Optimizing Emergency Department Throughput Using Best Practices to Improve Patient Flow
Emergency Department (ED) crowding and bottle necks are the reality of hospitals across the country. Patients seeking care and needing inpatient beds via the emergency rooms are facing delays with attaining the right level of care. Orchestrating a patient through an ED admission requires a multidisciplinary effort to provide safe, effective and efficient care. This quality improvement project conducted in a tertiary acute care hospital focused on Centers for Medicare and Medicaid metrics to measure Emergency Department (ED) throughput. This multidisciplinary initiative focused on reducing time stamps for patient arrival to the ED through departure to hospital or home. Outcomes showed a significant decrease in the time frame for patient arrival to being seen by a qualified provider, left without being seen rates, ED diversion, and ancillary department turnaround times. The interventions can be applied at other hospital based emergency departments
Opastuskierroksen elÀmyksellisyyttÀ on mahdollista siis kehittÀÀ entistÀ paremmaksi. Tuotteen kehittÀmiseksi työssÀ esitettiin sekÀ muutosehdotuksia ettÀ tÀysin uusia ideoita opastuksen toteutukseen, jotta elÀmyksen synty opastuskierroksen seurauksena vahvistuisi. The purpose of this thesis was to examine how a new guided tour of Valkeakoski, launched in 2015, meets the criteria and characteristics of experience. The purpose was also to study what the factors are that could make this tourism product more memorable. The objective of this study was to gather as much information as possible on the memorability of a guided tour and the opportunities to develop it a more memorable experience.
The approach of research in this thesis was qualitative and the data were collected through participatory observation on a guided tour, through an open interview of the person in charge of tourism in Valkeakoski and some brochures and e-mails. The data were analyzed using qualitative analysis and the main theory, experience triangle, developed by Sanna Tarssanen and Mika KylÀnen.
The assumption of this thesis was that a guided tour is memorable, because that adjective had been used when marketing the product. The findings of the study indicate that a guided tour is memorable according to the experience triangle, although it lacks some features of memorability. The study suggests some changes and new ideas for implementing the tour to develop the product
Assessing the effects of ANO7 mutations on mRNA splicing using a minigene splicing assay
Prostate cancer (PrCa) is the most common cancer type in men. Dysregulated splicing
is considered a hallmark of cancer, and PrCa has its own characteristic splicing
landscape. Alternative splicing (AS) enables production of multiple protein isoforms
from a single gene by altering splicing of exons and introns. AS outcomes can be
studied with reverse transcription PCR (RT-PCR) or a minigene splicing assay, which
allows comparison of wild type and mutant sequences in identical conditions.
This MSc study focused on two splicing mutations in ANO7, a PrCa susceptibility gene.
Variant rs77559646 in the splice donor site of intron 4 leads to exon skipping and
increased intronic expression presumably by disrupting binding of an essential splicing
factor. The variant was corrected by CRISPR-Cas9 base-editing, also producing a new
unwanted mutation upstream. This study aimed to determine whether normal splicing
would be restored regardless. Ultimately, RT-PCR analysis revealed that intron 3
expression was not reduced. Additionally, the region exhibited deletions related to Alu
repeat elements. Splicing assay showed that exon 4 splicing was improved, but only
slightly.
Variant rs78154103 in intron 7 causes cryptic splice site selection and partial intron
retention. The variantâs effect on splicing was studied with a minigene splicing assay
using a longer construct than in a previous study, allowing formation of endogenous
secondary structures, suspected of contributing to dysregulated splicing. As
anticipated, more exon skipping was detected compared to reference, but inclusion of
cryptic exon did not differ between the constructs, contrary to what was expected.EturauhassyöpÀ on miesten yleisin syöpÀ. EturauhassyövÀssÀ, kuten muissakin syö vissÀ, havaitaan hÀiriintynyttÀ esiaste-RNA:n silmukointia. Vaihtoehtoinen silmukointi
mahdollistaa useamman erilaisen proteiinituotteen tuoton yhdestÀ geenistÀ vaihtoeh toisilla eksoni-introni-yhdistelmillÀ. Vaihtoehtoista silmukointia voidaan tutkia esimerkik si RT-PCR-tekniikalla (reverse transcriptase - polymerase chain reaction) tai minigee nien silmukointianalyysillÀ (engl. minigene splicing assay), joka mahdollistaa villityypin ja mutanttisekvenssin vertailun identtisissÀ olosuhteissa.
TÀssÀ tutkielmassa tutkin kahta ANO7-eturauhassyöpÀalttiusgeenissÀ ilmenevÀÀ sil mukointimutaatiota. Variantti rs77559646 sijaitsee introni 4:n luovuttaja-alueella, josta
silmukointi tavallisesti alkaa. Variantti johtaa eksoni 4:n poissilmukointiin ja lisÀÀnty neeseen introni 3:n ekspressioon oletettavasti estÀmÀllÀ silmukointiin tarvittavan ribo nukleoproteiinin sitoutumisen. Mutaatio on korjattu CRISPR-Cas9 ba se-editing -tekniikalla, mutta samanaikaisesti syntyi uusi mutaatio ylÀjuosteessa. Tut kimuksen tavoitteena oli selvittÀÀ, palautuuko asianmukainen silmukointi tahattomasta
muutoksesta huolimatta.
Introni 7:ssÀ sijaitseva variantti rs78154103 aiheuttaa kryptisen silmukointikohdan akti vaation, johtaen kryptisen eksonin (eksoni + osa intronia) muodostumiseen. Variantin
vaikutusta silmukointiin tutkittiin minigeenien silmukointianalyysillÀ kÀyttÀen pidempÀÀ
minigeenikonstruktia, kuin aiemmassa tutkimuksessa. Pidempi sekvenssi mahdollistaa
RNA:n sekundaarirakenteiden muodostumisen, joiden rakennemuutosten epÀillÀÀn
aiheuttavan hÀiriintynyttÀ silmukointia kyseisellÀ geenialueella. Odotusten mukaisesti
variantti aiheutti lisÀÀntynyttÀ eksonin poistoa verrattuna villityyppiin. Kryptinen eksoni
kuitenkin ekspressoitui yhtÀ vahvasti molemmilla konstrukteilla, toisin kuin odotettiin
Nutritional genomics and personalized diet
Nutritional genetics is considered as the combination of nutrigenomics and nutrigenetics. Nutrigenomics is establishing the effects of ingested nutrients and other food components on gene expression and gene regulation. It will also determine the individual nutritional requirements based on the genetic makeup of the person (personalized diet) as well as the association between diet and chronic diseases which will help to understand the etiologic aspects of chronic diseases such as cancer, type-2 diabetes, obesity and cardiovascular disease (CVS). Nutrigenetics on the other hand identifies how the genetic makeup of a particular individual co-ordinates his or her response to variousdietary nutrients. It also reveals why and how people respond differently to the same nutrient. The present review will focus upon interaction of genetic background and diet with regard to development of such life threatening chronic conditions as obesity, cardiovascular disease (CVD), and cancer that are responsible for the majority of deaths in developed Western countries
Naisten representaatio Harriet Martineaun ohjesÀÀntöistÀ prostituutiota vastustavassa poliittisessa aktivismissa 1800-luvun Britanniassa.
The purpose of this thesis is to examine the representations of women in the activism of 19th-century freethinking feminists and the underlying motives thereof. I analyse the representations offered by Harriet Martineau in her writings on the Contagious Diseases Acts â legislation regulating prostitution â consisting of the four letters to the London Daily News in 1869, electoral placards and personal letters written c. 1871. In examining the different representations of women, I use content analysis to categorise Martineauâs rhetoric in her writings on the middle-class activists and lower-class sex workers.
The three key concepts for this thesis are freethought, gender, and representation. By freethought I refer to the ideology of the organized 19th-century secularist movement that lobbied for the separation of political, cultural, and moral life from religion. I use the term gender as theorized by Joan Scott, conceptualising it as a constitutive element of social relationships based on perceived differences and as a way of signifying relationships of power. In utilizing the term representation, I use Stuart Hallâs definition of representation as âthe process by which members of a culture use language to produce meaningâ.
The representations of women in Martineauâs writings on the Acts are varied and conflicting. I identify three representations of middle-class women: self-sacrificing patriots, moral guardians, and intellectual educators. Each is used to justify womenâs move into politics. The emphasis on self-sacrifice and moral guardianship employs the dominant ideology of femininity in which women were passive but at the centre of the morality of the nation. However, at the same time Martineau challenges the dominant idea of femininity in presenting the female activists as active agents and intelligent educators.
I identify three representations of lower-class sex workers: passive victims, sinners, and fellow English women. In the victimizing rhetoric, Martineau utilizes the common idea of the sex workerâs fallenness to evoke sympathy. However, she never differentiates between the sex workers by virtue, presenting all as victims of society. Most radically she portrays them as English women, equal subjects of the British law. This egalitarian representation challenges the hierarchical structure underpinning the victimizing rhetoric.
I suggest Martineauâs representations draw from two distinct aspects: her middle-class background and her freethinking mission of bettering society through education. Her moral background explains the more traditional depictions of women. However, I argue that her belief in necessarianism motivates her egalitarian view of sex workers as fellow Englishwomen. I suggest that Martineauâs over-arching agenda was to educate the public. This is apparent in her appeals to the duty of citizens to learn and enlighten others
Beginning of a New Era: Remote Recruitment Takeover : How has the COVID-19 pandemic and changing work affected recruitment process evolvement?
Needs, identity, and leadership: a theory of conflict and change
This PhD dissertation aims to inform theories of conflict and International Relations (IR) by using modified social psychological models of identification and leadership in which needs fulfilment plays a central role. The main hypothesis is that identification with groups and leaders is flexible on the lower needs levels and more lasting on the higher needs levels, and that leadership, to be adaptive, must on the lower needs levels be action-oriented and on the higher levels be relations-oriented. This hypothesis is used to inform group and system level theories. On the group level, the hypothesis reads that due to this pattern of individual identification, cohesive collective action and violence in
physiological deprivation requires coercive leadership to make up for the absence of
unity, while on the higher needs levels collective violence necessitates manipulative
leadership to make up for the absence of real deprivation. On the system level, the
hypothesis reads that since the dynamics of collective action depend on the level of
needs fulfilment and identification, change in the system can only be understood by
examining all three levels of analysis. The first two hypotheses (on the individual and
group level) are developed and demonstrated through qualitative case studies on the
conflicts of the Sudan/South Sudan and between the former Yugoslav republics. These
hypotheses are then used to reconcile the various conflicting theories on each level of
analysis as well as to create a comprehensive framework through which the various theories and concepts of IR can be seen as connected to a certain level of needs security/development, and thus as historically and regionally specific
Work-life balance in remote working conditions
Koronapandemian alettua etÀtyöstÀ tuli uusi normaali monille työntekijöille ympÀri maailmaa. PerinteisestÀ toimistosta kotitoimistoon siirtyminen oli nopea muutos, johon organisaatiot ja työtekijÀt eivÀt olleet tÀysin varautuneet. Muutos on vaikuttanut työntekijöihin monin eri tavoin.
LisÀksi osalla henkilöistÀ on esimies- taustaa.
Sleep and Menopause. Hormone Therapy and Sleep Deprivation
This study evaluated the effect of menopause, hormone therapy (HT) and aging on sleep. Further, the mechanisms behind these effects were examined by studying the associations between sleep and the nocturnal profiles of sleep-related hormones. Crosssectional study protocols were used to evaluate sleep in normal conditions and during recovery from sleep deprivation. The effect of initiation of HT on sleep and sleeprelated hormones was studied in a prospective controlled trial. Young, premenopausal and postmenopausal women were studied, and the methods included polysomnography, 24-h blood sampling, questionnaires and cognitive tests of attention.
Postmenopausal women were less satisfied with their sleep quality than premenopausal women, but this was not reflected in sleepiness or attention. The objective sleep quality was mainly similar in pre- and postmenopausal women, but differed from young women. The recovery mechanisms from sleep deprivation were relatively well-preserved after menopause. HT offered no advantage to sleep after sleep deprivation or under normal conditions. The decreased growth hormone (GH) and prolactin (PRL) levels after menopause were reversible with HT. Neither menopause nor HT had any effect on cortisol levels. In premenopausal women, HT had only minor effects on PRL and cortisol levels. The temporal link between GH and slow wave sleep (SWS) was weaker after menopause. PRL levels were temporally associated with sleep stages, and higher levels were seen during SWS and lower during rapid-eye-movement (REM) sleep.
Sleep quality after menopause is better determined by age than by menopausal state. Although HT restores the decreased levels of GH and PRL after menopause, it offers no advantage to sleep quality under normal conditions or after sleep deprivation.Siirretty Doriast
Addressing telecommuting in cyber security guidelines
Cyber security threats are becoming more common than before. New phenomena in society
include new cyber security threats which organisations and society should prepare for. One of
these phenomena is telecommuting. Telecommuting has its roots already in the 1970s, but it has
become increasingly popular during the last years. Especially the pandemic caused by Covid-19
has changed the way of working drastically. Pandemic and the social distancing forced many
organisations to have their employees working from home. Information technology has abled
telecommuting, but it has also brought some problems such as security issues. Cyber security
threats have increased and become more diverse during the mass telecommuting caused by Covid-19. Telecommuting has some special features that can increase cyber security threats and risks.
In this research the following cyber security threats relating to telecommuting were identified to
be most relevant: cyber attacks, social engineering, unauthorized access and physical security.
Previous literature has identified that there exist cyber security threats in telecommuting, but it
has remained unclear how organisations manage and mitigate these in practice. Many of the
identified threats relate to employeesâ unwanted behaviour. Employees are unaware of the threats
facing the organisation in telecommuting. Some employees have not been provided with proper
guidelines and instruction on secure way of working. Information security policies and guidelines
are important for maintaining cyber security in organisations. Policies can be even seen as the
basis for organisationâs cyber security. This research studied which guidelines could be applicable
in a telecommuting environment in order to mitigate the common cyber security threats. Most
prominent cyber security guidelines for telecommuting identified in this research were guidelines
for personal and mobile devices, guidelines for social engineering, guidelines for physical
security, network guidelines, password guidelines and guidelines for online meetings.
Case study of multiple cases was used as a method for this study. The cases are seven Finnish
universities. The empirical data consists of cyber security and telecommuting guidelines from the
universities. These guidelines were analysed by reflecting to the theoretical framework. The
analysis showed that especially guidelines for physical security and online meetings were lacking.
The presence of outsiders in the telecommuting environment was addressed poorly. Outsiders are
a threat both to physical and online meeting security as outsiders may see or hear confidential
things. In addition, guidelines were not addressing data labelling and information release. Threats
specific to Covid-19 were also addressed poorly even though cyber criminals have exploited the
pandemic. Guidelines seemed to be otherwise comprehensive. Threats that were addressed poorly
have been especially relevant during the pandemic which suggests that organisationsâ guidelines
are not quite up to date even though otherwise applicable. Organisations should review and update
their guidelines periodically and if a major change occurs in the operation environment.Kyberturvallisuusuhat ovat yleistymÀssÀ. Uudet ilmiöt tuovat mukanaan uusia
kyberturvallisuusuhkia, joihin organisaatioiden ja yhteiskunnan tulee varautua. Yksi nÀistÀ
ilmiöistÀ on etÀtyö.
Tietotekniikka on mahdollistanut
huono fyysinen turvallisuus.
Aikaisemmassa kirjallisuudessa on havaittu, ettÀ etÀtyöhön liittyy kyberturvallisuusuhkia, mutta
on jÀÀnyt epÀselvÀksi, miten organisaatiot hallitsevat ja vÀhentÀvÀt niitÀ kÀytÀnnössÀ. Monet
tunnistetuista uhista liittyvÀt työntekijöiden ei-toivottuun kÀyttÀytymiseen. TyöntekijÀt eivÀt
vÀlttÀmÀttÀ ole tietoisia etÀtyön uhista organisaatiolle. Osalle työntekijöistÀ ei ole myöskÀÀn
annettu asianmukaisia ohjeita kyberturvallisista työskentelytavoista. Tietoturvapolitiikat ja -
ohjeet ovat tÀrkeitÀ organisaatioiden kyberturvallisuuden yllÀpitÀmisessÀ. Politiikkoja voidaan
pitÀÀ jopa organisaation kyberturvallisuuden perustana. TÀssÀ tutkimuksessa selvitettiin,
minkÀlaisia ohjeita tarvitaan etÀtyössÀ yleisten kyberturvallisuusuhkien lieventÀmiseksi. TÀssÀ
tutkimuksessa tunnistetut kyberturvallisuusohjeet etÀtyöhön liittyivÀt henkilökohtaisten ja
mobiililaitteiden kÀyttöön, sosiaaliseen manipulointiin, fyysiseen turvallisuuteen, turvattomiin
verkkoihin, salasanoihin ja online-kokouksiin.
Tutkimusmetodina tÀssÀ tutkimuksessa kÀytettiin usean tapauksen tapaustutkimusta. Tapauksina
toimivat seitsemÀn suomalaista yliopistoa. Empiirinen data koostuu Suomessa toimivien
yliopistojen kyberturvallisuus- ja etÀtyöohjeista. NÀmÀ ohjeet analysoitiin teoreettiseen
viitekehyksen avulla ja siihen viitaten. Analyysi osoitti, ettÀ erityisesti fyysistÀ turvallisuutta ja
online-kokouksia koskevat ohjeet ovat puutteellisia. Ulkopuolisten lÀsnÀolo etÀtyöympÀristössÀ
on huomioitu huonosti.
