed to 246 radiotherapy departments across 35 SIOPE-affiliated countries. In addition to demographic data, 15 general items related to the organization of the radiotherapy process, and 10 radiotherapy-specific items were defined. For each of the 25 items, sum scores were calculated per center and country. Mann-Whitney U tests were used to analyze associations.RESULTS: Between March-June 2019, 121 departments (49 %) out of 31 countries (89 %) completed the survey. At center level, involvement of core disciplines in tumor boards (28 %), and integration of dedicated pediatric radiation therapy technologists (24 %) are limited, while rare & complex brachytherapy procedures are performed in many centers (23 %). For general and radiotherapy-specific items respectively, a relevant variation of sum scores was observed across countries (Δgeneral: ≤10 points; ΔRT_specific: ≤5 points) and among centers within a country (Δgeneral: ≤9 points; ΔRT_specific: ≤6 points). Sum scores for general and radiotherapy-specific items were higher in countries with a high-income (p < 0.01) and higher health development index (p < 0.01). A larger annual number of irradiated pediatric patients was associated with higher sum scores for general items (p < 0.01).CONCLUSION: This survey demonstrates the disparities in organization of pediatric radiotherapy departments between SIOPE-affiliated countries and centers within the same country. Investment is needed to reduce inequalities in pediatric radiotherapy care.</p
