    Assessment of Bacterial Contamination in Herbal Medicine Products Vended in Morogoro Municipality, Tanzania

    Traditional medicines are widely used in Tanzania; however, the microbiological safety of herbal medicine products (HMPs) is unknown. A cross-sectional study was conducted to determine microbial levels and antimicrobial susceptibility of bacteria isolated from HMPs vended in Morogoro Municipality. Fifty samples of HMPs were collected from vendors in six wards in the municipality. Bacterial contamination was determined through total viable count and bacterial isolation while susceptibility to the selected antimicrobials was determined by agar disc diffusion method. About 88% of the tested HMPs significantly (p<0.05) had higher total bacterial counts than WHO recommended levels. Ten percent of HMPs were contaminated with the pathogenic E. coli and 8% with S. aureus. The isolated bacteria were only susceptible to ciprofloxacin but were resistant to the rest of the tested antimicrobials at standard doses. Unhygienic handling practices and limited safety knowledge by the HMPs vendors was also observed. Use of the unregulated but vended HMPs may put the users at risk of acquiring infections with pathogenic and antimicrobial resistant bacteria that portends increased treatment challenges

    Disease behaviours of sows naturally infected with <i>Taenia solium</i> in Tanzania

    Neurocysticercosis (NCC) is a disease caused by the zoonotic parasite Taenia solium lodging in the central nervous system. Both humans and pigs can get NCC. The impact of the disease in pigs has so far been little explored. The aim of this study was to describe the effect of NCC on social and feeding behaviours as well as the pattern of activity as indicators of reduced welfare in naturally infected sows. In total 13 T. solium naturally infected and 15 non-infected control sows were videotaped for 2 consecutive weeks using close circuit television cameras at research facilities at Sokoine University of Agriculture, Morogoro, Tanzania. Videos were analysed at the beginning, in the middle and at the end of the 2 week recording period. For each time point, videos were analysed during feeding, while the enrichment was provided, and by recording every half an hour the sows’ behaviours performed over the course of a whole day. Sows with NCC spent significantly less time at the feeding trough, especially during the second half of the feeding period. Infected sows were also more passive e.g. lying and standing still significantly more during a whole day period and showed social isolation compared to non-infected control sows by performing behaviours more distant to their nearest neighbour. Results of this study indicated that NCC changed the behaviour of infected sows. The behavioural changes are indicative of decreased welfare. Efforts to reinforce the animal welfare aspect are needed as this has so far been neglected

    Severe seizures in pigs naturally infected with <i>Taenia solium</i> in Tanzania

    AbstractNeurocysticercosis (NCC) caused by Taenia solium is a serious neurological disease. In humans neurological symptoms have been thoroughly studied and documented, however, there is limited information on clinical signs in pigs infected with T. solium cysticerci. Among the scientific community, it is in fact believed that pigs with NCC rarely show neurological signs. The aim of this study was to describe clinical manifestations associated with NCC in pigs and correlate the manifestations to the number and distribution of cysticerci in brains of naturally infected pigs in Tanzania. Sixteen infected and 15 non-infected control pigs were observed for 14 days during daylight hours, and subsequently videotaped for another 14 consecutive days using close circuit television cameras. All occurrences of abnormal behaviour (trembling, twitching, mouth and ear paralysis, ataxia, dribbling, salivating, eye blinking, walking in circles) were recorded. At the end of the recording period, pigs were slaughtered and their brains dissected, cysticerci counted and locations noted. During the recording period, two infected pigs were observed having seizures. Some of the observed autonomic signs during a seizure were chewing motions with foamy salivation and ear stiffening. Motor signs included tonic muscle contractions followed by a sudden diminution in all muscle function leading to collapse of the animal. Stereotypic walking in circles was observed on several occasions. At dissection, both pigs had a high number of brain cysticerci (241 and 247 cysticerci). The two pigs with seizures were also older (36 months) compared to the others (18.3 months, ± 8.2 standard deviation). Results of this study have shown that pigs with NCC can develop clinical signs and suffer from seizures like humans with symptomatic NCC. Results of this study could potentially open up a new experimental pathway to explore the aetiology of neurological symptoms in humans with NCC associated epilepsy

    Survey of physicochemical characteristics and microbial contamination in selected food locally vended in Morogoro Municipality, Tanzania

    Biomed central Research notel 2015Background: Raw milk, raw fruit juice and raw fish are enriched with essential nutrients for human diet but are prone to microbial contamination along the value chain. This cross sectional study was conducted to assess physicochemical characteristics and microbial quality of raw milk, fruit juice and fish from food vendors in Morogoro Municipality, Tanzania. The physicochemical assessment of food samples was done by smell, colour, presence of debris, turbidity, consistence, pH and clot on alcohol test. Hygiene of food containers, personnel and the vending environment was also assessed. Qualitative and quantitative microbial assessment of food was done using standard laboratory protocols as described by Tanzania Bureau of Standards and International Systems of Standards. Results: Raw milk sold in Morogoro was of poor quality since was adulterated with water, contained sediments and clotted on alcohol test. Up to 63 % of the milk samples were contaminated with Escherichia coli and 60 % had higher total viable count (TVC) than the recommended values. Raw fruit juice was stored in dirty containers and sold under unhygienic environment. Seventy-three percent of juice samples had TVC beyond the recommendations while E. coli contamination rate was 63.3 %. The raw fish samples had started spoiling as depicted through sensory evaluation. E. coli contamination rate was 55 % and that of Campylobacter jejuni was 0.5 %. The mean TVC of raw fish was 8.1 (Log cfu/g) and 96.2 % of the fish samples had TVC beyond the recommended limits of 5.0 Log cfu/g. Conclusions: The physicochemical characteristics of food vended in Morogoro Municipality were of poor quality. The food had high bacterial contaminations. This situation poses health risks to the public and losses to food vendors due to spoilage. Stakeholders in food value chain should be educated on safe production and good hygienic practices. Routine quality and safety assessment of locally vended food, inspection of selling premises and regular health checkup of the personnel involved in food vending industry should be instituted

    Stepwise approach for the control and eventual elimination of Taenia solium as a public health problem

    Background: Taenia solium taeniosis/cysticercosis is a public health and agricultural problem, especially in low-income countries, and has been ranked the top foodborne parasitic hazard globally. In 2012, the World Health Organization published a roadmap that called for a validated strategy for T. solium control and elimination by 2015. This goal has not been met, and validated evidence of effective control or elimination in endemic countries is still incomplete. Measuring and evaluating success of control programmes remains difficult, as locally acceptable targets have not been defined as part of the 2012 roadmap nor from other sources, and the performance of tools to measure effect are limited. Discussion: We believe that an international agreement supported by the tripartite World Health Organization, Food and Agriculture Organization of the United Nations, and World Organisation for Animal Health is needed to facilitate endemic countries in publicising SMART (Specific, Measurable, Achievable/attainable, Relevant, Time-bound) country-level control target goals. These goals should be achievable through locally acceptable adoption of options from within a standardised intervention tool-kit', and progress towards these goals should be monitored using standardised and consistent diagnostics. Several intervention tools are available which can contribute to control of T. solium, but the combination of these - the most effective control algorithm - still needs to be identified. In order to mount control efforts and ensure political commitment, stakeholder engagement and funding, we argue that a stepwise approach, as developed for Rabies control, is necessary if control efforts are to be successful and sustainable. Conclusions: The stepwise approach can provide the framework for the development of realistic control goals of endemic areas, the implementation of intervention algorithms, and the standardised monitoring of the evaluation of the progress towards obtaining the control target goals and eventually elimination

    Spatial Clustering of Porcine Cysticercosis in Mbulu District, Northern Tanzania

    Taenia solium is a tapeworm that causes two different disease conditions. In its adult stage, it inhabits the small intestine of human, a condition known as taeniosis, which is characterised by mild symptoms including abdominal disconfort. In the larval stage, T. solium can infect humans and various animal species, mainly pigs, causing cysticercosis. Taeniosis is acquired through consumption of inadequately cooked infected meat, while cysticercosis is acquired through ingestion of tapeworm eggs in foodstuffs contaminated with faeces from a human tapeworm carrier. Cysticercosis of human central nervous tissues (neurocysticercosis) causes serious syndromes such as epilepsy. Transmission of T. solium is facilitated by several factors such as presence of tapeworm carriers, poor sanitation and poor pig husbandry, which allow pigs to access human faeces. Nevertheless, the role of these factors in parasite transmission may vary with different cultural settings. Following an incidence and a prevalence studies in a rural area of northern Tanzania, there was a significant spatial clustering of porcine cysticerocis, suggesting focal distribution of transmission risk factors, which could be targeted for interventions. The study also revealed that despite the low sensitivity of the lingual examination method to detect porcine cysticercosis, it could highlight the potential ‘hotspots’ of the infection
