    Transcatheter closure of iatrogenic Gerbode defect with an Amplatzer duct occluder in a 23-year-old patient

    A 23-year-old man was referred to our center with hematuria and hemolysis. The patient had undergone mitral and tricuspid valve replacement 3 months previously. Echocardiography and catheterization revealed a Gerbode-type ventricular septal defect. A decision was made to occlude the defect interventionally. The patient's hematuria ceased immediately safter the occlusion of the defect. © 2015 Japanese College of Cardiology

    The Role of Tawheedic Stimulus in the Conscience of Muslim Individuals

    Muslim individual’s worldview, culture and ethics are based on tawheed. In social, political and religious contexts, emotional state and motivational status of these individuals also inject perceptional premise to the way these individuals set value related priorities. The way Muslims see things, are mainly derived from faith, experience and family background. Values, knowledge and religious consciousness are therefore, the essence of tawheed in the mind of modern Muslim individuals. The key objective of this paper is to assess how these values are perceived by Muslim individuals in relation to the current world affairs and human relation within the socio-political and diverse worldviews. Muslim normative epistemological attitude towards tawheedic ethos in relation to the current reality of the technological and scientific development must be reassessed for further contextualization.     Menipisnya tanah pertanian akibat konversi (alih fungsi) lahan merupakan salah satu faktor penyebab keterpurukan sektor pertanian di Indonesia. Penyempitan lahan persawahan tidak hanya berdampak pada penurunan produksi padi tapi juga pada penghasilan masyarakat Indonesia yang bermata pencaharian sebagai petani. Kabupaten Kendal merupakan salah satu kabupaten di Jawa Tengah yang menjadi sentra penghasil padi dan memiliki luas lahan sawah yang cukup luas, namun adanya konversi Perubahan lahan sawah yang terjadi di Kabupaten ini sangat tinggi. Akibatnya, lahan sawah yang terdapat dikawasan tersebut contohnya Kecamatan Kaliwungu, Kecamatan Brangsong, dan Kecamatan Kota Kendal mengalami konversi atau Perubahan penggunaan lahan.Pada penelitian ini metode yang digunakan adalah teknik penginderaan jauh dan sistem informasi geografis dengan cara digitasi on screen. Citra yang digunakan adalah Citra Quickbird tahun 2010 dan Citra WorldView-2 tahun 2014. Setelah melakukan proses digitasi kemudian didapatkan luasan daerah yang terkonversi, lalu dianalisis menggunakan metode analisis tetangga terdekat untuk mengetahui pola persebaran lahan sawah dan pola daerah klasifikasi penggunaan lahan lainnya. Analisis sosial ekonomi digunakan untuk mengetahui dampak konversi lahan terhadap para petani.Berdasarkan pengolahan data dan hasil analisis didapatkan Perubahan luas lahan sawah di Kecamatan Kaliwungu, Kecamatan Brangsong dan Kecamatan Kota Kendal pada tahun 2010 sampai tahun 2014 sebesar 54,753 Ha. Pola persebaran luasan sawah tiap kecamatan pada tahun 2010 dan tahun 2014 adalah acak, sedangkan pola Perubahan luas pada tiap kecamatan adalah mengelompok. Dampak sosial ekonomi yang ditimbulkan dengan adanya konversi lahan sawah adalah semakin berkurangnya lahan sawah berpengaruh negatif terhadap nilai pendapatan petani, dimana USAha berproduksi padi di sawah tidak memberikan masukan yang mencukupi bagi kehidupan sehari hari petani, sehingga hal tersebut mendorong petani unuk menjual lahan sawahnya untuk beralih ke USAha lain.Kata Kunci : Sawah, Perubahan Lahan, Citra Quickbird, Citra WorldView-2ABSTRACTThe depletion of landagriculture due to the land conversion is one of the factors causing the deterioration of agricultural sector in Indonesia.The constriction of paddy fields is not only affected onthe decrease of rice production but also the income of Indonesian people who are farmers.Kendal is one of regencies in Central Java being rice production centers and has a quite spacious paddy field, but the conversion of paddy field in this county is really high. It might caused by several factors, including the increase of population and construction activities.As a result, for instance the paddy field which located in the area of the District Kaliwungu, District Brangsong and District Kendal get conversion or change of land use.The method that used in this research is the technique of remote sensing and geographic information system by digitize on screen. The image used is QuickBird Satellite Images 2010 and the WorldView-2 Satellite Images in 2014.After doing digitization process then obtained the extents of converted area, afterward analyzed using the nearest neighbor analysis method to determine the distribution pattern of paddy field and other patternof land use classification area. Socio-economic analysis is used to find out the impact of land conversion against the farmers.Based on the result of analyzing and data processing, it has been found a change of paddy field area in District Kaliwungu, District Brangsong and District Kendal in 2010 untill 2014 amounted 54,753 Ha.The pattern distribution of paddy field each district in 2010 and 2014 are random, while the the pattern of changing area in each district is clustered. Social economic impacts posed by the existence of conversion of paddy fields was reduced the negative effect wetland of farmer income, where rice production in rice's efforts do not provide sufficient input for life day farmers, so it encourages farmers to sell land it to move on to other business.     Caspian Sea level fluctuation and determination of setback line

    In the past 25 years, rising of the Caspian Sea level, part of a natural treat to the sea, has inundated and destroyed many buildings and arable lands and threatened many inhabitations in coastal areas. The main reason for these damages is that the law-setback has lost its efficiency and human activities have proceeded seaward. The goal of this study is to introduce a proper setback line for the southern coast of Caspian Sea on the basis of critical water elevation and the results of coastal vulnerability assessment to sea level rise. This setback contains vertical and horizontal buffers. The Coastal vulnerability index (CVI) method is used for coastal vulnerability assessment and is also used in the Geographic Information System. Five variables in two sub-indices were used in this method. The final map obtained from coastal vulnerability assessment divided the coastal zone into low, moderate, high and very high risk categories based on quartile ranges and visual inspection of data. A mean distance of very high risk category of vulnerability map from a second vertical buffer in each rural district was then proposed as a width of horizontal buffer in the same rural district

    Use of the Trendelenburg Position in the Porcine Model Improves Carotid Flow During Cardiopulmonary Resuscitation

    BACKGROUND: Cardiopulmonary resuscitation (CPR) is now widely used as a treatment for ventricular fibrillation, though numerous studies have shown the outcome of standard CPR to be dismal. Alternative methods of CPR, including interposed abdominal compression, constant aortic occlusion, and the use of intrathoracic pressure regulator, have been shown to increase cardiac output and affect the mortality rate of CPR.OBJECTIVES: Here we suggest the Trendelenburg position as yet another method of increasing cardiac output and therefore improving the effectiveness of chest compressions. We hypothesized that the use of the Trendelenburg position during CPR would increase cardiac output as measured by carotid blood flow.METHODS: We anaesthetized six pigs and measured their pre-arrest carotid flow rate for two minutes. We then induced ventricular fibrillation in those pigs and performed open-chest CPR on them. Post-arrest carotid blood flow was measured for two minutes each at 0 (supine position), 10, 20, and 30 degrees of head-down tilt in each pig. The mean carotid flow for each degree of tilt was compared to mean carotid flow at 0 degrees of tilt using a paired student t-test.RESULTS: We found an increase of up to 1.4-fold in carotid blood flow during CPR in the Trendelenburg position, though only 20 and 30 degrees of Trendelenburg showed a statistically significant increase from the 0 degrees of tilt in pigs.CONCLUSION: The Trendelenburg position can lead to increased blood flow through the carotid arteries during CPR in this pig model. Future studies should investigate whether this increased blood flow through the carotid arteries leads to improved brain perfusion and better neurologic outcomes
