    Approach to Assessing the Preparedness of Hospitals to Power Outages

    Within the secondary impacts of electricity blackouts, it is necessary to pay attention to facilities providing medical care for the population, namely the hospitals. Hospitals represent a key position in the provision of health care also in times of crisis. These facilities must provide constant care; it is therefore essential that the preparedness of such facilities is kept at a high level. The basic aim of this article is to analyse the preparedness of hospitals to power outages (power failures, blackouts) within a pilot study. On that basis, a SWOT analysis is used to determine strengths and weaknesses of the system of preparedness of hospitals to power outages and solutions for better security of hospitals are defined. The sample investigated consists of four hospitals founded by the Regional Authority (hospitals Nos. 1-4) and one hospital founded by the Ministry of Health of the Czech Republic (hospital No. 5). The results of the study shows that most weaknesses of the preparedness of hospitals are represented by inadequately addressed reserves of fuel for the main backup power supply, poor knowledge of employees who are insufficiently retrained, and old backup power supplies (even 35 years in some cases)

Firmou mi byly poskytnuty veškeré potřebné účetní výkazy, díky nimž mohla být bakalářská práce zpracována. Na základě zjištěných výsledků byly navrženy kroky, které by firmě mohly pomoci k lepším výsledkům.This thesis focuses on evaluating the financial situation of NEPA Ltd. company using the methods of financial analysis and the appropriate security information system. The company provided me with the necessary financial statements which helped me complete the thesis. Based on the results, there were certain steps proposed which could help Nepa company achieve better results.

Praktická část práce obsahuje také analýzu současného stavu podnikatelského subjektu i situace na chorvatském trhu. Závěr práce obsahuje finanční plán a definuje možná rizika, která by mohla ovlivnit celou investici.The diploma thesis deals with a proposal to build and operate a tourist resort in Croatia. The theoretical part explains all the important concepts and analyses, which are subsequently used in the analytical part. The empirical part also contains an analysis of the current state of the business entity and the situation on the Croatian market. Financial plan and potential risks are defined at the end of the thesis.

    Disertační práce se zabývá problematikou určování regionálních kritických prvků v oblasti zdravotnictví. Práce je koncipována do tří navazujících částí. V rámci teoretické části je provedena deskripce přístupů k určování prvků kritické infrastruktury v České republice, deskripce zdravotnické infrastruktury v ČR a deskripce současného stavu hodnocení kritických prvků v oblasti zdravotnictví ČR. Analytická část je orientována na analýzu přístupů k hodnocení kritičnosti ve vybraných zemích, a to se zaměřením na analýzu metod a kritérií využitelných pro systém určování kritických prvků. Následně navazuje výběr klíčových pracovišť nemocnice a jejich analýza. Stěžejní částí práce je návrh systému určování kritických prvků v nemocničním zařízení na úrovni regionu. V rámci tohoto systému je nadefinován jeho rámec vymezující komponenty, které jsou východisky pro definování procesu určování kritických prvků ve zdravotnictví. Na základě tohoto rámce je navržen algoritmus určování kritických prvků a jeho kritéria pro posuzování kritičnosti. V závěru práce je tento systém verifikován formou případové studie.The dissertation deals with the identification of regional critical elements in Healthcare. The dissertation is conceived into three subsequent parts. The theoretical part describes approaches to identify elements of the critical infrastructure in the Czech Republic, the Healthcare infrastructure in the Czech Republic, and also describes the current system of evaluation of elements of the critical infrastructure in Healthcare in the Czech Republic. The analytical part is directed on the analysis of approaches of the evaluation of criticality in selected countries, with the focus on the analysis of methods and criteria applicable to the system of determining the critical elements. The following part is focused on the selection of the key hospital departments and their analysis. The main part of this dissertation is the design of the system to determine the critical elements in hospital facilities at the regional level. The framework defining the components that are basic for the definition of the process of determining the critical elements in Healthcare are specified within the system. Based on this framework, the algorithm to determine the critical elements is designed along with the criteria to evaluate criticalness. Úvod práce popisuje současný stav kritické infrastruktury v Evropské unii se zaměřením na Českou republiku. Na tuto část navazuje deskripce zdravotnictví v České republice ve vztahu ke kritické infrastruktuře. Následně je provedena analýza problematiky vybraných částí ve zdravotnické kritické infrastruktuře. Zároveň byla provedena analýza vybraných zahraničních zemí se zaměřením na zdravotnickou kritickou infrastrukturu, která v závěrečné části poskytuje návrh k bezpečnostním opatřením zdravotnické kritické infrastruktury. Poslední část problematiky se zabývá analýzou traumatologického plánu fakultních nemocnic. Na základě deskripce a analýzy se závěrečná část diplomové práce zabývá návrhem bezpečnostních opatření pro zdravotnictví s návrhem nových odvětvových kritérií. V závěru práce jsou navrženy možnosti vybraných opatření směřujících k docílení zabezpečení odolnosti zdravotnické kritické infrastruktury.This thesis deals with the health of critical infrastructure and security. The introduction thesis describes the current condition of critical infrastructure in the European Union with a focus on the Czech Republic. This part is followed by the description of health care in the Czech Republic in relation to critical infrastructure. Subsequently, it analyzes the problems of selected parts of the health of critical infrastructure. At the same time, an analysis of the selected countries with a focus on critical health infrastructure in the final section provides a proposal of security measures of medical critical infrastructure. The last part of the issue analyzes the trauma plan faculty hospitals. Based on the description and analysis of the final part of the thesis describes the proposal of security measures for the health sector with the proposed new criteria.     Hyaluronan polymeric micelles for topical drug delivery

    Nanosized materials offer promising strategy for topical drug delivery due to their enhancing effect on drug percutaneous transport across the stratum corneum barrier. In this work, polymeric micelles made from hydrophobized hyaluronic acid (HA) were probed for skin delivery. Compared to non-polymeric micelle solutions containing similar drug amount, in vitro skin penetration analysis indicated 3 times larger deposition of drug in the epidermis and 6 times larger drug deposition in the dermis after 5 h of topical treatment in Franz diffusion cells. The drug deposition was further increased with prolonged time of topical treatment. Laser confocal microscopy revealed the accumulation of both, the HA forming the vehicle and the payload, in the epidermis and dermis. Although fluorescent labeling of the HA would suggest co-transport of the HA and the drug, loading FRET pair dyes in the micellar core clearly demonstrated gradual micelle disruption with increasing skin depth. Transcellular penetration was the predominant pathway for the loaded drug. The HA polymeric micelles also demonstrated increased bioactivity of loaded compound in vitro and in vivo. In addition, the loaded micelles were found to be stable in cream formulations and thus they have great potential for topical applications for cosmetic and pharmaceutical purposes. © 2016Internal Funding Agency of the Tomas Bata University in Zlin [IGA/FT/2016/001

    Retinoic Acid Grafted to Hyaluronic Acid Activates Retinoid Gene Expression and Removes Cholesterol from Cellular Membranes

    All-trans-retinoic acid (atRA) is a potent ligand that regulates gene expression and is used to treat several skin disorders. Hyaluronic acid (HA) was previously conjugated with atRA (HA-atRA) to obtain a novel amphiphilic compound. HA-atRA forms micelles that incorporate hydrophobic molecules and facilitate their transport through the skin. The aim of this study was to determine the influence of HA-atRA on gene expression in skin cells and to compare it with that of unbound atRA. Gene expression was investigated using microarrays and a luciferase system with a canonical atRA promoter. HA-atRA upregulated gene expression similarly to atRA. However, HA-atRA activated the expression of cholesterol metabolism genes, unlike atRA. Further investigation using HPLC and filipin III staining suggested that the treated cells induced cholesterol synthesis to replenish the cholesterol removed from the cells by HA-atRA. HA modified with oleate (HA-C18:1) removed cholesterol from the cells similarly to HA-atRA, suggesting that the cholesterol removal stemmed from the amphiphilic nature of the two derivatives. HA-atRA induces retinoid signaling. Thus, HA-atRA could be used to treat skin diseases, such as acne and psoriasis, where the combined action of atRA signaling and anti-inflammatory cholesterol removal may be potentially beneficial
