Designing nanoparticles during the drawing step
International audienceNanoparticles in the core of optical fibres are widely studied due to the opportunity they give to tailor spectroscopic properties. Such fibres are usually obtained by drawing at high temperature a preform containing nanoparticles. This study focuses on the effect of the fibre drawing on nanoparticles. We fabricated an MCVD optical preform by doping the porous layer with nanoparticles. The optical fibre was studied by a FIB/SEM tomography.Figure 1 is the volume reconstruction of the core of the optical fibre. The yellow phase represents nanoparticles inside the core of the optical fibre. This reconstruction shows evidences of break-up, elongation and coalescence of particles. These features will be discussed according to phenomena well known from the rheology of emulsions and polymers. It comes from a competition between viscous stresses of the flow and surface tension.Observation of these size-controlling phenomena occuring during fibre drawing offer new perspectives to tailor the size of nanoparticles and are therefore of great interest for light scattering issues

Urine proteomics in the diagnosis of stable angina.
BACKGROUND: We have previously described a panel of 238 urinary polypeptides specific for established severe coronary artery disease (CAD). Here we studied this polypeptide panel in patients with a wider range of CAD severity. METHODS: We recruited 60 patients who underwent elective coronary angiography for investigation of stable angina. Patients were selected for either having angiographic evidence of CAD or not (NCA) following coronary angiography (n = 30/30; age, 55 ± 6 vs. 56 ± 7 years, P = 0.539) to cover the extremes of the CAD spectrum. A further 66 patients with severe CAD (age, 64 ± 9 years) prior to surgical coronary revascularization were added for correlation studies. The Gensini score was calculated from coronary angiograms as a measure of CAD severity. Urinary proteomic analyses were performed using capillary electrophoresis coupled online to micro time-of-flight mass spectrometry. The urinary polypeptide pattern was classified using a predefined algorithm and resulting in the CAD238 score, which expresses the pattern quantitatively. RESULTS: In the whole cohort of patients with CAD (Gensini score 60 [40; 98]) we found a close correlation between Gensini scores and CAD238 (ρ = 0.465, P < 0.001). After adjustment for age (β = 0.144; P = 0.135) the CAD238 score remained a significant predictor of the Gensini score (β =0.418; P < 0.001). In those with less severe CAD (Gensini score 40 [25; 61]), however, we could not detect a difference in CAD238 compared to patients with NCA (-0.487 ± 0.341 vs. -0.612 ± 0.269, P = 0.119). CONCLUSIONS: In conclusion the urinary polypeptide CAD238 score is associated with CAD burden and has potential as a new cardiovascular biomarker
Assessment of PI3K/mTOR/AKT Pathway Elements to Serve as Biomarkers and Therapeutic Targets in Penile Cancer
The PI3K/mTOR/AKT pathway might represent an intriguing option for treatment of penile cancer (PeCa). We aimed to assess whether members of this pathway might serve as biomarkers and targets for systemic therapy. Tissue of primary cancer from treatment-naïve PeCa patients was used for tissue microarray analysis. Immunohistochemical staining was performed with antibodies against AKT, pAKT, mTOR, pmTOR, pS6, pPRAS, p4EBP1, S6K1 and pp70S6K. Protein expression was correlated with clinicopathological characteristics as well as overall survival (OS), disease-specific survival (DSS), recurrence-free survival (RFS) and metastasis-free survival (MFS). AKT inhibition was tested in two primarily established, treatment-naïve PeCa cell lines by treatment with capivasertib and analysis of cell viability and chemotaxis. A total of 76 patients surgically treated for invasive PeCa were included. Higher expression of AKT was significantly more prevalent in high-grade tumors and predictive of DSS and OS in the Kaplan–Meier analysis, and an independent predictor of worse OS and DSS in the multivariate regression analysis. Treatment with pan-AKT inhibitor capivasertib in PeCa cell lines induced a significant downregulation of both total AKT and pAKT as well as decreased cell viability and chemotaxis. Selected protein candidates of the mTOR/AKT signaling pathway demonstrate association with histological and survival parameters of PeCa patients, whereas AKT appears to be the most promising one
Reliability and clinical relevance of frozen-section diagnosis in maxillofacial surgery
Zur Evaluierung der Zuverlässigkeit von Schnellschnitten wurden retrospektiv
588 Schnellschnittanforderungen in Bezug auf ihre Aufgabenstellung analysiert.
Zur Beurteilung der prognostischen Relevanz wurden die Daten von 252
Tumoroperationen mit intraoperativ schnellschnittkontrollierten
Randverhältnissen nachverfolgt und die beobachteten Rezidivraten statistisch
in Bezug zu den Randbefunden gestellt. Es zeigte sich eine diagnostische Lücke
von rund 10 %, in der keine eindeutige Diagnose gestellt werden konnte. Diese
Lücke bezog sich ausschließlich auf Tumoren der Speicheldrüsen und des
lymphatischen Gewebes sowie auf Ameloblastome. Für die alleinige
Differenzierung zwischen benignen und malignen Prozessen war das Verfahren
jedoch auch für diese Tumorarten adäquat. Für Karzinome und Basaliome stellte
die intraoperative Schnellschnittdiagnose mit einer hohen Reproduzierbarkeit
ein geeignetes Kriterium zur intraoperativen Entscheidungsfindung dar. Zu 98 %
wurden die Ergebnisse der Lymphknotenbeurteilungen bestätigt. Allerdings
konnten mit der Schnellschnittdiagnostik nur 86 % aller Metastasen entdeckt
werden. Die Wahrscheinlichkeit, in der intraoperativen Randbeurteilung sowohl
ein eindeutiges als auch ein richtiges Ergebnis zu erhalten, betrug insgesamt
über 90 %. Die Fehlerrate der Schnellschnittergebnisse verdreifachte sich von
ursprünglich 5 % in der Erstoperation nach wiederholter Operation auf über 15
%. Primäre Randfreiheit im Schnellschnitt hatte dabei eine günstigere Prognose
als erst in einer nachfolgenden Operation erreichte Randfreiheit. Bei nicht
erreichter Randfreiheit im ersten Eingriff konnte durch Nachbestrahlung zwar
die Rezidivwahrscheinlichkeit gesenkt werden, erreichte aber die Prognose von
randfreien Nachresektionen bei weitem nicht. Nicht sicher und knapp freie
Randbefunde sowie Randbefunde mit prämalignen Veränderungen konnten nicht
wesentlich zur Senkung des Rezidivrisikos beitragen. Hier sollte bei
entsprechenden Schnellschnittbefunden intraoperativ bis zum Erreichen eines
eindeutig freien Randbefundes nachreseziert werden. Der prognostische Wert der
Randbefunde differierte in Abhängigkeit von der Lokalisation und Größe des
Tumors. Mundbodenkarzinome und Tumoren des Stadiums III und IV rezidivierten
nach Resektion mit freiem Rand überproportional häufig. Die Beobachtung legt
nahe, dass bei zunehmender Größe und Unzugänglichkeit des Operationsgebietes
randrepräsentative Proben schwieriger zu gewinnen sind. Die relativ hohe
Rezidivwahrscheinlichkeit der Alveolarfortsatzkarzinome ist in der
diagnostischen Lücke von Schnellschnitten im knöchernen Gewebe zu suchen; die
hohe Rezidivwahrscheinlichkeit von Basaliomen der Orbitaregion eher in einer
weniger radikal angelegten Operation.To evaluate the histological accuracy and clinical relevance, 588 requests for
intraoperative frozen section diagnosis (FS) were analysed for their tasks. To
evaluate prognostic relevance, margin diagnosis of 252 tumor-resections were
followed up and recurrence-rates were statistically analysed. In a „diagnostic
gap“ of about 10 %, diagnosis was deferred and could not help the surgeon to
make his decisions. This insecurity occurred only in tumors of salivary glands
and in tumors of lymphatic or odontogenic origin but not in tumors of the
skin. To differentiate between benign and malign lesions, FS was sufficient
for all type of tumors. Results of lymph-node diagnosis were accurate in 98 %
of all cases. Nevertheless 14 % of the metastases could not be recognised.
Diagnosis of the margins was doubt-free and accurate in more than 90 % of all
cases. The error rate in the cases of multiple surgery was 15 %, which was
three times higher than the 5 % error rate of first time surgery. Negative
margins in first time surgery had a better prognosis than negative margins
achieved in a further operation. Radiotherapy after positive margins could
diminish recurrence rate but results were poorer than prognosis of further
surgery till negative margins. Findings as: „malignant cells close to the
margins“, „premalign cells at the margin“ „uncertain negative margins“ did not
result in good prognosis. Therefore further surgery till negative margins is
recommended. Prognostic relevance differed depending on localisation and size
of the tumor. Carcinomas of the oral floor and tumors of stage III and IV with
negative margins lead to higher recurrence rates than other types of tumors.
That means, that representative margins are less reliable to expect in areas,
which are big sized and difficult to survey. High recurrence rate of
carcinomas of the alveolar ridge is due to the diagnostic gap of frozen
sections in bony structures; high recurrence rate of basalioma of the orbital
region is likely due to a less radical surgery intention
Clinical comparison of the management of the extraction socket before implantation
Einleitung Das Zahnfach unterliegt nach der Extraktion einer Atrophie mit
Veränderung der Abmessungen. Alveolarfortsatzerhaltende Maßnahmen sollen die
Ausgangssituation bei der späteren Implantatsetzung verbessern. Das Ziel
dieser randomisierten kontrollierten Studie ist die Bewertung des Nutzens
zweier unterschiedlicher Socket Preservation Maßnahmen. Methodik 120 Fälle von
Zahnentfernungen in der ästhetischen Zone wurden stratifiziert randomisiert
drei Studienarmen zugeteilt: Eine Gruppe erhielt eine Verfüllung der Alveole
mit einem porcinen Kollagenkegel und Abdeckung mit einer Membran. Die andere
Gruppe erhielt ein autologes Composit-Transplantat aus Tuberknochen mit
bedeckender Mukosa. In der Kontrollgruppe erfolgte die Heilung durch
Koagelbildung. Der Grund des Zahnverlustes, die Lokalisation und die
Defektsituation wurden dokumentiert und die Alveole vermessen. Zur Einteilung
der Defektsituation bei Extraktion und Implantation wird dafür ein
funktionelles Klassifikationschema vorgeschlagen: Bei Klasse I liegen alle
Alveolenwände im physiologischen Bereich unter der gedachten Schmelz-Zement-
Grenze. In Klasse II sind eine bis drei Regenerationsflächen höhenreduziert.
In Klasse III ist die Alveolenhöhe insgesamt reduziert. Nach 4 Monaten
erfolgte eine Frühimplantation. Zur Evaluation wurden die
Implantationswahrscheinlichkeit, die Defektsituation, metrische
Knochenrandveränderungen und die Notwendigkeit weiterer Knochenaufbauten
betrachtet. Die Kosteneffizienz der unterschiedlichen Maßnahmen wurde
berechnet. Ergebnisse In der Kollagengruppe war die
Implantationswahrscheinlichkeit nicht höher als in der Kontrollgruppe. Nur in
Fällen mit Parodontitis marginalis profunda fand sich ein Vorteil, der
statistisch nicht signifikant war. Der Verlust von Alveolarfortsatzbreite
konnte reduziert werden. Am offensichtlichsten war der stabilisierende Effekt
durch Kollagenkegel im entzündlich veränderten Parodontalgewebe. Signifikanz
wurde nicht erreicht. Für den routinemäßigen Einsatz in allen Fällen zeigte
sich eine negative Kosteneffizienz. In der Gruppe mit autologen Tuber-Os-
Mukosa Transplantaten war die Implantationswahrscheinlichkeit signifikant
höher als in der Kollagengruppe. In den Untergruppen ohne Parodontitis
marginalis profunda sowie in den Klassen I und III war die
Implantationswahrscheinlichkeit allerdings nur gering erhöht. Die klinische
Breite wurde in allen Untergruppen mit Ausnahme der Klasse III signifikant
verbessert. Es zeigte sich eine positive Kosteneffizienz für den Patienten und
eine ausreichende Profitabilität für den Behandler. Schlussfolgerung Für
Klasse I Defekte ohne entzündliche Veränderungen ist eine natürliche Abheilung
ausreichend. Bei entzündlichen Veränderungen des Parodonts reduziert ein
Kollagenkegel oder ein Tuber-Os-Mukosa Transplantat den Breitenverlust. Bei
Klasse II Defekten verbessert ein Transplantat das Knochenangebot und die
Implantationswahrscheinlichkeit. Bei Klasse III Defekten können Socket
Preservation Maßnahmen später notwendig werdende Knochenaufbauten nicht
verhindern.Objective The alveolar ridge undergoes atrophy and dimensional changes
subsequent to tooth removal. Preservation of the alveolar crest can enhance
the surgical site before implant placement. The aim of this randomized
controlled trial is to evaluate the benefits of two different socket
preservation measures. Methods 120 cases of tooth extraction in the esthetic
zone were randomly assigned to three groups: One test group received grafting
of a porcine collagen cone covered with a membrane. The other test group
received an autologous composite graft from bone of the maxillary tuberosity
with its attached mucosa. The control group underwent healing through blood
clot formation. The reason for tooth-loss, the localization and the defective
situation of the sockets were recorded. The dimensions were measured. In order
to grade the defective situations in extraction and implantation sites, a
functional classification scheme is proposed: Class I means that all four
walls of the socket reach the physiological height. In class II cases, one to
three walls of the host bone are diminished. In class III, the whole alveolus
shows a reduced height. After 4 months of healing, early implantation was
performed. The evaluation included the implantation probability, the defective
situation, marginal bone level changes and the need for additional
augmentation procedures. The cost-effectiveness of the different measures was
calculated. Results In the group with collagen, implantation probability was
less achievable as in the control group. Only cases with deep marginal
periodontitis resulted in an advantage which was not significant. The decrease
in clinical width could be reduced with collagen. What was mostly obvious was
the stabilizing effect in inflammatory periodontal tissue. For a standard use
in all cases, a negative cost-effectiveness was revealed. In the group with
autologous bone grafts, implantation was significantly more likely to be
achieved. Nevertheless, the results were only slightly superior in improving
the implantation probability in cases of non-inflammatory tooth loss and class
I and III defects. Clinical width was significantly improved in all subgroups
except class III defects. There was a positive cost-effectiveness ratio for
the patient and sufficient profitability for the practitioner. Conclusion For
class I defects without inflammatory change natural healing is sufficient.
With inflammatory changes in the periodontal tissue, a collagen cone or a
Tuber-Os-Mucosa graft reduces width loss. In class II defects the autograft
improves the bone width and the implantation probability. In class III
defects, socket preservation measures cannot prevent future bone augmentation
The first room-temperature X-ray absorption spectra of higher oxidation states of the tetra-manganese complex of photosystem II
AbstractThe manganese (Mn) complex of photosystem II catalyzes water oxidation. For the first time, its advancement through the reaction cycle was monitored by time-resolved X-ray absorption measurements at the Mn K-edge at room temperature. The complex was stepped through its four oxidation states by nano-second-laser flashes applied to samples exposed to the X-ray beam. Time courses of the X-ray fluorescence intensity were recorded during a flash sequence. Extended X-ray absorption fine-structure spectra were recorded with the S1, S2, and S3 oxidation states highly populated. The room temperature data is compatible with the formation of a third di-μ-oxo bridge between two Mn atoms upon the S2→S3 transition
