Percutaneous endoscopic gastrostomy: Indications, technique and complications at Groote Schuur Hospital

Estimating the incidence of equine viral arteritis and the sensitivity of its surveillance in the French breeding stock
Equine viral arteritis (EVA) may have serious economic impact on the equine industry. For this reason, it is monitored in many countries, especially in breeding stock, to avoid its spread during breeding activities. In France, surveillance is mainly based on serological tests, since mares are not vaccinated, but difficulties in interpreting certain series of results may impair the estimation of the number of outbreaks. In this study, we propose specific rules for identifying seroconversion in order to estimate the number of outbreaks that were detected by the breeding stock surveillance component (BSSC) in France between 2006 and 2013. A consensus among multidisciplinary experts was reached to consider seroconversion as a change in antibody titer from negative to at least 32, or as an eight-fold or greater increase in antibody level. Using these rules, 239 cases and 177 outbreaks were identified. Subsequently, we calculated the BSSC's sensitivity as the ratio of the number of detected outbreaks to the total number of outbreaks that occurred in breeding stock (including unreported outbreaks) estimated using a capture-recapture model. The total number of outbreaks was estimated at 215 (95% credible interval 195-249) and the surveillance sensitivity at 82% (CrI95% 71-91). Our results confirm EVA circulation in French breeding stock, show that neutralizing antibodies can persist up to eight years in naturally infected mares and suggest that certain mares have been reinfected. This study shows that the sensitivity of the BSSC is relatively high and supports its relevance to prevent the disease spreading through mating
Percolation model for structural phase transitions in LiHIO mixed crystals
A percolation model is proposed to explain the structural phase transitions
found in LiHIO mixed crystals as a function of the
concentration parameter . The percolation thresholds are obtained from Monte
Carlo simulations on the specific lattices occupied by lithium atoms and
hydrogen bonds. The theoretical results strongly suggest that percolating
lithium vacancies and hydrogen bonds are indeed responsible for the solid

Transmission Properties of the oscillating delta-function potential
We derive an exact expression for the transmission amplitude of a particle
moving through a harmonically driven delta-function potential by using the
method of continued-fractions within the framework of Floquet theory. We prove
that the transmission through this potential as a function of the incident
energy presents at most two real zeros, that its poles occur at energies
(), and that the
poles and zeros in the transmission amplitude come in pairs with the distance
between the zeros and the poles (and their residue) decreasing with increasing
energy of the incident particle. We also show the existence of non-resonant
"bands" in the transmission amplitude as a function of the strength of the

Accurate evaluation of the interstitial KKR-Green function
It is shown that the Brillouin zone integral for the interstitial KKR-Green
function can be evaluated accurately by taking proper care of the free-electron
singularities in the integrand. The proposed method combines two recently
developed methods, a supermatrix method and a subtraction method. This
combination appears to provide a major improvement compared with an earlier
proposal based on the subtraction method only. By this the barrier preventing
the study of important interstitial-like defects, such as an electromigrating
atom halfway along its jump path, can be considered as being razed.Comment: 23 pages, RevTe
HI in the Outskirts of Nearby Galaxies
The HI in disk galaxies frequently extends beyond the optical image, and can
trace the dark matter there. I briefly highlight the history of high spatial
resolution HI imaging, the contribution it made to the dark matter problem, and
the current tension between several dynamical methods to break the disk-halo
degeneracy. I then turn to the flaring problem, which could in principle probe
the shape of the dark halo. Instead, however, a lot of attention is now devoted
to understanding the role of gas accretion via galactic fountains. The current
cold dark matter theory has problems on galactic scales, such as
the core-cusp problem, which can be addressed with HI observations of dwarf
galaxies. For a similar range in rotation velocities, galaxies of type Sd have
thin disks, while those of type Im are much thicker. After a few comments on
modified Newtonian dynamics and on irregular galaxies, I close with statistics

Meeting Medicare requirements for transitional care: Do stroke care and policy align?
ObjectiveThis study (1) describes transitional care for stroke patients discharged home from hospitals, (2) compares hospitals' standards of transitional care with core transitional care management (TCM) components recognized by Medicare, and (3) examines the association of policy and hospital specialty designations with TCM implementation.MethodsHospitals participating in the Comprehensive Post-Acute Stroke Services (COMPASS) Study provided data on their hospital, stroke patient population, and standards of transitional care. Hospital-reported transitional care strategies were compared with the federal TCM definition (2-day follow-up, 14-day visit, non-face-to-face services). We examined the associations of TCM billing, stroke center certification, and Magnet nursing excellence designation with TCM implementation.ResultsTransitional care varied widely among 41 hospitals in North Carolina and no one strategy was universally applied or provided across hospitals. One third of hospitals met the TCM definition (37% provided telephone follow-up, 76% provided face-to-face provider follow-up, all provided a type of non-face-to-face support). There were no differences between groups (TCM met/not met) in hospital characteristics or transitional care resources and processes. Stroke center certification, Magnet designation, and use of TCM billing codes were not different for hospitals that did and did not meet the TCM definition.ConclusionsThere was substantial variation in the provision of strategies supporting stroke patients' transition home from the hospital. Supportive stroke care transitions are essential when more than 50% of stroke patients are discharged home and more than half experience moderate to severe strokes.
