    In-vitro Anti-bacterial Activity of Ethanolic and Aqueous Leaf Crude Extracts of Solanum Nigrum (Black Night Shade) of Bushenyi District - Uganda on Selected Enteric Bacteria

    This research was aimed to determine the antibacterial activity of ethanolic and aqueous crude extracts of Solanum nigrum leaves against some selected enteric bacteria. Fresh leaves of S. nigrum were collected from different garden of Bushenyi district Western Uganda and shade dried. Extraction was done by using standard methods. Phytochemical analyses of both ethanolic and aqueous crude extracts were also done. Antibacterial activities of both aqueous and ethanolic crude extracts were determined against clinical isolates of Escherichia coli, Klebsiella sp, and Shigella sp and Salmonella typhimurium by using agar well diffusion method and compared to the standard antibiotics Ciprofloxacin (5%2526micro%253Bg%252FmL) and Cotrimoxazole (25%2526micro%253Bg%252FmL). The results of phytochemicals analyses from this study revealed the presence of tannins, alkaloids, and saponins, reducing sugars, terpenoids and steroids from the two extracts. The ethanolic extract was effective only against E. coli at concentrations of 1, 0.5 and 0.25g%252FmL with 20.33%2526plusmn%253B0.33, 15.17%2526plusmn%253B0.17 and 8.33%2526plusmn%253B0.17 mm as mean %2526plusmn%253B SEM zones of inhibition respectively, while aqueous crude extract was effective against E. coli only at concentration of 1g%252FmL with 9.17%2526plusmn%253B01.7 mm as the mean %2526plusmn%253B SEM zone of inhibition. The ethanolic crude extract had lower MIC and MBC values of 250 mg%252FmL and 500 mg%252FmL respectively compared to the aqueous crude extract with MIC and MBC values of 500 mg%252FmL and %2526gt%253B1000 mg%252FmL respectively. The results of this study concluded that both ethanolic and aqueous crude extract of S. nigrum leaves had activity only against clinical E.coli. Ethanolic leaves crude extract of S. nigrum was more effective than the aqueous crude leaves extract. This may provide evidences for its usage as herbal remedy against enteric infections caused by E. coli

Second, real-time public health surveillance and monitoring of intervention coverage, and third, feedback of information to individuals, care providers and the public for transparent accountability. This series on eRegistries presents frameworks and tools to facilitate the development and secure operation of eRegistries for maternal and child health.In this first paper of the eRegistries Series we have used WHO frameworks and taxonomy to map how eRegistries can support commonly used electronic and mobile applications to alleviate health systems constraints in maternal and child health. A web-based survey of public health officials in 64 low- and middle-income countries, and a systematic search of literature from 2005-2015, aimed to assess country capacities by the current status, quality and use of data in reproductive health registries.eRegistries can offer support for the 12 most commonly used electronic and mobile applications for health. Countries are implementing health registries in various forms, the majority in transition from paper-based data collection to electronic systems, but very few have eRegistries that can act as an integrating backbone for health information. More mature country capacity reflected by published health registry based research is emerging in settings reaching regional or national scale, increasingly with electronic solutions. 66 scientific publications were identified based on 32 registry systems in 23 countries over a period of 10 years; this reflects a challenging experience and capacity gap for delivering sustainable high quality registries.Registries are being developed and used in many high burden countries, but their potential benefits are far from realized as few countries have fully transitioned from paper-based health information to integrated electronic backbone systems.     Enhanced infection prophylaxis reduces mortality in severely immunosuppressed HIV-infected adults and older children initiating antiretroviral therapy in Kenya, Malawi, Uganda and Zimbabwe: the REALITY trial

Introduction: Mortality from infections is high in the first 6 months of antiretroviral therapy (ART) among HIV‐infected adults and children with advanced disease in sub‐Saharan Africa. Whether an enhanced package of infection prophylaxis at ART initiation would reduce mortality is unknown. Methods: The REALITY 2×2×2 factorial open‐label trial (ISRCTN43622374) randomized ART‐naïve HIV‐infected adults and children >5 years with CD4 <100 cells/mm3. This randomization compared initiating ART with enhanced prophylaxis (continuous cotrimoxazole plus 12 weeks isoniazid/pyridoxine (anti‐tuberculosis) and fluconazole (anti‐cryptococcal/candida), 5 days azithromycin (anti‐bacterial/protozoal) and single‐dose albendazole (anti‐helminth)), versus standard‐of‐care cotrimoxazole. Isoniazid/pyridoxine/cotrimoxazole was formulated as a scored fixed‐dose combination. Two other randomizations investigated 12‐week adjunctive raltegravir or supplementary food. The primary endpoint was 24‐week mortality. Results: 1805 eligible adults (n = 1733; 96.0%) and children/adolescents (n = 72; 4.0%) (median 36 years; 53.2% male) were randomized to enhanced (n = 906) or standard prophylaxis (n = 899) and followed for 48 weeks (3.8% loss‐to‐follow‐up). Median baseline CD4 was 36 cells/mm3 (IQR: 16–62) but 47.3% were WHO Stage 1/2. 80 (8.9%) enhanced versus 108(12.2%) standard prophylaxis died before 24 weeks (adjusted hazard ratio (aHR) = 0.73 (95% CI: 0.54–0.97) p = 0.03; Figure 1) and 98(11.0%) versus 127(14.4%) respectively died before 48 weeks (aHR = 0.75 (0.58–0.98) p = 0.04), with no evidence of interaction with the two other randomizations (p > 0.8). Enhanced prophylaxis significantly reduced incidence of tuberculosis (p = 0.02), cryptococcal disease (p = 0.01), oral/oesophageal candidiasis (p = 0.02), deaths of unknown cause (p = 0.02) and (marginally) hospitalisations (p = 0.06) but not presumed severe bacterial infections (p = 0.38). Serious and grade 4 adverse events were marginally less common with enhanced prophylaxis (p = 0.06). CD4 increases and VL suppression were similar between groups (p > 0.2). Conclusions: Enhanced infection prophylaxis at ART initiation reduces early mortality by 25% among HIV‐infected adults and children with advanced disease. The pill burden did not adversely affect VL suppression. Policy makers should consider adopting and implementing this low‐cost broad infection prevention package which could save 3.3 lives for every 100 individuals treated
